1Golgi protein-73:A biomarker for assessing cirrhosis and prognosis of liver disease patients显示文摘BACKGROUND Reliable biomarkers of cirrhosis,hepatocellular carcinoma(HCC),or progression of chronic liver diseases are missing.In this context,Golgi protein-73(GP73)also called Golgi phosphoprotein-2,was originally defined as a resident Golgi type II transmembrane protein expressed in epithelial cells.As a result,GP73 expression was found primarily in biliary epithelial cells,with only slight detection in hepatocytes.However,in patients with acute or chronic liver diseases and especially in HCC,the expression of GP73 is significantly up-regulated in hepatocytes.So far,few studies have assessed GP73 as a diagnostic or prognostic marker of liver fibrosis and disease progression.AIM To assess serum GP73 efficacy as a diagnostic marker of cirrhosis and/or HCC or as predictor of liver disease progression.METHODS GP73 serum levels were retrospectively determined by a novel GP73 ELISA(QUANTA Lite®GP73,Inova Diagnostics,Inc.,Research Use Only)in a large cohort of 632 consecutive patients with chronic viral and non-viral liver diseases collected from two tertiary Academic centers in Larissa,Greece(n=366)and Debrecen,Hungary(n=266).Aspartate aminotransferase(AST)/Platelets(PLT)ratio index(APRI)was also calculated at the relevant time points in all patients.Two hundred and three patients had chronic hepatitis B,183 chronic hepatitis C,198 alcoholic liver disease,28 autoimmune cholestatic liver diseases,15 autoimmune hepatitis,and 5 with other liver-related disorders.The duration of follow-up was 50(57)mo[median(interquartile range)].The development of cirrhosis,liver decompensation and/or HCC during follow-up were assessed according to internationally accepted guidelines.In particular,the surveillance for the development of HCC was performed regularly with ultrasound imaging and alpha-fetoprotein(AFP)determination every 6 mo in cirrhotic and every 12 mo in non-cirrhotic patients.RESULTS Increased serum levels of GP73(>20 units)were detected at initial evaluation in 277 out of 632 patients(43.8%).GP73-seropositivity correlated at baseline with the presence of cirrhosis(96.4%vs 51.5%,P<0.001),decompensation of cirrhosis(60.3%vs 35.5%,P<0.001),presence of HCC(18.4%vs 7.9%,P<0.001)and advanced HCC stage(52.9%vs 14.8%,P=0.002).GP73 had higher diagnostic accuracy for the presence of cirrhosis compared to APRI score[Area under the curve(AUC)(95%CI):0.909(0.885-0.934)vs 0.849(0.813-0.886),P=0.003].Combination of GP73 with APRI improved further the accuracy(AUC:0.925)compared to GP73(AUC:0.909,P=0.005)or APRI alone(AUC:0.849,P<0.001).GP73 levels were significantly higher in HCC patients compared to non-HCC[22.5(29.2)vs 16(20.3)units,P<0.001)and positively associated with BCLC stage[stage 0:13.9(10.8);stage A:17.1(16.8);stage B:19.6(22.3);stage C:32.2(30.8);stage D:45.3(86.6)units,P<0.001]and tumor dimensions[very early:13.9(10.8);intermediate:19.6(18.4);advanced:29.1(33.6)units,P=0.004].However,the discriminative ability for HCC diagnosis was relatively low[AUC(95%CI):0.623(0.570-0.675)].Kaplan-Meier analysis showed that the detection of GP73 in patients with compensated cirrhosis at baseline,was prognostic of higher rates of decompensation(P=0.036),HCC development(P=0.08),and liver-related deaths(P<0.001)during follow-up.CONCLUSION GP73 alone appears efficient for detecting cirrhosis and superior to APRI determination.In combination with APRI,its diagnostic performance can be further improved.Most importantly,the simple GP73 measurement proved promising for predicting a worse outcome of patients with both viral and nonviral chronic liver diseases.Nikolaos K Gatselis Tamás Tornai Zakera Shums Kalliopi Zachou Asterios Saitis Stella Gabeta Roger Albesa Gary L Norman Mária Papp George N Dalekos 2020World Journal of Gastroenterology2020,26,34:21
2From Eliza to XiaoIce:challenges and opportunities with social chatbots显示文摘Conversational systems have come a long way since their inception in the 1960 s.After decades of research and development,we have seen progress from Eliza and Parry in the 1960 s and 1970 s,to task-completion systems as in the Defense Advanced Research Projects Agency(DARPA) communicator program in the 2000 s,to intelligent personal assistants such as Siri,in the 2010 s,to today's social chatbots like Xiao Ice.Social chatbots' appeal lies not only in their ability to respond to users' diverse requests,but also in being able to establish an emotional connection with users.The latter is done by satisfying users' need for communication,affection,as well as social belonging.To further the advancement and adoption of social chatbots,their design must focus on user engagement and take both intellectual quotient(IQ) and emotional quotient(EQ) into account.Users should want to engage with a social chatbot;as such,we define the success metric for social chatbots as conversation-turns per session(CPS).Using Xiao Ice as an illustrative example,we discuss key technologies in building social chatbots from core chat to visual awareness to skills.We also show how Xiao Ice can dynamically recognize emotion and engage the user throughout long conversations with appropriate interpersonal responses.As we become the first generation of humans ever living with artificial intelligenc(AI),we have a responsibility to design social chatbots to be both useful and empathetic,so they will become ubiquitous and help society as a whole.Heung-yeung SHUM Xiao-dong HE Di LI 2018Frontiers of Information Technology & Electronic Engineering2018,19,1:11

