1Histological and biochemical alterations in early-stage lobar ischemia-reperfusion in rat liver显示文摘AIM:To investigate the structural and biochemical changes in the early stage of reperfusion in the rat livers exposed to lobar ischemia-reperfusion (IR).METHODS: The median and left lobes of the liver were subjected to 60 min ischemia followed by 5, 10, 30, 45, 60 and 120 min reperfusion. Blood samples were taken at different time intervals to test enzyme activities and biochemical alterations induced by reperfusion. At the end of each reperfusion period, the animals were killed by euthanasia and tissue samples were taken for histological examination and immunohistochemistry. RESULTS: Cell vacuolation, bleb formation and focal hepatitis were the most important changes occur during ischemia. While some changes including bleb formation were removed during reperfusion, other alterations including portal hepatitis, inflammation and the induction of apoptosis were seen during this stage. The occurrence of apoptosis, as demonstrated by apoptotic cells and bodies, was the most important histological change during reperfusion. The severity of apoptosis was dependent on the time of reperfusion, and by increasing the time of reperfusion,the numbers of apoptotic bodies was significantly enhanced. The amounts of lactate dehydrogenase, alanine aminotransferase, aspartate aminotransferase, creatinine and urea were significantly increased in serum obtained from animals exposed to hepatic IR.CONCLUSION: Inflammation and subsequent apoptotic cell death were the most important changes in early-stage hepatic reperfusion injury, and the number of apoptotic bodies increased with time of reperfusion.Hossein Ali Arab Farhang Sasani Mohammad Hossein Rafiee Ahmad Fatemi Abbas Javaheri 2009World Journal of Gastroenterology2009,15,16:16
2Serum adipokines might predict liver histology findings in non-alcoholic fatty liver disease显示文摘AIM: To assess significance of serum adipokines to determine the histological severity of non-alcoholic fatty liver disease.METHODS: Patients with persistent elevation in serum aminotransferase levels and well-defined characteristics of fatty liver at ultrasound were enrolled. Individuals with a history of alcohol consumption, hepatotoxic medication, viral hepatitis or known liver disease were excluded. Liver biopsy was performed to confirm nonalcoholic liver disease(NAFLD). The degrees of liver steatosis, lobular inflammation and fibrosis were determined based on the non-alcoholic fatty liver activity score(NAS) by a single expert pathologist. Patients with a NAS of five or higher were considered to have steatohepatitis. Those with a NAS of two or lower were defined as simple fatty liver. Binary logistic regression was used to determine the independent association of adipokines with histological findings. Receiver operating characteristic(ROC) analysis was employed to determine cut-off values of serum adipokines to discriminate the grades of liver steatosis,lobular inflammation and fibrosis. RESULTS: Fifty-four participants aged 37.02 ± 9.82 were enrolled in the study. Higher serum levels of visfatin, IL-8, TNF-α levels were associated independently with steatosis grade of more than 33% [β = 1.08(95%CI: 1.03-1.14), 1.04(95%CI: 1.008-1.07), 1.04(95%CI: 1.004-1.08), P < 0.05]. Elevated serum IL-6 and IL-8 levels were associated independently with advanced lobular inflammation [β = 1.4(95%CI: 1.09-1.8), 1.07(95%CI: 1.003-1.15), P < 0.05]. Similarly, higher TNF-α, resistin, and hepcidin levels were associated independently with advanced fibrosis stage [β = 1.06(95%CI: 1.002-1.12), 19.86(95%CI: 2.79-141.19), 560.72(95%CI: 5.98-5255.33), P < 0.05]. Serum IL-8 and TNF-α values were associated independently with the NAS score, considering a NAS score of 5 as the reference value [β = 1.05(95%CI: 1.01-1.1), 1.13(95%CI: 1.04-1.22), P < 0.05]. 3Development of a time-dependent energy model to calculate the mining-induced stress over gates and pillars显示文摘Generally, longwall mining-induced stress results from the stress relaxation due to destressed zone that occurs above the mined panel. Knowledge of induced stress is very important for accurate design of adjacent gateroads and intervening pillars which helps to raise the safety and productivity of longwall mining operations. This study presents a novel time-dependent analytical model for determination of the longwall mining-induced stress and investigates the coefficient of stress concentration over adjacent gates and pillars. The model is developed based on the strain energy balance in longwall mining incorporated to a rheological constitutive model of caved materials with time-varying parameters. The study site is the Tabas coal mine of Iran. In the proposed model, height of destressed zone above the mined panel, total longwall mining-induced stress, abutment angle, induced vertical stress, and coefficient of stress concentration over neighboring gates and intervening pillars are calculated. To evaluate the effect of proposed model parameters on the coefficient of stress concentration due to longwall mining, sensitivity analysis is performed based on the field data and experimental constants. Also, the results of the proposed model are compared with those of existing models. The comparative results confirm a good agreement between the proposed model and the in situ measurements. According to the obtained results, it is concluded that the proposed model can be successfully used to calculate the longwall mining-induced stress. 