| 12 | Gallbladder carcinoma in a pregnant patient with Crohn's disease complicated with gallbladder involvement显示文摘Primary gallbladder (GB) carcinoma and Crohn's disease (CD) of the GB are individually rare. We present a case of a pregnant woman with CD found to have GB involvement and primary GB carcinoma. A 34-year-old female at 6 wk gestation with a 21 year history of CD of uncertain extent presented with 3 mo of diarrhea, urgency and abdominal pain. During work-up, she was found to have elevated transaminases and an abnormal alkaline phosphatase. Imaging revealed two gallbladder polyps both greater than 1 cm in size. Resection and histological evaluation was consistent with Crohn's involvement of the GB, poorly differentiated adenocarcinoma of the GB with invasion through the muscularis propria and matted lymph nodes in the porta hepatis positive for metastatic carcinoma (stage pT2N1). Six cases of CD involving the GB, two cases of primary GB carcinoma in CD, and ten cases of cholangiocarcinoma in pregnancy have been published.This is the only case that describes all three factors. Common features in CD of the GB include acute cholecystitis, ileal involvement, and presence independent of active intestinal disease. Common features in CD patients with GB malignancy include younger age of detection, a long history of CD, extensive colonic and ileal involvement of disease, the absence of cholelithiasis, and pre-existing gallbladder disease (primary sclerosing cholangitis and gallbladder polyps). Pregnancy is specific to this case. The role of CD in the development of GB malignancy is not well understood nor is the contribution of pregnancy to the spread of disease. | 13 | A Credit-Based Approach for Overcoming Free-Riding Behaviour in Peer-to-Peer Networks显示文摘The underlying premise of peer-to-peer(P2P)systems is the trading of digital resources among individual peers to facilitate file sharing,distributed computing,storage,collaborative applications and multimedia streaming.So-called free-riders challenge the foundations of this system by consuming resources from other peers without offering any resources in return,hindering resource exchange among peers.Therefore,immense effort has been invested in discouraging free-riding and overcoming the ill effects of such unfair use of the system.However,previous efforts have all fallen short of effectively addressing free-riding behaviour in P2P networks.This paper proposes a novel approach based on utilising a credit incentive for P2P networks,wherein a grace period is introduced during which free-riders must reimburse resources.In contrast to previous approaches,the proposed system takes into consideration the upload rate of peers and a grace period.The system has been thoroughly tested in a simulated environment,and the results show that the proposed approach effectively mitigates free-riding behaviour.Compared to previous systems,the number of downloads from free-riders decreased while downloads by contributing peers increased.The results also show that under longer grace periods,the number of downloads by fast peers(those reimbursing the system within the grace period)was greater than the number of downloads by slow peers. | Manal Hazazi Afnan Almousa Heba Kurdi Shiroq Al-Megren Shada Alsalamah | 2019 | Computers, Materials & Continua2019,,4: | 0 |
| 14 | HealthyBlockchain for Global Patients显示文摘An emerging healthcare delivery model is enabling a new era of clinical care based on well-informed decision-making processes.Current healthcare information systems(HISs)fall short of adopting this model due to a conflict between information security needed to implement the new model and those already enforced locally to support traditional care models.Meanwhile,in recent times,the healthcare sector has shown a substantial interest in the potential of using blockchain technology for providing quality care to patients.No blockchain solution proposed so far has fully addressed emerging cross-organization information-sharing needs in healthcare.In this paper,we aim to study the use of blockchain in equipping struggling HISs to cope with the demands of the new healthcare delivery model,by proposing HealthyBlockchain as a granular patient-centered ledger that digitally tracks a patient’s medical transactions all along the treatment pathway to support the care teams.The patient-centered ledger is a neutral tamper-proof trail timestamp block sequence that governs distributed patient information across the decentralized discrete HISs.HealthyBlockchain connects patients,clinicians,and healthcare providers to facilitate a transparent,trustworthy,and secure supporting platform. | Shada A.Alsalamah Hessah A.Alsalamah Thamer Nouh Sara A.Alsalamah | 2021 | Computers, Materials & Continua2021,,8: | 0 |
