| 1 | Hunt for the tipping point during endocrine resistance process in breast cancer by dynamic network biomarkers显示文摘Acquired drug resistance is the major reason why patients fail to respond to cancer therapies.It is a challenging task to deter.mine the tipping point of endocrine resistance and detect the associated molecules.Derived from new systems biology theory, the dynamic network biomarker (DNB) method is designed to quantitatively identify the tipping point of a drastic system transition and can theoretically identify DNB genes that play key roles in acquiring drug resistance.We analyzed time-course mRNA sequence data generated from the tamoxifen-treated estrogen receptor (ER)-positive MCF-7 cell line, and identified the tipping point of endocrine resistance with its leading molecules.The results show that there is interplay between gene mutations and DNB genes, in which the accumulated mutations eventually affect the DNB genes that subsequently cause the change of transcriptional landscape, enabling full-blown drug resistance. | 2 | Mutations in carboxy-terminal part of E2 including PKR/eIF2αphosphorylation homology domain and interferon sensitivity determining region of nonstructural 5A of hepatitis C virus 1b:Their correlation with response to interferon monotherapy and viral load显示文摘AIM: To study the amino acid substitutions in the carboxy (C)-terminal part of E2 protein and in the interferon (IFN) sensitivity determining region (ISDR) and their correlation with response to IFN and viral load in 85 hepatitis C virus (HCV)-lb-infected patients treated with IFN. METHODS: The C-terminal part of E2 (codons 617-711) including PKR/eIF2a phosphorylation homology domain (PePHD) and ISDR was sequenced in 85 HCV-1b-infected patients treated by IFN monotherapy. RESULTS: The amino acid substitutions in PePHD detected only in 4 of 85 patients were not correlated either with response to IFN or with viral load. The presence of substitutions in a N-terminal variable region (codons 617-641) in the C-terminal part of E2 was significantly correlated with both small viral load (33.9% vs 13.8%, P = 0.0394) and sustained response to IFN (25.0% vs 6.9 %, P = 0.0429). Four or more substitutions in ISDR were significantly correlated with both small viral load (78.6% vs 16.2%, P < 0.0001) and sustained response to IFN (85.7% vs 2.9%, P < 0.0001). In multivariate analysis, ISDR in nonstructural (NS) 5A (OR = 0.39, P < 0.0001) and N-terminal variable region (OR = 0.51, P = 0.039) was selected as the independent predictors for small viral load, and ISDR (OR = 39.0, P < 0.0001) was selected as the only independent predictor for sustained response. | 5 | Daikenchuto for postoperative adhesive small bowel obstruction:A systematic review and meta-analysis显示文摘AIM To assess the effectiveness of Daikenchuto for patients with postoperative adhesive small bowel obstruction(ASBO).METHODS A systematic search of Pub Med(MEDLINE),CINAHL,the Cochrane Library and Ichushi Web was conducted,and the reference lists of review articles were handsearched.The outcomes of interest were the incidence rate of surgery,the length of hospital days and mortality.The quality of the included studies,publication bias and between-study heterogeneity were also assessed.RESULTS Three randomized controlled trials(RCTs)and three retrospective cohort studies were selected for analysis.In the three RCTs,Daikenchuto significantly reduced the incidence of surgery(p OR=0.13;95%CI:0.03-0.50).Similarly,Daikenchuto significantly reduced the incidence of surgery(p OR=0.53;95%CI:0.32-0.87)in the three cohort studies.The length of hospital stay and mortality were not measured or described consistently.CONCLUSION The present meta-analysis demonstrates that administering Daikenchuto is associated with a lower incidence of surgery for patients with postoperative ASBO in the Japanese population.In order to better generalize these results,additional studies will be needed. | Tomohiko Ukai Satoru Shikata Ryuki Kassai Yousuke Takemura | 2016 | World Journal of Meta-Analysis2016，4，4： | 2 |

| 7 | The roles of activated protein C in experimental trauma models显示文摘Trauma-induced coagulopathy is classified into primary and secondary coagulopathy, with the former elicited by trauma and traumatic shock itself and the latter being acquired coagulopathy induced by anemia, hypothermia, acidosis, and dilution. Primary coagulopathy consists of disseminated intravascular coagulation and acute coagulopathy of trauma shock (ACOTS). The pathophysiology of ACOTS is the suppression of thrombin generation and neutralization of plasminogen activator inhibitor-1 mediated by activated protein C that leads to hypocoagulation and hyperfibrinolysis in the circulation.
