| 2 | Successful laparoscopic splenectomy after living-donor liver transplantation for thrombocytopenia caused by antiviral therapy显示文摘Although interferon (IFN) based therapy for recurrent hepatitis C virus (HCV) infection after liver transplantation has been widely accepted, it induces various adverse effects such as thrombocytopenia, resulting in its interruption. Recently, concomitant splenectomy at the time of living donor liver transplantation (LDLT) has been tried to overcome this problem, but this procedure leads to several complications such as excessive intraoperative bleeding and serious infection. A 60-year-old female received LDLT using a left lobe graft from her second son for liver failure caused by hepatitis C-related cirrhosis. Six months after LDLT, she was diagnosed as recurrent HCV infection by liver biopsy. IFN monotherapy was started from 7 mo after LDLT and her platelet count decreased to less than 50 000/μL, which thus made it necessary to discontinue the treatment. We decided to attempt laparoscopic splenectomy (LS) under general anesthesia. Since intra-abdominal findings did not show any adhesion formations around the spleen, LS could be successfully performed. After LS, since her platelet count immediately increased to 225 000/μL 14 d after operation, IFN therapy was restarted and we could convert the combination therapy of IFN and ribavirin, resulting in no detectable viral marker. | 3 | Gastric adenocarcinoma of fundic gland type spreading to heterotopic gastric glands显示文摘Herein, we present a case of gastric adenocarcinoma of fundic gland type(GA-FG) spreading to heterotopic gastric glands(HGG) in the submucosa. A 58-year-old man with epigastric pain was referred to our hospital and underwent an esophagogastroduodenoscopy. A Borrmann type II gastric cancer at the antrum and a 10 mm submucosal tumor-like lesion in the lesser curvature of the upper third of the stomach were detected. Histological examination of the biopsy specimens obtained from the submucosal tumorlike lesion suggested a GA-FG. Therefore, endoscopic submucosal dissection was performed as excisional biopsy, and histopathological examination of the resected specimen confirmed a GA-FG and HGG proximal to the GA-FG. Although the GA-FG invaded the submucosal layer slightly, the submucosal lesion of the GA-FG had a poor stromal reaction and was located just above the HGG in the submucosa. | 4 | Endoscopic submucosal dissection as excisional biopsy for anorectal malignant melanoma:A case report显示文摘BACKGROUND Anorectal malignant melanoma (AMM) is a rare disorder with an extremely poor prognosis. Although there is currently no consensus on the treatment methods for AMM, surgical procedures have been the most common treatment methods used until now. We recently encountered a case of AMM that we diagnosed using endoscopic submucosal dissection (ESD). To our knowledge, this is the first case of ESD for AMM, suggesting that ESD can potentially be a diagnostic and treatment method for AMM. CASE SUMMARY A 77-year-old woman visited our hospital with a chief complaint of anal bleeding and a palpable rectal mass. Colonoscopy revealed a 20-mm protruded lesion in the lower rectum. After obtaining biopsy specimens from the lesion, although a malignant rectal tumor was suspected, a definitive diagnosis was not made. Endoscopic ultrasonography revealed tumor invasion into the submucosal layer but not the muscular layer. Therefore, we performed an excisional biopsy using ESD. Immunohistochemical examination of the ESD-resected specimen revealed tumor cells positive for Human Melanin Black-45, Melan-A, and S-100. Moreover, the tumor cells lacked melanin pigment;thus, a diagnosis of amelanotic AMM was made. Although the AMM had massively invaded the submucosal layer and both lymphatic and venous invasion were present, we closely monitored the patient without any additional therapy on the basis of her request. Six months after ESD, local recurrence was detected, and the patient consented to wide local excision. | 5 | Familial pancreatic cancer: Concept, management and issues显示文摘Familial pancreatic cancer (FPC) is broadly defined as two first-degree-relatives with pancreatic cancer (PC) and accounts