| 1 | Risk factors and clinical characteristics of portal vein thrombosis after splenectomy in patients with liver cirrhosis显示文摘BACKGROUND:Portal vein thrombosis(PVT) is a potential lethal complication and may have negative influence on the prognosis after splenectomy in patients with liver cirrhosis.Prevention and timely detection of PVT are quite significant.There is a lack of knowledge about the clinical features and risk factors of PVT.Our study aimed to investigate the risk factors and clinical characteristics of PVT in order to figure out the high-risk individuals.METHODS:We collected the clinical data of 472 consecutive patients with non-neoplastic liver cirrhosis who had undergone splenectomy from January 2008 to December 2010 in our institution.Clinical and surgical characteristics of patients who developed PVT postoperatively and those who did not develop PVT were compared.Univariate and multivariate analyses of risk factors of PVT were performed.The mortality and rebleeding rate of the patients were also evaluated.RESULTS:Of the 472 patients,52 were excluded from the study.PVT developed in 71(71/420,16.9%) patients.Multivariate analysis revealed that wider preoperative portal vein diameter,postoperative thrombocytosis,prolonged prothrombin time and periesophagogastric devascularization were significantly correlated with PVT development [odds ratio(OR):5.701,2.807,1.850 and 2.090,respectively].The incidence of PVT in patients who took antiplatelet drugs was not lower than that in those who did not.Follow-up showed that patients in the PVT group had a tendency towards reduced overall survival but it was not statistically significant.Gastrointestinal bleeding occurred more often in the PVT group than that in the non-PVT group(P=0.044).CONCLUSIONS:Wider preoperative portal vein diameter,postoperative thrombocytosis,prolonged prothrombin time and periesophagogastric devascularization are independent risk factors of PVT.PVT is related with higher risk of postoperative gastrointestinal hemorrhage but has no significant impact on the overall survival. | Mu-Xing Li Xu-Feng Zhang Zheng-Wen Liu Yi Lv | 2013 | Hepatobiliary & Pancreatic Diseases International2013,12,5: | 57 |
| 2 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 4 | Wet Cupping Therapy Improves Local Blood Perfusion and Analgesic Effects in Patients with Nerve-Root Type Cervical Spondylosis显示文摘Objective: To observe wet cupping therapy(WCT) on local blood perfusion and analgesic effects in patients with nerve-root type cervical spondylosis(NT-CS). Methods: Fifty-seven NT-CS patients were randomly divided into WCT group and Jiaji acupoint-acupuncture(JA) group according a random number table. WCT group(30 cases) was treated with WCT for 10 min, and JA group(27 cases) was treated with acupuncture for 10 min. The treatment efficacies were evaluated with a Visual Analogue Scale(VAS). Blood perfusion at Dazhui(GV 14) and Jianjing(GB 21) acupoints(affected side) was observed with a laser speckle flowmetry, and its variations before and after treatment in both groups were compared as well. Results: In both groups, the VAS scores significantly decreased after the intervention(P<0.01), while the blood perfusion at the two acupoints significantly increased after intervention(P<0.05); however, the increasement magnitude caused by WCT was obvious compared with JA(P<0.05). | 5 | Nitrogen removal characteristics of heterotrophic nitrification-aerobic denitrification by Alcaligenes faecalis C16显示文摘Alcaligenes faecalis C16 was found to have the ability to heterotrophically nitrify and aerobically denitrify. In order to further understand its nitrogen removal ability and mechanism, the growth and ammonium removal response were investigated at different C/N ratios and ammonium concentrations in the medium with citrate and acetate as carbon source separately. Furthermore, experiments of nitrogen sources, production of nitrogen gas and enzyme assay were conducted. Results show that the bacterium converts NH+4-N and produces