1From NAFLD to MAFLD:a“redefining”moment for fatty liver disease显示文摘The term non-alcoholic fatty liver disease(NAFLD)was coined in 1980 to characterize a disease similar to alcoholic fatty liver disease that developed in patients without a history of excessive alcohol intake.[1]Morphologically,NAFLD is characterized by excess fatty infiltration of the liver in the absence of known causes of liver disease(eg,alcohol,autoimmune liver disease,viral hepatitis,etc).The clinical manifestations of NAFLD(both hepatic and extrahepatic)depend on the outcome of complex interactions between its primary drivers including poor lifestyle habits and diet,a dysfunctional microbiota,genetic predisposition,and environmental cues that result in metabolic dysfunction and liver disease.However,bringing all patients with their markedly different clinical courses under the NAFLD umbrella belies its complexity and implies a homogeneous disease state that then negatively impacts clinical management and a deeper understanding of pathogenesis.With advances in current knowledge on the spectrum of fatty liver diseases,it is apparent that the fourdecade-old outdated term NAFLD can no longer serve to usefully describe a highly heterogeneous disease.The disease as we understand it today not only impacts patients who consume alcohol and those who do not,but also potentially impacts all patients with any form of liver disease,by acting as a disease modifier.[2]Kenneth IZheng Jian-Gao Fan Jun-Ping Shi Vincent Wai-Sun Wong Mohammed Eslam Jacob George Ming-Hua Zheng 2020Chinese Medical Journal2020,,19:14
2Humoral immune response to circulating SARS-CoV-2 variants elicited by inactivated and RBD-subunit vaccines显示文摘SARS-CoV-2 variants could induce immune escape by mutations on the receptor-binding domain(RBD)and N-terminal domain(NTD).Here we report the humoral immune response to circulating SARS-CoV-2 variants,such as 501Y.V2(B.1.351),of the plasma and neutralizing antibodies(NAbs)elicited by CoronaVac(inactivated vaccine),ZF2001(RBD-subunit vaccine)and natural infection.Among 86 potent NAbs identified by high-throughput single-cell VDJ sequencing of peripheral blood mononuclear cells from vaccinees and convalescents,near half anti-RBD NAbs showed major neutralization reductions against the K417N/E484K/N501Y mutation combination,with E484K being the dominant cause.VH3-53/VH3-66 recurrent antibodies respond differently to RBD variants,and K417N compromises the majority of neutralizing activity through reduced polar contacts with complementarity determining regions.In contrast,the 242–244 deletion(242–244Δ)would abolish most neutralization activity of anti-NTD NAbs by interrupting the conformation of NTD antigenic supersite,indicating a much less diversity of anti-NTD NAbs than anti-RBD NAbs.Plasma of convalescents and CoronaVac vaccinees displayed comparable neutralization reductions against pseudo-and authentic 501Y.V2 variants,mainly caused by E484K/N501Y and 242–244Δ,with the effects being additive.Importantly,RBD-subunit vaccinees exhibit markedly higher tolerance to 501Y.V2 than convalescents,since the elicited anti-RBD NAbs display a high diversity and are unaffected by NTD mutations.Moreover,an extended gap between the third and second doses of ZF2001 leads to better neutralizing activity and tolerance to 501Y.V2 than the standard three-dose administration.Together,these results suggest that the deployment of RBD-vaccines,through a third-dose boost,may be ideal for combating SARS-CoV-2 variants when necessary,especially for those carrying mutations that disrupt the NTD supersite.Yunlong Cao Ayijiang Yisimayi Yali Bai Weijin Huang Xiaofeng Li Zhiying Zhang Tianjiao Yuan Ran An Jing Wang Tianhe Xiao Shuo Du Wenping Ma Liyang Song Yongzheng Li Xiang Li Weiliang Song Jiajing Wu Shuo Liu Xuemei Li Yonghong Zhang Bin Su Xianghua Guo Yangyang Wei Chuanping Gao Nana Zhang Yifei Zhang Yang Dou Xiaoyu Xu Rui Shi Bai Lu Ronghua Jin Yingmin Ma Chengfeng Qin Youchun Wang Yingmei Feng Junyu Xiao Xiaoliang Sunney Xie 2021Cell Research2021,31,7:12
