| 1 | Acute-on-chronic liver failure in a multi-ethnic Asian city:A comparison of patients identified by Asia-Pacific Association for the Study of the Liver and European Association for the Study of the Liver definitions显示文摘AIM To explore the applicability of the Asia-Pacific Association for the Study of the Liver(APASL) and European Association for the Study of the Liver(EASL) guidelines for acute-on-chronic liver failure(ACLF) in profiling patients and determining the outcome.METHODS Patients admitted to a tertiary hospital in Singapore with acute decompensation of liver disease from January 2004to July 2014 are screened for ACLF according to the APASL and EASL criteria. The patients' data(including basic demographics, information about existing chronic liver disease, information about the acute decompensation, relevant laboratory values during admission, treatment, and outcome) are retrospectively analyzed to determine the background, precipitating factors and outcome.RESULTS A total of 458 liver patients is analyzed, and 78 patients with ACLF are identified. Sixty-three patients(80.8%) meet the APASL criteria, 64 patients(82.1%) meet the EASL criteria, and 49 patients(62.8%) fulfilled both criteria. The most common causes of acute liver injury are bacterial infections(59.0%), hepatitis B flare(29.5%), and variceal bleeding(24.4%). The common aetiologies of the underlying chronic disease included hepatitis B(43.6%), alcoholic(20.5%) and cryptogenic(11.5%) liver disease. The overall mortality rate is 61.5%. Increased age, the number of organ failures(as per CLIF-SOFA score), peak creatinine, INR, and amylase levels are associated with increased mortality or the need for liver transplantation. 14.3% of patients undergo liver transplantation with a 100% 1-year survival rate. | 2 | Studies on the seasonal variations in the proximate composition of ascidians from the Palk Bay,Southeast coast of India显示文摘Objective:To investigate the seasonal fluctuations of the proximate composition of the ascidians muscle.Methods:The moisture content was estimated by drying 1 g of fresh tissue at a constant temperature at 105(?)for 24 h.The loss of weight was taken as moisture content.The total protein was estimated using the Biuret method.The total carbohydrate in dried sample was estimated spectrophotometrically following the phenol-sulphuric acid method.The lipid in the dried sample tissue was gravimetrically estimated following the chloroform-methanol mixture method.Ash content was determined gravimetrically by incinerating 1 g dried sample in muffle furnace at about 550℃for 6 h and results are expressed in percentage.Results:It was found very difficult to compare the monthly variations,as all the ten species,exhibited wide fluctuations in their proximate compositions.For the sake of convenience,average seasonal values were calculated by summing the monthly values.Conclusions:The proximate composition of the 10 commonly available ascidians showed high nutritive value and hence these groups especially solitary ascidians can be recommended for human consumption in terms of pickles,soup,curry and others after ensuring the safety of consumers. | Ananthan G Karthikeyan MM Selva Prabhu A Raghunathan C | 2012 | Asian Pacific Journal of Tropical Biomedicine2012,2,10: | 3 |
| 3 | Simultaneous colorectal and parenchymal-sparing liver resection for advanced colorectal carcinoma with synchronous liver metastases:Between conventional and mini-invasive approaches显示文摘The optimal timing of surgery in case of synchronous presentation of colorectal cancer and liver metastases is still under debate.Staged approach,with initial colorectal resection followed by liver resection(LR),or even the reverse,liver-first approach in specific situations,is traditionally preferred.Simultaneous resections,however,represent an appealing strategy,because may have perioperative risks comparable to staged resections in appropriately selected patients,while avoiding a second surgical procedure.In patients with larger or multiple synchronous presentation of colorectal cancer and liver metastases,simultaneous major hepatectomies may determine worse perioperative outcomes,so that parenchymal-sparing LR should represent the most appropriate option whenever feasible.Mini-invasive colorectal surgery has experienced rapid spread in the last decades,while laparoscopic LR has progressed much slower,and is usually reserved for limited tumours in favourable locations.Moreover,mini-invasive parenchymal-sparing LR is more complex,especially for larger or multiple tumours in difficult locations.It remains to be established if simultaneous resections are presently feasible with mini-invasive approaches or if we need further technological advances and surgical expertise,at least for more complex procedures.This review aims to critically analyze the current status and future perspectives of simultaneous resections,and the present role of the available miniinvasive techniques. | Emilio De Raffele Mariateresa Mirarchi Dajana Cuicchi Ferdinando Lecce Riccardo Casadei Claudio Ricci Saverio Selva Francesco Minni | 2020 | World Journal of Gastroenterology2020,26,42: | 3 |
