2Noninvasive investigations for non alcoholic fatty liver disease and liver fi brosis显示文摘Non-alcoholic fatty liver disease (NAFLD) includes a spectrum of diseases that have insulin resistance in common and are associated with metabolic conditions such as obesity, type 2 diabetes mellitus, and dyslipidemia. NAFLD ranges from simple liver steatosis, which follows a benign course, to nonalcoholic steatohepatitis (NASH), a more severe entity, with necroinflmmation and f ibrosis, which can progress to cryptogenic cirrhosis and end-stage liver disease. Liver biopsy remains the gold standard for evaluating the degree of hepatic necroinflammation and f ibrosis; however, several noninvasive investigations, such as serum biomarkers, have been developed to establish the diagnosis and also to evaluate treatment response. These markers are currently neither available in all centers nor validated in extensive studies. Examples include high-sensitivity C reactive protein and plasma pentraxin 3, which are associated with extensive liver f ibrosis in NASH. Interleukin-6 correlates with inflammation, and cytokeratin-18 represents a marker of hepatocyte apoptosis (prominent in NASH and absent in simple steatosis). 3Nonalcoholic fatty liver disease, metabolic risk factors, and hepatocellular carcinoma: An open question显示文摘Non-alcoholic liver disease(NAFLD) defines liver abnormalities ranging from simple steatosis to nonalcoholic steatohepatitis with or without cirrhosis development, occurring in the absence of significant alcohol consumption, use of teratogenic medication, or hereditary disorders.The association between NAFLD and metabolic syndrome is well documented and widely recognized.Obesity, type 2 diabetes mellitus(T2DM), and dyslipidemia are the most common metabolic risk factors associated with NAFLD.Among the components of metabolic syndrome, current evidence strongly indicates obesity and diabetes as hepatocellular carcinoma(HCC) risk factors.There is also growing evidence that suggests an increased risk of HCC in NAFLD patients, even surpassing other etiologies in some high-income countries.Epidemiologic data demonstrate a parallel rise in prevalence of obesity, diabetes, NAFLD, and HCC.As obesity and its related diseases have steadily afflicted larger populations, HCC incidence is expected to increase in the future.Pathophysiologic mechanisms that underlie NAFLD development and subsequent progression to nonalcoholic steatohepatitis and cirrhosis(insulin resistance and hyperinsulinemia, oxidative stress, hepatic stellate cell activation, cytokine/adipocytokine signaling pathways, and genetic and environmental factors) appear to play a significant role in the development of NAFLD-related HCC.However, a comprehensive view of molecular mechanisms linking obesity, T2 DM, and NAFLD-related HCC, as well as the exact sequence of molecular events, is still not understood in its entirety.Good-quality data are still necessary, and efforts should continue towards better understanding the underlying carcinogenic mechanisms of NAFLD-related HCC.In this paper, we aimed to centralize the most important links supporting these relationships, focusing on obesity, T2 DM, and NAFLD-related HCC, as well as point out the major gaps in knowledge regarding the underlying molecular mechanisms behind them.Letitia Adela Maria Streba Cristin Constantin Vere Ion Rogoveanu Costin Teodor Streba 2015World Journal of Gastroenterology2015,21,14:15

5Contrast-enhanced ultrasonography parameters in neural network diagnosis of liver tumors显示文摘AIM:To study the role of time-intensity curve(TIC) analysis parameters in a complex system of neural networks designed to classify liver tumors.METHODS:We prospectively included 112 patients with hepatocellular carcinoma(HCC)(n = 41),hypervascular(n = 20) and hypovascular(n = 12) liver metastases,hepatic hemangiomas(n = 16) or focal fatty changes(n = 23) who underwent contrast-enhanced ultrasonography in the Research Center of Gastroenterology and Hepatology,Craiova,Romania.We recorded full length movies of all contrast uptake phases and post-processed them offline by selecting two areas of interest(one for the tumor and one for the healthy surrounding parenchyma) and consecutive TIC analysis.The difference in maximum intensities,the time to reaching them and the aspect of the late/portal phase,as quantified by the neural network and a ratio between median intensities of the central and peripheral