| 1 | Rubber band ligation for 750 cases of symptomatic hemorrhoids out of 2200 cases显示文摘AIM: To study the results for the treatment of symptomatic hemorrhoids using rubber band ligation (RBL) method. METHODS: A retrospective study for 750 patients who came to the colorectal unit from June, 1998 to September, 2006, data was retrieved from archived fi les. RBL was performed using the Mc Gown applicator on an outpatient basis. The patients were asked to return to out-patient clinic for follow up at 2 wk, 1 mo, 6 mo and through telephone call every 6 mo for 2 years). RESULTS: After RBL, 696 patients (92.8%) were cured with no difference in outcome for second or third degree hemorrhoids (P = 0.31). Symptomatic recurrence was detected in 11.04% after 2 years. A total of 52 patients (6.93%) had 77 complications from RBL which required no hospitalization. Complications were pain, rectal bleeding and vaso-vagal symptoms(4.13%, 4.13% and 1.33% of patients, respectively). At 1 mo there were a significant improvement in mean SF-36 scores over baseline in five items, while after 2 years there were improvement in all items over baseline, but not significant. No significant manometeric changes after band ligation. CONCLUSION: RBL is a simple, safe and effective method for treating symptomatic second and third degree hemorrhoids as an out patient procedure with signifi cant improvement in quality of life. | 3 | Visual and refractive outcomes of posterior chamber phakic IOL in stable keratoconus显示文摘We evaluated the visual and refractive outcomes after phakic visian toric implantable collamer lens(ICL) insertion in stable keratoconus(KC). This retrospective study investigated toric ICL implantation in 14 eyes of 8 patients with stable KC. After 6 mo, the mean uncorrected distance visual acuity improved significantly from 0.77 to 0.15 logMAR. The mean best corrected distance visual acuity(BCDVA) improved from 0.18±0.1 to 0.15±0.1 logMAR. Fifty percent of eyes maintained their preoperative BCDVA; 42.8% gained one line. There was no statistical difference in high order or coma aberration. The mean refractive manifest spherical equivalent(MSE), mean refractive manifest spherical error, mean manifest astigmatism decreased significantly postoperatively. At 6 mo postoperatively, our achieved mean spherical equivalent was approximately 74%. No intraoperative or postoperative complications occurred. | 5 | Silicon micromirrors with three-dimensional curvature enabling lensless efficient coupling of free-space light显示文摘Miniaturized optical benches process free-space light propagating in-plane with respect to the substrate and have a large variety of applications,including the coupling of light through an optical fiber.High coupling efficiency is usually obtained using assembled micro-optical parts,which considerably increase the system cost and integration effort.In this work,we report a high coupling efficiency,monolithically integrated silicon micromirror with controlled three-dimensional(3D)curvature that is capable of manipulating optical beams propagating in the plane of the silicon substrate.Based on our theoretical modeling,a spherical micromirror with a microscale radius of curvature as small as twice the Gaussian beam Rayleigh range provides a 100%coupling efficiency over a relatively long optical path range.Introducing dimensionless parameters facilitates the elucidation of the role of key design parameters,including the mirror’s radii of curvature,independent of the wavelength.A micromachining method is presented for fabricating the 3D micromirror using fluorinated gas plasmas.The measured coupling efficiency was greater than 50%over a 200-mm optical path,compared to less than 10%afforded by a conventional flat micromirror,which was in good agreement with the model.Using the 3D micromirror,an optical cavity was formed with a round-trip diffraction loss of less than 0.4%,resulting in one order of magnitude enhancement in the measured quality factor.A nearly 100%coupling was also estimated when matching the sagittal and tangential radii of curvature of the presented micromirror’s surface.The reported class of 3D micromirrors may be an advantageous replacement for the optical lenses usually assembled in silicon photonics and optical benches by transforming them into real 3D monolithic systems while achieving wideband high coupling efficiency over submillimeter distances. | Yasser M Sabry Bassam Saadany Diaa Khalil Tarik Bourouina | 2013 | Light(Science & Applications)2013，2，1： | 1 |
