1Glutamine prevents oxidative stress in a model of mesenteric ischemia and reperfusion显示文摘AIM: To evaluate preventative effects of glutamine in an animal model of gut ischemia/reperfusion(I/R).METHODS: Male Wistar rats were housed in a controlled environment and allowed access to food and water ad libitum. Twenty male Wistar rats were divided into four experimental groups:(1) control group(control)- rats underwent exploratory laparotomy;(2) control + glutamine group(control-GLU)- rats were subjected to laparotomy and treated intraperitoneally with glutamine 24 and 48 h prior to surgery;(3) I/R group- rats were subjected to occlusion of the superior mesenteric artery for 30 min followed by 15 min of reperfusion; and(4) ischemia/reperfusion + glutamine group(G + I/R)- rats were treated intraperitoneally with glutamine 24 and 48 h before I/R. Local and systemic injuries were determined by evaluating intestinal and lung segments for oxidative stress using lipid peroxidation and the activity of superoxide dismutase(SOD), interleukin-6(IL-6) and nuclear factor kappa beta(NFkB) after mesenteric I/R. RESULTS: Lipid peroxidation of the membrane was increased in the animals subjected to I/R(P < 0.05). However, the group that received glutamine 24 and 48 h before the I/R procedure showed levels of lipid peroxidation similar to the control groups(P < 0.05). The activity of the antioxidant enzyme SOD was decreased in the gut of animals subjected to I/R when compared with the control group of animals not subjected to I/R(P < 0.05). However, the group that received glutamine 24 and 48 h before I/R showed similar SOD activity to both control groups not subjected to I/R(P < 0.05). The mean area of NF-kB staining for each of the control groups was similar. The I/R group showed the largest area of staining for NF-kB. The G + I/R group had the second highest amount of staining, but the mean value was much lower than that of the I/R group(P < 0.05). For IL-6, control and control-GLU groups showed similar areas of staining. The I/R group contained the largest area of IL-6 staining, followed by the G + I/R animals; however, this area was significantly lower than that of the group that underwent I/R without glutamine(P < 0.05). 2Glutamine prevents oxidative stress in a model of portal hypertension显示文摘AIM To evaluate the protective effects of glutamine in a model of portal hypertension(PH) induced by partial portal vein ligation(PPVL).METHODS Male Wistar rats were housed in a controlled environment and were allowed access to food and water ad libitum. Twenty-four male Wistar rats were divided into four experimental groups:(1) control group(SO)-rats underwent exploratory laparotomy;(2) control + glutamine group(SO + G)-rats were subjected to laparotomy and were treated intraperitoneally with glutamine;(3) portal hypertension group(PPVL)-rats were subjected to PPVL; and(4) PPVL + glutamine group(PPVL + G)-rats were treated intraperitoneally with glutamine for seven days. Local injuries were determined by evaluating intestinal segments for oxidative stress using lipid peroxidation and the activities of glutathione peroxidase(GPx), endothelial nitric oxide synthase(e NOS) and inducible nitric oxide synthase(i NOS) after PPVL.RESULTS Lipid peroxidation of the membrane was increased in the animals subjected to PH(P < 0.01). However, the group that received glutamine for seven days after the PPVL procedure showed levels of lipid peroxidation similar to those of the control groups(P > 0.05). The activity of the antioxidant enzyme GTx was decreased in the gut of animals subjected to PH compared with that in the control group of animals not subjected to PH(P < 0.01). However, the group that received glutamine for seven days after the PPVL showed similar GTx activity to both the control groups not subjected to PH(P > 0.05). At least 10 random, non-overlapping images of each histological slide with 200 × magnification(44 pixel = 1 μm) were captured. The sum means of all áreas, of each group were calculated. The mean areas of e NOS staining for both of the control groups were similar. The PPVL group showed the largest area of staining for e NOS. The PPVL + G group had the second highest amount of staining, but the mean value was much lower than that of the PPVL group(P < 0.01). For i NOS, the control(SO) and control + G(SO + G) groups showed similar areas of staining. 