1Enhanced recovery after surgery:Current research insights and future direction显示文摘Since the concept of enhanced recovery after surgery(ERAS)was introduced in the late 1990 s the idea of implementing specific interventions throughout the perioperative period to improve patient recovery has been proven to be beneficial. Minimally invasive surgery is an integral component to ERAS and has dramatically improved post-operative outcomes. ERAS can be applicable to all surgical specialties with the core generic principles used together with added specialty specific interventions to allow for a comprehensive protocol,leading to improved clinical outcomes. Diffusion of ERAS into mainstream practice has been hindered due to minimal evidence to support individual facets and lack of method for monitoring and encouraging compliance. No single outcome measure fully captures recovery after surgery,rather multiple measures are necessary at each stage. More recently the pre-operative period has been the target of a number of strategies to improve clinical outcomes,described as prehabilitation. Innovation of technology in the surgical setting is also providing opportunities to overcome the challenges within ERAS,e.g.,the use of wearable activity monitors to record information and provide feedback and motivation to patients peri-operatively. 4微接触钢板治疗长管状骨骨折临床疗效观察显示文摘目的 :报道苏格兰Dundee大学研制的微接触钢板生物力学性能及其治疗骨干骨折的临床疗效。方法 :将材料、尺寸完全一致的MCP、DCP、LC DCP行四点弯曲和扭转实验 ,评价其生物力学性能。临床观察对 19例长管状骨骨折进行MCP固定后的骨折愈合时间、骨痂生长情况 ,综合评价该钢板的优越性。结果 :MCP弯曲刚度显著强于DCP、LC DCP (P <0 0 5 ) ,扭转刚度弱于DCP (P <0 0 5 )但显著强于LC DCP (P <0 0 5 )。股骨、胫骨、肱骨骨折平均愈合时间分别为 12、 13、 10周 ,所有骨折均愈合 ,术后 6个月均弃拐行走或正常持物。结论 :MCP固定可靠 ,骨折愈合快 ,是治疗骨干骨折的较理想手段 ,值得在临床中推广应用。费军 余洪俊 杨欣建 Abel EW 孙俊 2004中国矫形外科杂志2004,12,5:15

6Reduction of the closure time of postoperative enterocutaneous fistulas with fibrin sealant显示文摘AIM: To assess whether the use of fibrin sealantshortens the closure time of postoperative enterocutaneous fistulas (ECFs). METHODS: The prospective case-control study included 70 patients with postoperative ECFs with an output of < 500 mL/d, a fistulous tract of > 2 cm and without any local complication. They were divided into study (n = 23) and control groups (n = 47). Esophageal, gastric and colocutaneous fistulas were monitored under endoscopic visualization, which also allowed fibrin glue application directly through the external hole. Outcome variables included closure time, time to resume oral feeding and morbidity related to nutritional support. RESULTS: There were no differences in mean age, fistula output, and follow-up. Closure-time for all patients of the study group was 12.5 ± 14.2 d and 32.5 ± 17.9 d for the control group (P < 0.001), and morbidity related to nutritional support was 8.6% and 42.5%, respectively (P < 0.01). In patients with colonic fistulas, complete closure occurred 23.5 ± 19.5 d after the first application of fibrin glue, and spontaneous closure was observed after 36.2 ± 22.8 d in the control group (P = 0.36). Recurrences were observed in 2 patients because of residual disease. One patient of each group died during follow-up as a consequence of septic complications related to parenteral nutrition. CONCLUSION: Closure time was significantly reduced with the use of fibrin sealant, and oral feeding was resumed faster. 10Surgical treatment of Lenke 5 adolescent idiopathic scoliosis: Comparison of anterior vs posterior approach显示文摘AIM To compare the posterior vs anterior approaches for fusion of Lenke 5 adolescent idiopathic scoliosis curves,matched for curve magnitude and for the distal level of fixation(dLOF) standardized to the third lumbar vertebrae(L3).METHODS A prospectively collected multicenter database was used for this retrospective comparative study. Our dependent variables included sagittal and coronal radiographic measurements, number of fused vertebrae, estimated blood loss, length of hospitalization and SRS total and individual domain scores at the two-year follow-up. Subject demographics were similar for all group comparisons.Independent t-test was used to compare groups for all analyses at P < 0.01.RESULTS For all matched cases of Lenke 5 curves, a selective approach was used only 50% of the time in cases undergoing a posterior fusion. When comparing a posterior selective approach to an anterior selective approach,surgeons utilizing a posterior approach fused significantly more levels than surgeons using an anterior approach with no other significant differences in radiographic or SRS outcomes(Ant = 4.8 ± 1.0 levels vs post = 6.1 ± 1.0 levels, P < 0.0001). When the dLOF was standardized to L3, the anterior approached provided significantly greater lumbar Cobb percent correction than the posterior approach(Ant = 69.1% ± 12.6% vs post =54.6% ± 16.4%, P = 0.004), with no other significant radiographic or SRS score differences between approaches.CONCLUSION Surgeons treating Lenke 5c curves with a posterior instrumentation and fusion vs an anterior approach include more motion segments, even with a selective fusion. 