| 1 | Pegylated interferon-alpha plus taurine in treatment of rat liver fibrosis显示文摘AIM: To investigate the antif ibrotic effects of peginterferon- alpha 2b and taurine on oxidative stress markers and hepatocellular apoptosis. METHODS: Sixty rats with CCl4-induced liver fibrosis were divided into 4 groups (n = 15). Group 1 was left for spontaneous recovery (SR). Groups 2-4 received peginterferon-alpha 2b, taurine, and their combination, respectively, for four weeks. Histological f ibrosis scores, histomorphometric analysis, tissue hydroxyproline, tissue MDA, GPx and SOD activities were determined. Activated stellate cells and hepatocellular apoptosis were also evaluated. RESULTS: The degree of f ibrosis decreased in all treatment groups compared to spontaneous recovery group. Taurine alone and in combination with peginterferon-alpha 2b reduced oxidative stress markers, but peginterferon-alpha 2b alone did not. Apoptotic hepatocytes and activated stellate cells were higher in groups 2-4 than in group 1. Combined taurine and peginterferon-alpha 2b further reduced fibrosis and increased activated stellate cell apoptosis, but could not improve oxidative stress more than taurine alone.CONCLUSION: Peginterferon-alpha 2b exerts anti- f ibrotic effects on rat liver fibrosis. It seems ineffective against oxidative stress in vivo. | 2 | Narrow pelvic inlet plane area and obesity as risk factors for anastomotic leakage after intersphincteric resection显示文摘BACKGROUND Intersphincteric resection(ISR)has been increasingly used as the ultimate sphincter-preserving procedure in extremely low rectal cancer.The most critical complication of this technique is anastomotic leakage.The incidence rate of anastomotic leakage after ISR has been reported to range from 5.1%to 20%.AIM To investigate risk factors for anastomotic leakage after ISR based on clinicopathological variables and pelvimetry.METHODS This study was conducted at Department of Colorectal Surgery,Japanese Red Cross Medical Center,Tokyo,Japan,with a total of 117 patients.We enrolled 117 patients with extremely low rectal cancer who underwent laparotomic and laparoscopic ISRs at our hospital.We conducted retrospective univariate and multivariate regression analyses on 33 items to elucidate the risk factors for anastomotic leakage after ISR.Pelvic dimensions were measured using threedimensional reconstruction of computed tomography images.The optimal cutoff value of the pelvic inlet plane area that predicts anastomotic leakage was determined using a receiver operating characteristic(ROC)curve.RESULTS We observed anastomotic leakage in 10(8.5%)of the 117 patients.In the multivariate analysis,we identified high body mass index(odds ratio 1.674;95%confidence interval:1.087-2.58;P=0.019)and smaller pelvic inlet plane area(odds ratio 0.998;95%confidence interval:0.997-0.999;P=0.012)as statistically significant risk factors for anastomotic leakage.According to the receiver operating characteristic curves,the optimal cutoff value of the pelvic inlet plane area was 10074 mm2.Narrow pelvic inlet plane area(≤10074 mm2)predicted anastomotic leakage with a sensitivity of 90%,a specificity of 85.9%,and an accuracy of 86.3%.CONCLUSION Narrow pelvic inlet and obesity were independent risk factors for anastomotic leakage after ISR.Anastomotic leakage after ISR may be predicted from a narrow pelvic inlet plane area(≤10074 mm2). | Akira Toyoshima Toshihiro Nishizawa Eiji Sunami Ryuji Akai Takahiro Amano Akiyoshi Yamashita Shin Sasaki Takeshi Endo Yoshihiro Moriya Osamu Toyoshima | 2020 | World Journal of Gastrointestinal Surgery2020，12，10： | 4 |

| 4 | Research on Torque Ripple Under Healthy and Open-Circuit Fault-Tolerant Conditions in a PM Multiphase Machine显示文摘In this paper,research into torque ripple production has been undertaken for both the healthy and open-circuit faulttolerant conditions of a five-phase permanent magnet(PM)machine by using the instantaneous power(I-Power)approach.When only the fundamental component of the phase currents is applied to the phase windings,it has been shown that the 9th and 11th harmonics of the back-electromotive force(back-EMF)causes torque ripples in a five-phase PM machine and its frequency is ten times the frequency of the fundamental phase currents.When the combined fundamental and third harmonic components of the phase currents are applied to the phase windings,it has been shown that the 7th and 13th harmonic of the back-electromotive force(back-EMF)causes additional torque ripples in a five-phase PM machine.These torque ripples under fault-tolerant conditions have been analyzed analytically,as well.It has been proven that there are interactions between the fundamental component of current and the third harmonic component of the back-EMF and vice versa.These interactions cause torque ripples.A finite element analysis(FEA)model of the five-phase PM machine has been done to validate the analytical results. | Ali Akay Paul Lefley | 2020 | CES Transactions on Electrical Machines and Systems2020，4，4： | 3 |