4Polarization-maintaining few mode fiber composed of a central circular-hole and an elliptical-ring core显示文摘We propose a novel waveguide design of polarization-maintaining few mode fiber(PM-FMF) supporting ≥10non-degenerate modes, utilizing a central circular air hole and a circumjacent elliptical-ring core. The structure endows a new degree of freedom to adjust the birefringence of all the guided modes, including the fundamental polarization mode. Numerical simulations demonstrate that, by optimizing the air hole and elliptical-ring core,a PM-FMF supporting 10 distinctive polarization modes has been achieved, and the effective index difference Δn_(eff) between the adjacent guided modes could be kept larger than 1.32 × 10^(-4) over the whole C +L band. 5Cartilage oligomeric matrix protein: A novel non-invasive marker for assessing cirrhosis and risk of hepatocellular carcinoma显示文摘AIM: To assess serum cartilage oligomeric matrix protein(COMP) as a marker of cirrhosis and risk of progression to hepatocellular carcinoma(HCC). METHODS: A COMP enzyme-linked immunosorbentassay was used to test 187 patients with chronic liver diseases at the time point of first evaluation. The selected patients included 72 with chronic hepatitis B infection, 75 with chronic hepatitis C infection, 22 with primary biliary cirrhosis, 7 with autoimmune hepatitis type 1, and 11 with alcoholic liver disease. Demographic, biochemical, histological and clinical characteristics of the patients were recorded at the first evaluation. One hundred and forty-seven patients were followed for a median [interquartile range(IQR)] duration of 96.5(102) mo. The clinical, biochemical and histological data, as well as the development of cirrhosis, HCC according to internationally accepted criteria and in case of death, a liver-related cause during the follow-up period, were recorded at the electronic database of our clinic. COMP determination was also performed in 43 healthy individuals who served as the control study group.RESULTS: COMP positivity(> 15 U/L) was detected in 22%-36% among chronic liver disease groups. Strikingly, almost 83% of COMP-positive patients were cirrhotic at baseline, independently of cause of liver disease. Among the patients who developed HCC during follow-up, 73.7%(14/19) were COMP positive at baseline. COMP positivity was significantly associated with older age(P < 0.001), advanced fibrosis(P = 0.001) and necroinflammatory activity(P = 0.001), higher aspartate aminotransferase(P < 0.001), alanine aminotransferase(P < 0.02), γ-glutamyl transpeptidase(P = 0.003), alkaline phosphatase(P = 0.001), bilirubin(P < 0.05), international normalized ratio(P = 0.002) and alpha-fetoprotein levels(P < 0.02), and lower albumin(P < 0.001), and platelet count(P = 0.008). COMP levels [median(IQR)] were significantly higher in cirrhotics compared to non-cirrhotics [13.8(7.9) U/L vs 9.8(4.6) U/L, respectively; P < 0.001]. On multivariate logistic regression analysis, COMP-positivity was independently associated only with cirrhosis(OR = 4.40, 95%CI: 1.33-14.69, P = 0.015). Kaplan-Meier analysis showed that COMP positivity was significantly associated with HCC development(P = 0.007) and higher incidence of liver-related death(P < 0.001). CONCLUSION: Elevated COMP levels are strongly associated with cirrhosis and HCC progression. 6Progress in Neural NLP:Modeling,Learning,and Reasoning显示文摘Natural language processing(NLP)is a subfield of artificial intelligence that focuses on enabling computers to understand and process human languages.In the last five years,we have witnessed the rapid development of NLP in tasks such as machine translation,question-answering,and machine reading comprehension based on deep learning and an enormous volume of annotated and unannotated data.In this paper,we will review the latest progress in the neural network-based NLP framework(neural NLP)from three perspectives:modeling,learning,and reasoning.In the modeling section,we will describe several fundamental neural network-based modeling paradigms,such as word embedding,sentence embedding,and sequence-to-sequence modeling,which are widely used in modern NLP engines.In the learning section,we will introduce widely used learning methods for NLP models,including supervised,semi-supervised,and unsupervised learning;multitask learning;transfer learning;and active learning.We view reasoning as a new and exciting direction for neural NLP,but it has yet to be well addressed.In the reasoning section,we will review reasoning mechanisms,including the knowledge,existing non-neural inference methods,and new neural inference methods.We emphasize the importance of reasoning in this paper because it is important for building interpretable and knowledgedriven neural NLP models to handle complex tasks.At the end of this paper,we will briefly outline our thoughts on the future directions of neural NLP.Ming Zhou Nan Duan Shujie Liu Heung-Yeung Shum 2020Engineering2020,6,3:6