| 4 | Small molecule SMU-CX24 targeting toll-like receptor 3 counteracts inflammation:A novel approach to atherosclerosis therapy显示文摘Toll-like receptor 3(TLR3),as an important pattern recognition receptor(PRR),dominates the innate and adaptive immunity regulating many acute and chronic inflammatory diseases.Atherosclerosis is proved as an inflammatory disease,and inflammatory events involved in the entire process of initiation and deterioration.However,the contribution of TLR3 to atherosclerosis remains unclear.Herein,we identified the clinical relevance of TLR3 upregulation and disease processes in human atherosclerosis.Besides,activation of TLR3 also directly led to significant expression of atherogenic chemokines and adhesion molecules.Conversely,silencing TLR3 inhibited the uptake of oxLDL by macrophages and significantly reduced foam cell formation.Given the aberrance in TLR3 functions on atherosclerosis progression,we hypothesized that TLR3 could serve as novel target for clinical atherosclerosis therapy.Therefore,we developed the novel ellipticine derivative SMU-CX24,which specifically inhibited TLR3(IC_(50)=18.87±2.21 nmol/L).In vivo,atherosclerotic burden was alleviated in Western diet fed ApoE^(−/−)mice in response to SMU-CX24 treatment,accompanying notable reductions in TLR3 expression and inflammation infiltration within atherosclerotic lesion.Thus,for the first time,we revealed that pharmacological downregulation of TLR3 with specific inhibitor regenerated inflammatory environment to counteract atherosclerosis progression,thereby proposing a new strategy and probe for atherosclerosis therapy. | Xiaohong Cen Baoqu Wang Yuqing Liang Yanlin Chen Yu Xiao Shaohua Du Kutty Selva Nandakumar Hang Yin Shuwen Liu Kui Cheng | 2022 | Acta Pharmaceutica Sinica B2022,12,9: | 2 |
| 5 | Antiproliferative role of Indig ofera aspalathoides on 20 methylcholanthrene induced fibrosarcoma in rats显示文摘Objective:To find out the anticancer effect of indigofera aspalathoides(L.aspalathoides)on20-methylcholanthrene induced fibrosarcoma in rats.Methods:Fibrosarcoma was induced in Wistar strain male albino rats by 2O-methylcholanthrene.Intraperitoneous(i.p.)administration of250 mg/kg body weight/day of aqueous extract of I.aspalathoides for 30 d effectively suppressed chemically induced tumors.Parameters such as body weight,liver and kidney weight,tumor weight,mean survival time,behavioral changes,blood glucose,blood glycogen and marker enzymes such as alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),acid phosphatase(ACP)and 5'-nucleiotidase(5'-NT)in serum,liver and kidney and lipid profiles such as total cholesterel,phospholipids,free fatty acids in liver and kidney of control and experimental animals were studied.Results:Fibrosarcoma bearing animals were ferocious and anxious.The mean survival time was found to increase after the treatment.The body weights were significantly decreased(P<0.001)in groupⅡfibrosarcoma animals which steadily increased after the treatment with I.aspalathoides.The liver and kidney weights were significantly increased whereas the tumor weights decreased as compared to the weights in untreated fibrosarcoma bearing rats.The blood glucose and the liver and kidney glycogen levels were found to decrease significantly(P<0.001)in groupⅡanimals.Elevated activities of marker enzymes were observed in serum,liver and kidney of fibrosarcoma bearing GroupⅡanimals which were normalize after I.aspalathoides treatment.In the liver and kidney of GroupⅡanimals the total cholesterol increased whereas the pbospbolipids and free fatty acid levels decreased(P<0.001)which were normalized after treatment.Conclusions:The treatment by I.aspalathoides on fihrosarcoma bearing rats has improved the levels of various parameters indicating its antiproliferative and anticancer activity. | Sivagnanam Selva Kumar Mudiganti Ram Krishna Rao Maruthaiveeran Periyasamy Balasubramanian | 2012 | Asian Pacific Journal of Tropical Biomedicine2012,2,12: | 2 |
| 6 | 高强度炎症反应诱导关节软骨特异性瓜氨酸化抗原表达显示文摘目的探讨关节软骨蛋白瓜氨酸表位的表达情况,以及瓜氨酸化表位对抗瓜氨酸蛋白抗体(ACPAs)的免疫应答的影响。方法体外诱导实验:实验组加不同浓度梯度的脂多糖、热灭活细菌(大肠杆菌、金色葡萄球菌)或二型胶原(CII)特异性单克隆抗体等不同刺激物诱导体外培养关节软骨蛋白瓜氨酸化,等量PBS作为对照组,利用特异性结合瓜氨酸化表位的单克隆抗体ACC4(IgG1)进行免疫组织化学染色,ACC1(IgG2a)作为阳性对照,Hy2.15(IgG1)和L243(IgG2a)分别作为同型阴性对照,显微镜下观察瓜氨酸化蛋白表达;体内诱导实验:SD大鼠右膝关节内注射不同浓度脂多糖作为实验组,左膝关节内注射等量PBS为对照组,3 d后取膝关节组织,制成冰冻切片,利用上述抗体进行免疫组化染色,显微镜下观察瓜氨酸化蛋白表达;建立不同种系的胶原诱导性关节炎(CIA)小鼠模型,评价其疾病发生率和严重程度,同时收集小鼠血清与高滴度抗瓜氨酸化抗体的类风湿性关节炎患者血清,采用液相芯片技术检测小鼠及类风湿患者血清中抗瓜氨酸化蛋白抗体水平,并用免疫组织化学检测关节炎小鼠关节软骨瓜氨酸化蛋白的表达水平。结果特异性结合瓜氨酸化CII表位的ACC4抗体未能结合至经不同诱导的关节软骨组织;与健康对照组相比,ACC4抗体和含高滴度的抗瓜氨酸化蛋白抗体的类风湿性关节炎患者血清中的抗体均能结合至CIA小鼠关节炎症关节软骨周围的滑膜组织;成功构建不同种系小鼠类风湿性关节炎模型,其发病率和疾病严重程度无统计学差异(P>0.05);液相芯片技术分析结果显示,小鼠血清中的抗瓜氨酸化抗体低表达或几乎不表达,但抗CII三螺旋结构多肽的抗体反应性较强。结论轻度急性炎症反应不足以引起关节软骨蛋白瓜氨酸化,其特异性抗原表位的表达需要高强度的炎症反应。二型胶原诱导的小鼠关节炎模型中炎症关节软骨蛋白可表达瓜氨酸化抗原表位,但瓜氨酸化抗原表位的表达不足以引起小鼠体内抗瓜氨酸化抗体的免疫应答,其免疫应答需要更多的刺激信号来诱导。 | 覃桂城 林晓吟 梁培彬 李艳鹏 周春 Kutty Selva Nandakumar Rikard Holmdahl | 2020 | 南方医科大学学报2020,40,8: | 2 |
