| 1 | Prognostic significance of the neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio in patients with stage Ⅲ and Ⅳcolorectal cancer显示文摘AIM To evaluate the prognostic value of the neutrophil-tolymphocyte ratio(NLR) and platelet-to-lymphocyte ratio(PLR) in patients with colorectal cancer(CRC).METHODS Between April 1996 and December 2010, medical records from a total of 1868 patients with CRC were retrospectively reviewed. The values of simple inflammatory markers including NLR and PLR in predicting the long-term outcomes of these patients were evaluated using Kaplan-Meier curves and Cox regression models.RESULTS The median follow-up duration was 46 mo(interquartile range, 22-73). The estimation of NLR and PLR was based on the time of diagnosis. In multivariate Cox regression analysis, high NLR (≥ 3.0) and high PLR(≥ 160) were independent risk factors predicting poor long-term outcomes in patients with stage Ⅲ and Ⅳ CRC. | 2 | Transparent-cap-fitted colonoscopy shows higher performance with cecal intubation time in difficult cases显示文摘AIM:To investigate the efficacy of cap-fitted colonoscopy(CFC) with regard to cecal intubation time.METHODS:Two hundred and ninety-five patients undergoing screening colonoscopy at Gospel Hospital,Kosin University College of Medicine were enrolled in this randomized controlled trial between January and December 2010.Colonoscopies were conducted by a single endoscopist.Patient characteristics including age,sex,body mass index,history of abdominal surgery,quality of preparation,and the presence of diverticulosis were recorded.RESULTS:One hundred and fifty patients were allocated into a CFC group and 145 into a non-CFC(NCF) group.Cecal intubations were achieved in all patients.Cecal intubation time in the CFC group was significantly shorter than in the NCF group for specific conditions:age ≥ 60 years,prior abdominal surgery,and poor bowel preparation.The number of detected adenomas was higher in the CFC group than in the NCF group(P = 0.040).CONCLUSION:CFC facilitated shortening of the cecal intubation time in difficult cases,and was more sensitive for detecting adenomas than was NCF. | Hyung Hun Kim Seun Ja Park Moo In Park Won Moon Sung Eun Kim | 2012 | World Journal of Gastroenterology2012,18,16: | 7 |
| 3 | Effects of vagus nerve preservation and vagotomy on peptide YY and body weight after subtotal gastrectomy显示文摘AIM:To investigate the relationship between the function of vagus nerve and peptide YY 3-36 and ghrelin levels after subtotal gastrectomy.METHODS:We enrolled a total of 16 patients who underwent subtotal gastrectomy due to gastric cancer.All surgeries were performed by a single skilled surgeon.We measured peptide YY 3-36,ghrelin,leptin,insulin,growth hormone levels,and body weight immediately before and one month after surgery.RESULTS:Vagus nerve preservation group showed less body weight loss and less increase of peptide YY 3-36 compared with vagotomy group(-5.56 ± 2.24 kg vs-7.85 ± 1.57 kg,P = 0.037 and 0.06 ± 0.08 ng/mL vs 0.19 ± 0.12 ng/mL,P = 0.021,respectively).Moreover,patients with body weight loss of less than 10% exhibited reduced elevation of peptide YY 3-36 level,typically less than 20% [6(66.7%) vs 0(0.0%),P = 0.011,odd ratio = 3.333,95% confidence interval(1.293,8.591)].CONCLUSION:Vagus nerve preservation contributes to the maintenance of body weight after gastrectomy,and this phenomenon may be related to the suppressed activity of peptide YY 3-36. | Hyung Hun Kim Moo In Park Sang Ho Lee Hyun Yong Hwang Sung Eun Kim Seun Ja Park Won Moon | 2012 | World Journal of Gastroenterology2012,18,30: | 6 |
| 4 | Significant risk and associated factors of active tuberculosis infection in Korean patients with inflammatory bowel disease using anti-TNF agents显示文摘AIM:To evaluate the incidence and risk factors of Korean tuberculosis(TB) infection in patients with inflammatory bowel disease(IBD) undergoing anti-TNF treatment.METHODS:The data of IBD patients treated with anti-TNFs in 13 tertiary referral hospitals located in the southeastern region of Korea were collected retrospectively.They failed to show response or were intolerant to conventional treatments,including steroids or immunomodulators.Screening measures for latent TB infection(LTBI)and the incidence and risk factors ofactive TB infection after treatment with anti-TNFs were identified.RESULTS:Overall,376 IBD patients treated with antiTNF agents were recruited(male 255,mean age of anti-TNF therapy 32.5±13.0 years);277 had Crohn’s disease,99 had ulcerative colitis,294 used infliximab,and 82 used adalimumab.Before anti-TNF treatment,screening tests for LTBI including an interferon gamma release assay or a tuberculin skin test were performed in 82.2%of patients.Thirty patients(8%)had LTBI.Sixteen cases of active TB infection including one TB-related