12Purification of solar cell silicon materials through filtration显示文摘Silicon is the material most commonly used in the manufacturing of photovoltaic (PV) cells. In the current study, laboratory experiments of purification of solar cell silicon materials through filtration are carried out. Inclusion removal from silicon was investigated. The purpose is to achieve clean silicon materials for solar cells. Silicon samples and filter samples were analyzed using microscope observation, EPMA, and X-ray detection. Silicon nitride (Si3N4) and silicon carbide (SiC) particles are the main non-metallic inclusions present in top-cut silicon scrap. Almost all inclusions larger than 10 μm can be removed from silicon by the porous foam filter. In mass fraction, more than 90% inclusions are removed. Si3N4 particles are mainly removed on the top surface of the filter, and SiC particles are mainly removed by entering the pores and attaching to the filter material. SiC inclusions are not only simply attached on the surface of the filter material, but are found also inside the filter material. There are SiC bridges near the filter materials. These bridges may fill the spaces between filter material, and this will further retard inclusions passing through the filter. Three-dimensional turbulent fluid flow and inclusion motion in the filter was calculated. 13Comparison of two remote sensing models for estimating evapotranspiration: algorithm evaluation and application in seasonally arid ecosystems in South Africa显示文摘Remote sensing tools are becoming increasingly important for providing spatial information on water use by different ecosystems. Despite significant advances in remote sensing based evapotranspiration(ET) models in recent years, important information gaps still exist on the accuracy of the models particularly in arid and semi-arid environments. In this study, we evaluated the Penman-Monteith based MOD16 and the modified Priestley-Taylor(PT-JPL) models at the daily time step against three measured ET datasets. We used data from two summer and one winter rainfall sites in South Africa. One site was dominated by native broad leaf and the other by fine leafed deciduous savanna tree species and C4 grasses. The third site was in the winter rainfall Cape region and had shrubby fynbos vegetation. Actual ET was measured using open-path eddy covariance systems at the summer rainfall sites while a surface energy balance system utilizing the large aperture boundary layer scintillometer was used in the Cape. Model performance varied between sites and between years with the worst estimates(R2<0.50 and RMSE>0.80 mm/d) observed during years with prolonged mid-summer dry spells in the summer rainfall areas. Sensitivity tests on MOD16 showed that the leaf area index, surface conductance and radiation budget parameters had the largest effect on simulated ET. MOD16 ET predictions were improved by:(1) reformulating the emissivity expressions in the net radiation equation;(2) incorporating representative surface conductance values;and(3) including a soil moisture stress function in the transpiration sub-model. Implementing these changes increased the accuracy of MOD16 daily ET predictions at all sites. However, similar adjustments to the PT-JPL model yielded minimal improvements. 17Valgus osteotomy for nonunion and neglected neck of femur fractures显示文摘Nonunion neck of femur can be a difficult problem to treat, particularly in the young, and is associated with high complication rates of avascular necrosis due to the precarious blood supply and poor biomechanics.The various treatment options that have been described can be broadly divided according to the aim of improving either biology or biomechanics. Surgeries aimed at improving the biology, such as vascularized fibula grafting, have good success rates but require high levels of expertise and substantial resources. A popular surgical treatment aimed at improving the biomechanics-valgus intertrochanteric osteotomyoptimizes conditions for fracture healing by converting shear forces across the fracture site into compressive forces. Numerous variations of this surgical procedure have been developed and successfully applied in clinical practice. As a result, the proximal femoral orientation for obtaining a good functional outcome has evolved over the years, and the present concept of altering the proximal femoral anatomy as little as possible has arisen. This technical objective supports attaining union as well as a good functional outcome, since excessive valgus can lead to increased joint reaction forces.