9On U-orthodox semigroups显示文摘As a generalization of an orthodox semigroup in the class of regular semigroups, a type W semigroup was first investigated by El-Qallali and Fountain. As an analogy of the type W semi-groups in the class of abundant semigroups, we introduce the U-orthodox semigroups. It is shown that the maximum congruence μ contained in HU on U-orthodox semigroups can be determined. As a consequence, we show that a U-orthodox semigroup S can be expressed by the spined product of a Hall semigroup WU and a V-ample semigroup (T,V ). 10Gut barrier failure biomarkers are associated with poor disease outcome in patients with primary sclerosing cholangitis显示文摘AIM To assess the prevalence of a panel of serologic markers that reflect gut barrier dysfunction in a mixed cohort of pediatric and adult primary sclerosing cholangitis(PSC) patients.METHODS Sera of 67 PSC patients [median age(range): 32(5-79) years, concomitant IBD: 67% and cirrhosis: 20%] were assayed for the presence of antibodies against to F-actin(AAA Ig A/Ig G) and gliadin(AGA Ig A/Ig G)] and for serum level of intestinal fatty acid-binding protein(I-FABP) by ELISA. Markers of lipopolysaccharide(LPS) exposure [LPS binding protein(LBP)] and various antimicrobial antibodies [anti-OMP Plus Ig A and endotoxin core Ig A antibody(Endo CAb)] were also determined. Poor disease outcome was defined as orthotopic liver transplantation and/or liver-related death during the follow-up [median: 99(14-106) mo]. One hundred and fifty-three healthy subjects(HCONT) and 172 ulcerative colitis(UC) patients were the controls. RESULTS A total of 28.4%, 28.0%, 9% and 20.9% of PSC patients were positive for AAA Ig A, AAA Ig G, AGA Ig A and AGA Ig G, respectively. Frequencies of AAA Ig A and AAA Ig G(P < 0.001, for both) and AGA Ig G(P = 0.01, for both) but not AGA Ig A were significantly higher compared to both of the HCONT and the UC groups. In survival analysis, AAA Ig A-positivity was revealed as an independent predictor of poor disease outcome after adjusting either for the presence of cirrhosis [HR = 5.15(1.27-20.86), P = 0.022 or for the Mayo risk score(HR = 4.24(0.99-18.21), P = 0.052]. AAA Ig A-positivity was significantly associated with higher frequency of antimicrobial antibodies(P < 0.001 for Endo Cab Ig A and P = 0.012 for anti-OMP Plus Ig A) and higher level of the enterocyte damage marker(median I-FABP_(AAA Ig A pos vs neg): 365 vs 166 pg/m L, P = 0.011), but not with serum LBP level. CONCLUSION Presence of Ig A type AAA identified PSC patients with progressive disease. 12Non-pulmonary allergic diseases and inflammatory bowel disease: A qualitative review显示文摘While the etiological underpinnings of inflammatory bowel disease(IBD) are highly complex, it has been not-ed that both clinical and pathophysiological similarities exist between IBD and both asthma and non-pulmonary allergic phenomena. In this review, several key points on common biomarkers, pathophysiology, clinical manifesta-tions and nutritional and probiotic interventions for both IBD and non-pulmonary allergic diseases are discussed.Histamine and mast cell activity show common behav-iors in both IBD and in certain allergic disorders. IgE also represents a key immunoglobulin involved in both IBD and in certain allergic pathologies, though these links require further study. Probiotics remain a critically important intervention for both IBD subtypes as well as multiple allergic phenomena. Linked clinical phenomena, especially sinonasal disease and IBD, are discussed. In addition, nutritional interventions remain an underuti-lized and promising therapy for modification of both al-lergic disorders and IBD. Recommending new mothers breastfeed their infants, and increasing the duration of breastfeeding may also help prevent both IBD and al-lergic diseases, but requires more investigation. 13The progress in optic nerve regeneration, where are we?显示文摘Optic nerve regeneration is an important area of research. It can be used to treat patients suffering from optic neuropathy and provides insights into the treatment of numerous neurodegenerative diseases. There are many hurdles impeding optic regeneration in mammals. The mammalian central nervous system is non-permissive to regeneration and intrinsically lacks the capacity for axonal regrowth. Any axonal injury also triggers a vicious cycle of apoptosis. Understanding these hurdles provides us with a rough framework to appreciate the essential steps to bring about optic nerve regeneration: enhancing neuronal survival, axon regeneration, remyelination and establishing functional synapses to the original neuronal targets. 