| 1 | Development of a quantum-dot-labelled magnetic immunoassay method for circulating colorectal cancer cell detection显示文摘AIM:To detect of colorectal cancer(CRC) circulating tumour cells(CTCs) surface antigens,we present an assay incorporating cadmium selenide quantum dots(QDs) in these paper.METHODS:The principle of the assay is the immunomagnetic separation of CTCs from body fluids in conjunction with QDs,using specific antibody biomarkers:epithelial cell adhesion molecule antibody,and monoclonal cytokeratin 19 antibody.The detection signal was acquired from the fluorescence signal of QDs.For the evaluation of the performance,the method under study was used to isolate the human colon adenocarcinoma cell line(DLD-1) and CTCs from CRC patients' peripheral blood.RESULTS:The minimum detection limit of the assay was defined to 10 DLD-1 CRC cells/mL as fluorescence was measured with a spectrofluorometer.Fluorescenceactivated cell sorting analysis and Real Time RT-PCR,they both have also been used to evaluate the performance of the described method.In conclusion,we developed a simple,sensitive,efficient and of lower cost(than the existing ones) method for the detection of CRC CTCs in human samples.We have accomplished these results by using magnetic bead isolation and subsequent QD fluorescence detection.CONCLUSION:The method described here can be easily adjusted for any other protein target of either the CTC or the host. | Maria Gazouli Anna Lyberopoulou Pericles Pericleous Spyros Rizos Gerassimos Aravantinos Nikolaos Nikiteas Nicholas P Anagnou Efstathios P Efstathopoulos | 2012 | World Journal of Gastroenterology2012,18,32: | 6 |
| 2 | Hemobilia as the initial manifestation of cholangiocarcinoma in a hemophilia B patient显示文摘Hemobilia is a rare manifestation of hemophilia and is usually iatrogenic following liver biopsy. There are only few reports of spontaneous hemobilia in hemophilia patients. Cholangiocarcinoma is a well-established cause of hemobilia. We describe a case of a 70-year-old male, with known haemophilia B and a past history of papillotomy, who presented with classical symptoms of hemobilia. The initial diagnostic work-up failed to demonstrate a potential cause of bleeding other than the coagulopathy. Three months later, he was readmitted to our hospital with a second episode of hemobilia. During the second work-up, a cholangiocarcinoma was diagnosed both by imaging studies and by a significant elevation of cancer antigen 19-9. | 3 | Cholangiocarcinoma:A 7-year experience at a single center in Greece显示文摘AIM: To evaluate survival rate and clinical outcome of cholangiocarcinoma.METHODS: The medical records of 34 patients with cholangiocarcinoma,seen at a single hospital between the years 1999-2006,were retrospectively reviewed.RESULTS: Thirty-four patients with a median age of 75 years were included.Seventeen (50%) had painless jaundice at presentation.Sixteen (47.1%) were perihilar,15 (44.1%) extrahepatic and three (8.8%) intrahepatic.Endoscopic retrograde cholangiography (ERCP) and/or magnetic resonance cholangiography (MRCP) were used for the diagnosis.Pathologic confirmation was obtained in seven and positive cytological examination in three.Thirteen patients had co-morbidities (38.2%).Four cases were managed with complete surgical resection.All the rest of the cases (30) were characterized as non-resectable due to advanced stage of the disease.Palliative biliary drainage was performed in 26/30 (86.6%).The mean follow-up was 32 mo (95% CI,20-43 mo).Overall median survival was 8.7 mo (95% CI,2-16 mo).The probability of 1-year,2-year and 3-year survival was 46%,20% and 7%,respectively.The survival was slightly longer in patients who underwent resection compared to those who did not,but this difference failed to reach statistical significance.Patients who underwent biliary drainage had an advantage in survival compared to those who did not (probability of survival 53% vs 0% at 1 year,respectively,P = 0.038).CONCLUSION: Patients with cholangiocarcinoma were usually elderly with co-morbidities and/or advanced disease at presentation.Even though a slight amelioration in survival with palliative biliary drainage was observed,patients had dismal outcome without resection of the tumor. | Alexandra Alexopoulou Aspasia Soultati Spyros P Dourakis Larissa Vasilieva Athanasios J Archimandritis | 2008 | World Journal of Gastroenterology2008,14,40: | 3 |
