| 1 | 瑞舒伐他汀治疗中国高胆固醇血症患者疗效和安全性的随机双盲多中心对照研究显示文摘目的评价瑞舒伐他汀治疗中国高胆固醇血症患者的疗效和安全性。方法采用随机、双盲、多中心研究。患者经6周筛选后符合 LDL-C≥4.14 mmol/L(160 mg/dl),<6.50 mmol/L(250 mg/dl)、TG<4.52 mmol/L(400 mg/dl)者以2:1随机接受瑞舒伐他汀10 mg/d 或阿托伐他汀10mg/d 治疗。12周后瑞舒伐他汀组 LDL-C 未达到 ATPⅢ治疗目标者,予瑞舒伐他汀20 mg 延续治疗8周。结果 304例进入随机治疗阶段,瑞舒伐他汀10 mg/d 组201例,阿托伐他汀10 mg/d 组103例。意向治疗人群290例,符合方案人群263例。瑞舒伐他汀组治疗12周后血 LDL-C 显著下降,下降幅度为45.6%,显著大于阿托伐他汀组的39.0%(P<0.001)。瑞舒伐他汀组患者 LDL-C 达标率也较阿托伐他汀组(78.0%比72.7%)有增高趋势,且在高危人群中优势更为明显(56.5%比35.0%),但差异未达到统计学意义。瑞舒伐他汀降低 TG(-22.8%)以及升高 HDL-C(+6.6%)和ApoA-1(+12.5%)的幅度与阿托伐他汀组差别无统计学意义(分别为-16.6%,+4.3%和+9.8%)。29例患者接受20 mg/d 瑞舒伐他汀延续治疗,22例完成治疗患者中10例(45.5%)LDL-C达标。研究中未发现药物相关的严重不良反应事件。结论本组研究显示瑞舒伐他汀10 mg 降低LDL-C 的疗效优于同等剂量的阿托伐他汀,治疗3个月安全性与之类似。 | Rosuvastatin Registration Clinical Trial Group.Cardiovascular Institute and Fu Wai Hospital,Peking Union Medical College and Chinese Academy of Medical Science,Beijing 100037,China | 2007 | 中华心血管病杂志2007,35,3: | 117 |

| 3 | Hyaluronic acid fragments evoke Kupffer cells via TLR4 signaling pathway显示文摘Kupffer cells, expressing toll-like receptor 4 (TLR4), play a central role in hepatic ischemia/reperfusion (I/R) injury. Hyaluronic acid (HA) fragments, degradative products of high-molecular-weight HA (HMW-HA), acquire the ability to activate immune cells under inflammatory conditions. Here we inves- tigated whether HA fragments could activate Kupffer cells and analyzed the underlying mechanism. Kupffer cells were isolated from wild-type mice (WT, C3H/HeN) and TLR4 mutant mice (C3H/HeJ) and HA fragments were produced by the methods of enzyme digestion and chromatography. Then Kupffer cells were stimulated by HA fragments or other control stimuli. The activation of Kupffer cells was estimated as the release of pro-inflammatory cytokines. The activation of p38 MAPK pathway of Kupffer cells was checked and blocking experiments were done as well. The results indicated that HA fragments acquired the ability to activate Kupffer cells in vitro, which was TLR4 dependent and not due to contamination of lipopolysaccharide. Experiments of p38 MAPK kinase inhibition by SB-203580 verified p38 MAPK was required in HA fragments induced Kupffer cells activation. | 5 | The abnormal expression of retinoic acid receptor-β,p53 and Ki67 protein,n normal,premalignant and malignant esophageal tissues显示文摘AIM: Esophageal cancer remains a significant healthiproblem worldwide. It is important to investigate alterationsin expression of retinoic acid receptor-β, p53 and Ki67proteins in esophageal carcinogenesis.METHODS: To find biomarkers for early identification ofesophageal cancer, we analyzed the retinoic acid receptor-j3,p53 protein and the proliferation marker Ki67 in surgicalspecimens of normal, mildly, and severely dysplastic andmalignant esophageal tissues by in situ hybridization ofRNA and immunohistochemistry.RESULTS: RAR-β was expressed in 94.3 %(33/35) of normalmucosae, 67.8 %(19/28) of the mild, 58.1% (18/31) of thesevere ieaions and 53.2 % (116/218) of tumor samples. RAR-β mRNA was expressed in 62.7 % (42/67), 55.1% (43/78) and29.2 % (7/24) of well, moderated and poorly differentiatedSSCs. The p53 and Ki67 proteins were 5.9 % (2/34) of thenormal mucosa. P53 and Ki67 stained positively in 10.7 % (3/28) and 21.4 % (6/28) of mild dysplasia, and 51.6 %(16/31)and 58. 1% (18/31) of severely dysplasia respectively.Samples from esophageal cancer showed no higher levers ofp53 and Ki67 expression than seen in severely dysplasticlesions. Methods Serum AGE levels were determined by AGE-ELISA, MTT method was used to determine lymphocyte proliferation, IL-2 activity was determined by a bioassay method. Spontaneous motor activity was used to detect mouse's neuromuscular movement, latency of step-through method was used to examine learning and memory abilities of mouse, colormetric assay was used to determine hydroxyproline concentration in mouse skin, pyrogallol autoxidation method was used to determine superoxide dismutase (SOD) activity of erythrocytes. Results Decreased levels of serum AGE, hydroxyproline concentration in mouse skin and spontaneous motor activity in D-galactose mouse aging model were detected after treated with ABP or LBP, while lymphocyte proliferation and IL-2 activity, learning and memory abilities,SOD activity of erythrocytes, were enhanced. | 7 | Analysis of gene expression profile induced by EMP-1 in esophageal