| 1 | Clinical usefulness of transpapillary removal of common bile duct stones by frequency doubled double pulse Nd:YAG laser显示文摘AIM: To study the efficacy and the safety of laser lithotripsy without direct visual control by using a balloon catheter in patients with bile duct stones that could not be extracted by standard technique. METHODS: The seventeen patients (7 male and 10 female; mean age 67.8 years) with difficult common bile duct (CBD) stones were not amenable for conventional endoscopic maneuvers such as sphincterotomy and mechanical lithotripsy were included in this study. Laser wavelengths of 532 nm and 1064 nm as a double pulse were applied with pulse energy of 120 mJ. The laser fiber was advanced under fluoroscopic control through the ERCP balloon catheter. Laser lithotripsy was continued until the fragment size seemed to be less than 10 mm. Endoscopic extraction of the stones and fragments was performed with the use of the Dormia basket and balloon catheter. RESULTS: Bile duct clearance was achieved in 15 of 17 patients (88%). The mean number of treatment sessions was 1.7 ± 0.6. Endoscopic stone removal could not be achieved in 2 patients (7%). Adverse effects were noted in three patients (hemobilia, pancreatitis, and cholangitis). In case 1,a 12-year-old boy had his necrotic,immature mandibular left second premolar treated with a revascularization technique. At a 24-month follow-up,periapical radiolucency had disappeared and thickening of the root wall was observed. In cases 2 and 3,a 10-year-old boy had his necrotic,immature,bilateral mandibular second premolars treated with the same modality. | 3 | Prediction of liver cirrhosis, using diagnostic imaging tools显示文摘Early diagnosis of liver cirrhosis is important. Ultrasoundguided liver biopsy is the gold standard for diagnosis of liver cirrhosis. However, its invasiveness and sampling bias limit the applicability of the method. Basic imaging for the diagnosis of liver cirrhosis has developed over the last few decades, enabling early detection of morphological changes of the liver by ultrasonography(US), computed tomography, and magnetic resonance imaging(MRI). They are also accurate diagnostic methods for advanced liver cirrhosis, for which early diagnosis is difficult. There are a number of ways to compensate for this difficulty, including texture analysis to more closely identify the homogeneity of hepatic parenchyma, elastography to measure the stiffness and elasticity of the liver, and perfusion studies to determine the blood flow volume, transit time, and velocity. Amongst these methods, elastography using US and MRI was found to be slightly easier, faster, and able to provide an accurate diagnosis. | 4 | Modeling hot deformation behavior of low-cost Ti-2Al-9.2Mo-2Fe beta titanium alloy using a deep neural network显示文摘Ti-2 Al-9.2 Mo-2 Fe is a low-cost β titanium alloy with well-balanced strength and ductility, but hot working of this alloy is complex and unfamiliar. Understanding the nonlinear relationships among the strain,strain rate, temperature, and flow stress of this alloy is essential to optimize the hot working process.In this study, a deep neural network(DNN) model was developed to correlate flow stress with a wide range of strains(0.025–0.6), strain rates(0.01–10 s^-1) and temperatures(750–1000℃). The model, which was tested with 96 unseen datasets, showed better performance than existing models, with a correlation coefficient of 0.999. The processing map constructed using the DNN model was effective in predicting the microstructural evolution of the alloy. | 6 | Biliary reflux detection in anomalous union of the pancreaticobiliary duct patients显示文摘AIM:To demonstrate the imaging findings of biliopancreatic and pancreatico-biliary reflux in patients with anomalous union of the pancreatico-biliary duct (AUPBD) on gadoxetic acid-enhanced functional magnetic resonance cholangiography (fMRC).METHODS:This study included six consecutive patients (two men and four women;mean age 47.5 years) with AUPBD.All subjects underwent endoscopic retrograde cholangiopancreatography (ERCP);one subject also underwent bile sampling of the common bile duct (CBD) to measure the amylase level because his gadoxetic acidenhanced fMRC images showed evidence of pancreatico-biliary reflux of pancreatic secretions.Of the five patients with choledochal cysts,four underwent pyloruspreserving pancreaticoduodenectomy.RESULTS:The five cases of choledochal cysts were classified as Todani classification Ⅰ.In three of the six patients with AUPBD,injected contrast media reached the distal CBD and pancreatic duct on delay images,suggesting biliopancreatic reflux.In two of these six