| 1 | Lepidium meyenii (Maca) improved semen parameters in adult men显示文摘Aim: The present study was designed to determine the effect of a 4-month oral treatment with tablets of Lepidium meyenii (Maca) on seminal analysis in nine adult normal men aged 24 - 44 years old. Methods: Nine men received tablets of Maca (1500 or 3000 mg/day) for 4 months. Seminal analysis was performed according to guidelines of the World Health Organization (WHO). Serum luteinizing hormone (LH), follicle stimulating hormone (FSH), prolactin (PRL), testosterone (T) and estradiol (E2) were measured before and after treatment. Results: Treatment with Maca resulted in increased seminal volume, sperm count per ejaculum, motile sperm count, and sperm motility. Serum hormone levels were not modified with Maca treatment. | 3 | A prospective analysis of factors that influence weight loss in patients undergoing radiotherapy显示文摘Malnutrition occurs frequently in patients with cancer.Indeed,a variety of nutritional and tumorrelated factors must be taken into account in these patients.Recognizing this relationship,we aimed to prospectively evaluate the risk factors that influence weight loss in patients undergoing radiotherapy with oral nutritional supplementation and dietetic counseling.Weight loss of 74 patients during radiotherapy and 1 month after treatment was analyzed.Parameters such as age,gender,tumor location,tumor stage,Eastern Cooperative Oncology Group performance status(ECOG PS)score,and the use of chemotherapy were analyzed to evaluate their influence on weight loss.All patients underwent oral nutritional supplementation and dietetic counseling.Forty-six(65.7%)patients lost weight,with a mean weight loss of(4.73±3.91)kg,during radiotherapy.At 1 month after treatment,45(66.2%)patients lost weight,presenting a mean weight loss of(4.96±4.04)kg,corresponding to a(6.84±5.24)%net reduction from their baseline weight.Head and neck cancer patients had a mean weight loss of(3.25±5.30)kg,whereas the remaining patients had a mean weight loss of(0.64±2.39)kg(P=0.028)during radiotherapy.In the multivariate analysis,the head and neck tumor location(P=0.005),use of chemotherapy(P=0.011),and ECOG PS score of 2-3(P=0.026)were considered independent risk factors.Nutritional status and parameters,such as tumor location(especially the head and neck),the use of chemotherapy,and the ECOG PS score,should be evaluated before radiotherapy because these factors can influence weight loss during radiotherapy and 1 month after treatment. | Jon Cacicedo Francisco Casquero Lorea Martinez-Indart Olga del Hoyo Alfonso Gomez de Iturriaga Arturo Navarro Pedro Bilbao | 2014 | Chinese Journal of Cancer2014,33,4: | 12 |
| 4 | Genetic, metabolic and environmental factors involved in the development of liver cirrhosis in Mexico显示文摘Liver cirrhosis(LC) is a chronic illness caused by inflammatory responses and progressive fibrosis. Globally, the most common causes of chronic liver disease include persistent alcohol abuse, followed by viral hepatitis infections and nonalcoholic fatty liver disease. However, regardless of the etiological factors, the susceptibility and degree of liver damage may be influenced by genetic polymorphisms that are associated with distinct ethnic and cultural backgrounds. Consequently, metabolic genes are influenced by variable environmental lifestyle factors, such as diet, physical inactivity, and emotional stress, which are associated with regional differences among populations. This Topic Highlight will focus on the genetic and environmental factors that may influence the metabolism of alcohol and nutrients in the setting of distinct etiologies of liver disease. The interaction between genes and environment in the current-day admixed population, Mestizo and Native Mexican, will be described. Additionally, genes involved in immune regulation, insulin sensitivity, oxidative stress and extracellular matrix deposition may modulate the degree of severity. In conclusion, LC is a complex disease. The onset, progression, and clinical outcome of LC among the Mexican population are influenced by specific genetic and environmental factors. Among these are an admixed genome with a heterogenic distribution of European, Amerindian and African ancestry; a high score of alcohol consumption; viral infections; a hepatopathogenic diet; and a high prevalence of obesity. | 7 | Selection of patients with hepatocellular carcinoma for livertransplantation:Past and future显示文摘The aim of liver transplantation(LT) for hepatocellular carcinoma(HCC) is to ensure a rate of disease-free survival similar to that of patients transplanted due to benign disease. Therefore, we are forced to adopt strict criteria when selecting candidates for LT and prioritizing patients on the waiting list(WL), to have clarified indications for bridging therapy for groups at risk for progression or recurrence, and to establish certain limits for downstaging therapies. Although the Milan criteria(MC) remain the standard and most employed criteria for indication of HCC patients for LT by far, in the coming years, criteria will be consolidated that take into account not only data regarding the size/volume and number of tumors but also their biology. This criteria will mainly include the alpha fetoprotein(AFP) values and, in view of their wide variability, any of the published logarithmic models for the selection of candidates for LT. Bridging therapy is necessary for HCC patients on the WL who meet the MC and have the possibility of experiencing a delay for LT greater than 6 mo or any of the known risk factors for recurrence. It is difficult to define single AFP values that would indicate bridging therapy(200, 300 or 400 ng/m L); therefore, it is preferable to rely on the criteria of a French AFP model score > 2. Other single indications for bridging therapy include a tumor diameter greater than 3 cm, more than one tumor, and having an AFP slope greater than 15 ng/m L per month or > 50 ng/m L for three months during strict monitoring while on the WL. When considering the inclusion of patients on the WL who do not meet the MC, it is mandatory to determine their eligibility for downstaging therapy prior to inclusion. The upper limit for this therapy could be one lesion up to 8 cm, 2-3 lesions with a total tumor diameter up to 8 cm, or a total tumor volume of 115 cm^3. Lastly, liver allocation and the prioritization of patients with HCC onthe WL should take