3Cardioembolic stroke:everything has changed显示文摘Historically,because of the difficulty of using warfarin safely and effectively,many patients with cardioembolic stroke who should have been anticoagulated were instead given ineffective antiplatelet therapy(or no antithrombotic therapy).With the arrival of new oral anticoagulants that are not significantly more likely than aspirin to cause severe haemorrhage,everything has changed.Because antiplatelet agents are much less effective in preventing cardioembolic stroke,it is now more prudent to anticoagulate patients in whom cardioembolic stroke is strongly suspected.Recent advances include the recognition that intermittent atrial fibrillation is better detected with more prolonged monitoring of the cardiac rhythm,and that percutaneous closure of patent foramen ovale(PFO)may reduce the risk of stroke.However,because in most patients with stroke and PFO the PFO is incidental,this should be reserved for patients in whom paradoxical embolism is likely.A high shunt grade on transcranial Doppler saline studies,and clinical clues to paradoxical embolism,can help in appropriate selection of patients for percutaneous closure.For patients with atrial fibrillation who cannot be anticoagulated,ablation of the left atrial appendage is an emerging option.It is also increasingly recognised that high levels of homocysteine,often due to undiagnosed metabolic deficiency of vitamin B_(12),markedly increase the risk of stroke in atrial fibrillation,and that B vitamins(folic acid and B_(12))do prevent stroke by lowering homocysteine.However,with regard to B_(12),methylcobalamin should probably be used instead of cyanocobalamin.Many important considerations for judicious application of therapies to prevent cardioembolic stroke are discussed.J David Spence 2018Stroke & Vascular Neurology2018,3,2:10
4RELATIONS OF INTRACRANIAL PRESSURE, CREATINE KINASE AND BRAINSTEM AUDITORY EVOKED POTENTIAL IN PATIENTS WITH TRAUMATIC BRAIN EDEMA显示文摘We studied the relations of intracranial pressure (ICP),creatine kinase (CK) and bralnstem auditory evoked potential (BAEP) in 44 patients with traumatic brain edema who were admitted to our hospital from June 1990 to February 1991. There were 30 males and 14 females, with age range from 9 to 67 years. The results showed that the abnormal BAEP could reflect the severity of cerebral edema in acute head injury and was related to ICP and serum CK levels. When ICP>30 mmHg (4kPa), the abnormality of BAEP was more obvious than that of the control group (P<0.05); the serum CK levels were also elevated markedly. In patients with ICP over and below 4kPa, the rate of abnormal BAEP was 38.46% and 77.78% respectively (P<0.05). The serum CK level in the normal group or in the group with moderate abnormality of BAEP was significantly different from that in the group with severe abnormality or lack of BAEP (274.8± 98.24 U/L vs 705.3± 364.27 U/L; P<0.001). 5Receptome profiling identifies KREMEN1 and ASGR1 as alternative functional receptors of SARS-CoV-2显示文摘Host cellular receptors play key roles in the determination of virus tropism and pathogenesis.However,little is known about SARS-CoV-2 host receptors with the exception of ACE2.Furthermore,ACE2 alone cannot explain the multi-organ tropism of SARS-CoV-2 nor the clinical differences between SARS-CoV-2 and SARS-CoV,suggesting the involvement of other receptor(s).Here,we performed genomic receptor profiling to screen 5054 human membrane proteins individually for interaction with the SARS-CoV-2 capsid spike(S)protein.Twelve proteins,including ACE2,ASGR1,and KREMEN1,were identified with diverse S-binding affinities and patterns.ASGR1 or KREMEN1 is sufficient for the entry of SARS-CoV-2 but not SARS-CoV in vitro and in vivo.SARS-CoV-2 utilizes distinct ACE2/ASGR1/KREMEN1(ASK)receptor combinations to enter different cell types,and the expression of ASK together displays a markedly stronger correlation with virus susceptibility than that of any individual receptor at both the cell and tissue levels.The cocktail of ASK-related neutralizing antibodies provides the most substantial blockage of SARS-CoV-2 infection in human lung organoids when compared to individual antibodies.Our study revealed an interacting host receptome of SARS-CoV-2,and identified ASGR1 and KREMEN1 as alternative functional receptors that play essential roles in ACE2-independent virus entry,providing insight into SARS-CoV-2 tropism and pathogenesis,as well as a community resource and potential therapeutic strategies for further COVID-19 investigations.Yunqing Gu Jun Cao Xinyu Zhang Hai Gao Yuyan Wang Jia Wang Juan He Xiaoyi Jiang Jinlan Zhang Guanghui Shen Jie Yang Xichen Zheng Gaowei Hu Yuanfei Zhu Shujuan Du Yunkai Zhu Rong Zhang Jianqing Xu Fei Lan Di Qu Guoliang Xu Yun Zhao Dong Gao Youhua Xie Min Luo Zhigang Lu 2022Cell Research2022,32,1:8
