2Impact of community-based interventions on HIV knowledge, attitudes, and transmission显示文摘In 2012,an estimated 35.3 million people lived with HIV,while approximately two million new HIV infections were reported.Community-based interventions(CBIs)for the prevention and control of HIV allow increased access and ease availability of medical care to population at risk,or already infected with,HIV.This paper evaluates the impact of CBIs on HIV knowledge,attitudes,and transmission.We included 39 studies on educational activities,counseling sessions,home visits,mentoring,women’s groups,peer leadership,and street outreach activities in community settings that aimed to increase awareness on HIV/AIDS risk factors and ensure treatment adherence.Our review findings suggest that CBIs to increase HIV awareness and risk reduction are effective in improving knowledge,attitudes,and practice outcomes as evidenced by the increased knowledge scores for HIV/AIDS(SMD:0.66,95%CI:0.25,1.07),protected sexual encounters(RR:1.19,95%CI:1.13,1.25),condom use(SMD:0.96,95%CI:0.03,1.58),and decreased frequency of sexual intercourse(RR:0.76,95%CI:0.61,0.96).Analysis shows that CBIs did not have any significant impact on scores for self-efficacy and communication.We found very limited evidence on community-based management for HIV infected population and prevention of mother-to-child transmission(MTCT)for HIV-infected pregnant women.Qualitative synthesis suggests that establishment of community support at the onset of HIV prevention programs leads to community acceptance and engagement.School-based delivery of HIV prevention education and contraceptive distribution have also been advocated as potential strategies to target high-risk youth group.Future studies should focus on evaluating the effectiveness of community delivery platforms for prevention of MTCT,and various emerging models of care to improve morbidity and mortality outcomes.Rehana A Salam Sarah Haroon Hashim H Ahmed Jai K Das Zulfiqar A Bhutta 2014Infectious Diseases of Poverty2014,3,1:3
3Predictors of thirty-day readmission in nonagenarians presenting with acute heart failure with preserved ejection fraction: a nationwide analysis显示文摘BACKGROUD Acute heart failure with preserved ejection fraction(HFpEF)is a common but poorly studied cause of hospital admissions among nonagenarians.This study aimed to evaluate predictors of thirty-day readmission,in-hospital mortality,length of stay,and hospital charges in nonagenarians hospitalized with acute HFpEF.METHODS Patients hospitalized between January 2016 and December 2018 with a primary diagnosis of diastolic heart failure were identified using ICD-10 within the Nationwide Readmission Database.We excluded patients who died in index admission,and discharged in December each year to allow thirty-day follow-up.Univariate regression was performed on each variable.Vari-ables with P-value<0.2 were included in the multivariate regression model.RESULTS From a total of 45,393 index admissions,43,646 patients(96.2%)survived to discharge.A total of 7,437 patients(15.6%)had a thirty-day readmission.Mean cost of readmission was 43,265 United States dollars(USD)per patient.Significant predictors of thirty-day readmission were chronic kidney disease stage III or higher[adjusted odds ratio(aOR)=1.20,95%CI:1.07−1.34,P=0.002]and diabetes mellitus(aOR=1.18,95%CI:1.07−1.29,P=0.001).Meanwhile,female(aOR=0.90,95%CI:0.82−0.99,P=0.028)and palliative care encounter(aOR=0.27,95%CI:0.21−0.34,P<0.001)were associated with lower odds of readmission.Cardiac arrhythmia(aOR=1.46,95%CI:1.11−1.93,P=0.007)and aortic stenosis(aOR=1.36,95%CI:1.05−1.76,P=0.020)were amongst predictors of in-hospital mortality.CONCLUSIONS In nonagenarians hospitalized with acute HFpEF,thirty-day readmission is common and costly.Chronic co-morbidities predict poor outcomes.Further strategies need to be developed to improve the quality of care and prevent the poor outcome in nonagenarians.Ahmed Maraey Mahmoud Salem Nabila Dawoud Mahmoud Khalil Ahmed Elzanaty Hadeer Elsharnoby Ahmed Younes Ahmed Hashim Amit Alam 2021Journal of Geriatric Cardiology2021,18,12:3

6Role of endoscopic ultrasound and endoscopic ultrasound-guided tissue acquisition in diagnosing hepatic focal lesions显示文摘BACKGROUND Endoscopic ultrasonography(EUS)has become an established method in diagnostic and therapeutic procedures in gastroenterology;however,it has recently gained a growing role in hepatology.AIM To evaluate the role of EUS features,strain elastography(SE),and EUS-tissue acquisition in diagnosing hepatic focal lesions(HFLs)that could affect further management.METHODS This cross-sectional study included 215 patients with pancreatic,biliary,or gastrointestinal malignancies referred for EUS examination.HFLs were identified in 43 patients(20%),and EUSguided tissue acquisition was performed from these lesions.RESULTS EUS features were highly sensitive(100%)but much less specific(57%)in diagnosing HFLs;the overall accuracy was 94%.Real-time elastography was also very sensitive(97%)but less specific(67%)in diagnosing HFLs;however,the overall accuracy was 92%.EUS tissue acquisition was extremely sensitive(100%)and specific(100%),with a 100%overall diagnostic accuracy.CONCLUSION The diagnostic utility of EUS-guided tissue acquisition was extremely accurate in diagnosing HFLs.EUS characteristics and real-time SE accurately predicted the histological diagnosis of both benign and malignant HFLs.Hussein Hassan Okasha Hanane Delsa Abdelmoneim Alsawaf Ahmed Morad Hashim Hani M Khattab Dalia Abdelfatah Abeer Abdellatef Amr Albitar 2023World Journal of Methodology2023,13,4:1

18An Efficient IIoT-Based Smart Sensor Node for Predictive Maintenance of Induction Motors显示文摘Predictive maintenance is a vital aspect of the industrial sector,and the use of Industrial Internet of Things(IIoT)sensor nodes is becoming increasingly popular for detecting motor faults and monitoring motor conditions.An integrated approach for acquiring,processing,and wirelessly transmitting a large amount of data in predictive maintenance applications remains a significant challenge.This study presents an IIoT-based sensor node for industrial motors.The sensor node is designed to acquire vibration data on the radial and axial axes of the motor and utilizes a hybrid approach for efficient data processing via edge and cloud platforms.The initial step of signal processing is performed on the node at the edge,reducing the burden on a centralized cloud for processing data from multiple sensors.The proposed architecture utilizes the lightweight Message Queue Telemetry Transport(MQTT)communication protocol for seamless data transmission from the node to the local and main brokers.The broker’s bridging allows for data backup in case of connection loss.The proposed sensor node is rigorously tested on a motor testbed in a laboratory setup and an industrial setting in a rice industry for validation,ensuring its performance and accuracy in real-world industrial environments.The data analysis and results from both testbed and industrial motors were discussed using vibration analysis for identifying faults.The proposed sensor node is a significant step towards improving the efficiency and reliability of industrial motors through realtime monitoring and early fault detection,ultimately leading to minimized unscheduled downtime and cost savings.Majida Kazmi Maria Tabasum Shoaib Arshad Aziz Hashim Raza Khan Saad Ahmed Qazi 2023Computer Systems Science & Engineering2023,47,10:0