mortality occurred during 801 personyears(PY)follow-up(1997.4 cases per 100000 PY)after anti-TNF treatment.LTBI(OR=5.76,95%CI:1.57-21.20,P=0.008)and WBC count<5000 mm3(OR=4.5,95%CI:1.51-13.44,P=0.007)during follow-up were identified as independently associated risk factors.CONCLUSION:Anti-TNFs significantly increase the risk of TB infection in Korean patients with IBD.The considerable burden of TB and marked immunosuppression might be attributed to this risk. | Eun Soo Kim Geun Am Song Kwang Bum Cho Kyung Sik Park Kyeong Ok Kim Byung Ik Jang Eun Young Kim Seong Woo Jeon Hyun Seok Lee Chang Heon Yang Yong Kook Lee Dong Wook Lee Sung Kook Kim Tae Oh Kim Jonghun Lee Hyung Wook Kim Sam Ryong Jee Seun Ja Park Hyun Jin Kim | 2015 | World Journal of Gastroenterology2015,21,11: | 5 |
| 5 | Bevacizumab as a second- or later-line of treatment for metastatic colorectal cancer显示文摘AIM: To determine the efficacy of bevacizumab in patients with metastatic colorectal cancer (MCRC) who have failed prior chemotherapy without bevacizumab. METHODS: Between March 2002 and June 2010, 40 patients in South Korea with MCRC who were treated with bevacizumab plus chemotherapy as a second or later-line treatment were analyzed retrospectively for their overall response rate (ORR), overall survival (OS), and progression-free survival (PFS). The tumor responses were assessed using the RECIST (Response Evaluation Criteria in Solid Tumors) guidelines. RESULTS: All of the patients had progressed under prior chemotherapy without bevacizumab. Three patients (7.5%) exhibited an ORR, twenty one patients (52.5%) exhibited stable disease (SD), and fifteen patients (37.5%) exhibited disease progression. The median duration of the OS and PFS were 14.0 and 6.13 mo respectively. The median OSs were 16.60, 14.07 and 13.00 mo for second-line, third-line and fourth- or later-line treatments, respectively. The median PFSs were 7.23, 7.30 and 3.87 mo for the second-line, third-line and fourth- or later-line treatments, respectively. | 6 | Extensive frontal focused ultrasound Check for updates mediated blood-brain barrier opening for the treatment of Alzheimer's disease:a proof-of-concept study显示文摘Background:Focused ultrasound(FUS)-mediated blood-brain barrier(BBB)opening has shown efficacy in removal of amyloid plaque and improvement of cognitive functions in preclinical studies,but this is rarely reported in clinical studies.This study was conducted to evaluate the safety,feasibility and potential benefits of repeated extensive BBB opening.Methods:In this open-label,prospective study,six patients with Alzheimer's disease(AD)were enrolled at Severance Hospital in Korea between August 2020 and September 2020.Five of them completed the study.FUS-mediated BBB opening,targeting the bilateral frontal lobe regions over 20 cm^(3),was performed twice at three-month intervals.Magnetic resonance imaging,^(18)F-Florbetaben(FBB)positron emission tomography,Caregiver-Administered Neuropsychiatric Inventory(CGA-NPI)and comprehensive neuropsychological tests were performed before and after the procedures.Results:FUS targeted a mean volume of 21.1±2.7 cm^(3)and BBB opening was confirmed at 95.7%±9.4%of the targeted volume.The frontal-to-other cortical region FBB standardized uptake value ratio at 3 months after the procedure showed a slight decrease,which was statistically significant,compared to the pre-procedure value(-1.6%,0.986 vs 1.002,P=0.043).The CGA-NPI score at 2 weeks after the second procedure significantly decreased compared to baseline(2.2±3.0 vs 8.6±6.0,P=0.042),but recovered after 3 months(5.2±5.8 vs 8.6±6.0,P=0.89).No adverse effects were observed.Conclusions:The repeated and extensive BBB opening in the frontal lobe is safe and feasible for patients with AD.In addition,the BBB opening is potentially beneficial for amyloid removal in AD patients. | So Hee Park Kyoungwon Baik Seun Jeon Won Seok Chang Byoung Seok Ye Jin Woo Chang | 2021 | Translational Neurodegeneration2021,10,4: | 3 |
| 7 | Impact of Fusobacterium nucleatum in the gastrointestinal tract on natural killer cells显示文摘BACKGROUND Gut microbial dysbiosis contributes to the development and progression of colorectal cancer(CRC).Natural killer(NK)cells are involved in early defense mechanisms to kill infective pathogens and tumor cells by releasing chemokines and cytokines.To better understand the relationship between the gut microbiome and CRC,it was hypothesized here that a high abundance of Fusobacterium nucleatum(F.nucleatum)in the gastrointestinal tract could cause reduced NK cell activity.AIM To identify associations between gastrointestinal tract F.nucleatum levels and NK cell activity.METHODS In vitro experiments were performed on NK cells treated with F.nucleatum,Peptostreptococcus anaerobius,and Parvimonas micra to identify the effects of gut