1p16 promoter hypermethylation:A useful serum marker for early detection of gastric cancer显示文摘AIM: To determine p16 promoter hypermethylation in gastric tumoral tissue and serum samples, its impact on p16-protein expression, and correlation with clinical and histological features. METHODS: Samples were obtained from 52 histologically confirmed cases of gastric adenocarcinoma. Gastric tissue and serum of 50 age- and sex-matched individuals with normal gastroscopy and biopsy were obtained as control samples. Methylation-specific polymerase chain reaction (MSP) was used to evaluate methylation status of p16 promoter. p16-protein expression was analyzed by immunohistochemical staining on paraffin-embedded sections. RESULTS: Methylation was detected in 44.2% (23/52) of tumoral tissues. 60.9% of them were also methylated in serum, i.e., 26.9% of all patients (14/52). Methylation was not detected in tissue and sera of control samples. p16-protein expression was decreased in 61.5% of cases (32/52), and was significantly associated with promoter hypermethylation (P < 0.001). Methylation was significantly more frequent in higher pathological grades (P < 0.05). Methylation was not associated with other clinicopathological features and environmental factors including H pylori infection and smoking. CONCLUSION: p16 promoter hypermethylation is an important event in gastric carcinogenesis. It is the principle mechanism of p16 gene silencing. It is related to malignant tumor behavior. 2Expression of cell adhesion molecule CD44 in gastric adenocarcinoma and its prognostic importance显示文摘AIM: To evaluate the relation of cluster of differentiation 44 (CD44) expression with clinicopathological features of gastric adenocarcinoma, and also its effect on prognosis with an emphasis on the differences between intestinal and diffuse types. METHODS: From 2000 to 2006, 100 patients with gastric adenocarcinoma, who had undergone total or subtotal gastrectomy without any prior treatment, were studied. Haematoxylin & eosin (HE) staining was used for histological evaluation, including the type (Lauren's classifi cation) and grading of the tumor. The expression of CD44 in the gastric adenocarcinoma mucosa and the adjacent mucosa were determined by immunohistochemistry. The survival analysis was obtained using the Kaplan-Meier test. RESULTS: Of 100 patients, 74 (74%) patients were male. The tumors were categorized as intestinal type (78%) or diffuse type (22%). Sixty-five percent of patients were CD44-positive. CD44 expression was not detected in normal gastric mucosa. Rather, CD44 was more commonly expressed in the intestinal subtype (P = 0.002). A signifi cant relation was seen between the grade of tumor and the expression of CD44 (P = 0.014). The survival analysis showed a poor prognosis of patients with CD44-positive tumors (P = 0.008); and this was more prominent in the intestinal (P = 0.001) rather than diffuse type. CONCLUSION: Cell adhesion molecule CD44 is highly expressed in gastric adenocarcinoma. CD44 expression is correlated with a poor prognosis in patients with the intestinal type of gastric adenocarcinoma. 3Risk factors for hilar cholangiocarcinoma:A case-control study in China显示文摘AIM:To study the association between hilar cholangiocarcinoma(HC) and pre-existing medical conditions. METHODS:Three hundred and thirteen HC patients admitted to the Eastern Hepatobiliary Surgery Hospital(Shanghai,China) in 2000-2005 and 608 healthy controls were enrolled in this study.Association between HC and pre-existing medical conditions was studied with their adjusted odds ratio(OR) calculated by logistic regression analysis. RESULTS:The prevalence of choledocholithiasis(adjusted OR=2.704,P=0.039) ,hepatolithiasis(adjusted OR=3.278,P=0.018) ,cholecystolithiasis(adjusted OR =4.499,P<0.0001) ,cholecystectomy(adjusted OR =7.012,P=0.004) ,biliary ascariasis(adjusted OR= 7.188,P=0.001) ,liver fluke(adjusted OR=10.088,P =0.042) and liver schistosomiasis(adjusted OR=9.913,P=0.001) was higher in HC patients than in healthy controls. 