| 1 | Intraocular lens power calculation in eyes with previous corneal refractive surgery显示文摘Background:This review aims to explain the reasons why intraocular lens(IOL)power calculation is challenging in eyes with previous corneal refractive surgery and what solutions are currently available to obtain more accurate results.Review:After IOL implantation in eyes with previous LASIK,PRK or RK,a refractive surprise can occur because (i)the altered ratio between the anterior and posterior corneal surface makes the keratometric index invalid;(ii)the corneal curvature radius is measured out of the optical zone;and (iii)the effective lens position is erroneously predicted if such a prediction is based on the post-refractive surgery corneal curvature.Different methods are currently available to obtain the best refractive outcomes in these eyes,even when the perioperative data(i.e.preoperative corneal power and surgically induced refractive change)are not known.In this review,we describe the most accurate methods based on our clinical studies.Conclusions:IOL power calculation after myopic corneal refractive surgery can be calculated with a variety of methods that lead to relatively accurate outcomes,with 60 to 70%of eyes showing a prediction error within 0.50 diopters. | Giacomo Savini Kenneth J.Hoffer | 2018 | Eye and Vision2018，5，1： | 10 |
| 2 | Medical treatment for gastro-entero-pancreatic neuroendocrine tumours显示文摘Gastro-entero-pancreatic neuroendocrine neoplasms(GEPNENs) represents a various family of rare tumours. Surgery is the first choice in GEP-NENs patients with localized disease whilst in the metastatic setting many other treatment options are available. Somatostatin analogues are indicated for symptoms control in functioning tumours. Furthermore they may be effective to inhibit tumour progression. GEP-NENs pathogenesis has been extensively studied in the last years therefore several driver mutations pathway genes have been identified as crucial factors in their tumourigenesis. GEP-NENs can over-express vascular endothelial growth factor(VEGF), basic-fibroblastic growth factor, transforming growth factor(TGF-α and-β), platelet derived growth factor(PDGF), insulin-like growth factor-1(IGF-1) and their receptors PDGF receptor, IGF-1 receptor, epidermal growth factor receptor, VEGF receptor, and c-kit(stem cell factor receptor) that can be considered as potential targets. The availability of new targeted agents, such as everolimus and sunitinib that are effective in advanced and metastatic pancreatic neuroendocrine tumours, has provided new treatment opportunities. | 3 | Road induced edge effects on a forest bird community in tropical Asia显示文摘Background: Edge effects cause changes in bird community richness, abundance, and/or distribution within a landscape, but the avian guilds most influenced can vary among regions. Although Southeast Asia has the highest rates of deforestation and projected species loss, and is currently undergoing an explosive growth in road infrastructure, there have been few studies of the effects of forest edges on avian communities in this region.Methods: We examined avian community structure in a dry evergreen forest in northeastern Thailand adjacent to a five-lane highway. We evaluated the richness and abundance of birds in 11 guilds at 24 survey points on three parallel transects perpendicular to the edge. At each point, 10-min surveys were conducted during February-August 2014 and March-August 2015. Vegetation measurements were conducted at 16 of the bird survey points and ambient noise was measured at all 24 survey points.Results: We found a strongly negative response to the forest edge for bark-gleaning, sallying, terrestrial, and understory insectivores and a weakly negative response for arboreal frugivore-insectivores, foliage gleaning insectivores, and raptors. Densities of trees and the percentage canopy cover were higher in the interior, and the ambient noise was lower. In contrast, arboreal nectarivore-insectivores responded positively to the forest edge, where there was a higher vegetation cover in the ground layer, a lower tree density, and a higher level of ambient noise.Conclusion: Planners should avoid road development in forests of high conservation value to reduce impacts on biodiversity. | 4 | Gastro-entero-pancreatic neuroendocrine tumors:Is now time for a new approach?显示文摘Gastro-entero-pancreatic tumors(GEP-NETs) are rare neoplasms often characterized by an overexpression of somatostatin receptors.Thus,radiolabeled somatostatin analogues have showed an increasing relevance both in diagnosis and treatment,especially in low-and intermediate-differentiated GEP-NETs.These evidences have led to a growing development of new functional imaging techniques as 68 Ga-DOTATATE positron emission tomography/computed tomography(PET/CT) proved useful in the management of these neoplasms.However these tumors have a heterogeneous behavior also modifying their aggressiveness through time.Therefore sometimes 18 F-fluorodeoxyglucose PET/CT appears to be more appropriate to obtain a better assessment of the disease.According to these considerations,the combination of different functional imaging techniques should be considered in the management of GEP-NETs patients allowing clinicians to choose the tailored therapeutic approach among available options. | Rossana Berardi Mariangela Torniai Agnese Savini Silvia Rinaldi Stefano Cascinu | 2016 | World Journal of Clinical Oncology2016，7，2： | 2 |