areas were analyzed by a feed forward back propagation multi-layer neural network which was trained to classify data into five distinct classes,corresponding to each type of liver lesion.RESULTS:The neural network had 94.45% training accuracy(95% CI:89.31%-97.21%) and 87.12% testing accuracy(95% CI:86.83%-93.17%).The automatic classification process registered 93.2% sensitivity,89.7% specificity,94.42% positive predictive value and 87.57% negative predictive value.The artificial neural networks(ANN) incorrectly classified as hemangyomas three HCC cases and two hypervascular metastases,while in turn misclassifying four liver hemangyomas as HCC(one case) and hypervascular metastases(three cases).Comparatively,human interpretation of TICs showed 94.1% sensitivity,90.7% specificity,95.11% positive predictive value and 88.89% negative predictive value.The accuracy and specificity of the ANN diagnosis system was similar to that of human interpretation of the TICs(P = 0.225 and P = 0.451,respectively).Hepatocellular carcinoma cases showed contrast uptake during the arterial phase followed by wash-out in the portal and first seconds of the late phases.For the hypovascular metastases did not show significant contrast uptake during the arterial phase,which resulted in negative differences between the maximum intensities.We registered wash-out in the late phase for most of the hypervascular metastases.Liver hemangiomas had contrast uptake in the arterial phase without agent wash-out in the portallate phases.The focal fatty changes did not show any differences from surrounding liver parenchyma,resulting in similar TIC patterns and extracted parameters.CONCLUSION:Neural network analysis of contrastenhanced ultrasonography-obtained TICs seems a promising field of development for future techniques,providing fast and reliable diagnostic aid for the clinician.Costin Teodor Streba Mihaela Ionescu Dan Ionut Gheonea Larisa Sandulescu Tudorel Ciurea Adrian Saftoiu Cristin Constantin Vere Ion Rogoveanu 2012World Journal of Gastroenterology2012,18,32:11
6Proteomic analysis of mitochondria reveals a metabolic switch from fatty acid oxidation to glycolysis in the failing heart显示文摘This work characterizes the mitochondrial proteomic profile in the failing heart and elucidates the molecular basis of mitochondria in heart failure. Heart failure was induced in rats by myocardial infarction, and mitochondria were isolated from hearts by differential centrifugation. Using two-dimensional gel electrophoresis and matrix-assisted laser desorption/ionization-time of flight mass spectrometry, a system biology approach was employed to investigate differences in mitochondrial proteins between normal and failing hearts. Mass spectrometry identified 27 proteins differentially expressed that involved in energy metabolism. Among those, the up-regulated proteins included tricarboxylic acid cycle enzymes and pyruvate dehydrogenase complex subunits while the down-regulated proteins were involved in fatty acid oxidation and the OXPHOS complex. 10Coexistence of normal, super-, and hyper-deformation in nuclei: A study with angular momentum projection method显示文摘Angular-momentum-projected energy surface calculations for A ≈ 110 nuclei indicate three distinct energy minima occurring at different angular-momenta. These correspond to normal, super-, and hy- per-deformed shapes coexisting in one nucleus. 110Pd is studied in detail, with a quantitative prediction on super- and hyper-deformed spectra by the Projected Shell Model calculation. 11Studies of low-lying states of even-even Xe isotopes within the nucleon pair approximation显示文摘We study the relevance of nucleon pairs with higher spins, in addition to spin-zero and spin-two pairs (i.e., S D pairs), in low-lying states of even-even Xe nuclei. By including those higher-spin pairs in our configuration, we recalculate energy level schemes, elec trical quadrupole moments, magnetic moments and electric quadrupole transition rates, for low-lying states of even-even 128-142Xe nuclei. 