3Modern surgical strategies for perianal Crohn's disease显示文摘One of the most challenging phenotypes of Crohn’s disease is perianal fistulizing disease(PFCD).It occurs in up to 50%of the patients who also have symptoms in other parts of the gastrointestinal tract,and in 5%of the cases it occurs as the first manifestation.It is associated with severe symptoms,such as pain,fecal incontinence,and a significant reduction in quality of life.The presence of perianal disease in conjunction with Crohn’s disease portends a significantly worse disease course.These patients require close monitoring to identify those at risk of worsening disease,suboptimal biological drug levels,and signs of developing neoplasm.The last 2 decades have seen significant advancements in the management of PFCD.More recently,newer biologics,cell-based therapies,and novel surgical techniques have been introduced in the hope of improved outcomes.However,in refractory cases,many patients face the decision of having a stoma made and/or a proctectomy performed.In this review,we describe modern surgical management and the most recent advances in the management of complex PFCD,which will likely impact clinical practice.Gilmara Pandolfo Zabot Ornella Cassol Rogerio Saad-Hossne Willem Bemelman 2020World Journal of Gastroenterology2020,26,42:2

10Epidemiology of inflammatory bowel diseases in the state of Rio Grande do Sul,Brazil显示文摘BACKGROUND This is the first study on the epidemiology of inflammatory bowel diseases(IBDs)in Rio Grande do Sul(RS),the southernmost state of Brazil with the country’s fifth largest population.Crohn’s disease(CD)and ulcerative colitis(UC)are collectively termed IBDs.They have high incidence and prevalence rates in highincome countries,although in recent years there has been a change in the classic geographical distribution of IBDs,with growing rates in traditionally lowincidence regions.AIM To estimate the incidence and prevalence of IBDs in the RS state,Brazil,between 2014 and 2019.METHODS This is a cross-sectional descriptive observational study.Patients with IBD who had initiated treatment and met the inclusion criteria of the RS state free drug distribution program were included.Data were obtained from registration or renewal records of the RS state specialty pharmacy.The male,female,and total populations were estimated according to mid-year data from the Brazilian Institute of Geography and Statistics,which served as a reference for calculating the incidence and prevalence rates of IBDs during the study period.Results were described using mean,standard deviation,and range.RESULTS We included 1082 patients with IBD,of whom 57.5%were female and 42.5% were male.Patients with CD accounted for 72.45% of the sample,and those with UC accounted for 27.54%.IBD prevalence during the study period was 9.51 per 100000 population,of which 6.89 corresponded to people with CD and 2.62,to people with UC.Incidence rates per 100000 population/year were 2.54 in 2014,2.61 in 2015,1.91 in 2016,0.80 in 2017,0.83 in 2018,and 0.96 in 2019.The mean IBD incidence rate per 100000 population was 1.61,of which 1.17 corresponded to CD and 0.44,to UC.The mean age was 41 years,and patients were mostly aged 30-40 years.Prevalence by region was higher in the state capital metropolitan area:12.69 per 100000 population.CONCLUSION Our results demonstrated an IBD prevalence of 9.51% and incidence of 1.61 per 100000 population.The patients were predominantly female,and CD was more prevalent than UC.Ornella Sari Cassol Gilmara Pandolfo Zabot Rogerio Saad-Hossne Alexandre Padoin 2022World Journal of Gastroenterology2022,28,30:0
11Use of fly and bottom ashes from a thermoelectrical plant in the synthesis of geopolymers:Evaluation of reaction efficiency显示文摘The use of by-products as raw materials in the manufacturing of industrial products has risen in the last years because of environmental considerations.One example is the use of coal ashes from thermalelectrical plants in the production of geopolymer–a green cement made by mixing aluminosilicate with alkaline activator.In this study,fly and bottom ashes from a thermal-electrical unit were used as sources of aluminosilicate in the synthesis of geopolymers.A mixture of sodium hydroxide(10 mol/L)and sodium silicate(SiO_(2)/Na_(2)O ratio of 2.2)was used as the alkaline activator.The type(fly or bottom ash)and content of the ash were the variables in the synthesis.The ashes were characterized by X-ray fluorescence(XRF),X-ray diffraction(DRX),particle size distribution(PSD),specific surface area(BET),and thermal analysis(DTA/TGA).The ash-based geopolymer samples were measured to obtain their compressive strength after curing.The evolution of the geopolymerization process was also assessed based on final alkali concentration measurements.The results show that it is possible to obtain geopolymers using coal ashes as raw materials with high solid content.The compressive strength for the bottom ash geopolymer after 90 days of curing is 35 MPa.The low concentration of unreacted alkalis after curing(1.5×10^(-3)e 3.5×10^(-3)M)corresponds to high efficiency of the geopolymerization reaction.Leidy J.Jaramillo Nieves Fabio Elyseu Silviany Goulart Mariana de Souza Pereira Erick Zabot Valvassori Adriano Michael Bernardin 2021Energy Geoscience2021,2,2:0