4Prediction of representative deformation modulus of longwall panel roof rock strata using Mamdani fuzzy system显示文摘Deformation modulus is the important parameter in stability analysis of tunnels, dams and mining structures. In this paper, two predictive models including Mamdani fuzzy system(MFS) and multivariable regression analysis(MVRA) were developed to predict deformation modulus based on data obtained from dilatometer tests carried out in Bakhtiary dam site and additional data collected from longwall coal mines. Models inputs were considered to be rock quality designation, overburden height, weathering,unconfined compressive strength, bedding inclination to core axis, joint roughness coefficient and fill thickness. To control the models performance, calculating indices such as root mean square error(RMSE),variance account for(VAF) and determination coefficient(R2) were used. The MFS results show the significant prediction accuracy along with high performance compared to MVRA results. 8Application of Modified Silica Coated Magnetite Nanoparticles for Removal of Iodine from Water Samples显示文摘The adsorption of iodine onto silica coated magnetite nanoparticles(im-SCMNPs) that modified with imidazole was investigated for removal of high concentrations of iodine from wastewater. Modified silica magnetite nanoparticles showed high efficiency in removing iodine from wastewater samples. The optimum pH for iodine removal was 7.0-8.0. The adsorption capacity was evaluated using both the Langmuir and Freundlich adsorption isotherm models. The size of the produced magnetite nanoparticles was determined by X-ray diffraction analysis and scanning electron microscopy. Synthesized magnetite nanoparticles showed the high adsorption capacity and would be a good method to increase adsorption efficiency for the removal of iodine in a wastewater treatment process. 9Rapunzel syndrome:The unsuspected culprit显示文摘Trichobezoar is a rare intriguing disorder in which swallowed hairs accumulates in the stomach. Being indigestible and slippery, it could not be propulsed and becomes entrapped within the stomach. Large amounts can thus accumulate over the years forming a hair ball. Rapunzel syndrome is a variant where hair accumulation reaches the small gut and beyond in some cases. Although the syndrome has been known for many years, only 24 cases have been reported in the literature and the discovery of a new case is always surprising. In this report, we present two cases discovered within a period of three months. One of them was pregnant and had small bowel intussusception and perforation, a very rare combination. We hereby add two more cases to the literature. 13Hepatitis B virus genotypes in southwest Iran: Molecular, serological and clinical outcomes显示文摘AIM: To investigate the associations of hepatitis B virus (HBV) genotype with HBeAg and anti-HBe status, alanine aminotransferase (ALT) levels and HBV-DNA detection in different groups of HBV-infected patients in southwest Iran. METHODS: A total of 89 HBsAg-positive serum samples were collected from the same number of patients. All sera were then investigated to determine HBV DNA and serological markers. For all the polymerase chain reaction (PCR)-positive samples, biochemical, histopathological assays and genotyping were also performed. RESULTS: Genotype D was the only type of HBV foundin different clinical forms of acute and chronic infections. There was a high prevalence of HBeAg-negative HBV- infected patients with chronic hepatitis (52.7%). Out of 55 patients with chronic hepatitis, seven (12.7%) were diagnosed with cirrhosis. A significant association between the presence of anti-HBe antibody and an increase in ALT level, among either HBeAg-negative (P = 0.01) or HBeAg-positive (P = 0.026) patients, was demonstrated. No significant differences were observed between the clinical outcomes of HBeAg-positive and -negative individuals (P = 0.24). CONCLUSION: Genotype D has been recognized as the only type of HBV found in different clinical forms of HBV infections, including cirrhosis, among the residents of southwest Iran. 15Alunite processing method selection using the AHP and TOPSIS approaches under fuzzy environment显示文摘Alunite is the most important non bauxite resource for alumina. Various methods have been proposed and patented for processing alunite, but none has been performed at industrial scale and no technical,operational and economic data is available to evaluate methods. In addition, selecting the right approach for alunite beneficiation, requires introducing a wide range of criteria and careful analysis of alternatives.In this research, after studying the existing processes, 13 methods were considered and evaluated by 14 technical, economic and environmental analyzing criteria. Due to multiplicity of processing methods and attributes, in this paper, Multi Attribute Decision Making methods were employed to examine the appropriateness of choices. The Delphi Analytical Hierarchy Process(DAHP) was used for weighting selection criteria and Fuzzy TOPSIS approach was used to determine the most profitable candidates. 