| 2 | Benefit of combination β-blocker and endoscopic treatment to prevent variceal rebleeding: A meta-analysis显示文摘AIM: To determine whether the association of β-blockers with endoscopic treatment is superior to endoscopic treatment alone for the secondary prophylaxis of oesophageal variceal bleeding. METHODS: Randomised controlled trials comparing sclerotherapy (SCL) with SCL plus β-blockers (BB) or banding ligation (BL) with BL plus BB were identif ied.Main outcomes were overall and 6, 12 and 24 mo rebleeding rates, as well as overall and 6, 12 and 24 mo mortality. Two statistical methods were used: Yusuf-Peto, and Der Simonian and Laird. Inter-trial heterogeneity was systematically taken into account. RESULTS: Seventeen randomised controlled trials were included, 14 with SCL and 3 with BL. Combination β-blocker and endoscopic treatment signif icantly reduced rebleeding rates at 6, 12 and 24 mo and overall [odds ratio (OR): 2.20, 95% conf idence interval (CI): 1.69-2.85, P<0.0001] compared to endoscopic treatment alone. Mortality at 24 mo was signif icantly lower for the combined treatment group (OR: 1.83, 95% CI:1.16-2.90, P= 0.009), as well as overall mortality (OR: 1.43, 95% CI:1.03-1.98, P= 0.03). | 7 | Influence of Garlic Intercropping or Active Emitted Volatiles in Releasers on Aphid and Related Beneficial in Wheat Fields in China显示文摘In order to develop biological control of aphids by a 'push-pull'approach, intercropping using repellent emitting plants was developed in different crop and associated plant models. Garlic is one of the potential plant that could be inserted in crops to decrease the pest occurrence in neighboring crop plots. In this study, field works were conducted in wheat fields in Langfang Experimental Station, Hebei Province in China from October 2009 to July 2010 during wheat developmental season. The effect of wheat intercropping with garlic but also the volatiles emission on the incidence of the English grain aphid, Sitobion avenae Fabricius (Homoptera: Aphididae) was assessed. Natural beneficial occurrence and global yields in two winter wheat varieties that were susceptible or resistant to cereal aphid were also determined comparing to control plots without the use of garlic plant intercrop nor semiochemical releaser in the fields. S. avenae was found to be lower in garlic oil blend treatment (GOB), diallyl disulfide treatment (DD) and wheat-garlic intercropping treatment (WGI) when compared to the control plots for both two varieties (P<0.01). Both intercropping and application of volatile chemicals emitted by garlic could improve the population densities of natural enemies of cereal aphid, including ladybeetles and mummified aphids. Ladybeetle population density in WGI, GOB and mummified aphids densities in WGI, DD were significantly higher than those in control fields for both two varieties (P<0.05). There were significant interactions between cultivars and treatments to the population densities of S. avenae. The 1 000-grain weight and yield of wheat were also increased compared to the control. | 14 | 胆道闭锁患儿Kasai术后远期预后研究——63例带自体肝患者生存20年评估显示文摘目的评估胆道闭锁患儿Kasai术后远期预后。方法1968年~1983年,法国巴黎Bicêtre医院儿外科共对271例胆道闭锁患儿行Kasai手术或其变异术式,术后随访20年,对带自体肝生存患者进行综合性评估。结果271例患者中,63例(占23%)带自体肝者至少生存20年。肝功能结果显示:21例总胆红素指标正常,其中12例γ-GT和转氨酶指标正常;44例有门脉高压表现;19例仍有晚期胆道感染发作;6例并发胆道结石;61例患者肝穿刺活检提示有不同程度肝硬化表现。20岁之后,2例死于肝功能衰竭;10例已行肝移植;4例正等待肝移植。结论胆道闭锁患儿Kasai术后带自体肝20年生存率仅为23%,大部分患儿转肝移植治疗。即使带自体肝生存患儿随访各项指标也不令人满意。因此密切关注肝脏情况,尽早发现并发症,对决定进一步治疗措施,改善预后,具有积极的意义。 | 朱坚 韩天权 Frédéric GAUTHIER | 2005 | 临床小儿外科杂志2005,4,6: | 7 |