6Deceleration Capacity--A Novel Measure for Autonomic Nervous System in Patients with Vasovagal Syncope on Tilt-table Testing显示文摘This study was to investigate the changes of autonomic nerve function and hemodynamics in patients with vasovagal syncope(VVS) during head-up tilt-table testing(HUT). HUT was performed in 68 patients with unexplained syncope and 18 healthy subjects served as control group. According to whether bradycardia, hypotension or both took place during the onset of syncope, the patients were divided during the test into three subgroups: vasodepressor syncope(VD), cardioinhibitory syncope(CI) and mixed syncope(MX) subgroups. Heart rate, blood pressure, heart rate variability(HRV), and deceleration capacity(DC) were continuously analyzed during HUT. For all the subjects with positive responses, the normalized low frequency(LFn) and the LF/HF ratio markedly decreased whereas normalized high frequency(HFn) increased when syncope occurred. Syncopal period also caused more significant increase in the power of the DC in positive groups. These changes were more exaggerated compared to controls. All the patients were indicative of a sympathetic surge in the presence of withdrawal vagal activity before syncope and a sympathetic inhibition with a vagal predominance at the syncopal stage by the frequency-domain analysis of HRV. With the measurements of DC, a decreased vagal tone before syncope stage and a vagal activation at the syncopal stage were observed. The vagal tone was higher in subjects showing cardioinhibitory responses at the syncopal stage. 7FGF1^(△HBS) prevents diabetic cardiomyopathy by maintaining mitochondrial homeostasis and reducing oxidative stress via AMPK/Nur77 suppression显示文摘As a classically known mitogen,fibroblast growth factor 1(FGF1)has been found to exert other pleiotropic functions such as metabolic regulation and myocardial protection.Here,we show that serum levels of FGF1 were decreased and positively correlated with fraction shortening in diabetic cardiomyopathy(DCM)patients,indicating that FGF1 is a potential therapeutic target for DCM.We found that treatment with a FGF1 variant(FGF1^(△HBS))with reduced proliferative potency prevented diabetes-induced cardiac injury and remodeling and restored cardiac function.RNA-Seq results obtained from the cardiac tissues of db/db mice showed significant increase in the expression levels of anti-oxidative genes and decrease of Nur77 by FGF1AHBS treatment.Both in vivo and in vitro studies indicate that FGF1^(△HBS) exerted these beneficial effects by markedly reducing mitochondrial fragmentation,reactive oxygen species(ROS)generation and cytochrome c leakage and enhancing mitochondrial respiration rate and β-oxidation in a 5;AMP-activated protein kinase(AMPK)/Nur77-dependent manner,all of which were not observed in the AMPK null mice.The favorable metabolic activity and reduced proliferative properties of FGF1^(△HBS) testify to its promising potential for use in the treatment of DCM and other metabolic disorders.Dezhong Wang Yuan Yin Shuyi Wang Tianyang Zhao Fanghua Gong Yushuo Zhao Beibei Wang Yuli Huang Zizhao Cheng Guanghui Zhu Zengshou Wang Yang Wang Jun Ren Guang Liang Xiaokun Li Zhifeng Huang 2021Signal Transduction and Targeted Therapy2021,6,4:6