17Effects of Gaussian filter in processing GRACE data: Gravity rate of change at Lhasa,southern Tibet显示文摘In this paper, the spatial gravity distribution over Tibetan Plateau and the gravity rate of change at Lhasa for different Gaussian filter radii are computed using GRACE data. Results show that the estimate of the gravity rate of change is spatial radius-dependent of the Gaussian filter. The GRACE-estimated gravity rate of change agrees well with the surface measured one. In other words, the GRACE-estimated gravity rate of change has a limited value as that obtained by surface measurement when the spatial filter radius reaches zero. Then numerical simulations are made for different spatial radii of the Gaussian filter to investigate its behaviors when applied to surface signals. Results show that the estimate of a physical signal is filter-radius dependent. If the computing area is equal to or less than the mass area, especially for a uniformly distributed mass, the estimate gives an almost correct result, no matter what filter radius is used. The estimate has large error because of the signal leakage caused by harmonic truncation if the computing area is much bigger than the mass distribution (or inverse for a small mass anomaly). If a mass anomaly is too small, it is difficult to recover it from space observation unless the filter radius is extremely small. If the computing point (or area) is outside the mass distribution, the estimated result is almost zero, particularly for small filter radii. These properties of the Gaussian filter are helpful in applying GRACE data in different geophysical problems with different spatial position and geometrical size. We further discuss physical sources causing the scalar gravity change at Lhasa. Discussions indicate that the gravity rate of change at Lhasa is not caused by the present-day ice melting (PDIM) (or Little Ice Age, LIA) effect because no ice melting occurs in Lhasa city and nearby. The gravity rate of change is attributable mainly to tectonic deformation associated with the Indian Plate collision. 18Chinese topical herbal medicine gives additive effect on pharmaceutical agent on fracture healing显示文摘OBJECTIVE:To investigate the efficacy on the combination of oral strontium ranelate(Sr R)with a topical Chinese herbal paste on facilitation of fracture healing.METHODS:An open fracture was created at the mid-shaft of the right tibia of rat.A herbal pastecalled CDR containing Honghua(Flos Carthami),Chuanxuduan(Radix Dipsaci Asperoidis)and Dahuang(Radix Et Rhizoma Rhei Palmati)was prepared.The rats were treated with either CDR topically on the fracture site,or Sr R orally,or their combinations.Bone turnover biochemical markers in serum were measured.Microarchitecture of the fracture was analyzed using micro-CT after 14 and 28 d,followed by histomorphometrical analysis.RESULTS:Micro-computed tomography analysis revealed that the combined treatment of CDR with600 mg/g Sr R significantly increased the total callus density,mineralized callus volume fraction,mineralized callus mineral content and mineralized callus density of the callus after 28 d of treatment.This result was consistent with the histomorphometrical analysis on the osteoid volume.Analysis of biochemical markers showed that the combined treatments reduced the bone resorption that occurs temporarily after fracture.CONCLUSION:This study demonstrated that the combined treatment of oral Sr R and topical CDR is effective to promote fracture healing by their additive effect on promoting bone formation and retarding bone resorption.Siu Wing Sum Shiu Hoi Ting Shum Wai Ting Ko Chun Hay Lau Clara Bik San Hung Leung Kim Leung Ping Chung 2019Journal of Traditional Chinese Medicine2019,39,6:4
19Design of photonic crystal fiber with elliptical air-holes to achieve simultaneous high birefringence and nonlinearity显示文摘A new type of V-shaped photonic crystal fiber with elliptical air-holes is proposed to realize simultaneous high birefringence and nonlinearity at a wavelength of 1.55μm.The full vector finite element method was adopted to investigate its characteristics,including birefringence,nonlinearity,and dispersion.The PCF exhibited a very high birefringence of 2.89×10^(-2) and very high nonlinear coefficient of 102.69 W^(-1)·km^(-1).In particular,there were two zero-dispersion wavelengths(ZDWs)in the visible(X:640-720 nm and Y:730-760 nm)and near-infrared regions(X:1050-1606 nm and Y:850-1500 nm).The combination of high birefringence and nonlinearity allowed the PCF to maintain the polarization state and generate a broadband super continuum,with potential applications in nonlinear optics.刘敏 侯静云 杨虚 赵昺玥 Ping Shum 2018Chinese Physics B2018,27,1:4