cancer cells using cDNA Microarray显示文摘AIM: To obtain human esophageal cancer cell EC9706 stably expressed epithelial membrane protein-1 (EMP-1) with integrated eukaryotic plasmid harboring the open reading frame (ORF) of human EMP-1, and then to study the mechanism by which EMP-1 exerts its diverse cellular action on cell proliferation and altered gene profile by exploring the effect of EMP-1.METHODS: The authors first constructed pcDNA3.1/mychis expression vector harboring the ORF of EMP-1 and then transfected it into human esophageal carcinoma cell line EC9706. The positive clones were analyzed by Western blot and RT-PCR. Moreover, the cell growth curve was observed and the cell cycle was checked by FACS technique. Using cDNA microarray technology, the authors compared the gene expression pattern in positive clones with control. To confirm the gene expression profile, semi-quantitative RT-PCR was carried out for 4 of the randomly picked differentially expressed genes. For those differentially expressed genes,classification was performed according to their function and cellular component.RESULTS: Human EMP-1 gene can be stably expressed in ECg706 cell line transfected with human EMP-1. The authors found the cell growth decreased, among which S phase was arrested and G1 phase was prolonged in the transfected positive clones. By cDNA microarray analysis, 35 genes showed an over 2.0 fold change in expression level after transfection, with 28 genes being consistently up-regulated and 7 genes being down-regulated. Among the classified genes, almost half of the induced genes (13 out of 28 genes) were related to cell signaling, cell communication and particularly to adhesion.CONCLUSION: Overexpression of human EMP-1 gene can inhibit the proliferation of EC9706 cell with S phase arrested and G1 phase prolonged. | 8 | Effects of transmitters and interleukin-10 on rat hepatic fibrosis induced by CCl_4显示文摘AIM: To study the effects of transmitters ET, AgII, PGI2,CGRP and GG on experimental rat hepatic fibrosis and the antifibrogenic effects of IL-10.METHODS: One hundred SD rats were randomly divided into 3 groups: control group (N): intraperitoneal injection a week; IL-10 treated group (E): besides same dosage of the third week. In the fifth, the seventh and the ninth week,rats in three groups were selected randomly to collect plasma and liver tissues. The levels of ET, AgII, PGI2, CGRP and GG were assayed by radioimmunoassay (RIA). The liver fibrosis was observed with silver staining.RESULTS: The hepatic fibrosis was developed with the increase of the injection frequency of CCl4. The ET, AgII, PGI2, CGRP The histological examination showed that the degrees of the rats liver fibrosis in group E were lower than those in group C.CONCLUSION: The transmitters ET, AgII, PGI2, CGRP and GG play a significant role in the rat hepatic fibrosis induced by CCl4. | 10 | Intergrin gene expression profiles of human hepatocellular carcinoma显示文摘AIM: To investigate gene expression profiles of intergringenes in hepatocellular carcinoma (HCC) through theusage of Atlas Human Cancer Array membranes, semi-quantitative reverse transcription polymerase chainreaction (RT-PCR) and Northern blot.METHODS: Hybridization of cDNA array membrane wasperformed with α 32P-labeled cDNA probes synthesizedfrom RNA isolated from hepatocellular carcinoma andadjacent non-cirrhotic liver. AtlasImage, which is asoftware specific to array, was used to analyze theresult. RT-PCR of 24 pairs specimen and Northern blotof 4 pairs specimen were used to confirm the expressionpattern of some intergrin genes identified by Atlasarrays hybridization.RESULTS: Among 588 genes spotted in membrane, 17genes were related to intergrin. Four genes were up-regulated, such as intergrin alpha8, beta1, beta7 andbeta8 in HCC. Whereas there were no genes down-regulated in HCC. RT-PCR and Northern blot analysisof intergrin beta1 gene gave results consistent withcDNA array findings.CONCLUSION: Investigation of these intergrin genesshould help to disclose the molecular mechanism of thecell adhesion, invasive and metastasis of HCC. A fewgenes are reported to have changed in HCC for the firsttime. The quick and high-throughout method of profilinggene expression by cDNA array provides us overviewof key factors that may involved in HCC, and may findthe clue of the study of HCC metastasis