| 1 | 丹参素对大鼠肝缺血再灌注损伤的保护作用显示文摘目的 观察丹参素对大鼠肝缺血再灌注损伤的保护作用.方法 60只SD大鼠随机分成3组(每组20只):假手术(Sham)组、缺血再灌注(I/R)组、丹参预处理(Sal+I/R)组.解剖出肝脏中叶、左叶的门静脉、肝动脉以及胆管,应用小血管夹钳夹后造成肝脏左、中叶缺血,1h后恢复血流,构建缺血再灌注模型.假手术(Sham)组按同样步骤解剖,但不阻断血流.丹参预处理(Sal+I/R)组于缺血前60 min经尾静脉注射丹参素40 mg/kg.再灌注6h后,各组随机各取10只大鼠,经下腔静脉收集血液标本及留取肝组织标本.再灌注12h后同样方法处理剩余大鼠.测血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平.检测肝脏组织中谷胱甘肽过氧化物酶(GPX)和超氧化物歧化酶(SOD)的水平.苏木素-伊红(HE)染色,光镜观察肝脏组织病理学的变化.结果 Sal+ I/R组血清中ALT、AST、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平[6 h:(350.47 ±69.65) U/L、(500.25±76.12) U/L、(165.32±23.46) ng/L、(65.31±9.52) ng/L;12 h:(250.47±36.78) U/L、(355.25±59.12) U/L、(130.65±34.31) ng/L、(103.38±18.46) ng/L]均低于I/R组[6 h:(550.34±82.31) U/L、(702.28±106.48) U/L、(300.13±49.28) ng/L、(95.55 ±29.21) ng/L;12 h:(490.34 ±89.68) U/L、(550.28±64.23) U/L、(260.00±60.08) ng/L、(180.32±38.12) ng/L,P=0.019];Sal+ I/R组肝脏组织中GPX和SOD的水平[6 h:(52.34±11.59)、(63.00±19.23) pg/ml;12 h:(68.32±21.45)、(81.00±17.65) pg/ml]高于I/R组[6 h:(30.12±5.12)、(43.00 ±7.45) pg/ml;12 h:(39.58 ±3.16)、(54.00±8.46) pg/ml,P=0.029];光镜下Sal+ I/R组肝脏组织损伤较I/R组轻.结论 丹参素可降低大鼠肝脏缺血再灌注损伤后血清IL-6、TNF-α的水平,提高肝组织中GPX和SOD的水平,减轻缺血再灌注后肝脏的损伤程度. | 黄庆先 贾秀梅 闫博 王雪峰 刘小方 Akash Masaud | 2018 | 中华实验外科杂志2018,35,5: | 6 |

| 5 | Imaging-based algorithmic approach to gallbladder wall thickening显示文摘Gallbladder(GB) wall thickening is a frequent finding caused by a spectrum of conditions. It is observed in many extracholecystic as well as intrinsic GB conditions. GB wall thickening can either be diffuse or focal. Diffuse wall thickening is a secondary occurrence in both extrinsic and intrinsic pathologies of GB, whereas, focal wall thickening is mostly associated with intrinsic GB pathologies. In the absence of specific clinical features, accurate etiological diagnosis can be challenging. The survival rate in GB carcinoma(GBC) can be improved if it is diagnosed at an early stage, especially when the tumor is confined to the wall. The pattern of wall thickening in GBC is often confused with benign diseases, especially chronic cholecystitis, xanthogranulomatous cholecystitis, and adenomyomatosis. Early recognition and differentiation of these conditions can improve the prognosis. In this minireview, the authors describe the patterns of abnormalities on various imaging modalities(conventional as well as advanced) for the diagnosis of GB wall thickening. | 7 | Complementary roles of interventional radiology and therapeutic endoscopy in gastroenterology显示文摘Acute upper and lower gastrointestinal bleeding, enteral feeding, cecostomy tubes and luminal strictures are some of the common reasons for gastroenterology service. While surgery was initially considered the main treatment modality, the advent of both therapeutic endoscopy and interventional radiology have resulted in the paradigm shift in the management of these conditions. In this paper, we discuss the patient's work up, indications, and complementary roles of endoscopic and angiographic management in the settings of gastrointestinal bleeding, enteral feeding, cecostomy tube placement and luminal strictures. These conditions often require multidisciplinary approaches involving a team of interventional radiologists, gastroenterologists and surgeons. | 9 | Radiation-induced in vitro mutagenesis system for salt tolerance and other agronomic characters in sugarcane(Saccharum officinarum L.)显示文摘Gamma ray-induced in vitro mutagenesis and selection for salt(NaC l) tolerance were investigated in sugarcane(Saccharum officinarum L.). Embryogenic callus cultures were irradiated(10 to 80 Gy) and subjected to in vitro selection by exposure of irradiated callus to NaC l(0, 50, 100,150, 200, and 250 mmol L-1). Increasing NaC l concentrations resulted in growth reduction and increased membrane damage. Salt-selected callus lines were characterized by the accumulation of proline, glycine betaine, and Na+and K+concentration. Higher accumulation of proline and glycine betaine was observed in NaC l stressed callus irradiated at 20 Gy. Na+concentration increased and K+concentration decreased with increasing salt level. Irradiated callus showed50–60% regeneration under NaC l stress, and in vitro-regenerated plants were acclimatized in the greenhouse, with 80–85% survival. A total of 138 irradiated and salt-selected selections were grown to maturity and their agronomic