patients,a band-like filling defect was noted in the CBD on pre-fatty meal images,which decreased in size on delayed post-fatty meal images,suggesting pancreatico-biliary reflux of pancreatic secretions,and the bile sampled from the CBD in one patient had an amylase level of 113 000 IU/L.In one of the six patients with AUPBD,contrast media did not reach the distal CBD due to multiple CBD stones.CONCLUSION:Gadoxetic acid-enhanced fMRC successfully demonstrated biliopancreatic reflux of bile and pancreatico-biliary reflux of pancreatic secretions in patients with AUPBD with and without choledochal cysts. | Suk Keu Yeom Seung Wha Lee Sang Hoon Cha Hwan Hoon Chung Bo Kyung Je Baek Hyun Kim Jong Jin Hyun | 2012 | World Journal of Gastroenterology2012，18，9： | 5 |

| 8 | Analysis on sports and recreation activity-related eye injuries presenting to the Emergency Department显示文摘AIM:To investigate the incidence and general characteristics of sports-related eye injuries in patients visiting the Emergency Department. METHODS: A cross-sectional,multi-center,observational study.Patients with an injured eye who visited the Emergency Department at one of nine hospitals in Korea were enrolled.All data were prospectively collected between March and September 2010 using a questionnaire.Eye injuries that occurred during risky sports were examined by gender and age.Additionally,the rate of open globe injuries that occurred with and without protective eyewear was examined for each activity.Continuous variables were compared using Student's t-test and categorical variables were compared using Chi-square test.RESULTS: A total of 446 patients had sports-related eye injuries.Teenagers(10-19 years old) and young adults(20-29 years old) had the most eye injuries.Eye injuries accounted for 0.2% of Emergency Department patients.Baseball was the most common cause of sports-related eye injuries,followed by soccer and hiking.Protective gear was worn by 9.4% of all patients.Patients that were 30-39 years of age had the highest rate of protective gear use,followed by patients that were 40-49 years of age.The proportion of sports-related eye injuries that were open-globe injuries was highest for soccer and hiking.CONCLUSION: Although injuries were most common in patients below the age of 10 years,these patients had the lowest rate of protective eyewear use.Injuries in adults over 40 years of age most commonly occurred during hiking,but the rate of protective eyewear use was low.Young athletes should be educated on and provided with protective eyewear and policies protective gear use should be established.For older adults,eye protection should be encouraged,especially during hiking. | Sungbae Moon Hyun Wook Ryoo Jae Yun Ahn Jung Bae Park Kang Suk Seo Sang Do Shin Kyoung Jun Song Kang Hyun Lee In Sool Yoo Jin Seong Cho Hyun Ho Ryu Tae Oh Jeong Seok Ran Yeom Young Taek Kim Sung Ok Hong | 2016 | International Journal of Ophthalmology(English edition)2016，9，10： | 3 |

The integration of technology into society also results in an increasing desire to control who and what has access to sensitive information, especially for vulnerable people including children and the elderly. With blockchain rise as a technology that can revolutionize the world, it is now possible to have an immutable audit trail of locational data over time. By controlling the process through inexpensive equipment in the home, it is possible to control whom has access to such personal data. This paper presents a block-chain based family security system for outdoor tracking and in-house monitoring of users’ activities via sensors to detect anomalies in users’ daily activities with the integration of Artificial Intelligence (AI). For outdoor tracking the locations of the consenting family members’ smart phones are logged and stored in a private blockchain which can be accessed through a node installed in the family home on a computer. The data for the whereabouts and daily activities of family members stays securely within the family unit and does not go to any third-party organizations. A Self-Organizing Maps (SOM) based smart contract is used for anomaly detection in users’ daily activities in a smart home, which notifies emergency contact or other family members in case of anomaly detection. The approach described in this paper contributes to the development of in-house data processing for outdoor tracking, and daily activities monitoring and prediction without any third-party hardware or software. The system is implemented at a small scale with one miner, two user nodes and several device nodes, as a proof of concept;the technical feasibility is discussed along with the limitations of the system.
