1抗表皮生长因子受体和抗血管内皮生长因子药物在非小细胞肺癌治疗中的作用显示文摘采用以铂类药物为基础联合细胞毒性药物作为非小细胞肺癌(non-small-cell lung cancer,NSCLC)的标准化一线治疗的疗效已达到相应的平台。作用于特定通路的新型分子靶向药物逐渐成为治疗NSCLC的有效药物;某些药物的III期试验已取得阳性结果。值得注意的是,有研究证实,采用抗血管内皮生长因子(vascular endothelial growth factor,VEGF)抗体贝伐珠单抗抑制VEGF通路,以及采用表皮生长因子受体(epidermal growth factor receptor,EGFR)的小分子酪氨酸激酶抑制剂厄洛替尼或单克隆抗体(西妥昔单抗)靶向作用于EGFR通路作为一线或二线治疗方案,均可延长晚期肺癌患者的生存期。可导致肿瘤细胞存活和增殖的信号处理过程不同,为靶向作用于多种信号通路为有效的抗癌治疗策略提供了依据。因此,分子靶向药物的合理联用可能达到更好的临床疗效,并可成为对标准化疗耐药或不能耐受标准化疗的患者的可替代的治疗选择。目前的挑战在于明确应探寻哪些分子实体,以及它们的最佳联合方案。本综述旨在探讨贝伐珠单抗联合厄洛替尼抑制血管生成和EGFR信号作为治疗NSCLC的有效的非化疗方案的潜在临床疗效。其它可阻滞EGFR和血管生成通路以及互补信号通路的新型药物的联合具有独特的作用机制和较轻的毒副作用,有可能为晚期NSCLC患者的个体化治疗选择提供更多的治疗方案。Corey LANGER Jean-Charles SORIA 南娟(翻译) 丁燕(校对) 2010中国肺癌杂志2010,13,9:16
2Aquaporins:Their role in cholestatic liver disease显示文摘This review focuses on current knowledge on hepato-cyte aquaporins(AQPs)and their significance in bile formation and cholestasis.Canalicular bile secretion results from a combined interaction of several solute transporters and AQP water channels that facilitate water flow in response to the osmotic gradients created. During choleresis,hepatocytes rapidly increase their canalicular membrane water permeability by modulating the abundance of AQP8.The question was raised as to whether the opposite process,i.e.a decreased canalicular AQP8 expression would contribute to the development of cholestasis.Studies in several experimental models of cholestasis,such as extrahepatic obstructive cholestasis,estrogen-induced cholestasis, and sepsis-induced cholestasis demonstrated that the protein expression of hepatocyte AQP8 was impaired. In addition,biophysical studies in canalicular plasma membranes revealed decreased water permeability associated with AQP8 protein downregulation.The combined alteration in hepatocyte solute transporters and AQP8 would hamper the efficient coupling of osmotic gradients and canalicular water flow.Thus cholestasis may result from a mutual occurrence of impaired solute transport and decreased water permeability.Guillermo L Lehmann Maria C Larocca Leandro R Soria Raúl A Marinelli 2008World Journal of Gastroenterology2008,14,46:8
3Clinical impact of confocal laser endomicroscopy in the management of gastrointestinal lesions with an uncertain diagnosis显示文摘AIM To evaluate the clinical impact of confocal laser endomicroscopy(CLE) in the diagnosis and management of patients with an uncertain diagnosis.METHODS A retrospective chart review was performed.Patients who underwent CLE between November 2013 and October 2015 and exhibited a poor correlation between endoscopic and histological findings were included.Baseline characteristics,indications,previous diagnostic studies,findings at the time of CLE,clinical management and histological results were analyzed.Interventions based on CLE findings were also analyzed.We compared the diagnostic accuracy of CLE and target biopsies of surgical specimens.RESULTS A total of 144 patients were included.Of these,51%(74/144) were female.The mean age was 51 years old.In all,41/144(28.4%) lesions were neoplastic(13 bile duct,10 gastric,8 esophageal,6 colonic,1 duodenal,1 rectal,1 ampulloma and 1 pancreatic).The sensitivity,specificity,positive predictive value,negative predictive value,and observed agreement when CLE was used to detect N-lesions were 85.37%,87.38%,72.92%,93.75% and 86.81%,respectively.Cohen's Kappa was 69.20%,thus indicating good agreement.Changes in management were observed in 54% of the cases.CONCLUSION CLE is a new diagnostic tool that has a significant clinical impact on the diagnosis and treatment of patients with uncertain diagnosis.Carlos Robles-Medranda Maria Vargas Jesenia Ospina Miguel Puga-Tejada Manuel Valero Miguel Soria Gladys Bravo Carlos Robles-Jara Hannah Pitanga Lukashok 2017World Journal of Gastrointestinal Endoscopy2017,9,8:6

7在与心肌的梗塞介绍的就医的病人的心率的预示的意义显示文摘 AIM: To investigate the prognostic significance of resting heart rate in patients with acute coronary syndrome (ACS), independent of other known factors. METHODS: Patients 40 years of age or older who had been admitted with acute coronary syndrome (ACS) to one of the 94 hospitals participating in the Prevalence of Peripheral Arterial Disease in Patients with Acute Coronary Syndrome (PAMISCA) study were included. Patients were divided into two groups based on their resting heart rate (HR ≥ or < 70 bpm). Complications were recording during a follow-up period of 1 year. RESULTS: There were 1054 ACS patients analyzed (43.5% with ST segment elevation and 56.5% without elevation). Mean age was 66.6 ± 11.7 years, 70.6% were male and 29.4% of subjects were female. During follow-up, more patients in the HR ≥ 70 bpm group were hospitalized for heart failure and they also had a higher mortality rate. In the multivariate analysis, a heart rate of ≥ 70 bpm was independently related to overall mortality during the follow-up period (hazard ratio 2.5; 95% confidence interval, 1.26-4.97, P = 0.009). 8Heuristics of node selection criteria to assess robustness of world airport network显示文摘The world airport network(WAN) is one of the networked infrastructures that shape today's economic and social activity, so its resilience against incidents affecting the WAN is an important problem. In this paper, the robustness of air route networks is extended by defining and testing several heuristics to define selection criteria to detect the critical nodes of the WAN.In addition to heuristics based on genetic algorithms and simulated annealing, custom heuristics based on node damage and node betweenness are defined. The most effective heuristic is a multiattack heuristic combining both custom heuristics. 11Regulation of mitochondrial function and endoplasmic reticulum stress by nitric oxide in pluripotent stem cells显示文摘Mitochondrial dysfunction and endoplasmic reticulum stress(ERS) are global processes that are interrelated and regulated by several stress factors. Nitric oxide(NO) is a multifunctional biomolecule with many varieties of physiological and pathological functions, such as the regulation of cytochrome c inhibition and activation of the immune response, ERS and DNA damage; these actions are dose-dependent. It has been reported that in embryonic stem cells, NO has a dual role, controlling differentiation, survival and pluripotency, but the molecular mechanisms by which it modulates these functions are not yet known. Low levels of NO maintain pluripotency and induce mitochondrial biogenesis. It is well established that NO disrupts the mitochondrial respiratory chain and causes changes in mitochondrial Ca^(2+) flux that induce ERS. Thus, at high concentrations, NO becomes a potential differentiation agent due to the relationship between ERS and the unfolded protein response in many differentiated cell lines. Nevertheless, many studies have demonstratedthe need for physiological levels of NO for a proper ERS response.