NH2 OH during the growing phase and nitrite accumulation is its distinct metabolic feature. A. faecalis C16 is able to tolerate not only high ammonium concentration but also high C/N ratio, and the ammonium tolerance is associated with carbon source and C/N ratio. The nitrogen balance under different conditions shows that approximately28%–45% of the initial ammonium is assimilated into the cells, 44%–60% is denitrified and several percent is converted to nitrification products. A. faecalis C16 cannot utilize hydroxylamine, nitrite or nitrate as the sole nitrogen source for growth. However, nitrate can be used when ammonium is simultaneously present in the medium. A possible pathway for nitrogen removal by C16 is suggested. | 6 | Extrahepatic portacaval shunt via a magnetic compression technique: A cadaveric feasibility study显示文摘AIM: To explore the anatomical feasibility of portacaval shunt using a magnetic compression technique(MCT) in cadavers.METHODS: Computed tomography(CT) images of 30 portal hypertensive patients were obtained.The diameters of the portal vein(PV),the inferior vena cava(IVC),and distance between the two structures were measured.Similar measurements were performed on 20 adult corpses.The feasibility of portacaval shunt based on those measurements was analyzed.First stage of the extrahepatic portacaval shunt using MCT was performed on five cadavers.Specifically,the PV and IVC were exposed through an abdominal incision of the cadavers.The parent magnet was introduced from the femoral vein and was delivered into the IVC by an anchor wire and a 5F Cook catheter.The daughter magnet was introduced into the PV through the splenic vein using aninterventional guide wire.When the daughter magnet met the parent magnet,they automatically clipped together and the first stage of the portacaval shunt was set up.RESULTS: The average diameters of the PV and the IVC measured from the 30 CT image were 14.39 ± 2.36 mm and 18.59 ± 4.97 mm,respectively,and the maximum and minimum distances between the PV and the IVC were 9.79 ± 4.56 mm and 9.50 ± 4.79 mm,respectively.From 20 cadavers,the average diameters of the PV and the IVC were 14.48 ± 1.47 mm and 24.71 ± 2.64 mm,and the maximum and minimum distances between the PV and the IVC were 10.14 ± 1.70 mm and 8.93 ± 1.17 mm,respectively.The distances between the PV and the IVC from both the CT images and the cadavers were within the effective length of portacaval anastomosis using MCT(30.30 ± 4.19 mm).The PV and IVC are in close proximity to each other with no intervening tissues or structures in between.Simulated surgeries of the first stage using MCT on five cadavers was successfully performed.CONCLUSION: Anatomically,extrahepatic portacaval shunt employing MCT is highly feasible in humans. | Xiao-Peng Yan Wen-Yan Liu Jia Ma Jian-Peng Li Yi Lv | 2015 | World Journal of Gastroenterology2015,21,26: | 24 |
| 7 | Electroacupuncture improves gut barrier dysfunction in prolonged hemorrhagic shock rats through vagus anti-inflammatory mechanism显示文摘AIM:To investigate whether electroacupuncture(EA)at Zusanli(ST36)prevents intestinal barrier and remote organ dysfunction following prolonged hemorrhagic shock through a vagus anti-inflammatory mechanism.METHODS:Sprague-Dawley rats were subjected to about 45%of total blood volume loss followed by delayed fluid replacement(DFR)with Ringer lactate 3h after hemorrhage.In a first study,rats were randomly divided into six groups:(1)EAN:EA at non-channel acupoints followed by DFR;(2)EA:EA at ST36 after hemorrhage followed by DFR;(3)VGX/EA:vagotomy(VGX)before EA at ST36 and DFR;(4)VGX/EAN:VGX before EAN and DFR;(5)α-bungarotoxin(α-BGT)/EA:intraperitoneal injection ofα-BGT before hemorrhage,followed by EA at ST36 and DFR;and(6)α-BGT/EAN group:α-BGT injection before hemorrhage followed by EAN and DFR.Survival and mean arterial pressure(MAP)were monitored over the next 12 h.In a second study,with the same grouping and treatment,cytokine levels in plasma and intestine,organ parameters,gut injury score,gut