for 4%-10% of PC. Several genetic syndromes, including Peutz-Jeghers syndrome, hereditary pancreatitis, hereditary breast-ovarian cancer syndrome(HBOC), Lynch syndrome, and familial adenomatous polyposis (FAP), also have increased risks of PC, but the narrowest definition of FPC excludes these known syndromes. When compared with other familial tumors, proven genetic alterations are limited to a small proportion (<20%) and the familial aggregation is usually modest. However, an ethnic deviation (Ashkenazi Jewish>Caucasian) and a younger onset are common also in FPC. In European countries, 'anticipation' is reported in FPC families, as with other hereditary syndromes; a trend toward younger age and worse prognosis is recognized in the late years. The resected pancreases of FPC kindred often show multiple pancreatic intraepithelial neoplasia (Pan IN) foci, with various K-ras mutations, similar to colorectal polyposis seen in the FAP patients. As with HBOC patients, a patient who is a BRCA mutation carrier with unresectable pancreatic cancer (accounting for 0%-19% of FPC patients) demonstrated better outcome following platinum and Poly (ADP-ribose) polymerase inhibitor treatment. Western countries have established FPC registries since the 1990 s and several surveillance projects for highrisk individuals are now ongoing to detect early PCs. Improvement in lifestyle habits, including non-smoking, is recommended for individuals at risk. | 7 | Counter-Rotating Type Tidal Stream Power Unit Boarded on Pillar(Performances and Flow Conditions of Tandem Propellers)显示文摘The authors have invented the unique counter-rotating type tidal stream power unit composed of the tandem propellers and the double rotational armature type peculiar generator without the traditional stator. The front and the rear propellers counter-drive the inner and the outer armatures of the peculiar generator, respectively. The unit has the fruitful advantages that not only the output is sufficiently higher without supplementary equipment such as a gearbox, but also the rotational moment hardly act on the pillar because the rotational torque of both propellers/armatures are counter-balanced in the unit. This paper discusses experimentally the performances of the power unit and the effects of the propeller rotation on the sea surface. The axial force acting on the pillar increases naturally with the increase of not only the stream velocity but also the drag of the tandem propellers. Besides, the force vertical to the stream also acts on the pillar, which is induced from the Karman vortex street and the dominant frequencies appear owing to the front and the rear propeller rotations. | 11 | A comparative review of petrogenetic processes beneath the Cameroon Volcanic Line: Geochemical constraints显示文摘The origin and petrogenesis of the Cameroon Volcanic Line(CVL),composed of volcanoes that form on both the ocean floor and the continental crust,are difficult to understand because of the diversity,heterogeneity,and nature of available data.Major and trace elements,and Sr-Nd-Pb isotope data of volcanic rocks of the CVL spanning four decades have been compiled to reinterpret their origin and petrogenesis.Volcanic rocks range from nephelinite,basanite and alkali basalts to phonolite,trachyte and rhyolite with the presence of a compositional gap between Si O258e64 wt.%.Similarities in geochemical characteristics,modeled results for two component mixing,and the existence of mantle xenoliths in most mafic rocks argue against significant crustal contamination.Major and trace element evidences indicate that the melting of mantle rocks to generate the CVL magma occurred dominantly in the garnet lherzolite stability field.Melting models suggest small degree(<3%)partial melting of mantle bearing(6e10%)garnet for Mt.Etinde,the Ngaoundere Plateau and the Biu Plateau,and<5%of garnet for the oceanic sector of the CVL,Mt.Cameroon,Mt.Bambouto,Mt.Manengouba and the Oku Volcanic Group.The Sr-Nd-Pb isotope systematics suggest that mixing in various proportions of Depleted MORB Mantle(DMM)with enriched mantle 1 and 2(EM1 and EM2)could account for the complex isotopic characteristics of the CVL lavas.Low Mg number(Mg#100 Mg O/(Mg O t Fe