microbiome species on NK cells.Following 24 and 48 h of treatment,NK cell counts were measured.In parallel studies,C57BL/6 mice were given broadspectrum antibiotics in their drinking water to reduce resident gut flora.After 3 wk,the mice received the various bacterial species or phosphate-buffered saline(PBS)via oral gavage every 2 d for 6 wk.At the study end,blood samples were acquired to perform NK cell activity assessment and cytokine analysis.Intestinal tissues were collected and analyzed via immunohistochemistry(IHC).RESULTS The data show that after 3 wk of broad-spectrum antibiotic treatment,levels of total bacteria and F.nucleatum were markedly decreased in mice.Gavage of F.nucleatum significantly decreased NK cell activity relative to the activities of cells from mice treated with antibiotics only and PBS.The administration of F.nucleatum decreased the proportion of NK46+cells based on IHC staining and increased the production of interleukin-1βand tumor necrosis factor-α.CONCLUSION High levels of F.nucleatum in the gastrointestinal tract reduced NK cell activity in mice,and the decrease in NK cell activity might be affected by increased proinflammatory cytokines after F.nucleatum treatment. | Yeon Ji Kim Bu Kyung Kim Seun Ja Park Jae Hyun Kim | 2021 | World Journal of Gastroenterology2021,27,29: | 3 |
| 8 | Second-line bismuth-containing quadruple therapy for Helicobacter pylori eradication and impact of diabetes显示文摘AIM To investigate Helicobacter pylori (H. pylori) eradication rates using second-line bismuth-containing quadruple therapy and to identify predictors of eradication failure.METHODS This study included 636 patients who failed firstline triple therapy and received 7d of bismuthcontaining quadruple therapy between January 2005 and December 2015. We retrospectively demonstrated H. pylori eradication rates with respect to the year of therapy as well as demographic and clinical factors. H. pylori eradication was confirmed by a 13C-urea breath test or a rapid urease test at least 4 wk after the completion of bismuth-based quadruple therapy: proton pump inhibitor, metronidazole, bismuth, and tetracycline.RESULTS The overall eradication rates by intention-to-treat analysis and per-protocol analysis were 73.9%( 95% CI : 70.1 %- 77.4%) and 94.5% (95% CI : 92.4%-96.5%), respectively. Annual eradication rates from 2005 to 2015 were 100.0%, 92.9%, 100.0%, 100.0%, 100.0%, 97.4%, 100.0%, 93.8%, 84.4%, 98.9%, and 92.5%, respectively, by per-protocol analysis. | 9 | Early-stage primary signet ring cell carcinoma of the colon显示文摘Primary signet ring cell carcinoma of the colorectum detected at an early stage is very rare; most cases are detected at an advanced stage. Therefore, its progno-sis is poorer than that of ordinary colorectal cancer. A 56-year-old Korean man was seen at this hospital for management of signet ring cell carcinoma of the co-lon. Colonoscopic examination revealed a Ⅱa-like, ill-defined and flatly elevated 9-mm residual tumor in the cecum. Endoscopic mucosal resection was preformed. Pathological examination of the resected specimen re-vealed signet ring cell carcinoma that had invaded the lamina propria without venous or perineural invasion. Abdominal computed tomography (CT) and positron CT showed no evidence of primary lesions or distant me-tastasis. An additional laparoscopic right-hemicolectomy was performed; no residual tumor or lymph node me-tastasis was found. | 11 | Assessment of colon polyp morphology: Is education effective?显示文摘AIM To determine the inter-observer variability for colon polyp morphology and to identify whether education can improve agreement among observers.METHODS For purposes of the tests, we recorded colonoscopy video clips that included scenes visualizing the polyps. A total of 15 endoscopists and 15 nurses participated in the study. Participants watched 60 video clips of the polyp morphology scenes and then estimated polyp morphology(pre-test). After education for 20 min, participants performed a second test in which the order of 60 video clips was changed(post-test). To determine if the effectiveness of education was sustained, four months later, a third, follow-up test was performed with the same participants.RESULTS The overall Fleiss' kappa value of the inter-observer agreement was 0.510 in the pre-test, 0.618 in the post-test, and 0.580 in the follow-up test. The overall diagnostic accuracy of the estimation for polyp morphology in the pre-, post-, and follow-up tests was 0.662, 0.797, and 0.761, respectively. After education, the inter-observer agreement and diagnostic accuracy of all participants improved. However, after four months, the inter-observer agreement and diagnostic accuracy of expert groups were markedly decreased, and those of beginner and nurse groups remained similar to pre-test levels.