13Diagnostic challenges in non-cirrhotic portal hypertension-porto sinusoidal vascular disease显示文摘Non-cirrhotic portal hypertension consists of a group of diseases characterized by signs and complications of portal hypertension,which differ from cirrhosis through histological alterations,hemodynamic characterization and,clinical outcome.Because of the similarities in clinical presentation and imaging signs,frequently these patients,and particularly those with porto-sinusoidal vascular disease(PSVD),are misdiagnosed as having liver cirrhosis and thus raising difficulties in their diagnosis.The most challenging differentiation to be considered is between PSVD and cirrhosis and,although not pathognomonic,liver biopsy is still the standard of diagnosis.Although they still require extended validation before being broadly used,new non-invasive methods for the diagnosis of porto-sinusoidal vascular disease,like transient elastography,contrast-enhanced ultrasound or metabolomic profiling,have shown promising results.Another issue is the differentiation between PSVD and chronic extrahepatic portal vein obstruction,especially now when it is known that 40%of patients suffering from PSVD develop portal vein thrombosis.In this particular case,once the portal vein thrombosis occurred,the diagnosis of PSVD is impossible according to the current guidelines.Moreover,so far,the differentiation between PSVD and sinusoidal obstruction syndrome has not been clear so far in particular circumstances.In this review we highlighted the diagnostic challenges regarding the PSVD,as well as the current techniques used in the evaluation of these patients.Oana Nicoara-Farcau Ioana Rusu Horia Stefanescu Marcel Tanțau Radu Ion Badea Bogdan Procopet 2020World Journal of Gastroenterology2020,26,22:4
14Systematic study of survival probability of excited superheavy nuclei显示文摘The stability of excited superheavy nuclei (SHN) with 100 Z 134 against neutron emission and fission is investigated by using a statistical model. In particular, a systematic study of the survival probability against fission in the 1n-channel of these SHN is made. The present calculations consistently take the neutron separation energies and shell correction energies from the calculated results of the finite range droplet model which predicts an island of stability of SHN around Z = 115 and N = 179. It turns out that this island of stability persists for excited SHN in the sense that the calculated survival probabilities in the 1n-channel of excited SHN at the optimal excitation energy are maximized around Z = 115 and N = 179. 15CALCULATION OF 3-D TRANSIENT TEMPERATURE FIELD DURING LASER TRANSFORMATION HARDENING PROCESS显示文摘A three-dimensional transient heat transfer model for laser transformation hardening process has been developed in this paper. The finite size of the laser treated sample, the surface heat loss of the sample, the latent heat of phase transformation and the temperature dependence of thermal properties of materials were considered. The heat source was considered as a moving Gaussian heat flux with a constant velocity. Three-dimension unequally spatial grid explicit finite difference equations, alternating direction implicit finite difference equations and implicit finite difference equations were deduced respectively. Three programs to calculate the temperature field were developed using Fortran language. The transient temperature fields of C22, 42CrMo, C60 steel samples during laser transformation hardening process were calculated using these programs, and the widths and depths of laser transformation hardening zones were also predicted. C22, 42CrMo, C60 steel samples were treated by CO2 laser,the widths and depths of laser transformation hardening zones of these samples were also measured experimentally. 17A mechanism for enhanced photocatalytic activity of nano-size silver particle modified titanium dioxide thin films显示文摘Ag-TiO2 nanostructured thin films with silver volume fraction of 0–20% were deposited on silicon and quartz substrates by RF magnetron sputtering and annealed in ambient air at 950°C for 1 h. The phase structure and surface topography of the films were characterized by X-ray diffractometer and transmission electron microscope. Photocatalytic activity of the films was evaluated by light induced degradation of methyl orange (C14H14N3NaO3S) solution using a high pressure mercury lamp as lamp-house. The relation of photocatalytic activity and silver content was studied in detail. It was found that silver content influences phase structure of TiO2 thin films, and silver in the films is metallic Ag (Ag0). With increasing silver content from 0 to 20 vol%, photocatalytic activity of the films increases first and then decreases. A suitable amount (2.5–5 vol%) silver addition can significantly enhance the photocatalytic activity of TiO2 films.