O))and Ni,Cr and Co contents of the CVL mafic lavas reveal their crystallization from fractionated melts.The absence of systematic variation in Nb/Ta and Zr/Hf ratios,and Sr-Nd isotope compositions between the mafic and felsic lavas indicates progressive evolution of magmas by fractional crystallization.Trace element ratios and their plots corroborate mantle heterogeneity and reveal distinct geochemical signatures for individual the CVL volcanoes. | Asobo N.E.Asaah Tetsuya Yokoyama Festus T.Aka Tomohiro Usui Mengnjo J.Wirmvem Boris Chako Tchamabe Takeshi Ohba Gregory Tanyileke J.V.Hell | 2015 | Geoscience Frontiers2015，6，4： | 3 |

| 14 | A standardized technique for safe pancreaticojejunostomy:Pair-Watch suturing technique显示文摘AIM:To prevent pancreatic leakage after pancreaticoje-junostomy,we designed a new standardized technique that we term the 'Pair-Watch suturing technique'.METHODS:Before anastomosis,we imagine the faces of a pair of watches on the jejunal hole and pancreatic duct.The first stitch was put between 9 o'clock of the pancreatic side and 3 o'clock of the jejunal side,and a total of 7 stitches were put on the posterior wall,followed by the 5 stitches on the anterior wall.Using this technique,twelve stitches can be sutured on the first layer anastomosis regardless of the caliber of the pancreatic duct.In all cases the amylase activity of the drain were measured.A postoperative pancreatic fistula was diagnosed using postoperative pancreatic fistula grading.RESULTS:From March 2007 to July 2008,29 consecutive cases underwent pancreaticojejunostomy using this technique.Pathologic examination results showed pancreatic carcinoma(n=14),intraductal papillary-mucinous neoplasm(n=10),intraductal papillary-mucinous carcinoma(n=1),carcinoma of ampulla of Vater(n=1),carcinoma of extrahepatic bile duct(n=1),metastasis of renal cell carcinoma(n=1),and duodenal carcinoma(n=1).Pancreaticojejunal anastomoses using this technique were all watertight during the surgical procedure.The mean diameter of main pancreatic duct was 3.4 mm(range 2-7 mm).Three patients were recognized as having an amylase level greater than 3 times the serum amylase level,but all of them were diagnosed as grade A postoperative pancreatic fistula grading and required no treatment.None of the cases developed complications such as hemorrhage,abdominal abscess,and pulmonary infection.There was no postoperative mortality.CONCLUSION:Our technique is less complicated than other methods and very secure,providing reliable anastomosis for any size of pancreatic duct. | Yoshinori Azumi Shuji Isaji Hiroyuki Kato Yuu Nobuoka Naohisa Kuriyama Masashi Kishiwada Takashi Hamada Shugo Mizuno Masanobu Usui Hiroyuki Sakurai Masami Tabata | 2010 | World Journal of Gastrointestinal Surgery2010，2，8： | 2 |

| 16 | Mesenchymal stem cells for treatment of aortic aneurysms显示文摘An aortic aneurysm(AA) is a silent but life-threatening disease that involves rupture. It occurs mainly in aging and severe atherosclerotic damage of the aortic wall. Even though surgical intervention is effective to prevent rupture, surgery for the thoracic and thoraco-abdom-inal aorta is an invasive procedure with high mortality and morbidity. Therefore, an alternative strategy for treatment of AA is required. Recently, the molecular pathology of AA has been clarified. AA is caused by an imbalance between the synthesis and degradation of extracellular matrices in the aortic wall. Chronic inflam-mation enhances the degradation of matrices directly and indirectly, making control of the chronic inflamma-tion crucial for aneurysmal development. Meanwhile, mesenchymal stem cells(MSCs) are known to be ob-tained from an adult population and to differentiate into various types of cells. In addition, MSCs have not only the potential anti-inflammatory and immunosuppres-sive properties but also can be recruited into damagedtissue. MSCs have been widely used as a source for celltherapy to treat various diseases involving graft-versus-host disease, stroke, myocardial infarction, and chronicinflammatory disease such as Crohn's disease clinically.Therefore, administration of MSCs might be availableto treat AA using anti-inflammatory and immnosup-pressive properties.