and moleculartargets of anti-metastasis therapy. | 12 | Expression of insulin-like growth factor 1 and insulin-like growth factor 1 receptor and its intervention by interleukin-10 in experimental hepatic fibrosis显示文摘AIM: To study the expression of IGF-1 and IGF-1R and its intervention by interleuldn-10 in the course of experimental hepatic fibrosis.METHODS: Hepatic fibrosis was induced in rats by carbon tetrachloride intoxication and liver specimens were taken from the rats administered CCl4 with or without IL-10 treatment and the animals of the control group.Immunoreactivities for insulin-like growth factor-1 (IGF-1)and IGF-1 receptor(IGF-1R) were demonstrated by immunohistochemistry, and their intensities were evaluated in different animal groups.RESULTS: The positive levels for IGF-1 and IGF-1R were increased with the development of hepatic fibrosis, with the positive signals localized in cytoplasm and/or at the plasmic membrane of hepatocytes. The positive signals of IGF-1 and IGF-1R were observed more frequently (P<0.01) in the CCl4-treated group (92.0 % and 90.0 %) compared to those in the control group. The positive signals decreased significantly (P<0.05) in IL-10-treated group. | 15 | Expression and signifi cance of TLR4 and HIF-1α in pancreatic ductal adenocarcinoma显示文摘AIM:To investigate the expression of toll-like receptor(TLR) 4,nuclear factor-κB(NF-κB) p65 and hypoxiainducible transcription factor 1α(HIF-1α) in pancreatic ductal adenocarcinoma and their clinical significance.METHODS:The mRNA of TLR4 and HIF-1α were investigated by real-time polymerase chain reaction in 30 cases of pancreatic ductal adenocarcinoma and its adjacent tissues,and expression of TLR4,NF-κB p65 and HIF-1α protein were detected by immunohistochemistry in 65 cases of pancreatic ductal adenocarcinoma tissues and 38 cases of corresponding adjacent tissues.The relationship between TLR4 or HIF-1α and pathologic features,as well as the association between TLR4 and HIF-1α,were also analyzed.Kaplan-Meier method was used to assess the impact of expression of TLR4 and HIF-1α on survival of patients with pancreatic cancer.RESULTS:The relative quantif ication of TLR4 and HIF-1α mRNA in tumor tissues was 0.81±0.10 and 0.87±0.11,respectively,signif icantly higher than that in adjacent tissues(0.81±0.10 vs 0.70±0.16,P=0.002;0.87±0.11 vs 0.68±0.13,P=0.000).The protein expression of TLR4,NF-κB p65 and HIF-1α in tumor tissues was 69.20%,66.15% and 70.80%,respectively,being signif icantly higher than that in adjacent normal tissues(69.20% vs 39.50%,P=0.003;66.15% vs 31.58%,P=0.001;70.80% vs 36.80%,P=0.001).There was no signif icant correlation between TLR4 or HIF-1α expression and the age,gender,tumor location,the degree of tumor differentiation in the patients(P>0.05).However,there was signif icant correlation between the expression of TLR4 or HIF-1α and tumor size,lymph node metastasis,venous invasion and clinical staging(P<0.05).The expression of TLR4 and HIF-1α had a signif icant impact on survival of patients with pancreatic adenocarcinoma.CONCLUSION:TLR4,NF-κB p65 and HIF-1α are overexpressed in pancreatic adenocarcinoma,TLR4 may be partly involved in up-regulating HIF-1α,and both synergestically promote development of pancreatic adenocarcinoma. | Jian-Jun Zhang,He-Shui Wu,Lin Wang,Yuan Tian,Jing-Hui Zhang,Hai-Long Wu Department of Pancreatic Surgery,Union Hospital,Tongji Medical College,Huazhong University of Science and Technology,Wuhan 430022,Hubei Province,China Department of Pediatrics,Union Hospital,Tongji Medical College,Huazhong University of Science and Technology,Wuhan 430022,Hubei Province,China Laboratory of General Surgery,Union Hospital,Tongji Medical College,Huazhong University of Science and Technology,Wuhan 430022,Hubei Province,China | 2010 | World Journal of Gastroenterology2010,16,23: | 21 |
