| 1 | NIHSS绝对值变化与NIHSS变化率对脑梗死患者残疾预测能力的比较研究显示文摘目的探讨美国国立卫生研究院卒中量表(NIHSS)绝对值的变化与NIHSS变化率对脑梗死患者功能残疾的预测价值。方法采用前瞻性设计对10个临床研究中心急性期脑梗死患者的数据进行分析,筛选出入院当天、发病90d NIHSS评分和BI数据资料完整患者953例,按入院时NIHSS评分分为轻、中、重度,以BI作为功能残疾疗效判定'金标准',应用ROC比较NIHSS绝对值变化与NIHSS变化率对患者功能残疾的判断价值。结果轻、中、重度患者AUC曲线下面积分别为0.60、0.76、0.93,与AUC曲线下面积=0.50比较,中、重度患者差异有统计学意义(P<0.05)。NIHSS变化率在轻、中、重度患者中分别为0.76、0.85、0.97,与AUC曲线下面积=0.50比较,差异均有明显统计学意义(P<0.01)。脑梗死患者良好功能结局的最佳NIHSS绝对值变化的界值在轻、中、重度患者中分别为2.5、4.5、14.5;敏感性分别为0.57、0.92、1.00,特异性分别为0.61、0.50、0.89;NIHSS变化率的界值在轻、中、重度患者中分别为70.8%、74.7%、84.9%;敏感性分别为0.63、0.82、1.00,特异性分别为0.78、0.75、0.94。结论 NIHSS绝对值变化、NIHSS变化率对患者功能残疾的预测能力随着神经功能缺损程度的增加而增大,而后者对结局预测能力更佳。 | 王平平 高颖 刘璐 马斌 曹克刚 辛喜艳 张杨 ShaPour Badiee Aval Baghyahi | 2011 | 中风与神经疾病杂志2011,28,2: | 52 |

| 3 | Microstructural evolution and mechanical properties of friction stir-welded C71000 copper–nickel alloy and 304 austenitic stainless steel显示文摘Dissimilar joints comprised of copper–nickel and steel alloys are a challenge for manufacturers in modern industries, as these metals are not thermomechanically or chemically well matched. The present study investigated the effects of tool rotational speed and linear speed on the microstructure and mechanical properties of friction stir-welded C71000 copper–nickel and 340 stainless steel alloys using a tungsten carbide tool with a cylindrical pin. The results indicated that a rotational-to-linear speed ratio of 12.5 r/mm did not cause any macro defects, whereas some tunneling defects and longitudinal cracks were found at other ratios that were lower and higher. Furthermore, chromium carbide was formed on the grain boundaries of the 304 stainless steel near the shoulder zone and inside the joint zone, directing carbon and chromium penetration toward the grain boundaries. | 7 | Effectiveness of two conventional methods for seismic retrofit of steel and RC moment resisting frames based on damage control criteria显示文摘This study investigates the efficiency of two types of rehabilitation methods based on economic justification that can lead to logical decision making between the retrofitting schemes. Among various rehabilitation methods, concentric chevron bracing(CCB) and cylindrical friction damper(CFD) were selected. The performance assessment procedure of the frames is divided into two distinct phases. First, the limit state probabilities of the structures before and after rehabilitation are investigated. In the second phase, the seismic risk of structures in terms of life safety and financial losses(decision variables) using the recently published FEMA P58 methodology is evaluated. The results show that the proposed retrofitting methods improve the serviceability and life safety performance levels of steel and RC structures at different rates when subjected to earthquake loads. Moreover, these procedures reveal that financial losses are greatly decreased, and were more tangible by the application of CFD rather than using CCB. Although using both retrofitting methods reduced damage state probabilities, incorporation of a site-specific seismic hazard curve to evaluate mean annual occurrence frequency at the collapse prevention limit state caused unexpected results to be obtained. | 10 | ROC曲线分析NIHSS的变化率预测脑梗死患者功能残疾的可行性显示文摘目的通过对美国国立卫生研究院卒中量表(NIHSS)评分变化率的分析试图为脑梗死临床结局评价方法的选择提供一种新的思路与方法。方法本项研究采取前瞻性设计对来自10个临床研究中心急性期脑梗死住院患者的数据内容进行分析,筛选出入院当天NIHSS评分、发病90 d NIHSS评分和BI数据资料完整患者953例,将患者按入院时NIHSS评分分为3级:轻度(〈7分)、中度(7分-15分)、重度(〉15分),以BI作为功能残疾的疗效判定"金标准",发病90 d BI≥95分为预后良好、BI〈95分为预后不良的分界点将BI转换为二分类变量(残疾与否),应用ROC曲线了解NIHSS的变化率对脑梗死患者发病90天功能残疾的判断价值。结果本研究发现NIHSS变化率预测脑梗死患者发病90 d功能残疾的AUC曲线下面积均〉0.70,轻度患者为0.76、中度患者为0.85,重度患者为0.97,与AUC曲线下面积=0.50进行比较,差异有统计学意义(P≤0.001)。预测脑梗死患者良好功能结局的最佳NIHSS变化率的截断值在轻度、中度、重度神经功能缺损的患者分别为70.8%、74.7%、84.9%;其灵敏度在三者分别为63.0%、81.8%、100.0%,特异度分别为78.1%、75.4%、94.3%。结论 NIHSS的变化率对脑梗死患者发病90 d功能残疾具有较好的预测能力,且这种预测能力随着神经功能缺损程度的增加而增大。 | 王平平 高颖 刘璐 马斌 曹克刚 辛喜艳 Shapour Badiee Aval Baghyahi | 2011 | 中西医结合心脑血管病杂志2011,9,4: | 2 |