By making use of advanced CMOS technology,complex miniaturized circuits can be easily realized on a large scale and at a low cost covering visible to mid-IR wavelengths. In this paper we present our recent work on the development of silicon and Si3N4-based photonic integrated circuits for various spectroscopic sensing applications. We report our findings on waveguide-based absorption, and Raman and surface enhanced Raman spectroscopy. | 16 | Genotype phenotype classification of hepatocellular adenoma显示文摘Studies that compare tumor genotype with phenotype have provided the basis of a new histological/molecular classification of hepatocellular adenomas. Based on two molecular criteria (presence of a TCF1/HNF1α or β-catenin mutation), and an additional histological criterion (presence or absence of an inflammatory infiltrate), subgroups of hepatocellular adenoma can be defined and distinguished from focal nodular hyperplasia. Analysis of 96 hepatocellular adenomas performed by a French collaborative network showed that they can be divided into four broad subgroups: the fi rst one is defi ned by the presence of mutations in TCF1 gene inactivating the hepatocyte nuclear factor 1 (HNF1α); the second by the presence of β-catenin activating mutations; the category without mutations of HNF1α or β-catenin is further divided into 2 subgroups depending on the presence or absence of in? ammation. Therefore, the approach to the diagnosis of problematic benign hepatocytic nodules may be entering a new era directed by new molecular information. It is hoped that immunohistological tools will improve significantly diagnosis of liver biopsy in our ability to distinguish hepatocellular adenoma from focal nodular hyperplasia (FNH), and to delineate clinically meaningful entities within each group to define the best clinical management. | 17 | Genetic Variation Among the Geographic Population of the Grain Aphid, Sitobion avenae (Hemiptera: Aphididae) in China Inferred from Mitochondrial COI Gene Sequence显示文摘In order to characterize the genetic relationship of the geographic populations of Sitobion avenae (Hemiptera: Aphididae) in China, a 588 bp region of the mitochondrial cytochrome oxidase subunit I (mtDNA-COI) gene was sequenced and analyzed among the different geographic populations. 269 individuals were collected from 17 localities in different wheat-growing areas in China that covered most of the range reported for this species. Within the sequence among these geographic populations, 15 polymorphic sites defined 16 distinct haplotypes, ranging in sequence divergence from 0.2% (one nucleotide) to 1.7% (10 nucleotides). Of the 15 variable sites, 12 were transitional substitutions, 2 were transversional substitutions and 1 was transitional and transversional substitution. Phylogenetic analysis showed that all haplotypes were highly interconnected with each other, in absence of phylogeographic structing. Each of 8 haplotypes was found only at one locality, and the other haplotypes were the widespread distributed in the different localities. The higher genetic diversity was found in the northern China populations than that in the southern China populations. METHODS: A prospective, non-comparative, and nonrandomized trial was performed and registered on Eudra CT database(2011-002953-80) and on www.trial.gov(NCT01398098). All patients had a validated indication for colonoscopy and a preparation using sodium phosphate(NaP) tablets. The day of the procedure, patients took 4 tablets with 250 mL of water every 15 min, three times. The gastric volume wasestimated every 15 min from computed antral surfaces and weight according to the formula of Perlas et al(Anesthesiology, 2009). Colonoscopy was performed within the 6 h following the last intake.RESULTS: Thirty patients were prospectively included in the study from November 2011 to May 2012. The maximum volume of the antrum was 212 mL, achieved 15 min after the last intake. 24%, 67% and 92% of subjects had an antral volume below 20 mL at 60, 120 and 150 min, respectively. 81% of patients had a Boston score equal to 2 or 3 in each colonic segment. No adverse events leading to treatment discontinuation were reported.CONCLUSION: Gastric volume evaluation appeared to be a simple and reliable method for the assessment of gastric emptying.