17热带水蛭Hirudinaria manillensis睡液提取物的体内降糖作用(英文)显示文摘The anticoagulant effect of leech saliva was traditionally employed in the treatment of diabetes mellitus complications such as peripheral vascular complications.This study was carried out to examine the effect of leech saliva extract(LSE)on blood glucose levels in alloxan-induced diabetic rats.First,LSE was collected from leeches which were fed on a phagostimulatory solution.Second,total protein concentration was estimated using the Bradford assay.Third,diabetic rats were injected subcutaneously(sc)with LSE at doses of 500 and 1 000μg·kg 1body weight(bw).Other diabetic rats were injected sc with insulin at doses of 10 and 20 U·kg 1bw.Another group was injected simultaneously with LSE(250μg·kg 1bw)and insulin(10 U·kg 1bw).Fasting blood glucose(FBG)concentrations were monitored during a study period of eight hours at regular intervals.Findings showed that both doses of LSE resulted in a significant and gradual decrease in FBG starting from 10%18%downfall after two hours of injection reaching the maximal reduction activity of 58%after eight hours.Remarkably,LSE was sufficient to bring the rats to a near norm-glycemic state.The high dose of insulin induced a severe hypoglycemic condition after 2 4 h of injection.The lower dose was able to decline FBG for 2 6 h in rats which became diabetic again after 8 h.On the other hand,the concurrent injection of low doses of LSE and insulin produced a hypoglycemic effect with all rats showing normal FBG levels.Taken together,these findings indicated that the subcutaneous injection of LSE of the medicinal Malaysian leech was able to provide better glycemic control compared with insulin.Moreover,the synergism between LSE and insulin suggests that LSE could be utilized as an adjuvant medication in order to reduce insulin dosage or to achieve better control of blood glucose.Abdualkader Abdualrahman Mohammed Ghawi Abbas Mohammad Alaama Mohamed Awang Mohamed Merzouk Ahmed 2013中国天然药物2013,11,5:3
18Cutaneous leishmaniasis in Iran: Results from an epidemiological study in urban and rural provinces显示文摘Objective: To examine the prevalence and clinical manifestations of cutaneous leishmaniasis(CL) in Iran.Methods: This study was conducted in Iran between 2011 and 2013. Sampling, preparing, developing, and fixing of suspicious skin lesions were completed in healthcare centers in 31 Iranian provinces as well as in the Academic Reference Laboratory and the National Reference Laboratory. The information was then analyzed at the Ministry of Health's Information Management Center of Contagious Diseases.Results: Over a three-year period, the number of people identified with CL was 56 546.The highest incidence was reported in 2011(27.5 per 100 000). Wet CL accounted for 43.7% of cases while 43.3% resulted from sporotrichoid leishmaniasis. The results showed that there was a higher incidence of CL due to Leishmania major(50.2%) than to Leishmania tropica. The results of this study found that the highest incidence of CL had happened respectively in Ilam, Fars and, Khorasan Razavi Provinces between 2011 and 2013.Conclusions: Although the incidence of the disease is declining, CL is still a public health concern and disease control protocols need to be established. 19Climate change and potential distribution of zoonotic cutaneous leishmaniasis in Central Iran: Horizon 2030 and 2050显示文摘Objective: To investigate and predict the effects of climate change on the potential distribution of the main vector and reservoir hosts of the disease in Yazd province in the future.Methods: Distribution data for vector and reservoir hosts of zoonotic cutaneous leishmaniasis in Yazd province were obtained from earlier studies conducted in the area.MaxEnt ecological niche modeling was used to predict environmental suitability.BCC-CSM1-1(m) model and two climate change scenarios, RCP 4.5 and RCP 8.5 were used for horizons 2030 and 2050 climate projections.Future projections were based on data of a regional climate change model.Results: With both scenarios in 2030 and 2050, the results of jackknife test indicated that the mean temperature of wettest quarter and temperature annual range had the greatest effect on the model for the vector and the reservoir hosts, respectively.Conclusions: The climate conditions are the major determinants of zoonotic cutaneous leishmaniasis incidence rate in Yazd Province.These climate conditions provide favorable habitats for ease transmission of zoonotic cutaneous leishmaniasis in this endemic area.Habitats suitability for the vector and reservoir will be expanding in the coming years compared with the current conditions, such that, in horizon 2030 & 2050, the probability of the presence of the vector and reservoir within 38 580 and 37 949 km^2, respectively, from Yazd province is above 60%.Moreover, an increase is predicted in the presence of the vector in the western parts and the reservoir in the northern and central parts of the province in the future.Understanding the role of environmental and bioclimatic factors in zoonotic cutaneous leishmaniasis occurrence can provide a guide for policy-makers in the creation and implementation of more effective policies for prevention and control.Babak Shiravand Ahmad Ali Hanafi-Bojd Abbas Ali Dehghani Tafti Mohammad Reza Abai Ali Almodarresi Masoud Mirzaei 2019Asian Pacific Journal of Tropical Medicine2019,12,5:2