8Influences of Refined Konjac Meal on the Levels of Tissue Lipids and the Absorption of Four Minerals in Rats显示文摘This paper reports a study on the hypocholesterolemic effect of the refined konjac meal (RKM) containing about 80% glucomannan prepared from the tubers of Amorphophallus konjac K.Koch.Male and female Sprague-Dawley rats, 5 weeks old, were divided into five groups and fed a normal basal diet, a hypercholesterolemic diet (control diet), and three test diets (RKM added to the control diet at levels of 2.5.5, and 10%, respectively) for 12 weeks.The results obtained indicate that RKM could markedly lower the cholesterol levels in the serum and the liver of rats eating hypercholesterolemic diets.At the end of the 4th week of the feeding experiment, the serum cholesterol levels in the 5 and the 10% RKM groups and the liver cholesterol level in the 10% RKM group were significantly lower than those in the control groups.At the end of the 12th week, the serum cholesterol levels of all the three RKM groups were lowered to a normal level as was the liver cholesterol level of the 10% RKM group.The lipolropic effect of RKM was also confirmed by histopathologic examination of the livers.In addition to the hypocholesterolemic effects, RKM diets also increased stool bulk.Minor effects on the absorption and utilization of Ca, Fe, Zn, and Cu were found. 1990 Academic Press.inc.HOU YUN-HUA, ZHANG LI-SHI, ZHOU HONG-MING, WANG RUI-SHU, AND ZHANG YIN-ZHU School of Public Health, West China University of Medical Sciences, Chengdu,China 1990Biomedical and Environmental Sciences1990,3,3:6
9Localized bursting of mesocarp cells triggers catastrophic fruit cracking显示文摘The so-called rain-cracking of sweet cherry fruit severely threatens commercial production.Simple observation tells us that cuticular microcracking(invisible)always precedes skin macrocracking(visible).The objective here was to investigate how a macrocrack develops.Incubating detached sweet cherry fruit in deionized water induces microcracking.Incubating fruit in D2O and concurrent magnetic resonance imaging demonstrates that water penetration occurs only(principally)through the microcracks,with nondetectable amounts penetrating the intact cuticle.Optical coherence tomography of detached,whole fruit incubated in deionized water,allowed generation of virtual cross-sections through the zone of a developing macrocrack.Outer mesocarp cell volume increased before macrocracks developed but increased at a markedly higher rate thereafter.Little change in mesocarp cell volume occurred in a control zone distant from the crack.As water incubation continued,the cell volume in the crack zone decreased,indicating leaking/bursting of individual mesocarp cells.As incubation continued still longer,the crack propagated between cells both to form a long,deep macrocrack.Outer mesocarp cell turgor did not differ significantly before and after incubation between fruit with or without macrocracks;nor between cells within the crack zone and those in a control zone distant from the macrocrack.The cumulative frequency distribution of the logtransformed turgor pressure of a population of outer mesocarp cells reveals all cell turgor data followed a normal distribution.The results demonstrate that microcracks develop into macrocracks following the volume increase of a few outer mesocarp cells and is soon accompanied by cell bursting.Eckhard Grimm Jan Hahn Daniel Pflugfelder Moritz Jonathan Schmidt Dagmar van Dusschoten Moritz Knoche 2019Horticulture Research2019,6,1:5
10MUTAGENICITY AND CARCINOGENICITY OF FISH SAUCE FROM A COUNTY WITH THE HIGH RISK FOR GASTRIC CANCER IN CHINA显示文摘The mutagenicity and carcinogenicity of fish sauce (FS) sample from Changle County, a high gastric cancer mortality (113.20/105) area, were investigated with the biologic short-term tests and laboratory animal experiment. The results showed that the extract of FS was markedly direct mutagenic toward S. typhimurium TA100, induced high sister chromatid exchanges (SCE) and micronucleus (MN) in V79 cells after nitrosation with sodium nitrite. But the non-nitrosated FS did not. The nitrosated fish sauce (NFS) also induced SOS in E. coli PQ37 and alkylation of calfthymus DNA. The potency of NFS to induce unscheduled DNA synthesis (UDS) in human normal gastric mucosal cells was increased about fivefold compared with FS. When the NFS extract was given to newborn rats by gavage, dys-plasia and adenocaroinoma were induced in the glandular stomach in the 4th and 16th experimental week, respectively. 