| 1 | Treatment of young patients with lupus nephritis using calcineurin inhibitors显示文摘Recent advances in the management of lupus nephritis,together with earlier renal biopsy and selective use of aggressive immunosuppressive therapy,have contributed to a favorable outcome in children and adolescents with systemic lupus erythematosus(SLE).Nevertheless,we believe that a more effective and less toxic treatment is needed to attain an optimal control of the activity of lupus nephritis.Recent published papers and our experiences regarding treatment of young patients with lupus nephritis using calcineurin inhibitors are reviewed.Although it has been reported that intermittent monthly pulses of intravenous cyclophosphamide(IVCY)are effective for preserving renal function in adult patients,CPA is a potent immunosuppressive agent that induces severe toxicity,including myelo-and gonadal toxicity,and increases the risk of secondary malignancy.Thus,treatment for controlling lupus nephritis activity,especially in children and adolescents,remains challenging.Cyclosporine A(Cs A)and tacrolimus(Tac)are T-cell-specific calcineurin inhibitors that prevent the activation of helper T cells,thereby inhibiting thetranscription of the early activation genes of interleukin(IL)-2 and suppressing T cell-induced activation of tumor necrosis factor-α,IL-1βand IL-6.Therefore,both drugs,which we believe may be less cytotoxic,are attractive therapeutic options for young patients with lupus nephritis.Recently,a multidrug regimen of prednisolone(PDN),Tac,and mycophenolate mofetile(MMF)has been found effective and relatively safe in adult lupus nephritis.Since the mechanisms of action of MMF and Tac are probably complementary,multidrug therapy for lupus nephritis may be useful.We propose as an alternative to IVCY,a multidrug therapy with mizoribine,which acts very similarly to MMF,and Tac,which has a different mode of action,combined with PDN for pediatric-onset lupus nephritis.We also believe that a multidrug therapy including Cs A and Tac may be an attractive option for young patients with SLE and lupus | Hiroshi Tanaka Kazushi Tsuruga Tomomi Aizawa-Yashiro Shojiro Watanabe Tadaatsu Imaizumi | 2012 | World Journal of Nephrology2012，1，6： | 8 |
| 2 | Acquired amegakaryocytic thrombocytopenia previously diagnosed as idiopathic thrombocytopenic purpura in a patient with hepatitis C virus infection显示文摘We report the first case of a patient with hepatitis C virus(HCV) infection and idiopathic thrombocytopenic purpura(ITP), who later developed acquired amegakaryocytic thrombocytopenia(AAMT), with autoantibodies to the thrombopoietin(TPO) receptor(c-Mpl). A 64-year-old woman, with chronic hepatitis C, developed severe thrombocytopenia and was diagnosed with ITP. She died of liver failure. Autopsy revealed cirrhosis and liver carcinoma. In the bone marrow, a marked reduction in the number of megakaryocytes was observed, while other cell lineages were preserved. Therefore, she was diagnosed with AAMT. Additionally, autoantibodies to c-Mpl were detected in her serum. Autoantibodies to c-Mpl are one of the causes of AAMT, acting through inhibition of TPO function, megakaryocytic maturation, and platelet formation. HCV infection induces several autoantibodies. HCV infection might also induce autoantibodies to c-Mpl, resulting in the development of AAMT. | 3 | The spectral changes of partially coherent light focused by an apertured lens with chromatic aberration显示文摘The spectral changes of a partially coherent polychromatic light focused by an apertured lens with chromatic aberration are investigated. It is demonstrated that the spectrum in the focused field is different from that in the aperture. Comparing with the spectrum in the aperture, the spectrum in the focused field shifts to lower or higher frequency, which is defined as a spectral shift. The influence of chromatic aberration of the lens, the coherence of the partially coherent light in the aperture, the radius of the aperture, and the spectral width of the spectrum of the aperture on the spectral shift are investigated in detail. | 5 | Clinical and computed tomography findings of appendiceal diverticulitis vs acute appendicitis显示文摘AIM: To study the clinical features and computed tomography(CT) findings of appendiceal diverticulitis vs acute appendicitis.METHODS: We retrospectively reviewed the records of 451 patients who had undergone appendectomy inour in stitution from January 2007 to September 2012. Patient demographics, clinical features, pathological findings, and surgical outcomes were analyzed. We also compared preoperative CT images of 25 patients with appendiceal diverticulitis with those of 25 patients with acute appendicitis.RESULTS: Among 451 patients, 44(9.7%) were diagnosed to have appendiceal diverticulitis and 398(86.9%) to have acute appendicitis. Patients with appendiceal diverticulitis were older(59 vs 37 years, P < 0.001) and had a longer duration of the illness(4.0 d vs 1.0 d, P < 0.001). Perforation rates in patients with appendiceal diverticulitis were higher(68% vs 27%, P < 0.001). The appendix could be visualized in only 13 patients(52%) among the appendiceal diverticulitis cases, but in all acute appendicitis cases. CT findings suggestive of appendiceal diverticulitis included the absence of fluid collection in the appendix(84% vs 12%, P < 0.001), absence of appendicolith(92% vs 52%, P = 0.005), and formation of abscess(68% vs 16%, P < 0.001). Appendiceal diverticula were identified in 6 patients(24%).CONCLUSION: Among patients who had undergone appendectomy, 9.7% had appendiceal diverticulitis.