| 16 | An implantable rat liver tumor model for experimental transarterial chemoembolization therapy and its imaging features显示文摘AIM: To establish an ideal implantable rat liver tumor model for interventional therapy study and examine its angiographic signs and MRI, CT features before and after embolization. METHODS: Forty male Wistar rats were implanted with Walker256 tumor in the left lateral lobe of liver. Digital subtraction angiography (DSA) and transarterial chemoembolization were performed on day 14 after implantation. Native computer tomography (CT, n=8) and native magnetic resonance (MR,n=40) were performed between the day 8 and day 21 after implantation. The radiological morphological characteristics were correlated with histological findings.RESULTS: Successful implantation was achieved in all forty rats, which was confirmed by CT and MRI. MR allowed tumor visualization from day 8 while CT from day 11 after implantation. The tumors were hypodensity on CT, hypointense on MR T1-weighted and hyperintense on T2-weighted. The model closely resembled human hepatocardnoma in growth pattem and the lesions were rich in vasculature on angiography and got its filling mainly from the hepatic artery. Before therapy, tumor size was 211.9±48.7 mm3. No ascites, satellite liver nodules or lung metastasis were found. One week after therapy, tumor size was 963.6±214.8 mm3 in the control group and 356.5±78.4mm3 in TACE group. Ascites (4/40), satellite liver nodules (7/40) or lung metastasis (3/40) could be seen on day 21.CONCLUSION: Walker-256 tumor rat model is suitable for the interventional experiment. | 17 | Influence of transarterial chemoembolization on angiogenesis and expression of vascular endothelial growth factor and basic fibroblast growth factor in rat with Walker-256 transplanted hepatoma:An experimental study显示文摘AIM: After transarterial chemoembolization (TACE), the residual cancer cells are under extensive hypoxic or even anoxic environment. Hypoxia can lead to adaptive responses.For example, angiogenesis will help these cells survive. In this study, we examined the effect of TACE on angiogenesis and expression of vascular endothelial growth factor (VEGF)and basic fibroblast growth factor (b-FGF) and to assess their relevance to Walker-256 transplanted hepatoma.METHODS: Male Wistar rats were inoculated with Walker256 tumor in the left lobe of liver. Angiography and transarterial chemoembolization were performed at d14 after transplantation. Sixty rats bearing walker-256 transplanted hepatoma were randomly divided into control group, arterial infusion group and TACE group. Each group consisted of twenty rats. Normal saline, 5-Fu, 5-Fu and lipiodol were infused through hepatic artery respectively. Two weeks after the infusion, staining of factor Ⅷ, VEGF and b-FGF was performed by immunohistochemistry method in routine paraffin-embeded sections. Microvessel density(MVD) was counted in endothelial cells with positive factor Ⅷ. Their expression levels were analyzed in conjunction with the pathologic features.RESULTS: While a smaller tumor volume was found in TACE group (F=37.818, P<0.001), no statistical differences between MVD and expression of VEGF and b-FGF were found among the 3 groups. MVD of the control group, chemotherapy group and chemoemoblization group was 80.84±24.24,83.05±20.29 and 85.20±23.91 (F=0.193, P=0.873),respectively. The positive expression of VEGF and b-FGF was 75 %, 75 % , 85 % (X2=0.449, P=0.799) and 30 %,25 %, 30 % (X2=0.141, P=0.922), respectively. | 19 | Hepatitis C virus infections and genotypes in China显示文摘Objective: To define the conditions of hepatitis C vi-rus (HCV) infections, and geographic and demo-graphic distributions of genotypes in China.Methods: HCV infected patients were selected fromindividuals with different patterns of liver diseasesand high risk populations in different parts of China.Genotypes of HCV in some isolates were further ana-lyzed, based on the data from our laboratory studiesand some carefully selected published literatures.Results: The anti-HCV positive rates were 9.7% inpatients with acute hepatitis, 13.3% in patients withchronic hepatitis, 18.3% in patients with hepatocel-lular carcinoma, 33.0% in patients with liver cirrho-sis, and 43.2% in patients with posttransfusionalhepatitis (average, 16.2% in patients with liver di-seases). The anti-HCV positive rates in the high riskpopulations were 36.4% in leukemic patients,43.0% in hemodialysis patients, 12.7% in blood do-nors, 64.1% in drug abusers, 13.1% in prostitutes,and 2.57% in naturally healthy people. At least 4clades (clades 1, 2, 3 and 6) of HCV were found inChina with different geographic and demographicdistributions. Genotype 1b was the most widely dis-tributed genotype, and genotype 3 was mainly foundin Yunnan Province, Southwest China.Conclusion: China has a high incidence of HCV in-fection.