11Anti-PD-L1/TGF-BR fusion protein (SHR-1701) overcomes disrupted lymphocyte recovery-induced resistance to PD-1/PD-L1 inhibitors in lung cancer显示文摘Background:Second-generation programmed cell death-protein 1/pro-grammed death-ligand 1(PD-1/PD-L1)inhibitors,such as bintrafusp alfa(M7824),SHR-1701,and YM101,have been developed to simultaneously block PD-1/PD-L1 and transforming growth factor-beta/transforming growth factor-beta receptor(TGF-P/TGF-βR).Consequently,it is necessary to identify predictive factors of lung cancer patients who are not only resistant to PD-1/PD-LI inhibitors but also sensitive to bifunctional drugs.The purpose of this study was to search for such predictors.Methods:Multivariable Cox regression was used to study the association between the clinical outcome of treatment with PD-1/PD-L1 inhibitors and lym-phocyte recovery after lymphopenia in lung cancer patients.Murine CMT167 lung cancer cells were engineered to express the firefly luciferase gene and implanted orthotopically in the lung of syngeneic mice.Bioluminescence imag-ing,flow cytometry,and immunohistochemistry were employed to detrmine response to immunotherapy and function of tumor-infiltrating immune cells.Results:For lung cancer patients treated with anti-PD-1/PD-LI antibodies,poor lymphocyte recovery was associated with a shorter progression-free survival(PFS;P<0.001),an accumulation of regulatory T cells(Tregs),and an elimi-nation of CD8+T cells in the peripheral blood.Levels of CD8+T cells and Treg cells were also imbalanced in the tumors and peripheral immune organs of mice with poor lymphocyte recovery after chemotherapy.Moreover,these mice failed to respond to anti-PD-1 antibodies but remained sensitive to the anti-PD-LI/TGF-βR fusion protein(SHR-1701).Consistently,SHR-1701 but not anti-PD-1 antibod-ies,markedly enhanced IFN-γproduction and Ki-67 expression in peripheral CD8+T cells from patients with impaired lymphocyte recovery.Conclusions:Lung cancer patients with poor lymphocyte recovery and suffer-ing from persistent lymphopenia after previous chemotherapy are resistant to anti-PD-1/PD-L1 antibodies but might be sensitive to second-generation agents such as SHR-1701.Bo Cheng Kaikai Ding Pengxiang Chen Jianxiong Ji Tao Luo Xiaofan Guo Wei Qiu Chunhong Ma Xue Meng Jian Wang Jinming Yu Yuan Liu 2022Cancer Communications2022,42,1:5
12Identification of miR-515-3p and its targets,vimentin and MMP3,as a key regulatory mechanism in esophageal cancer metastasis:functional and clinical significance显示文摘Metastasis is the main factor of treatment failure in cancer patients,but the underlying mechanism remains to be elucidated and effective new treatment strategies are urgently needed.This study aims to explore novel key metastasis-related microRNAs(miRNAs)in esophageal squamous cell carcinoma(ESCC).By comparing miRNA profiles of the highly metastatic ESCC cell sublines,we established through serial in vivo selection with the parental cells,we found that the expression level of miR-515-3p was lower in ESCC tumor tissues than adjacent normal tissues,further decreased in metastatic tumors,and moreover,markedly associated with advanced stage,metastasis and patient survival.The in vitro and in vivo assays suggested that miR-515-3p could increase the expression of the epithelial markers as well as decrease the expression of the mesenchymal markers,and more importantly,suppress invasion and metastasis of ESCC cells.Mechanistically,we revealed that miR-515-3p directly regulated vimentin and matrix metalloproteinase-3(MMP3)expression by binding to the coding sequence and 3′untranslated region,respectively.In addition,the data from whole-genome methylation sequencing and methylation-specific PCR indicated that the CpG island within miR-515-3p promoter was markedly hypermethylated in ESCC cell lines and ESCC tumor tissues,which may lead to deregulation of miR-515-3p expression in ESCC.Furthermore,our preclinical experiment provides solid evidence that systemic delivery of miR-515-3p oligonucleotide obviously suppressed the metastasis of ESCC cells in nude mice.Taken together,this study demonstrates that miR-515-3p suppresses tumor metastasis and thus represents a promising prognostic biomarker and therapeutic strategy in ESCC.Hui-Fang Hu Wen Wen Xu Wei-Xia Zhang Xin Yan Yang-Jia Li Bin Li Qing-Yu He 2020Signal Transduction and Targeted Therapy2020,5,1:5