permeability to 4 kDa FITC-dextran,and expression and distribution of tight junction protein ZO-1 were evaluated.RESULTS:MAP was significantly lowered after blood loss;EA at ST36 improved the blood pressure at corresponding time points 3 and 12 h after hemorrhage.EA at ST36 reduced tumor necrosis factor-αand interleukin(IL)-6 levels in both plasma and intestine homogenates after blood loss and DFR,while vagotomy or intraperitoneal injection ofα-BGT before EA at ST36reversed its anti-inflammatory effects,and EA at ST36did not influence IL-10 levels in plasma and intestine.EA at ST36 alleviated the injury of intestinal villus,the gut injury score being significantly lower than that of EAN group(1.85±0.33 vs 3.78±0.59,P<0.05).EA at ST36 decreased intestinal permeability to FITCdextran compared with EAN group(856.95 ng/mL±90.65 ng/mL vs 2305.62 ng/mL±278.32 ng/mL,P<0.05).EA at ST36 significantly preserved ZO-1 protein expression and localization at 12 h after hemorrhage.However,EA at non-channel acupoints had no such effect,and abdominal vagotomy andα-BGT treatment could weaken or eliminate the effects of EA at ST36.Besides,EA at ST36 decreased blood aminotransferase,MB isoenzyme of creatine kinase and creatinine vs EAN group at corresponding time points.At the end of 12-h experiment,the survival rate of the EA group was significantly higher than that of the other groups.CONCLUSION:EA at ST36 attenuates the systemic inflammatory response,protects intestinal barrier integrity,improves organ function and survival rate after hemorrhagic shock via activating the cholinergic antiinflammatory mechanism. | Ming-Hua Du Hong-Min Luo Sen Hu Yi Lv Zhi-Long Lin Li Ma | 2013 | World Journal of Gastroenterology2013,19,36: | 21 |
| 8 | Sutureless Intestinal Anastomosis with a Novel Device of Magnetic Compression Anastomosis显示文摘Objective To explore the feasibility and efficiency of a novel magnetic compression anastomats(MCAs) in intestinal anastomosis.Methods A total of 36 male mongrel canines underwent intestinal anastomosis using traditional hand-sewn(n=18) or a novel MCAs(n=18).We compared the anastomosis time,postoperative complications,bursting strength of anastomoses,gross appearance,and pathology between two groups at each time-point of follow-up.Results The mean anastomosis time with MCAs was significantly less than that with hand-sewn(8.50±1.95 vs.31.1±4.32 minutes,P<0.001).The blood stools and intussusceptions occurred in both groups during follow-up period.Only 1 mongrel canine receiving intestinal anastomosis by MCAs experienced anastomotic leakage.The average bursting pressure of anastomoses obtained from mongrel canines undergoing intestinal anastomosis by MCAs was significantly higher than that by traditional hand-sewn at 1 week's follow-up time(P<0.05).Gross appearance of the anastomoses constructed by MCAs was relatively smoother and flatter.Pathological evalution of anastomoses revealed that general inflammation was greater in hand-sewn anastomoses than magnetic anastomosis.Conclusion The magnetic compression anastomat is a safe and effective device of sutureless intestinal anastomosis in canine models. | Chao Fan Jia Ma Hong-ke Zhang Rui Gao Jian-hui Li Liang Yu Zheng Wu Yi Lv | 2011 | Chinese Medical Sciences Journal2011,26,3: | 20 |
| 9 | Surveillance-based evidence:elimination of schistosomiasis as a public health problem in the Peoples’Republic of China显示文摘Background:A steady progress on schistosomiasis control in the Peoples’Republic of China(P.R.China)was achieved and broadened into the twelve-year medium and long term national plan(MLNP)which marled the implementation of an integrated control strategy across all endemic areas in P.R.China in 2004.To understand the endemic trends of schistosomiasis to assess the effectiveness of an integrated strategy,we conducted an analysis of schistosomiasis surveillance data spanned from 2005 to 2015.Methods:The schistosomiasis sentinel surveillance data from sentinel sites were collected and analyzed from 2005 to 2015.In