| 1 | Clinical outcome in patients with hilar malignant strictures type Ⅱ Bismuth-Corlette treated by minimally invasive unilateral versus bilateral endoscopic biliary drainage显示文摘BACKGROUND:Stenting of malignant hilar strictures remains a standard endoscopic treatment in patients with unresectable tumors.The aim of this two-center prospective study was to compare unilateral versus bilateral drainage in hilar malignant stenosis Bismuth-Corlette type II.METHODS:During a 3-year period,a total of 49 patients with hilar tumors(Bismuth-Corlette type II) were referred for endoscopic treatment,following the criteria of unresectability.Ultrasound,computed tomography scan and magnetic resonance cholangiopancreatography(MRCP) were previously performed in all patients in order to facilitate endoscopic retrograde cholangiopancreatography(ERCP).The stricture was first passed by the hydrophilic guide-wire and then contrast medium was injected.Mechanical bile duct dilation was performed,followed by plastic stent placement only in the liver lobe which was previously opacified.The procedures were performed under conscious sedation.The patients were followed up for the next 12 months with a stent exchange every 3 months.Primary outcome was assessed by patient survival in the first 12 months after the procedure.RESULTS:All 49 patients were treated with ERCP while 39(79.59%) had successful stent placement.Among these,32 had hilar cholangiocarcinoma(82%) and 7(18%) had gallbladder cancer.Two groups of patients had Bismuth II strictures:A,21 patients(54%) with unilateral contrast injection and drainage,and B,18(46%) with bilateral contrast injection and drainage.A total of 57 plastic stents were used(10 Fr,89%;11.5 Fr,11%).Group B showed a lower bilirubin level 7 days after the procedure(P=0.008).Early complications were cholangitis(3 patients,2 in group A and 1 in group B) and acute pancreatitis(2 patients,1 each in A and B) with no statistical difference between the groups.Late complications were stent migration(5 patients,1 in A and 4 in B) and stent clogging(6 patients,2 in A and 4 in B) showing a significant difference between the groups(P<0.01).The first stent replacement after 3 months was successful in 87% of patients(four died due to disease progression and one due to cardiopulmonary insufficiency) showing no statistical difference between the groups.At 6 months follow-up,72% patients survived,with no statistical difference between the groups.A final follow-up(12 months) showed the survival rate of 18%(4 patients from group A and 3 from group B)(P>0.05).CONCLUSIONS:A minimally invasive approach,based on the criterion that every bile duct being opacified needs to be drained,is associated with a lower incidence of early complications.Considering that the clinical outcome measured by bilirubin level was lower in patients with bilateral drainage 7 days after the procedure,we assumed that drainage of 50% or more of the liver volume leads to sufficient drainage effectiveness. | Milutin Bulajic Nikola Panic Miodrag Radunovic Radisav Scepanovic Radoslav Perunovic Predrag Stevanovic Tatjana Ille Maurizio Zilli Mirko Bulajic | 2012 | Hepatobiliary & Pancreatic Diseases International2012,11,2: | 8 |

| 4 | Characterization particulate matter from several Chinese cooking dishes and implications in health effects显示文摘Cooking fume produced by oil and food at a high temperature releases large amount of fine particulate matter(PM) which have a potential hazard to human health. This chamber study investigated particle emission characteristics originated from using four types of oil(soybean oil, olive oil, peanut oil and lard) and different kinds of food materials(meat and vegetable). The corresponding emission factors(EFs) of number, mass, surface area and volume for particles were discussed. Temporal variation of size-fractionated particle concentration showed that olive oil produced the highest number PM concentration for the entire cooking process. Multiple path particle dosimetry(MPPD) model was performed to predict deposition in the human respiratory tract. Results showed that the pulmonary airway deposition fraction was the largest. It was also found that particles produced from olive oil led to the highest deposition. | 6 | Epidemiology of hepatitis C in Croatia in the European context显示文摘We analyzed prevalence, risk factors and hepatitis C virus(HCV) genotype distribution in different population groups in Croatia in the context of HCV epidemiology in Europe, with the aim to gather all existing information on HCV infection in Croatia which will be used to advise upon preventive measures. It is estimated that 35000-45000 of the Croatian population is chronically infected with HCV. Like in other European countries, there have been changes in the HCV epidemiology in Croatia over the past few decades. In some risk groups(polytransfused and hemodialysis patients), a significant decrease in the HCV prevalence was observed after the introduction of routine HCV screening of blood/blood products in 1992. Injecting