13Controlling resistant hypertension显示文摘Resistant hypertension(failure to achieve target blood pressures with three or more antihypertensive drugs including a diuretic)is an important and preventable cause of stroke.Hypertension is highly prevalent in China(>60%of persons above age 65),and only~6%of hypertensives in China are controlled to target levels.Most strokes occur among persons with resistant hypertension;approximately half of strokes could be prevented by blood pressure control.Reasons for uncontrolled hypertension include(1)non-compliance;(2)consumption of substances that aggravated hypertension,such as excess salt,alcohol,licorice,decongestants and oral contraceptives;(3)therapeutic inertia(failure to intensify therapy when target blood pressures are not achieved);and(4)diagnostic inertia(failure to investigate the cause of resistant hypertension).In China,an additional factor is lack of availability of appropriate antihypertensive therapy in many healthcare settings.Sodium restriction in combination with a diet similar to the Cretan Mediterranean or the DASH(Dietary Approaches to Stop Hypertension)diet can lower blood pressure in proportion to the severity of hypertension.Physiologically individualised therapy for hypertension based on phenotyping by plasma renin activity and aldosterone can markedly improve blood pressure control.Renal hypertension(high renin/high aldosterone)is best treated with angiotensin receptor antagonists;primary aldosteronism(low renin/high aldosterone)is best treated with aldosterone antagonists(spironolactone or eplerenone);and hypertension due to overactivity of the renal epithelial sodium channel(low renin/low aldosterone;Liddle phenotype)is best treated with amiloride.The latter is far more common than most physicians suppose.J David Spence 2018Stroke & Vascular Neurology2018,3,2:4
14Optimal dual antiplatelet therapy strategy in elderly patients with acute coronary syndrome显示文摘One out of three hospitalizations for acute coronary syndrome(ACS)involve nowadays elderly patients,carrying together a significant burden of comorbidities and a higher risk of complications.In particular,both ischemic and haemorrhagic risk are markedly enhanced in advanced age,and strictly interconnected,challenging the management of dual antiplatelet therapy(DAPT)in these patients.The recent development of several therapeutic options in terms of duration and combination of antiplatelet agents have offered a wider spectrum of opportunities for a more individualized approach in the management of DAPT after an ACS,although the criteria for the selection of the most appropriate strategy in each patient still lack validation.In particular,dose-adjustment,early aspirin discontinuation,laboratory-driven tailoring and shorter or extended DAPT have been addressed with promising safety and efficacy results.The present review provides an updated overview on the emerging evidencefrom randomized clinical trials and subanalyses dedicated to the management of DAPT in elderly patients presenting with ACS.Monica Verdoia Rocco Gioscia Giuseppe De Luca 2021Journal of Geriatric Cardiology2021,18,3:4

19Changes of PTH, 1,25(OH)_2D_3 and Biochemistry in Renal Osteodystrophy.显示文摘The changes of plasma PTH, 1,25(OH)2D3, serum calcium, phosphorus and AKP were observed in 55 uremic patients with renal osteodystrophy. The results showed that: in all types of renal osteodystrophy, plasma PTH were markedly higher than normal (P<0.001), while plasma 1,25(OHH)2D3 and serum calcium were lower than normal (P<0.001). In low bone-turnover uremic osteodystrophy (LTO), plasma phosphorus was markedly lower than that of high bone-turnover uremic osteodystrophy (HTO) and mixe d uremic osteodystrophy (MO) (P<0.05), and plasma 1,25(OH)2D3 was significantly lower than that of HTO (P<0.05). In HTO and MO, serum phosphorus was much higher than mormal (P<0.001). Plasma PTH had a negative linear correlation with plasma 1,25(OH)2D3 (P<0.05), a positive linear correlation with serum AKP (P<0.05), but no linear correlation with the duration of hemodialysis, serum calcium, serum aluminum and bone aluminum content.
