| 1 | Prognostic and predictive biomarkers in metastatic colorectal cancer anti-EGFR therapy显示文摘AIM: To reviewing genetic and epigenetic make-up of metastatic colorectal cancers(m CRCs) addicted to epidermal growth factor receptor(EGFR) signalling.METHODS: The present study summarizes the potential value of prognostic and predictive biomarkers in selecting m CRC patients treated with anti-EGFR therapy. A meta-analysis was performed using a systematic search of Pub Med, Medline and Web of Science to identify eligible papers until March 21 st, 2016 using these following terms: ‘‘colorectal cancer'', 'predictive biomarkers' ', 'anti-EGFR therapy', 'KRAS', 'NRAS'', 'PIK3CA', 'TP53', 'PTEN', ‘‘EGFR', 'MET', 'HER2', 'epiregulin', 'amphiregulin', 'prognostic biomarkers', 'BRAF', 'miR NA' and 'antibody-dependent cell-mediated cytotoxicity(ADCC) activity'. Two investigators independently evaluated and extracted data from each identified studies based on selected criteria of inclusion and exclusion. RESULTS: The introduction of agents targeting EGFR such as cetuximab and panitumumab increased overall survival of mC RCs. Nevertheless, it has firstly became evident that response rates to cetuximab regimens in unselected patient populations were typically lower than 30%. Clinical data confirmed the predictive value of RAS mutations for resistance to cetuximab and panitumumab leading to the license of these monoclonal antibodies exclusively for the management of patients with RAS-wild type colorectal cancers. So far the identification of predictive biomarkers have generated interesting, though preliminary and, at times, conflicting data on the importance of tumour mR NA levels of EGFR ligands, of activating mutations in other genes such as NRAS and PIK3 CA. | 2 | Thiopurine-methyltransferase variants in inflammatory bowel disease:Prevalence and toxicity in Brazilian patients显示文摘AIM:To analyze the prevalence of thiopurine-methyltransferase(TPMT)genotypes and their associationwith drug toxicity in inflammatory bowel disease(IBD)patients from southeastern Brazil.METHODS:A total of 219 consecutive patients with IBD,of which 146 had Crohn’s disease and 73 had ulcerative colitis,regularly seen at the outpatient unit of the Division of Gastroenterology at the University Hospital Pedro Ernesto of the State University of Rio de Janeiro,a tertiary referral center,were enrolled in this study from February 2009 to January 2011.We analyzed the presence of major TPMT genetic variants(TPMT*2,*3A,*3C)in IBD patients by means of a specific allele and RFLP-PCR.Genomic DNA was isolated from peripheral blood leukocytes by proteinase-K/Sodium Dodecyl Sulfate digestion and phenol-chloroform extraction.TPMT*2(C238G),TPMT*3A(G460A/A719G),and TPMT*3C(A719G)genotypes were detected by real-time polymerase chain reaction followed by direct sequencing with specific primers.Clinical data were systematically recorded,and correlated with the genotype results.RESULTS:The distribution of the selected TPMT gene polymorphism TPMT*2(C238G),TPMT*3A(G460A/A719G),and TPMT*3C(A719G)genotypes was 3.6%,5.4%,and 7.7%of the patients,respectively.Among the side effects recorded from patients taking azathioprine,14 patients presented with pancreatitis and/or an elevation of pancreatic enzymes,while 6 patients had liver toxicity,and 2 patients exhibited myelosuppression/neutropenia.TPMT polymorphisms were detected in 37/219 patients(8 heterozygous for*2,11 heterozygous for*3A,and 18 heterozygous for*3C).No homozygotic polymorphisms were found.Despite the prevalence of the TPMT*3C genotype,no differences among the genotype frequencies were significant.Although no association was detected regarding myelotoxicity or hepatotoxicity,a trend towards the elevation of pancreatic enzymes was observed for TPMT*2 and TPMT*3C genotypes.CONCLUSION:The prevalence of TPMT genotypes was high among Brazilian patients.Variants genes*2and*3C may be associated with azathioprine pancreatic toxicity in a IBD southeastern Brazilian population. | Ana Teresa P Carvalho Barbara C Esberard Renata S B Fróes Davy C M Rapozo Ana B Grinman Tatiana A Simo Juliana C V C Santos Antonio José V Carneiro Luis Felipe Ribeiro-Pinto Heitor S P de Souza | 2014 | World Journal of Gastroenterology2014，20，12： | 3 |
| 3 | Triiodothyronine and breast cancer显示文摘The thyroid hormones(THs), triiodothyronine(T3) and thyroxine(T4), are essential for survival; they are involved in the processes of development, growth, and metabolism. In addition to hyperthyroidism or hypothyroidism, THs are involved in other diseases. The role of THs in the development and differentiation of mammary epithelium is well established; however, their specific role in the pathogenesis of breast cancer(BC) is controversial. Steroid hormones affect many human cancers and the abnormal responsiveness of the mammary epithelial cells to estradiol(E2) in particular is known to be an important cause for the development and progression of BC. The proliferative effect of T3 has been demonstrated in various types of cancer. In BC cell lines, T3 may foster the conditions for tumor proliferation and increase the effect of cell proliferation by E2; thus, T3 may play a role in the developmentand progression of BC. Studies show that T3 has effects similar to E2 in BC cell lines. | 6 | Clinical significance of'anti-HBc alone'in human immunodeficiency virus-positive patients显示文摘AIM:To determine the prevalence and clinical relevance of isolated antibodies to hepatitis B core antigen as the only marker of infection('anti-HBc alone')among human immunodeficiency virus(HIV) type-1 infected patients.Occult hepatitis B infection frequency was also evaluated. METHODS:Three hundred and forty eight histories from 2388 HIV-positive patients were randomly reviewed.Patients with serological markers of hepatitis B virus(HBV)infection were classified into three groups:past hepatitis,'anti-HBc alone'and chronic hepatitis.Determination of DNA from HBV,and RNA and genotype from hepatitis C virus(HCV)were performed on'anti-HBc alone'patients. RESULTS:One hundred and eighty seven(53.7%) HIV-positive patients had markers of HBV infection: 118 past infection(63.1%),14 chronic hepatitis (7.5%)and 55'anti-HBc alone'(29.4%).Younger age[2.3-fold higher per every 10 years younger;95% confidence intervals(CI)1.33-4.00]and antibodies to HCV infection[odds ratio(OR)2.87;95%CI 1.10-7.48]were factors independently associated with the'anti-HBc alone'pattern.No differences in liver disease frequency were detected between both groups. Serum levels of anti-HBs were not associated with HCV infection(nor viral replication or HCV genotype),or with HIV replication or CD4 level.No'anti-HBc alone' patient tested positive for HBV DNA.
