| 1 | Complications of endoscopic retrograde cholangiography in the post-MRCP era: A tertiary center experience显示文摘AIM: To evaluate our experience in endoscopic retrograde cholangio-pancreatography (ERCP) in terms of fulfilling the ASGE guidelines in indications, positive findings,and complications in the post-magnetic resonance cholangiopancreatography (MRCP) era.METHODS: Between November 2001 and February 2003, consecutive ERCP cases were prospectively evaluated with regard to the indications, findings, cannulation techniques,devices used during the procedure, sedation given,duration of procedure, and complications. These data were entered in a database for subsequent processing and analysis.RESULTS: Of 336 cases, 21.4% were diagnostic and 78.6% therapeutic ERCP. The indications for ERCP fulfilled the ASGE guidelines in 323 cases (96.1%). Suspected bile duct stone was the most frequent indication (26.8%), and this was followed by cholangitis (24.4%), dilated common bile duct (14.9%), and cholestatic jaundice (13.4%).Cannulation success rate was 94%. Biliary sphincterotomy was performed in 175 (52.1%) patients. Repeated ERCP was performed on 31.5% of the patients. Overall, the complication rate was 9.8% with 0.3% being procedurerelated mortality. The complications were pancreatitis (5.4%), bleeding (0.8%), cholangitis (2.4%) and others (1.5%). | 2 | Effect of intermetallic compounds on the fracture behavior of dissimilar friction stir welding joints of Mg and Al alloys显示文摘Joining Mg to Al is challenging because of the deterioration of mechanical properties caused by the formation of intermetallic compounds(IMCs) at the Mg/Al interface. This study aims to improve the mechanical properties of welded samples by preventing the fracture location at the Mg/Al interface. Friction stir welding was performed to join Mg to Al at different rotational and travel speeds. The microstructure of the welded samples showed the IMCs layers containing Al12Mg17(γ) and Al3Mg2(β) at the welding zone with a thickness(< 3.5 μm). Mechanical properties were mainly affected by the thickness of the IMCs, which was governed by welding parameters. The highest tensile strength was obtained at 600 r/min and 40 mm/min with a welding efficiency of 80%. | 18 | Outcome of lamivudine-resistant hepatitis B virus is generally benign except in cirrhotics显示文摘AIM: We set to determine factors that determine clinical severity after the development of resistance.METHODS: Thirty-five Asian patients with genotypic lamivudine resistance were analyzed in three groups:13/35 (37%) were non-cirrhotics with normal pre-treatment ALT (Group IA), 12/35 (34%) were non-cirrhotics with elevated pre-treatment ALT (Group IB), and 10/35 (29%)were cirrhotics (Group Ⅱ). Patients were followed for a median of 98 wk (range 26-220) after the emergence of genotypic resistance.RESULTS: Group IA patients tended to retain normal ALT.Group IB patients showed initial improvement of ALT with lamivudine but 9/12 patients (75%) developed abnormal ALT subsequently. On follow-up however, this persisted in only 33%. Group Ⅱ patients also showed improvement while on treatment, but they deteriorated with the emergence of resistance with 30% death from decompensated liver disease. Pretreatment ALT levels and CPT score (in the cirrhotic group) were predictive of clinical resistance and correlated with peak ALT levels and CPT score.CONCLUSION: The phenotype of lamivudine-resistant HBV correlated with the pretreatment phenotype.