these sentinel sites,residents aged 6 years or above were screened annually by indirect hemagglutination assay(IHA),while only antibody positives were followed by stool examination either Kato-katz method(KK)and/or hatching technique(HT).Domestic animals raised in sentinel sites were examined by HT for confirming the infection of schistosomes.Snail investigation was conducted each year through systematic sampling method combined with environmental sampling method.The snails collected from field were tested by microscopic dissection method.The infection rates of schistosomes in residents,domestic animals and snails,as well as the indicators reflecting the snails’distribution were calculated and analyzed.ANOVA analysis was used to examine the changes of the number of eggs per gram feces in population and Chi-square test was used to examine any change in proportions among groups.Results:A total of 148902 residents from sentinel sites attended this study and 631676 blood samples were examined by IHA test during the 11 covered years.The annual average antibody positive rates presented a significant decrease trends,from 17.48%(95%CI:17.20–17.75%)in 2005 to 5.93%(95%CI:5.71–6.15%)(χ2=8890.47,P<0.001)in 2015.During 2005–2015,the average infection rate of schistosomes in residents declined from 2.07%(95%CI:1.96–2.17%)to 0.13%(95%CI:0.09–0.16%),accompanied by significant decrease of infection intensity in population.In 2015,the stool positives were only found in farmers,fishermen and boatmen with infection rate of 0.16%(95%CI:0.11–0.20%),0.17%(95%CI:0–0.50%)respectively.The infection rate of schistosomes in domestic animals dropped from 9.42%(538/5711,95%CI:8.66–10.18%)to 0.08%(2/2360,95%CI:0–0.20%)from 2005 to 2015.Infections were found in eight species of domestic animals at the beginning of surveillance while only two cattle were infected in 2015.Totally 98 ha of new snail habitats were found,while 94.90%(93/98)distributed in lake and marshland regions.The percentage of frames with snails decreased from 16.96%(56884/335391,95%CI:16.83–17.09%)in 2005 to 4.28%(18121/423755,95%CI:4.22–4.34%)in 2014,with a slightly increase in 2015.Meanwhile,the infection rate of schistosomes in snails was decreased from 0.26%(663/256531,95%CI:0.24–0.28%)to zero during 2005–2015.Conclusions:The infection rate of schistosomes declined significantly,providing evidence that the goal of the MLNP was achieved.Elimination of schistosomiasis as a public health problem defined as WHO was also reached in P.R.China nationwide.Surveillance-response system should be improved and strengthened to realize the final goal of schistosomiasis elimination. | Jing Xu Shi-Zhu Li Li-Juan Zhang Robert Bergquist Hui Dang Qiang Wang Shan Lv Tian-Ping Wang Dan-Dan Lin Jian-Bing Liu Guang-Hui Ren Kun Yang Yang Liu Yi Dong Shi-Qing Zhang Xiao-Nong Zhou | 2020 | Infectious Diseases of Poverty2020,9,3: | 20 |
| 10 | Fast magnetic reconstruction of the portal vein with allogeneic blood vessels in canines显示文摘BACKGROUND:The resection and reconstruction of large vessels,including the portal vein,are frequently needed in tumor resection.Warm ischemia before reconstruction might have deleterious effects on the function of some vital organs and therefore,how to reconstruct the vessels quickly after resection is extremely important.The present study was to introduce a new type of magnetic compression anastomosis(MCA)device to establish a quick non-suture anastomosis of the portal vein after resection in canines.METHODS:The new MCA device consists of a pair of titanium alloy and neodymium-ferrum-boron magnet(Ti-Nd Fe B)composite rings.The Nd Fe B magnetic ring as a core of the device was hermetically sealed inside the biomedical titanium alloy case.Twelve canines were divided into two groups:a MCA group in which the end-to-end anastomoses was made with a new device after resection in the portal vein and a traditional manual suture(TMS)group consisted of 6 canines.The anastomosis time,anastomotic patency and quality were investigated at week 