1Tai Ji Quan as antihypertensive lifestyle therapy:A systematic review and meta-analysis显示文摘Background:Professional health organizations are not currently recommending Tai Ji Quan alongside aerobic exercise to treat hypertension.We aimed to examine the efficacy of Tai Ji Quan as antihypertensive lifestyle therapy.Methods:Tai Ji Quan interventions published in English and Chinese were included when they involved healthy adults,reported pre-and postintervention blood pressure(BP),and had a non-exercise/non-diet control group.We systematically searched 11 electronic databases for studies published through July 31,2018,yielding 31 qualifying controlled trials.We(1)evaluated the risk of bias and methodological study quality,(2)performed meta-regression analyses following random-effects assumptions,and(3)generated additive models representing the largest possible clinically relevant BP reductions.Results:Participants(n=3223)were middle-aged(56.6±15.1 years of age,mean±SD)adults with prehypertension(systolic BP(SBP)=136.9±15.2 mmHg,diastolic BP(DBP)=83.4±8.7 mmHg).Tai Ji Quan was practiced 4.0±1.4 sessions/week for 54.0§10.6 min/session for 22.3±20.2 weeks.Overall,Tai Ji Quan elicited significant reductions in SBP(-11.3 mmHg,95%CI:-14.6 to-8.0;d+=-0.75)and DBP(-4.8 mmHg,95%CI:-6.4 to-3.1;d+=-0.53)vs.control(p<0.001).Controlling for publication bias among samples with hypertension,Tai Ji Quan trials published in English elicited SBP reductions of 10.4 mmHg and DBP reductions of 4.0 mmHg,which was half the magnitude of trials published in Chinese(SBP reductions of 18.6 mmHg and DBP reductions of 8.8 mmHg).Conclusion:Our results indicate that Tai Ji Quan is a viable antihypertensive lifestyle therapy that produces clinically meaningful BP reductions(i.e.,10.4 mmHg and 4.0 mmHg of SBP and DBP reductions,respectively)among individuals with hypertension.Such magnitude of BP reductions can lower the incidence of cardiovascular disease by up to 40%.Yin Wu Blair T.Johnson Shiqi Chen Yiyang Chen Jill Livingston Linda S.Pescatello 2021Journal of Sport and Health Science2021,10,2:11

14Guiding phosphorus stewardship for multiple ecosystem services显示文摘The essential role of phosphorus(P)for agriculture and its impact on water quality has received decades of research attention.However,the benefits of sustainable P use and management for society due to its downstream impacts on multiple ecosystem services are rarely acknowledged.We propose a conceptual framework-the“phosphorus-ecosystem services cascade”(PESC)-to integrate the key ecosystem processes and functions that moderate the relationship between P released to the environment from human actions and ecosystem services at distinct spatial and temporal scales.Indirect pathways in the cascade via soil and aquatic processes link anthropogenic P to biodiversity and multiple services,including recreation,drinking water provision,and fisheries.As anthropogenic P cascades through catchments,it often shifts from a subsidy to a stressor of ecosystem services.Phosphorus stewardship can have emergent ecosystem service co-benefits due to synergies with other societal or management goals(e.g.,recycling of livestock manures and organic wastes could impact soil carbon storage).Applying the PESC framework,we identify key research priorities to align P stewardship with the management of multiple ecosystem services,such as incorporating additional services into agri-environmental P indices,assessing how widespread recycling of organic P sources could differentially impact agricultural yields and water quality,and accounting for shifting baselines in P stewardship due to climate change.Ultimately,P impacts depend on site-specific agricultural and biogeophysical contexts,so greater precision in targeting stewardship strategies to specific locations would help to optimize for ecosystem services and to more effectively internalize the downstream costs of farm nutrient management.Graham K.MacDonald Helen P.Jarvie Paul J.A.Withers Donnacha G.Doody Bonnie L.Keeler Philip M.Haygarth Laura T.Johnson Richard W.McDowell Michael K.Miyittah Stephen M.Powers Andrew N.Sharpley Jianbo Shen Douglas R.Smith Michael N.Weintraub Tiequan Zhang 2016Ecosystem Health and Sustainability2016,2,12:0

17A phaseⅡprospective trial of photobiomodulation therapy in limiting oral mucositis in the treatment of locally advanced head and neck cancer patients显示文摘Objective:This study aimed to compare the historical incidence rate of severe oral mucositis(OM)in head and neck cancer patients undergoing definitive concurrent chemoradiation therapy(CRT)versus a prospective cohort of patients with locally advanced head and neck squamous cell carcinoma(HNSCC)treated with prophylactic photobiomodulation therapy(PBMT).Methods:This US-based,institutional,single-arm,phaseⅡprospective clinical trial was initiated in 50 patients(age≥18 years,Karnofsky Performance Scale Index>60,with locally advanced HNSCC(excluding oral cavity)receiving definitive or adjuvant radiation therapy(RT)with concurrent platinum-based chemotherapy(CT).PBMT was delivered three times per week throughout RT utilizing both an intraoral as well extraoral delivery system.Primary outcome measure was incidence of severe OM utilizing both the National Cancer Institute Common Toxicity Criteria,version 4.0(NCI-CTCAE)Grade≥3 and the World Health Organization Mucositis Grading Scale(WHO)Grade≥3 versus historical controls;secondary outcome measures included time to onset of severe OM following therapy initiation.Results:At baseline,all patients included in final analysis(N=47)had OM Grade 0.Average RT and CT dose was(66.3±5.1)Gy and(486.1±106.8)mg/m ^(2),respectively.Severe OM was observed in 11 of 47 patients(23%,confidence interval:12,38).OM toxicity grade trended upward during treatment,reaching a maximum at 7 weeks(WHO:1.8 vs.NCI-CTCAE:1.7).Subsequently,OM grade returned to baseline 3 months following completion of RT.The mean time to onset of severe OM was(35±12)days.The mean time to resolution of severe OM was(37±37)days.Conclusions:Compared to historical outcomes,PBMT aides in decreasing severe OM in patients with locally advanced HNSCC.PBMT represents a minimally invasive,prophylactic intervention to decrease OM as a major treatment-related side effect.Janielle K.-A.Taylor Leila J.Mady Khalil Baddour Uzoma K.Iheagwara Shuyan Zhai James P.Ohr Daniel P.Zandber Vikram C.Gorantla Robert L.Ferris Seungwon Kim Umamaheswar Duvvuri Mark W.Kubik Shaum Sridharan Jonas T.Johnson Karen D.Holeva Annette E.Quinn David A.Clump 2022World Journal of Otorhinolaryngology-Head and Neck Surgery2022,8,4:0