| 5 | Muscle hematoma:A critically important complication of alcoholic liver cirrhosis显示文摘An iliopsoas hematoma can occur either spontaneously or secondary to trauma or bleeding tendency due to hemophilia and anticoagulant therapy.Although liver cirrhosis is commonly associated with coagulopathy, iliopsoas hematoma is very rare.We herein,present a case of bilateral iliopsoas hematoma in a patient with alcoholic cirrhosis,and review the literature on muscle hematoma associated with cirrhosis.A 56-year-old man with alcoholic cirrhosis was admitted in a state of shock with anemia.The cause of anemia could not be detected,and the patient was treated conservatively. The site of bleeding was not detected with either gastroduodenal endoscopy or upper abdominal computed tomography,the latter of which did not include the iliopsoas muscle.He died on the 10th day of admission and bilateral iliopsoas hematomas were found on autopsy.An iron stain was positive in the iliopsoas muscle.Eight cases of muscle hematoma associated with cirrhosis,including the present case, were found in a review of the literature.Four of these cases involved the rectus abdominis muscle,3 involved the iliopsoas muscle and 1 involved combined muscles.Alcoholic cirrhosis accounted for 75%of the cases.One case(12.5%)was associated with virus- related cirrhosis,and another with combined virus- and alcohol-related cirrhosis.The mortality rate was 75%despite early diagnosis and low risk scores for cirrhosis.Muscle hematoma in patients with cirrhosis isclosely related to alcoholism,and the mortality rate of the condition is extremely high.In conclusion,muscle hematoma should be recognized as an important complication of cirrhosis. | Chiyo Sugiyama Akifumi Akai Noriyoshi Yamakita Tsuneko Ikeda Keigo Yasuda | 2009 | World Journal of Gastroenterology2009，15，35： | 2 |
| 6 | Comparison of intravitreal anti-vascular endothelial growth factor agents and treatment results in Irvine-Gass syndrome显示文摘AIM:To compare the efficacy of bevacizumab,ranibizumab,and aflibercept in pseudophakic cystoid macular edema(CME)patients with Irvine-Gass syndrome(IGS).METHODS:This study is designed as retrospective consecutive case series.Those who developed postoperative pseudophakic CME that refractory to topical treatment and were treated with anti-vascular endothelial growth factor(VEGF)agents included in the study.Optical coherence tomography(OCT)examination including central macular thickness(CMT),total macular volume(TMV),retinal nerve fiber layer(RNFL),ganglion cell layer(GCL)and choroidal thickness(ChT)measurements at the baseline,1 st,3 rd and 6 th month controls were performed.RESULTS:Fifty-nine eyes of 59 patients with CME and other healthy eyes of the patients(Control group)were evaluated.There were 22 eyes of 22 patients in the bevacizumab group(group 1),19 eyes of 19 patients in the ranibizumab group(group 2),and 18 eyes of 18 patients in the aflibercept group(group 3).There was no difference in terms of age,gender,axial length,IOP,and spherical equivalent values.The baseline subfoveal and mean ChT were higher in the IGS group.The difference between the baseline and sixth month values of subfoveal and mean ChT were compared in the CME groups,thinning was observed in all three groups.GCL was thinner in the patient group at the 6 th month of treatment.The resolution time of CME was observed faster in group 1.CONCLUSION:All three anti-VEGF agents seem to be effective in CME but bevacizumab appears to be slightly more cost-effective than the other two alternatives. | Fahrettin Akay Mehmed Ugur Isιk Berkay Akmaz Yusuf Ziya Güven | 2020 | International Journal of Ophthalmology(English edition)2020，13，10： | 2 |
| 7 | Determination of fire-access zones along road networks in firesensitive forests显示文摘Wild forest fires are one of the greatest environmental disasters affecting forest resources. Along the coastal zone of the Mediterranean region in Turkey,forested areas are classified as first-degree, fire-sensitive areas. Every year, thousands of hectares of forests have been destroyed in Turkey. In this study, fire-access zones were determined in the Mediterranean forests of Turkey, by utilizing geographic information systems(GIS) technology. The effective reach distance of fire hoses from both sides of roads was considered in order to delineate fireaccess zones. The effective reach distance can vary based on the technical capabilities and hydraulic capacity of fire trucks(minimum and maximum pressures on water pump);terrain structures(uphill, downhill and flat); and ground slope. These factors and their influences were studied in fire sensitive forest areas located in the eastern Mediterranean city of Kahramanmaras? in Turkey. First, terrain structures on both sides of the road network and groundslope classes were determined based on GIS data layers.Then, fire access zones were delineated according to water pressure data, terrain structures, and ground-slope classes.The results indicated that 69.30 % of the forested areas were within the fire-access zones, while the rest of the forest was out of reach the fire hoses. The accessible areas were also calculated for forested areas with different firesensitivity degrees. The accessible areas were 69.59, 69.96,and 67.16 % for the forested areas that are sensitive to fires at the first, second, and third degrees, respectively.
