Helicobacter pylori infection- recent developments in diagnosis显示文摘Considering the recommended indications for Helicobacter pylori(H.pylori)eradication therapy and the broad spectrum of available diagnostic methods,a reliable diagnosis is mandatory both before and after eradication therapy.Only highly accurate tests should be used in clinical practice, and the sensitivity and specificity of an adequate test should exceed 90%.The choice of tests should take into account clinical circumstances, the likelihood ratio of positive and negative tests, the cost-effectiveness of the testing strategy and the availability of the tests. This review concerns some of the most recent developments in diagnostic methods of H.pylori infection, namely the contribution of novel endoscopic evaluation methodologies for the diagnosis of H.pylori infection, such as magnifying endoscopy techniques and chromoendoscopy. In addition, the diagnostic contribution of histology and the urea breath test was explored recently in specific clinical settings and patient groups.Recent studies recommend enhancing the number of biopsy fragments for the rapid urease test.Bacterial culture from the gastric biopsy is the gold standard technique, and is recommended for antibiotic susceptibility test.Serology is used for initial screening and the stool antigen test is particularly used when the urea breath test is not available, while molecular methods have gained attention mostly for detecting antibiotic resistance. | Ana Isabel Lopes Filipa F Vale Mónica Oleastro | 2014 | World Journal of Gastroenterology2014,20,28: | 12
Evaluation of biodegradable electric conductive tube-guides and mesenchymal stem cells显示文摘AIM: To study the therapeutic effect of three tubeguides with electrical conductivity associated to mesenchymal stem cells(MSCs) on neuro-muscular regeneration after neurotmesis.METHODS: Rats with 10-mm gap nerve injury were tested using polyvinyl alcohol(PVA), PVA-carbon nanotubes(CNTs) and MSCs, and PVA-polypyrrole(PPy). The regenerated nerves and tibialis anterior muscles were processed for stereological studies after 20 wk. The functional recovery was assessed serially for gait biomechanical analysis, by extensor postural thrust, sciatic functional index and static sciatic functionalindex(SSI), and by withdrawal reflex latency(WRL). In vitro studies included cytocompatibility, flow cytometry, reverse transcriptase polymerase chain reaction and karyotype analysis of the MSCs. Histopathology of lung, liver, kidneys, and regional lymph nodes ensured the biomaterials biocompatibility. RESULTS: SSI remained negative throughout and independently from treatment. Differences between treted groups in the severity of changes in WRL existed, showing a faster regeneration for PVA-CNTs-MSCs(P < 0.05). At toe-off, less acute ankle joint angles were seen for PVA-CNTs-MSCs group(P = 0.051) suggesting improved ankle muscles function during the push off phase of the gait cycle. In PVA-PPy and PVA-CNTs groups, there was a 25% and 42% increase of average fiber area and a 13% and 21% increase of the 'minimal Feret's diameter' respectively. Stereological analysis disclosed a significantly(P < 0.05) increased myelin thickness(M), ratio myelin thickness/axon diameter(M/d) and ratio axon diameter/fiber diameter(d/D; g-ratio) in PVA-CNT-MSCs group(P < 0.05). CONCLUSION: Results revealed that treatment with MSCs and PVA-CNTs tube-guides induced better nerve fiber regeneration. Functional and kinematics analysis revealed positive synergistic effects brought by MSCs and PVA-CNTs. The PVA-CNTs and PVA-PPy are promising scaffolds with electric conductive properties, bio- and cytocompatible that might prevent the secondary neurogenic muscular atrophy by improving the reestablishment of the neuro-muscular junction. | Jorge Ribeiro Tiago Pereira Ana Rita Caseiro Paulo Armada-da-Silva Isabel Pires Justina Prada Irina Amorim Sandra Amado Miguel Franca Carolina Goncalves Maria Ascensao Lopes Jose Domingos Santos Dina Morais Silva Stefano Geuna Ana Lúcia Luís Ana Colette Maurício | 2015 | World Journal of Stem Cells2015,7,6: | 2

Autoimmune gastritis presenting as iron deficiency anemia in childhood显示文摘AIM: To characterize clinical, laboratorial, and histological profile of pediatric autoimmune gastritis in the setting of unexplained iron deficiency anemia investigation.METHODS: A descriptive, observational study including pediatric patients with a diagnosis of autoimmune gastritis(positive parietal cell antibody and gastric corpus atrophy) established in a 6 year period(2006-2011) in the setting of refractory iron deficiency anemia(refractoriness to oral iron therapy for at least 6 mo and requirement for intravenous iron therapy) investigation, after exclusion of other potentially contributing causes of anemia. Helicobacter pylori(H. pylori) infection and anti-secretory therapy were also excluded. Data were retrospectively collected from clinical files, including: demographic data(age, gender, and ethnic background), past medical history, gastrointestinal symptoms, familial history, laboratorial evaluation(Hb, serum ferritin, serum gastrin, pepsinogen Ⅰ/ pepsinogen Ⅱ, B12 vitamin, intrinsic factor autoantibodies, thyroid autoantibodies, and anti-transglutaminase antibodies), and endoscopic and histological findings(HE, Periodic Acid-Schiff/Alcian blue, gastrin, chromogranin A and immunochemistry analysis for CD3, CD20 and CD68). Descriptive statistical analysis was performed(mean, median, and standard deviation).RESULTS: We report a case-series concerning 3 girls and 2 boys with a mean age of 13.6 ± 2.8 years(3 Caucasian and 2 African). One girl had type Ⅰ diabetes. Familial history was positive in 4/5 cases, respectively for autoimmune thyroiditis(2/5), sarcoidosis(1/5) and multiple myeloma(1/5). Laboratorial evaluation on admission included: Hb: 9.5 ± 0.7 