1电针激活迷走神经-肾上腺轴的神经解剖学基础显示文摘电针刺激 (ES) 通过躯体感觉自主神经反射来调控机体的生理功能(如抑制严重的炎症)。二十世纪七十年代有学者认为针刺反射具有身体区域特异性(穴位特异性)。例如,低强度 (0.5 mA,10 Hz) ES 小鼠后肢穴位足三里 (ST36) 可激活迷走神经-肾上腺抗炎通路,而 ES 腹部天枢穴 (ST25) 无效,这种现象的神经解剖学基础尚不清楚。最近,在一项新的研究中,来自哈佛大学医学院、中国中医科学院和复旦大学的研究人员阐明了 ES 激活迷走神经-肾上腺轴抗炎通路的神经解剖学机制,并发现了由 PROKR2^(Cre)标记的感觉神经元对激活迷走神经-肾上腺轴至关重要。特异性敲除小鼠表达 PROKR2^(Cre)标记的 DRG 神经元,低强度电针 (0.5 mA) 刺激后肢 ST36 穴位不产生任何反应,不能使肾上腺释放儿茶酚胺类神经递质,也无法抑制细菌内毒素脂多糖 (LPS) 所诱发的全身炎症;而高强度电针 (3.0 mA)刺激后肢穴位 ST36 和腹部穴位 ST25 可激活交感神经抗炎通路。该研究还发现,进一步运用交叉遗传的方法特异性诱导光敏感通道蛋白 CatCh 表达于 PROKR2^(Cre)标记的神经末梢,可以激活迷走神经-肾上腺抗炎通路,但不能激活交感神经-肾上腺抗炎通路。此外,PROKR2^(Cre)神经纤维的分布可以预测低强度 ES 产生抗炎作用的身体区域(穴位特异性)。该研究为穴位选择和特异性激活自主神经通路提供了神经解剖学基础。尽管现代解剖学没有文献支持经络的物理存在,但针灸确实可以通过躯体神经反射来达到治疗效果。这些反射始于背根神经节 (DRG) 和/或三叉神经节周围神经的激活,然后从脊髓向大脑传递感觉信息,进而激活外周自主神经通路,最终对机体进行调控。该研究解读了为什么 ES 足三里 (ST36) 可激活迷走-肾上腺轴,而刺激腹部天枢穴 (ST25) 无任何效果,ES 刺激 ST25反而激活了脊髓交感神经通路。该研究认为迷走神经-肾上腺抗炎通道是由感觉神经介导,是后肢特有的 PROKR2^(Cre)标记的 DRG 神经元(一种 Cre 介导的重组标记神经元具有 PROKR2 持续或瞬时表达的作用)激活的,这些神经纤维密集分布在后肢深层筋膜组织。陈紫悦(译) 韩清见(校) Liu SB Wang ZF Su YS 2022中国疼痛医学杂志2022,28,4:6