24 postoperatively.RESULTS:The portal vein was reconstructed successfully in all of the animals and they all survived.The duration of portal vein anastomosis was significantly shorter in the MCA group than in the TMS group(8.16±1.25 vs 36.24±2.17 min,P<0.05).Portography and ultrasound showed that the blood flow was normal without angiostenosis or thrombosis in all of the canines.Hematoxylin-eosin staining and electron microscope scanning showed in contrast to the TMS group,MCA anastomotic intimal was much smoother with more regularly arranged endothelial cells at week 24 postoperatively. | Shan-Pei Wang Xiao-Peng Yan Fei Xue Ding-Hui Dong Xu-Feng Zhang Feng Ma Hao-Hua Wang Yi Lv | 2015 | Hepatobiliary & Pancreatic Diseases International2015,14,3: | 20 |
| 11 | Novel magnetic compression technique for establishment of a canine model of tracheoesophageal fistula显示文摘BACKGROUND Clinically,tracheoesophageal fistula(TEF)is lack of effective surgical strategies.One reason is due to the lack of appropriate animal models of acquired TEF,which is usually complex and difficult.Recently,the magnetic compression technique has been applied for digestive tract anastomosis or vascular anastomosis in animals.In this study,an animal model of TEF in dogs was developed by using the magnetic compression technique,hoping to provide a new method for mimicking TEF.AIM To establish a TEF model in dogs by using the magnetic compression technique.METHODS Six male beagles were used as models with two Nd-Fe-B permanent magnets for TEF.The parent magnet and the daughter magnet were placed in the cervical esophagus and trachea,respectively.The anterior wall of the esophagus and the posterior wall of the trachea were compressed when the two magnets coupled.After 4-6 d,the necrotic tissue between the two magnets fell off and the parent and daughter magnets disengaged from the target location,leaving a fistula.Gastroscopy/bronchoscopy,upper gastrointestinal contrast study,and histological analysis were performed.RESULTS The establishment of the TEF model in all six beagles was successful.The average time of magnet placement was 4.33±1.11 min(range,3-7 min).Mean time for the magnets to disengage from the target location was 4.67±0.75 d(range,4-6 d).TEFs were observed by gastroscopy/bronchoscopy and esophageal angiography.The gross anatomical structure of the esophagus and the trachea was in good condition.There was no esophageal mucosa or pseudostratified ciliated columnar epithelium at the site of the fistula according to histological analysis.CONCLUSION It is simple,feasible,and minimally invasive to use the magnetic compression technique for the establishment of the TEF model in dogs. | Yi Gao Rong-Qian Wu Yi Lv Xiao-Peng Yan | 2019 | World Journal of Gastroenterology2019,25,30: | 20 |
| 12 | Effect of ulinastatin combined rivaroxaban on deep vein thrombosis in major orthopedic surgery显示文摘Objective:To explore the effect of ulinastatin(UTI) continuous infusion combined Rivaroxaban on the deep vein thrombosis in patients undergoing major orthopedic surgery.Methods:Forty-five patients undergoing major orthopedic surgery were randomly divided into three groups:ulinastatin continuous infusion(Uc) group,ulinastatin single injection(Us) group and control(C) group.All patients received patient-controlled intravenous analgesia(PCIA) after operation,and took Rivaroxaban 10 mg orally 12 hours after operation.Ulinastatin(5 000 U/kg)was given intravenously to both Uc and Us groups preoperatively.Group C was given isometric normal saline,group Uc was pumped UTI continuous intravenously at the end of surgery(10 000U/kg) to 48 hours through PCIA pump.The values of hematocrit(HCT),thrombomodulin(TM),Interleukin(IL-6),thrombin-antithrombin complex(TAT),D-Dimer(D-D) were normally tested before surgery(T1),at the end of the surgery(T2),12 hours(T3).24 hours(T4) and 48 hours(T5)after surgery.Results:Compared with T1,there was an upward tendency in TM,IL-6,TAT,and D-D after operation in group C group(P <0.05).The values of them were significandy increased from nearly 24-hour after surgery in Us group(P<0.05).In group Uc.there were no significant changes in these indices after operation(P>0.05).Conclusions:During the perioperative period,ulinastatin continuous infusion combined Rivaroxaban can correct blood hypercoagulability through different approaches in patients undergoing major orthopedic surgery. | Xi Yu Yi Tian Ka Wang Ying-Lin Wang Guo-Yi Lv Guo-Gang Tian | 2014 | Asian Pacific Journal of Tropical Medicine2014,7,11: | 17 |

