| 2 | Improving immunization capacity in Ethiopia through continuous quality improvement interventions:a prospective quasi-experimental study显示文摘Background:Strong scientific evidence is needed to support low-income countries in building effective and sustainable immunization programs and proactively engaging in global vaccine development and implementation initiatives. This study aimed to implement and evaluate the effectiveness of system-wide continuous quality improvement(CQI)interventions to improve national immunization programme performance in Ethiopia. Methods:The study used a prospective,quasi-experimental design with an interrupted time-series analysis to collect data from 781 government health sectors(556 healthcare facilities,196 district health offices, and 29 zonal health departments)selected from developing and emerging regions in Ethiopia.Procedures included baseline quality assessment of immunization programme and services using structured checklists;immunization systems strengthening using onsite technical support, training, and supportive supervision interventions in a Plan-Do-Check-Act cycle over 12 months;and collection and analysis of data at baseline and at the 6th and 12th month of interventions using statistical process control and the t-test.Outcome measures were the coverage of the vaccines pentavalent 3,measles,Bacillus Calmette-Guérin vaccine(BCG),Pneumococcal Conjugate Vaccine(PCV),as well as full vaccination status;while process measures were changes in human resources, planning, service delivery, logistics and supply, documentation, coordination and collaboration, and monitoring and evaluation.Analysis and interpretation of data adhered to SQUIRE 2.0 guidelines. Results:Prior to the interventions, vaccination coverage was low and all seven process indicators had an aggregate score of below 50%,with significant differences in performance at healthcare facility level between developing and emerging regions(P=0.0001).Following the interventions, vaccination coverage improved significantly from 63.6%at baseline to 79.3%for pentavalent(P=0.0001),62.5 to 72.8%for measles(P=0.009),62.4 to 73.5%for BCG(P=0.0001),65.3 to 81.0%for PCV(P=0.02),and insignificantly from 56.2 to 74.2%for full vaccination. All seven process indicators scored above 75%in all regions, with no significant differences found in performance between developing and emerging regions. Conclusions:The CQI interventions improved immunization capacity and vaccination coverage in Ethiopia, where the unstable transmission patterns and intensity of infectious diseases necessitate for a state of readiness of the health system at all times. The approach was found to empower zone, district, and facility-level health sectors to exercise accountability and share ownership of immunization outcomes. While universal approaches can improve routine immunization, local innovative interventions that target local problems and dynamics are also necessary to achieve optimal coverage. | Tsegahun Manyazewal Alemayehu Mekonnen Tesfa Demelew Semegnew Mengestu Yusuf Abdu Dereje Mammo Workeabeba Abebe Belay Haffa Daniel Zenebe Bogale Worku Amir Aman Setegn Tigabu | 2018 | Infectious Diseases of Poverty2018,7,1: | 1 |

| 6 | Community knowledge, attitude and practice towards cutaneous leishmaniasis endemic area Ochello,Gamo Gofa Zone,South Ethiopia显示文摘Objective: To assess the knowledge, attitude and practice of the community related to cutaneous leishmaniasis(CL) in an endemic area Ochello, Gamo Gofa Zone, South Ethiopia. Methods: We conducted community based cross-sectional survey among residents in Ochello from November to December 2014. The study area was purposely selected based on previous reports on endemicity of CL. Using simple random sampling technique, a total of 392 household participants were selected in the study area Ochello. Structured questionnaire was used to collect the data. Regarding the responses given to knowledge, attitude and practice, a score of 1 was given for each right response and 0 for unsure responses. Data were double entered and analysis was conducted using SPSS version 20 statistical software.Descriptive statistics that include frequency and percentage were used to analyze the results. Results: In total, 392 individuals were participated in our study where 225(57.4%) of the participants were males and 167(42.6%) were females. Of all the total participants, 265(67.6%) had heard of the disease, and 127(32.4%) responded that they did not know CL.Based on the scoring results, 265(67.6%) participants were knowledgeable about CL. Out of 265 participants who heard about CL, most of them [215(54.8%)] had the attitude that CL was a problem in their area and had no positive attitude towards the treatment of CL.Approximately, 215(54.8%) replied that CL was preventable. Majority of the respondents did not sleep outdoors and did not practice sleeping near vegetation with or without bed net. Conclusions: The current finding indicated that the inhabitants of Ochello developed good awareness and encouraging attitude regarding CL. However, their prevention and control practice was very low. Hence, the result of this study calls for organized community awareness creation through various means. | Nigatu Kebede Alemayehu Worku Ahmed Ali Abebe Animut Yohannes Negash Wondwossen Abebe Gebreyes Abhay Satoskar | 2016 | Asian Pacific Journal of Tropical Biomedicine2016,6,7: | 0 |
| 7 | The burden of and risk factors for active trachoma in the North and South Wollo Zones of Amhara Region,Ethiopia:a crosssectional study显示文摘Background:Trachoma is a disease of the eye, caused by the bacteria Chlamydia trachomatis, which can lead to blindness if left untreated.Ethiopia is one of the most trachoma-affected countries in the world. The objective of this study was to determine the prevalence of and associated risk factors for active trachoma among children in selected woredas of North and South Wollo Zones in Amhara Region,Ethiopia. Methods:This study was a community-based,cross-sectional study, which was conducted from October to December 2014 among children aged 1-8.A four-stage random cluster sampling technique was employed to select the study areas and participants. From each selected household, one child was clinically assessed for active trachoma.A structured questionnaire was used to collect sociodemographic, behavioral, and clinical data.Multivariate logistic regression analysis was used to analyze the association between predictor variables and active trachoma. Results:The overall prevalence of active trachoma among 1358 children was found to be 21.6%(95%CI:19.4-23.8%).When analyzed by the presence or absence of individual WHO simplified system signs of active trachoma, trachomatous inflammation-follicular cases constituted18%(95%CI:15.9-20.2%),while 4.7%(95%CI:3.6-5.8%)were trachomatous inflammation-intense cases.Ocular discharge(aOR=5.2;95%CI:3.3-8.2),nasal discharge(aOR=1.8;95%CI:1.2-2.7),time taken to fetch water(aOR=0.02;95%CI:0.01-0.05),frequency of hand and face washing(aOR=4.4;95%CI:1.1-17.8),and access to a latrine(aOR=0.006;95%CI:0.001-0.030)were found to be independently associated with the presence of active trachoma. Conclusions:There is a high burden of active trachoma among children in the study areas.Lack of personal hygiene and limited access to a safe water supply and latrines were associated with increased prevalence of active trachoma. In order to reduce the burden of active trachoma, facial cleanliness and environmental improvement components of the SAFE strategy should be upgraded in the study areas. | Beselam Tadesse Alemayehu Worku Abera Kumie Solomon Abebe Yimer | 2017 | Infectious Diseases of Poverty2017,6,1: | 0 |