drug users(IDUs) still represent a group with the highest risk for HCV infection with prevalence ranging from 29% to 65%. Compared to the prevalence in theCroatian general population(0.9%), higher prevalence rates were found in prison populations(8.3%-44%), human immunodeficiency virus-infected patients(15%), persons with high-risk sexual behavior(4.6%) and alcohol abusers(2.4%). Low/very low prevalence was reported in children and adolescents(0.3%) as well as in blood donors(0%-0.009%). In addition, distribution of HCV genotypes has changed due to different routes of transmission. In the general population, genotypes 1 and 3 are most widely distributed(60.4%-79.8% and 12.9%-47.9%, respectively). The similar genotype distribution is found in groups with high-risk sexual behavior. Genotype 3 is predominant in Croatian IDUs(60.5%-83.9%) while in the prison population genotypes 3 and 1 are equally distributed(52.4% and 47.6%). | 7 | Comparison of light absorption and oxidative potential of biodiesel/diesel and chemicals/diesel blends soot particles显示文摘Soot particles,mainly coming from fuel combustion,affect climate forcing through absorbing light and also result in adverse human health outcomes.Though biodiesel or additives blending with diesel was considered environmentally friendly,the understanding on absorbing and oxidative capacity of soot emitted from them are still unclear.The watersoluble organic carbon(WSOC)content,surface chemical structure,light absorption and oxidative potential(OPDTT)of soot from biodiesel/diesel and chemicals/diesel blends were investigated utilizing total organic carbon analyzer,X-ray photoelectron spectrometer,ultraviolet–visible spectrophotometry and dithiothreitol(DTT)assay.The differences and correlations between soot properties were statistically analyzed.Chemicals/diesel blends soot owned significantly higher WSOC content,ratio of mass absorbing efficiency(MAE)in250 and 365 nm(E2/E3),OPDTT,and higher surface carbonyl content.Coconut biodiesel/diesel blends soot contained evidently higher aromatic carbon–oxygen single bond(ArC–O)content,and higher MAE365.The individual comparison of biodiesel/diesel blends showed20%coconut biodiesel blend owned the lowest WSOC,E2/E3 and OPDTT,while highest ArC–O and MAE365,representing strongest absorbing properties.Association analysis showed OPDTTwas significantly positively correlated with WSOC.Further,the evident negative correlation between MAE365 and OPDTT was observed.Our results showed coconut biodiesel/diesel blends soot induced lower levels of oxidative potential,whereas absorption of light was higher,which have far reaching consequences on climate forcing.Therefore,it is important to evaluate the balance point between light-absorbing properties and oxidative potential,under the wide use of biodiesel. | Yu Kuang Yi Guo Jiaqi Chai Jing Shang Jiali Zhu Svetlana Stevanovic Zoran Ristovski | 2020 | Journal of Environmental Sciences2020,32,1: | 2 |
| 8 | Epidemiology of hepatitis E in South-East Europe in the 'One Health' concept显示文摘The significance of hepatitis E virus(HEV)as an important public health problem is rising.Until a decade ago,cases of HEV infection in Eur-ope were mainly confined to returning travelers,but nowadays,hepatitis E represents an emerging zoonotic infection in many European countries.The aim of this manuscript is to perform a systematic review of the published literature on hepatitis E distribution in humans,animals and environmental samples('One Health'concept)in the South-Eastern European countries.Comparison of the available data showed that the anti-HEV seroprevalence in the South-Eastern Europe varies greatly,depending on the population studied,geographical area and methods used.The IgG seroprevalence rates in different population groups were found to be 1.1%-24.5%in Croatia,up to 20.9%in Bulgaria,5.9-%17.1%in Romania,15%in Serbia,up to 9.7%in Greece and 2%-9.7%in Albania.Among possible risk factors,older age was the most significant predictor for HEV seropositivity in most studies.Higher seroprevalence rates were found in animals.HEV IgG antibodies in domestic pigs were detected in 20%-54.5%,29.2%-50%,38.94%-50%and 31.1%-91.7%in Serbia,Bulgaria,Romania and Croatia,respectively.In wild boars seroprevalence rates were up to 10.3%,30.3%and 31.1%in Romania,Slovenia and Croatia,respectively.A high HEV RNA prevalence in wild boars in some countries(Croatia and Romania)indicated that wild boars may have a key role in the HEV epidemiology.There are very few data on HEV prevalence in environmental samples.HEV RNA was detected in 3.3%and 16.7%surface waters in Slovenia and Serbia,respectively.There is no evidence of HEV RNA in sewage systems in this region.The available data on genetic characterization show that human,animal and environmental HEV strains mainly belong to the genotype 3. | Anna Mrzljak Petra Dinjar-Kujundzic Lorena Jemersic Jelena Prpic Ljubo Barbic Vladimir Savic Vladimir Stevanovic Tatjana Vilibic-Cavlek | 2019 | World Journal of Gastroenterology2019,25,25: | 2 |