g/d L; serum ferritin: 4.0 ± 0.9 ng/m L; serum gastrin: 393 ± 286 pg/m L; low pepsinogen Ⅰ/ pepsinogen Ⅱ ratio in 1/5 patients; normal vitamin B12 levels(analyzed in 3 patients). Endoscopy findings included: duodenal nodularity(2/5) and gastric fold softening(2/5), and histological evaluation showed corpus atrophic gastritis with lymphocytic infiltration(5/5), patchy oxyntic gland mononuclear cell infiltration(5/5), intestinal and/or pseudo-pyloric metaplasia in corpus mucosa(4/5), and enterochromaffin cell hyperplasia(4/5). Immunochemistry for gastrin on corpus biopsies was negative in all cases. Duodenal histology was normal. All biopsies were negative for H. pylori(Giemsa staining and cultural examination). CONCLUSION: We highlight autoimmune gastritis as a diagnosis to be considered when investigating refractory iron deficiency anemia in children, particularly in the setting of a personal/familial history of autoimmune disease, as well as the diagnostic contribution of a careful immunohistological evaluation. | Cristina Gon?alves Maria Emília Oliveira Ana M Palha Anabela Ferr?o Anabela Morais Ana Isabel Lopes | 2014 | World Journal of Gastroenterology2014,20,42: | 1
Successful endoscopic resolution of a large gastric bezoar in a child显示文摘Bezoars are masses or concretions of indigestible materials found in the gastrointestinal tract, usually in the stomach.Case reports of childhood gastric bezoars(particularly phytobezoars)are rare.In this age group they represent a therapeutic challenge, because of the combination of hard consistency and great size. The present report concerns an 8-year-old boy with a history of high fruit intake, presenting with abdominal complaints due to a large gastric phytobezoar.Successful endoscopic fragmentation coupled with suction removal was accomplished, using a standard-channel endoscope.Although laborious,it has been shown to be an efficacious and safe procedure, completed in one session.Endoscopic techniques for pediatric bezoar management may thus be cost effective, taking into account the avoidance ofsurgery, the length of the hospital stay and the number of endoscopic sessions. | Sara Azevedo Joo Lopes António Marques Paula Mourato Lucília Freitas Ana Isabel Lopes | 2011 | World Journal of Gastrointestinal Endoscopy2011,3,6: | 1
High-resolution multi-scale air pollution system:Evaluation of modelling performance and emission control strategies显示文摘A high percentage of the world's population lives in areas where air pollutant concentrations exceed the World Health Organization guidelines. This work aims to develop and test,a high-resolution multi-scale air pollution modelling system by integrating a set of adequate tools. This system is able to provide detailed air pollutant concentrations in urban areas and support air quality management strategies through a better identification of different atmospheric processes. It also allows furthering the design and assessment of air pollution control measures for a specific area.To evaluate its performance and suitability, the system was applied to the Macao Special Administrative Region(SAR),China, one of the most densely populated areas on earth, during a winter period when this area is affected by high levels of Particulate Matter(PM).Although the developed system tends to underestimate the PM concentrations,it revealed a good performance in reproducing the temporal and spatial air pollution patterns.Several exceedances of the Chinese air quality standards were calculated and high population exposure to PM pollution was estimated. The tested urban atmospheric emission reduction scenarios have shown air quality improvements, indicating that emission reduction measures at urban level should focus on the domestic sector. However,it is crucial to implement joint pollution prevention strategies with neighbouring regions to improve the air quality in Macao SAR.The approach developed in this work can support policymakers in defining new strategies to reduce atmospheric pollution in urban areas. | Diogo Lopes Joana Ferreira Sandra Rafael Ka In Hoi Xin Li Yuan Liu Ka-Veng Yuen Kai Meng Mok Ana Isabel Miranda | 2024 | Journal of Environmental Sciences2024,,3: | 0
Motivations for and barriers to innovation in non-profit organizations: The case of nursing homes in Northern Portugal显示文摘To contribute to the empirical literature on innovation in non-profit organizations, this study examines innovation in Private Social Solidarity Institutions with social responses for the elderly in Northern Portugal. It enriches the literature by clarifying the concept of innovation applied to non-profit organizations, which represents a perspective different from the business context. Moreover,it is important to support the innovation decisionmaking process in similar non-profit organizations. For this study, the population was surveyed through questionnaires based on the PUBLIN research project, which has already been applied in different countries, and the targets were employees of organizations with social responses for the elderly. Our sample is small, with 28 respondent institutions and 51 employees, but it is representative,as it includes institutions from all districts in Northern Portugal. The study employed a quantitative approach, particularly factor analysis, multiple linear regression, and correlation analyses, using scales already applied in other dimensions of the social sector,to understand the effect of each innovation dimension/determinant on creativity and innovation in nonprofit organizations. The results show that in these organizations with social responses for the elderly, learning orientation,risk-taking,and job satisfaction have a positive impact on innovation. | Ana Rita Gonçalves Lopes Aldina Isabel de Azevedo Correia Alexandra Maria da Silva Braga | 2024 | International Journal of Innovation Studies2024,8,1: | 0