into account the recently described HCC model for end-stage liver disease, which considers hepatic function, HCC size and the number and the log of AFP values. | 10 | Effect of alcoholic extract of Lepidium meyenii (Maca) on testicular function in male rats显示文摘Aim: To evaluate the effect of the alcoholic extract of Lepidium meyenii (Maca) on the spermatogenesisin male rats. Methods: In Holtzman rats, Maca alcoholic extract (5 %) was given by oral route at doses of 48 mg/day or 96 mg/day for 7 days, 14 days and 21 days. Testicular function was assessed by measurements of lengths of different stages of seminiferous epithelia and by epididymal sperm count. Results: Ethanolic extract of Maca increased the length of stages IX-XI of seminiferous epithelium at treatment day 7, day 14 and day 21. Progression of spermatogenesis was evident only after day 21 when lengths of stages XII-XIV of seminiferous epithelium were increased; at day 7 and day 14, no important change in spermatogenesis was observed. Epididymal sperm count was increased with 48 mg/day at all times. With 96 mg/day an increase in sperm count was observed at day 7, but it was reduced at day 14 and day 21 of treatment. Serum testosterone levels were not affected. Conclusion: The alcoholic extract of Maca activates onset ant progression of spermatogenesis at 48 mg/day or 96 mg/day in rats. | Gustavo F. In real structures, two or more sites or types of damage can be present at the same time. It has been shown that one kind of damaged condition can interfere with the detection of another kind of damage, leading to an incorrect assessment about the structure condition. Identifying combined damage on structures still represents a challenge for condition monitoring, because the reliable identification of a combined damaged condition is a difficult task. Thus, this work presents a fusion of methodologies, where a single wavelet-packet and the empirical mode decomposition (EMD) method are combined with artificial neural networks (ANNs) for the automated and online identification-location of single or multiple-combined damage in a scaled model of a five-bay truss-type structure. Results showed that the proposed methodology is very efficient and reliable for identifying and locating the three kinds of damage, as well as their combinations. | 16 | Accuracy of transnasal endoscopy with a disposable esophagoscope compared to conventional endoscopy显示文摘AIM: To assess feasibility of unsedated esophagoscopy using a small-caliber disposable transnasal esophagosco-py and to compare its accuracy with standard endoscopy.METHODS: We prospectively included subjects who were referred for upper endoscopy. All subjects un-derwent transnasal endoscopy with E.G. Scan^(TM). The disposable probe has a 3.6 mm gauge and at its distal end there is a 6 mm optical capsule, with a viewing angle of 125°. Patients underwent conventional endos-copy after the completion of E.G. Scan^(TM). We describe the findings detected by the E.G. Scan^(TM) and calculate the diagnostic accuracy, sensitivity, specificity, positive predictive value, negative predictive value and Kappa index for esophageal diagnosis.RESULTS: A total of 96 patients(54 women), mean age of 50.12 years(14 to 79), were evaluated. In all cases we were able to perform esophagoscopy with E.G.Scan^(TM). The average realization time was 5 min. A total of 58 alterations were detected in the esophagus, 49 gastric abnormalities and 13 duodenal abnormalities. We found that for esophageal varices, E.G. Scan^(TM) has sensitivity, specificity and diagnostic accuracy of 95%, 97% and 97%, respectively. Kappa coefficients were 0.32 for hiatal hernia, 0.409 for erosive gastroesopha-geal reflux disease, 0.617 for Barrett's esophagus, and 0.909 for esophageal varices.CONCLUSION: Esophagoscopy with E.G. Scan? is a well-tolerated, fast and safe procedure. | 17 | Drought and Spatiotemporal Variability of Forest Fires Across Mexico显示文摘Understanding the spatiotemporal links between drought and forest fire occurrence is crucial for improving decision-making in fire management under current and future climatic conditions. We quantified forest fire activity in Mexico using georeferenced fire records for the period of 2005–2015 and examined its spatial and temporal relationships with a multiscalar drought index, the Standardized Precipitation-Evapotranspiration Index(SPEI). A total of 47975 fire counts were recorded in the 11-year long study period, with the peak in fire frequency occurring in 2011. We identified four fire clusters, i.e., regions where there is a high density of fire records in Mexico using the Getis-Ord G spatial statistic. Then, we examined fire frequency data in the clustered regions and assessed how fire activity related to the SPEI for the entire study period and also for the year 2011. Associations between the SPEI and fire frequency varied across Mexico and fire-SPEI relationships also varied across the months of major fire occurrence and related SPEI temporal scales. In particular, in the two fire clusters located in northern Mexico(Chihuahua, northern Baja California), drier conditions over the previous 5 months triggered fire occurrence. In contrast, we did not observe a significant relationship between drought severity and fire frequency in the central Mexico cluster, which exhibited the highest fire frequency. We also found moderate fire-drought associations in the cluster situated in the tropical southern Chiapas where agriculture activities are the main causes of forest fire occurrence. However, certain high allelic frequencies of several nutrientrelated polymorphisms may have been achieved by past gene-nutrient interactions. Such interactions may have promoted the positive selection of variants adapted to regional food sources. At present, the unbalanced diet composition of the Mexicans has led the country to a 70% prevalence rate of overweightness and obesity due to substantial changes in food habits, among other factors. International guidelines and intervention strategies may not be adequate for all populations worldwide because they do not consider disparities in genetic and environmental factors, and thus there is a need for differential prevention and management strategies. Here, we provide the rationale for an intervention strategy for the prevention and management of obesity-related diseases such as nonalcoholic steatohepatitis based on a regionalized genome-based diet.