performance was evaluated under normal and saline conditions. Of these, 18 mutant clones were characterized for different agro-morphological characters and some of the mutant clones exhibited improved sugar yield with increased Brix%,number of millable canes, and yield. | 12 | T4 cervical esophageal cancer cured with modern chemoradiotherapy: A case report显示文摘BACKGROUND T4 esophageal cancer portends a poor prognosis,particularly when it is complicated by a tracheoesophageal fistula(TEF)either resulting from disease or occurring as a complication of treatment.Patients with TEF that occurs during treatment are commonly treated with palliative intent because fistula-associated treatment complications such as aspiration pneumonia and mediastinitis are associated with high morbidity and mortality.To date,there is no clear evidence on the optimal treatment of T4 esophageal cancer,particularly when a TEF formation occurs.CASE SUMMARY A 67-year-old gentleman who presented with dysphagia and weight loss.Endoscopy and imaging revealed a T4N1M0 cervical esophageal squamous cell carcinoma.He received image-guided intensity-modulated radiation therapy,with concurrent weekly carboplatin(area under curve 2 mg/mL per minute)and paclitaxel(50 mg/m2 of body surface area).One week after treatment initiation(16.2 Gy thus far),he developed cough on swallowing.A TEF was detected on image-guided radiation therapy using cone-beam computed tomography during the treatment course,for which a tracheal stent was inserted.After discussing the risks and morbidity of continuing treatment,he resumed chemoradiotherapy with an additional radiation dose of 45 Gy in 25 fractions.Three months after completion of chemoradiotherapy,he developed an esophageal stricture that required esophageal stenting and dilatation.The patient remains cancer-free at two year on follow-up.Complete response of esophageal cancer was evident on post-treatment endoscopy and computed tomography imaging,with successful closure of TEF.CONCLUSION This case highlights that successful curative treatment for esophageal cancer complicated by a TEF is possible using novel chemotherapeutic regimens and modern radiation technologies. | Chia Ching Lee Chong Ming Yeo Wee Khoon Ng Akash Verma Jeremy CS Tey | 2020 | World Journal of Clinical Cases2020,8,10: | 2 |
| 13 | Hydrogen sulfide intervention in cystathionine-β-synthase mutant mouse helps restore ocular homeostasis显示文摘AIM: To investigate the applications of hydrogen sulfide(H2 S) in eye-specific ailments in mice.METHODS: Heterozygous cystathionine-β-synthase(CBS+/-) and wild-type C57 BL/6 J(WT) mice fed with or without high methionine diet(HMD) were administered either phosphate buffered saline(PBS) or the slow-release H2 S donor: GYY4137. Several analyses were performed to study GYY4137 effects by examining retinal lysates for key protein expressions along with plasma glutamate and glutathione estimations. Intraocular pressure(IOP) was monitored during GYY4137 treatment; barium sulfate and bovine serum albumin conjugated fluorescein isothiocyanate(BSA-FITC) angiographies were performed for examining vasculature and its permeability post-treatment. Visionguided behavior was also tested employing novel object recognition test(NORT) and light-dark box test(LDBT) recordings.RESULTS: CBS deficiency(CBS+/-) coupled with HMD led disruption of methionine/homocysteine(Hcy) metabolism leading to hyperhomocysteinemia(HHcy) in CBS+/-mice as reflected by increased Hcy, and s-adenosylhomocysteine hydrolase(SAHH) levels. Unlike CBS, cystathionine-γ lyase(CSE), methylenetetrahydrofolate reductase(MTHFR) levels which were reduced but compensated by GYY4137intervention. Heightened oxidative and endoplasmic reticulum(ER) stress responses were mitigated by GYY4137 effects along with enhanced glutathione(GSH) levels. Increased glutamate levels in CBS+/-strain were prominent than WT mice and these mice also exhibited higher IOP that was lowered by GYY4137 treatment. CBS deficiency also resulted in vision-guided behavioral impairment as revealed by NORT and LDBT findings. Interestingly, GYY4137 was able to improve CBS+/-mice behavior together with lowering their glutamate levels. Blood-retinal barrier(BRB) appeared compromised in CBS+/-with vessels' leakage that was mitigated in GYY4137 treated group.