| 14 | Electroacupuncture activates enteric glial cells and protects the gut barrier in hemorrhaged rats显示文摘AIM:To investigate whether electroacupuncture ST36 activates enteric glial cells,and alleviates gut inflammation and barrier dysfunction following hemorrhagic shock.METHODS:Sprague-Dawley rats were subjected to approximately 45% total blood loss and randomly divided into seven groups:(1) sham:cannulation,but no hemorrhage;(2) subjected to hemorrhagic shock(HS);(3) electroacupuncture(EA) ST36 after hemorrhage;(4) vagotomy(VGX)/EA:VGX before hemorrhage,then EA ST36;(5) VGX:VGX before hemorrhage;(6) a-bungarotoxin(BGT)/EA:intraperitoneal injection of a-BGT before hemorrhage,then EA ST36; and(7) a-BGT group:a-BGT injection before hemorrhage.Morphological changes in enteric glial cells(EGCs) were observed by immunofluorescence,and glial fibrillary acidic protein(GFAP; a protein marker of enteric glial activation) was evaluated using reverse transcriptase polymerase chain reaction and western blot analysis.Intestinal cytokine levels,gut permeability to 4-k Da fluorescein isothiocyanate(FITC)-dextran,and the expression and distribution of tight junction protein zona occludens(ZO)-1 were also determined.RESULTS:EGCs were distorted following hemorrhage and showed morphological abnormalities.EA ST36 attenuated the morphological changes in EGCs at 6 h,as compared with the VGX,a-BGT and HS groups.EA ST36 increased GFAP expression to a greater degree than in the other groups.EA ST36 decreased intestinal permeability to FITC-dextran(760.5 ± 96.43 ng/m L vs 2466.7 ± 131.60 ng/m L,P < 0.05) and preserved ZO-1 protein expression and localization at 6 h afterhemorrhage compared with the HS group.However,abdominal VGX and a-BGT treatment weakened or eliminated the effects of EA ST36.EA ST36 reduced tumor necrosis factor-a levels in intestinal homogenates after blood loss,while vagotomy or intraperitoneal injection of a-BGT before EA ST36 abolished its antiinflammatory effects.CONCLUSION:EA ST36 attenuates hemorrhageinduced intestinal inflammatory insult,and protects the intestinal barrier integrity,partly via activation of EGCs. | Sen Hu Zeng-Kai Zhao Rui Liu Hai-Bin Wang Chun-Yu Gu Hong-Min Luo Huan Wang Ming-Hua Du Yi Lv Xian Shi | 2015 | World Journal of Gastroenterology2015,21,5: | 15 |
| 15 | Overexpression of microRNA-124 promotes the neuronal differentiation of bone marrow-derived mesenchymal stem cells显示文摘microRNAs(miRNAs) play an important regulatory role in the self-renewal and differentiation of stem cells. In this study, we examined the effects of miRNA-124(miR-124) overexpression in bone marrow-derived mesenchymal stem cells. In particular, we focused on the effect of overexpression on the differentiation of bone marrow-derived mesenchymal stem cells into neurons. First, we used GeneChip technology to analyze the expression of miRNAs in bone marrow-derived mesenchymal stem cells, neural stem cells and neurons. miR-124 expression was substantially reduced in bone marrow-derived mesenchymal stem cells compared with the other cell types. We constructed a lentiviral vector overexpressing miR-124 and transfected it into bone marrow-derived mesenchymal stem cells. Intracellular expression levels of the neuronal early markers β-III tubulin and microtubule-associated protein-2 were significantly increased, and apoptosis induced by oxygen and glucose deprivation was reduced in transfected cells. After miR-124-transfected bone marrow-derived mesenchymal stem cells were transplanted into the injured rat spinal cord, a large number of cells positive for the neuronal marker neurofilament-200 were observed in the transplanted region. The Basso-Beattie-Bresnahan locomotion scores showed that the motor function of the hind limb of rats with spinal cord injury was substantially improved. These results suggest that miR-124 plays an important role in the differentiation of bone marrow-derived mesenchymal stem cells into neurons. | 17 | Choledochojejunostomy with an innovative magneticcompressive anastomosis:How to determine optimalpressure?