| 1 | Rotator cuff tears: An evidence based approach显示文摘Lesions of the rotator cuff(RC) are a common occurrence affecting millions of people across all parts of the globe. RC tears are also rampantly prevalent with an agedependent increase in numbers. Other associated factors include a history of trauma, limb dominance, contralateral shoulder, smoking-status, hypercholesterolemia, posture and occupational dispositions. The challenge lies in early diagnosis since a high proportion of patients are asymptomatic. Pain and decreasing shoulder power and function should alert the heedful practitioner in recognizing promptly the onset or aggravation of existing RC tears. Partial-thickness tears(PTT) can be bursalsided or articular-sided tears. Over the course of time, PTT enlarge and propagate into full-thickness tears(FTT) and develop distinct chronic pathological changes due to muscle retraction, fatty infiltration and muscle atrophy. These lead to a reduction in tendon elasticity and viability. Eventually, the glenohumeral joint experiences a series of degenerative alterations- cuff tear arthropathy. To avert this, a vigilant clinician must utilize and corroborate clinical skill and radiological findings to identify tear progression. Modern radio-diagnostic means of ultrasonography and magnetic resonance imaging provide excellent visualization of structural details and are crucial in determining further course of action for these patients. Physical therapy along with activity modifications, antiinflammatory and analgesic medications form the pillars of nonoperative treatment. Elderly patients with minimal functional demands can be managed conservatively and reassessed at frequent intervals. Regular monitoring helps in isolating patients who require surgical interventions. Early surgery should be considered in younger, active and symptomatic, healthy patients. In addition to being costeffective, this helps in providing a functional shoulder witha stable cuff. An easily reproducible technique of maximal strength and sturdiness should by chosen among the armamentarium of the shoulder surgeon. Grade 1 PTTs do well with debridement while more severe lesions mandate repair either by trans-tendon technique or repair following conversion into FTT. Early repair of repairable FTT can avoid appearance and progression of disability and weakness. The choice of surgery varies from surgeon-to-surgeon with arthroscopy taking the lead in the current scenario. The double-row repairs have an edge over the single-row technique in some patients especially those with massive tears. Stronger, costeffective and improved functional scores can be obtained by the former. Both early and delayed postoperative rehabilitation programmes have led to comparable outcomes. Guarded results may be anticipated in patients in extremes of age, presence of comorbidities and severe tear patters. | 2 | Metal organic framework-combustion: A one-pot strategy to NiO nanoparticles with excellent anode properties for lithium ion batteries显示文摘NiO nanoparticles with average particles size of 30 nm are synthesized using a one-pot metal–organic framework-combustion(MOF-C) technique, for use as an anode material in rechargeable lithium ion batteries(LIBs). The structural and electronic properties of these nanoparticles are studied using various techniques, including powder X-ray diffraction(PXRD), transmission electron microscopy(TEM), scanning electron microscopy(SEM), X-ray photoelectron spectroscopy(XPS), and N_2 adsorption/desorption studies. The as-synthesized NiO nanoparticles sustained reversible stable capacities of 748 and 410 mAh/g at applied current densities of 500 and 1000 m A/g, respectively, after 100 cycles. Furthermore, the anode displays a notable rate capability, achieving a stable capacity of ~200 mAh/g at a high current density of10 A/g. These results indicate that the size of the NiO nanoparticles and their high surface area influence their electrochemical properties. | 3 | Phase-pure Na3V2(PO4)2F3 embedded in carbon matrix through a facile polyol synthesis as a potential cathode for high performance sodium-ion batteries显示文摘In this study,a pseudo-layered Na super-ionic conductor of Na3V2(PO4)2F3 (NVPF)/C cathode for sodium-ion batteries is prepared successfully using a facile polyol refluxing process without any impurity phases.The X-ray diffraction and Rietveld refinement results confirm that NVPF possesses tetragonal NASICON-type lattice with a space group of P42/mnm.In this preparative method,polyol is utilized as a solvent as well as a carbon source.The presence of nanosized NVPF particles in the carbon network is confirmed by field-emission scanning electron microscopy (FE-SEM) and high-resolution transmission electron microscopy (HR-TEM).The existence of carbon is analyzed by Raman scattering and elemental analysis.When applied as a Na-storage material in a potential window of 2.0-4.3 V,the electrode exhibits two flat voltage plateaus at 3.7 and 4.2 V with an electrochemically active V^3+/V^4+ redox couple.In addition,Na3V2(PO4)2F3/C composite achieved a retention capacity of ~ 88% even after 1,500 cycles at 15 C.Moreover,at high current densities of 30 and 50 C,Na3V2(PO4)2F3/C cathode retains the specific discharge capacities of 108.4 and 105.9 mAh·g-1,respectively,revealing the structural stability of the material prepared through a facile polyol refluxing method. | Sohyun Park Jinju Song Seyeon Kim Balaji Sambandam Vinod Mathew Sungjin Kim Jeonggeun Jo Seokhun Kim Jaekook Kim | 2019 | Nano Research2019,12,4: | 4 |
| 4 | Proximal fibular osteotomy:Systematic review on its outcomes显示文摘BACKGROUND The morbidity and burden of knee osteoarthritis affecting millions of lives worldwide has created a constant pursuit in finding the ideal treatment for knee osteoarthritis.There has been a paradigm shift in the surgical treatment of osteoarthritis ever since the initial description of Volkmann’s tibial osteotomy.This review focuses on one such recent procedure,the proximal fibular osteotomy(PFO)for medial compartment knee osteoarthritis.This review encompasses the history,evidence,risk factors,outcomes and technical considerations of PFO.AIM To understand the evidence and its techniques,and whether this could be an alternative solution to the problem of knee osteoarthritis in the developing world.METHODS The phrases“proximal fibular osteotomy”and“knee osteoarthritis”were searched(date of search December 20,2019)on PubMed to identify articles evaluating the biomechanical and clinical outcomes of PFO in patients with knee osteoarthritis.A total of 258 were retrieved.After reviewing the summary of the texts,22 articles written in English were marked for abstract review.Articles that were case studies or cadaver experiments were excluded.The abstracts of the remaining articles were read,and only those that focused on the history,outcomes of case studies and technical considerations of PFO were included in the review.A total of 12 articles were included in this review.RESULTS At least six studies reported improvement in the visual analogue scale(VAS)from the average preoperative VAS score[6.32,95%confidence interval(CI)=(4.05,8.59)]to average postoperative VAS score[1.23,95%CI:(-1.20,3.71)],which was statistically significant.Similarly,the American Knee Society Score(KSS)functional score improved from an average preoperative KSS functional score[43.11,95%CI:(37.83,48.38)]to postoperative KSS functional score[66.145,95%CI:(61.94,70.35)],which was statistically significant.The femorotibial angle improved by around 7º,and the hip knee ankle angle improved by around 6º.CONCLUSION With the existing data,it seems that PFO is a viable option for treating medial joint osteoarthritis in selected patients.Long term outcome studies and progression of disease pathology are some of the important parameters that need to be addressed by use of multicenter randomized controlled trials. | Munis Ashraf Prabhudev Prasad Purudappa Vishaal Sakthivelnathan Senthilnathan Sambandam VaratharajMounsamy | 2020 | World Journal of Orthopedics2020,11,11: | 3 |
