| 1 | Interactive effect of dissolved organic matter and phenanthrene on soil enzymatic activities显示文摘The investigation of dissolved organic matter(DOM) on urease,catalase and polyphenol oxidase activity in a phenanthrene(Phe)contaminated soil was conducted under laboratory incubation conditions.Values of soil enzymatic activity depended mainly on incubation time.In the initial 16 days,urease activity increased,and was followed by a decrease.In the initial 8 days,catalase activity decreased and then increased.Variation of polyphenol oxidase activity was just the reverse of catalase activity.After 30 days of incubation,no pronounced difference among treatments with Phe,Phe and DOM,and control were detected in urease,catalase and polyphenol oxidase activity.Phe might inhibit urease and catalase,and stimulate polyphenol oxidase.DOM could improve inhibition of Phe in soil urease and catalase activity during the initial period of applying DOM.Nevertheless,DOM had no significant effect on polyphenol oxidase activity in the Phe contaminated soil.There was a negative correlation between catalase and polyphenol oxidase(r =-0.761***),and catalase and urease(r =-0.554**).Additionally,a positive correlation between polyphenol oxidase and urease was also detected(r = 0.701***).It is implied that the formed DOM after application of organic wastes into soils may counteract the inhibition of polycyclic aromatic hydrocarbons in soil enzyme activities. | Xinhua Zhan,Wenzhu Wu,Lixiang Zhou,Jianru Liang,Tinghui Jiang College of Resources and Environmental Sciences,Nanjing Agricultural University,Nanjing 210095,China.Greenstar Plant Products Inc.,9430 198 St,Langley,BC,Canada V1M3C8. | 2010 | Journal of Environmental Sciences2010,22,4: | 10 |
| 2 | Lean techniques for the improvement of patients' flow in emergency department显示文摘BACKGROUND: Emergency departments(EDs) face problems with overcrowding, access block, cost containment, and increasing demand from patients. In order to resolve these problems, there is rising interest to an approach called 'lean' management. This study aims to(1) evaluate the current patient flow in ED,(2) to identify and eliminate the non-valued added process, and(3) to modify the existing process.METHODS: It was a quantitative, pre- and post-lean design study with a series of lean management work implemented to improve the admission and blood result waiting time. These included structured re-design process, priority admission triage(PAT) program, enhanced communication with medical department, and use of new high sensitivity troponin-T(hsTnT) blood test. Triage waiting time, consultation waiting time, blood result time, admission waiting time, total processing time and ED length of stay were compared.RESULTS: Among all the processes carried out in ED, the most time consuming processes were to wait for an admission bed(38.24 minutes; SD 66.35) and blood testing result(mean 52.73 minutes, SD 24.03). The triage waiting time and end waiting time for consultation were significantly decreased. The admission waiting time of emergency medical ward(EMW) was significantly decreased from 54.76 minutes to 24.45 minutes after implementation of PAT program(P<0.05).CONCLUSION: The application of lean management can improve the patient flow in ED. | 3 | Effects of Delayed Second Cochlear Implant显示文摘Objective Since Helms' successful bilateral cochlear implantation with good results in 1996, there have been increasing number of reports on bilateral cochlear implantation. Most second device have been implantated within one year after the first. Considering effects of long time auditory deprivation, it is not clear whether a delayed second cochlear implant serves to add additional benefits and how it may interact with central nervous system plasticity. Methods Three cases who received delayed second cochlear implants at People's Hospital of Peking University from 2002 to 2005 were reviewed. The interval between the first and second implants was longer than 2 years in all three patients. Sound perception, and unilateral/bilateral speech discrimination in quiet and noise were evaluated. In addition, GAP detection test was conducted in one patient. Results In one case, having both implants on provided improved performance compared to using only one implant both in quiet and noise. Presumably due to visual interference from lip-reading or short interval between second implant and testing, one patient showed no improvement from using the second implant either in quiet or noise, while the last case demonstrated additional benefits from the second implant only in quiet. In all three patients, performance in recognizing the four tones in Mandarin was superior over word recognition. Conclusions Considerable plasticity in the cerebral auditory center is preserved, despite long acoustic deprivation in some children who have received unilateral cochlear implant. Delayed second implants can result in significant improvements in some of these children. Visual interference from lip-reading may be an obstacle during retraining. | 5 | Cilostazol for secondary stroke prevention:systematic review and meta-analysis显示文摘Background Stroke is one of the leading causes of death worldwide.Cilostazol,an antiplatelet and phosphodiesterase 3 inhibitor,has not been clearly established for ischaemic stroke use.We aim to determine the efficacy and safety of cilostazol for secondary stroke prevention.Methods MEDLINE,EMBASE,Cochrane Library,Web of Science and ClinicalTrials.gov were searched from inception to 25 September 2020,for randomised trials comparing the efficacy and safety of cilostazol monotherapy or dual therapy with another antiplatelet regimen or placebo,in patients with ischaemic stroke.Version 2 of the Cochrane risk-of-bias tool for randomised trials(RoB 2)was used to assess study quality.This meta-analysis was reported in line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)statement.Results Eighteen randomised trials comprising 11429 participants were included in this meta-analysis.Most trials possessed low risk of bias and were of low heterogeneity.Cilostazol significantly reduced the rate of ischaemic stroke recurrence(risk ratio,RR=0.69,95%CI 0.58 to 0.81),any stroke recurrence(RR=0.64,95%CI 0.54 to 0.74)and major adverse cardiovascular events(RR=0.67,95%CI 0.56 to 0.81).Cilostazol did not significantly decrease mortality(RR=0.90,95%CI 0.64 to 1.25)or increase the rate of good functional outcome(Modified Rankin Scale score of 0–1;RR=1.07,95%CI 0.95 to 1.19).Cilostazol demonstrated favourable safety profile,significantly reducing the risk of intracranial haemorrhage(RR=0.46,95%CI 0.31 to 0.68)and major haemorrhagic events(RR=0.49,95%CI 0.34 to 0.70).Conclusions Cilostazol demonstrated superior efficacy and safety profiles compared with traditional antiplatelet regimens such as aspirin and clopidogrel for secondary stroke prevention but does not appear to affect functional outcomes.Future randomised trials can be conducted outside East Asia,or compare cilostazol with a wider range of antiplatelet agents. | Choon Han Tan Andrew GR Wu Ching-Hui Sia Aloysius ST Leow Bernard PL Chan Vijay Kumar Sharma Leonard LL Yeo Benjamin YQ Tan | 2021 | Stroke & Vascular Neurology2021,6,3: | 2 |