4Stool-based DNA testing,a new noninvasive method for colorectal cancer screening,the first report from Iran显示文摘AIM:To detect tumor-associated DNA changes in stool samples among Iranian patients with colorectal cancer(CRC)compared to healthy individuals using BAT-26,p16 hypermethylation and long DNA markers.METHODS:Stool DNA was isolated from 45 subjects including 25 CRC patients and 20 healthy individuals using a new,fast and easy extraction method.Long DNA associated with tumor was detected using polymerase chain reaction method.Microsatellite studies were performed utilizing denaturating polyacrylamide gel to determine the instability of BAT-26.Methylation status of p16 promoter was analyzed using methylation-specific PCR(MSP).RESULTS:The results showed a significant difference in existence of long DNA(16 in patients vs 1 in controls,P < 0.001)and p16(5 in patients vs none in controls,P = 0.043)in the stool samples of two groups.Long DNA was detected in 64% of CRC patients;whereas just one of the healthy individuals was positive for Long DNA.p16 methylation was found in 20% of patients and in none of healthy individuals.Instability of BAT-26 was not detected in any of stool samples.CONCLUSION:We could detect colorectal cancer related genetic alterations by analyzing stool DNA witha sensitivity of 64% and 20% and a specificity of 95% and 100% for Long DNA and p16 respectively.A non-invasive molecular stool-based DNA testing can provide a screening strategy in high-risk individuals.However,additional testing on more samples is necessary from Iranian subjects to determine the exact specificity and sensitivity of these markers.Mohammad Reza Abbaszadegan Alireza Tavasoli Arash Velayati Hamid Reza Sima Hassan Vosooghinia Mehdi Farzadnia Hamid Asadzedeh Mehran Gholamin Ezzat Dadkhah Azadeh Aarabi 2007World Journal of Gastroenterology2007,13,10:14

6Structure Styles of Mesozoic-Cenozoic U-bearing Rock Series in Northern China显示文摘In Northern China,sandstone-type uranium(U) deposits are mostly developed in MesozoicCenozoic basins. These U deposits are usually hosted in unvarying horizons within the basins and exhibit typical U-forming sedimentary associations,which is referred to as U-bearing rock series. This study describes the structural features of U-bearing rock series within the main Mesozoic-Cenozoic Uproducing continental basins in Kazakhstan,Uzbekistan,and Russia in the western segment of the Central Asian Metallogenic Belt(CAMB),and Northern China in the eastern segment of the CAMB. We analyze the basic structural conditions and sedimentary environments of U-bearing rock series in Northern China and classify their structural styles in typical basins into river valley,basin margin,and intrabasin uplift margin types. The intrabasin uplift margin structural style proposed in this study can be used to indicate directions for the exploration of sandstone-type U deposits hosted in the center of a basin. 7Fatty liver index vs waist circumference for predicting non-alcoholic fatty liver disease显示文摘AIM: To determine the discriminatory performance of fatty liver index(FLI) for non-alcoholic fatty liver disease(NAFLD).METHODS: The data of 5052 subjects aged over 18 years were analyzed. FLI was calculated from body mass index, waist circumference(WC), triglyceride, and gamma glutamyl transferase data. Logistic regression analysis was conducted to determine the association between FLI and NAFLD. The discriminatory performance of FLI in the diagnosis of NAFLD was evaluated by receiver operating characteristic analysis. Area under the curves(AUCs) and related confidence intervals were estimated. Optimal cutoff points of FLI in the diagnosis of NAFLD were determined based on the maximum values of Youden's index.RESULTS: The mean age of men and women in the study population were 44.8 ± 16.8 and 43.78 ± 15.43, respectively(P = 0.0216). The prevalence of NAFLD was 40.1% in men and 44.2% in women(P < 0.0017). FLI was strongly associated with NAFLD, so that even a one unit increase in FLI increased the chance of developing NAFLD by 5.8%(OR = 1.058, 95%CI: 1.054-1.063, P < 0.0001). Although FLI showed good performance in the diagnosis of NAFLD(AUC = 0.8656(95%CI: 0.8548-0.8764), there was no significant difference with regards to WC(AUC = 0.8533, 95%CI: 0.8419-0.8646). The performance of FLI was not significantly different between men(AUC = 0.8648, 95%CI: 0.8505-0.8791) and women(AUC = 0.8682, 95%CI: 0.8513-0.8851). The highest performance with regards to age was related to the 18-39 age group(AUC = 0.8930, 95%CI: 0.8766-0.9093). 