显示文摘AIM: To investigate the optimal magnetic pressure and provide a theoretical basis for choledochojejunostomy magnetic compressive anastomosis(magnamosis).METHODS: Four groups of neodymium-iron-boron magnets with different magnetic pressures of 0.1, 0.2, 0.3 and 0.4 MPa were used to complete the choledochojejunostomy magnamosis. Twenty-six young mongrel dogs were randomly divided into five groups: four groups with different magnetic pressures and 1 group with a hand-suture anastomosis. Serum bilirubin levels were measured in all groups before and 1 wk, 2 wk, 3 wk, 1 mo and 3 mo after surgery. Daily abdominal X-ray fluoroscopy was carried out postoperatively to detect the path and the excretion of the magnet. The animals were euthanized at 1 or 3 mo after the operation, the burst pressure was detected in each anastomosis, and the gross appearance and histology were compared according to the observation.RESULTS: The surgical procedures were all successfully performed in animals. However, animals of group D(magnetic pressure of 0.4 MPa) all experienced complications with bile leakage(4/4), whereas half of animals in group A(magnetic pressure of 0.1 MPa) experienced complications(3/6), 1 animal in the manual group E developed anastomotic stenosis, and animals in group B and group C(magnetic pressure of 0.2 MPa and 0.3 MPa, respectively) all healed well without complications. These results also suggested that the time required to form the stoma was inversely proportional to the magnetic pressure; however, the burst pressure of group A was smaller than those of the other groups at 1 mo(187.5 ± 17.7 vs 290 ± 10/296.7 ± 5.7/287.5 ± 3.5, P < 0.05); the remaining groups did not differ significantly. | 20 | Irisin attenuates intestinal injury,oxidative and endoplasmic reticulum stress in mice with L-arginine-induced acute pancreatitis显示文摘BACKGROUND Acute pancreatitis(AP)is often associated with intestinal injury,which in turn exaggerates the progression of AP.Our recent study has shown that a low level of serum irisin,a novel exercise-induced hormone,is associated with poor outcomes in patients with AP and irisin administration protects against experimental AP.However,the role of irisin in intestinal injury in AP has not been evaluated.AIM To investigate the effect of irisin administration on intestinal injury in experimental AP.METHODS AP was induced in male adult mice by two hourly intraperitoneal injections of Larginine.At 2 h after the last injection of L-arginine,irisin(50 or 250μg/kg body weight)or 1 mL normal saline(vehicle)was administered through intraperitoneal injection.The animals were sacrificed at 72 h after the induction of AP.Intestinal injury,apoptosis,oxidative and endoplasmic reticulum(ER)stress were evaluated.RESULTS Administration of irisin significantly mitigated intestinal damage,reduced apoptosis,and attenuated oxidative and ER stress in AP mice.In addition,irisin treatment also effectively downregulated serum tumor necrosis factor-alpha and interleukin-6 levels and alleviated injury in the pancreas,liver and lung of AP mice.CONCLUSION Irisin-mediated multiple physiological events attenuate intestinal injury following an episode of AP.Irisin has a great potential to be further developed as an effective treatment for patients with AP. | Yi-Fan Ren Meng-Zhou Wang Jian-Bin Bi Jia Zhang Lin Zhang Wu-Ming Liu Sha-Sha Wei Yi Lv Zheng Wu Rong-Qian Wu | 2019 | World Journal of Gastroenterology2019,25,45: | 12 |