9Psychosocial stress and liver disease status显示文摘'Psychosocial stress' is an increasingly common concept in the challenging and highly-demanding modern society of today. Organic response to stress implicates two major components of the stress system, namely the hypothalamic-pituitary-adrenal axis and the sympathetic nervous system. Stress is anamnestically reported by patients during the course of disease, usually accompanied by a decline in their overall health status. As the mechanisms involving glucocorticoids and catecholamines have been deciphered, and their actions on immune cell function deeper understood, it has become clear that stress has an impact on hepatic inflam-matory response. An increasing number of articles have approached the link between psychosocial stress and the negative evolution of hepatic diseases. 10Experimental Analysis on Shielding Effect of the Grounding Electrodes Under Impulse-current显示文摘Generally,the flow of a lightning impulse current from a grounding electrode into ground is a very complicated process determined by many factors.In order to analyze the mechanism of the impulse current dissipating in the earth from grounding electrode,the experiments that had been carried out by other authors almost used a single horizontal grounding wire or vertical grounding rod for sake of simplicity.However,in practical conditions,most of the grounding systems are constructed of grounding electrodes with branches in different directions.In this study,basing on the principle of dimensional similarity,impulse simulation experiments are performed on the common ground electrodes with conductor branches.This paper focuses on analyzing the impulse current dispersal regularity of different branches when injecting at one point.Comparing with the leakage current distribution of a single ground electrode,it is found that the leakage currents along the branches increase with the distance to the current feed point,and the more conductors near the injection point,the more uneven the leakage current distribution is.This work indicates that shielding effect should be taken into account when analyzing the impulse characteristics of grounding electrodes.YANG Cai-wei SIMA Wen-xia YUAN Tao YANG Qing 2008高电压技术2008,34,12:7
11DAMAGE DETECTION IN STRUCTURES USING MODIFIED BACK-PROPAGATION NEURAL NETWORKS显示文摘A nonparametric structural damage detection methodology based on neural networks method ispresented for health monitoring of structure-unknown systems.In this approach appropriate neural networksare trained by use of the modal test data from a ‘healthy’structure.The trained networks which are subse-quently fed with vibration measurements from the same structure in different stages have the capability of rec-ognizing the location and the content of structural damage and thereby can monitor the health of the structure.A modified back-propagation neural network is proposed to solve the two practical problems encountered bythe traditional back-propagation method,i.e.,slow learning progress and convergence to a false local mini-mum.Various training algorithms,types of the input layer and numbers of the nodes in the input layer areconsidered.Numerical example results from a 5-degree-of-freedom spring-mass structure and analyses on theexperimental data of an actual 5-storey-steel-frame demonstrate that neural-networks-based method is a robustprocedure and a practical tool for the detection of structural damage,and that the modified back-propagationalgorithm could improve the computational efficiency as well as the accuracy of detection.Sima Yuzhou 2002Acta Mechanica Solida Sinica2002,15,4:6

14Correlation Analysis Algorithm for Transmission Line Fault Location Based on Travelling Wave显示文摘This paper introduced correlation method to locate transmission line fault.First it described the principle of transmission line fault location based on traveling waves.The principle of correlation analysis is introduced,then the method using correlation analysis in fault location is given.Transmission line model is established with EMTP-ATP.Basing on the model,some kinds of fault are simulated.The feasibility of this algorithm is proved based on simulation results.By comparing with the classical wavelet analysis,this paper gave the advantages of this algorithm in two cases:noise influence suppression and accuracy of near distance fault location.Experiment is established to simulate transmission line grounding fault.The experiment result showed the correlation algorithm's validity.All the analysis result indicated that the correlation algorithm have a high precision.DU Lin PANG Jun SIMA Wen-xia 2008高电压技术2008,34,12:5

16Inoperable esophageal cancer and outcome of palliative care显示文摘AIM:To determine the outcome of esophageal cancer patients referred for palliative care,in Gorgan and Gonbad gastrointestinal clinics,northeast of Iran. METHODS:This cross-sectional study was done on inoperable esophageal cancer cases referred to gastrointestinal clinics in Gorgan and Gonbad city(2005-2006) . Demographic data were collected during the procedure and cases were followed up every one month. Improvement proportion was calculated with 95% confidence interval,to determine the rate of improvement. Survival analysis and Kaplan-Meier methods were used to estimate the duration of palliative care effectiveness. RESULTS:We recruited 39 cases into the study. Squamous cell carcinoma was the most prevalent(92.3%) . The middle third of the esophagus was involved predominantly(51.3%) . Dilation was the most preferred method(89.7%) and stenting was done in 4 cases. Decreasing dysphagia score was not related to palliation method or pathology type of carcinoma. Age of the patients was significantly related to the improvement of dysphagia score. Mean survival time was 137.6 d and median was 103 d. 17Identification of the involvement of LOXL4 in generation of keratocystic odontogenic tumors by RNA-Seq analysis显示文摘Keratocystic odontogenic tumors(KCOT) are benign, locally aggressive intraosseous tumors of odontogenic origin. KCOT have a higher stromal microvessel density(MVD) than dentigerous cysts(DC) and normal oral mucosa. To identify genes in the stroma of KCOT involved in tumor development and progression, RNA sequencing(RNA-Seq) was performed using samples from KCOT and primary stromal fibroblasts isolated from gingival tissues. Seven candidate genes that possess a function potentially related to KCOT progression were selected and their expression levels were confirmed by quantitative PCR, immunohistochemistry and enzyme-linked immunosorbent assay. Expression of lysyl oxidase-like 4(LOXL4), the only candidate gene that encodes a secreted protein, was enhanced at both the mRNA and protein levels in KCOT stromal tissues and primary KCOT stromal fibroblasts compared to control tissues and primary fibroblasts(P,0.05). In vitro, high expression of LOXL4 could enhance proliferation and migration of the human umbilical vein endothelial cells(HUVECs). There was a significant, positive correlation between LOXL4 protein expression and MVD in stroma of KCOT and control tissues(r50.882). 19Endoscopic management and prevention of migrated esophageal stents显示文摘The use of self-expandable metallic stents has increased recently to palliate inoperable esophageal neoplasia and also in the management of benign strictures. Migration is one of the most common complications after stent placement and the endoscopist should be able to recognize and manage this situation. Several techniques for managing migrated stents have been described, as well as new techniques for preventing stent migration. Most stents have a 'lasso' at the upper flange which facilitates stent repositioning or removal. An overtube, endoloop and large polypectomy snare may be useful for the retrieval of stents migrated into the stomach. External fixation of the stent with Shim's technique is efficient in preventing stent migration. 20青蒿琥酯治疗妊娠期重症疟疾患者的疗效观察显示文摘目的:探讨青蒿琥酯(Artesunate)治疗妊娠期重症疟疾的疗效。方法:对2008年8月至2009年12月到非洲赤道几内亚共和国首都马拉博医院住院治疗的60例妊娠期重症疟疾患者,采用注射用青蒿琥酯缓慢静脉注射,每天一次注射60?,连用3-5d,第1天剂量加倍,对高原虫密度(红细胞感染率≥10%)的患者,第1天首剂120?,注射后4-6h,再注射一次60?。静脉注射3-5d后,当患者病情好转时,改口服青蒿琥酯片,2次/d,一次50mg,直至完成7d疗程。取外周血制作厚薄血涂片行显微镜检查治疗前后的疟原虫情况。结果:全部患者给药后疟疾临床症状消失,外周血疟原虫密度均有一定程度的下降,绝大部分患者疟原虫消失,经治疗一个疗程后,60例患者中有58例治愈,治愈率96.7%。结论:注射用青蒿琥酯静脉注射及口服青蒿琥酯片对妊娠期重症疟疾患者的治疗效果满意,且无明显不良反应,值得临床推广应用。刘瑞爱 邓敏茹 Antonio Sima Eworo Okomo 2010河北医学2010,16,7:3