| 4 | Modern diagnostic approaches to cholangiocarcinoma显示文摘BACKGROUND:Cholangiocarcinoma is a very aggressive tumor with poor survival.Therefore,early diagnosis and surgical resection are of paramount importance.Its diagnosis is difficult because access to the tumor is not easy.Biopsy is possible only for intrahepatic cholangiocarcinoma,which accounts for 10% of cases.Routine brush cytology from endoscopic retrograde cholangiopancreatography(ERCP) has a high specificity of 100% but unfortunately a low sensitivity of 30%.In this review we briefly describe new diagnostic techniques applicable to ERCP brush cytology specimens and targeting the genetic background of the disease,in particular fluorescence in situ hybridization(FISH) and digital image analysis(DIA).DATE SOURCES:The PubMed database up to 2011 was used for the retrieval of relevant articles.The search terms FISH,fluorescence in situ hybridization,DIA,digital image analysis and cholangiocarcinoma were used.Both original and review articles were used.RESULTS:FISH identifies cells with chromosomal abnormalities,mainly numerical aberrations,using a mixture of fluorescencelabeled probes.FISH offers a higher sensitivity than routine cytology,retaining a high level of specificity.The DIA criterion for malignancy is demonstration of aneuploidy.This technique increases the sensitivity to 40%,but the specificity remains low.Preliminary data from application to other tumors suggest that combination of FISH and DIA may be of further benefit.CONCLUSIONS:The new techniques offer a significantly enhanced diagnostic efficacy in the evaluation of ERCP brush specimens.Apart from contributing to a more timely diagnosis,their wider application to cholangiocarcinoma may also facilitate the genetic study of the disease and add to our understanding of oncogenesis at the molecular level,with the prospect of identifying targets for novel therapeutic interventions. | Larisa E Vasilieva Stefanos I Papadhimitriou Spyros P Dourakis | 2012 | Hepatobiliary & Pancreatic Diseases International2012,11,4: | 2 |
| 5 | Translation and validation of the Greek chronic liver disease questionnaire显示文摘AIM:To translate into Greek and validate the chronic liver disease questionnaire (CLDQ).METHODS:Two hundred and six consecutive adult patients with the diagnosis of a chronic liver disease from 2 general hospitals in Athens were enrolled in the study from May to September 2008.In order to assess their quality of life (QOL) the CLDQ was applied.The instrument was translated from English,back translated and reviewed in focus groups within the framework of a large multicenter study.The measurements that were performed included:2 independent sample t tests,oneway analysis of variance,reliability coefficients,explanatory factor analysis using a varimax rotation and the principal components method.RESULTS:One hundred and twenty five (61%) patients were men,half were aged 40-59 years and > 33% were > 60 years old.Among the patients,48 (23%) were hospitalized and 97 (47%) were cirrhotic according to the Child-Pugh score.The internal consistency of the Greek CLDQ version using Cronbach's alpha coefficient was found to be 0.93.Exploratory factor analysis identified 7 domains accounting for 65% of the variance of CLDQ items and only partially overlapping with those found in the original version.The area under the receiver operating characteristics curve was calculated at 0.813 and the logistic estimate for the threshold score of 167.50 provided a sensitivity of 74.3% and a specificity of 71.6% for the model.CONCLUSION:Our data confirmed the validity of the Greek version of the CLDQ in identifying the QOL among patients with chronic liver disease. | Zoi Kollia Evridiki Patelarou Victoria Vivilaki Eleni Kollia Florentia Kefou Ioannis Elefsiniotis Spyros P Dourakis Hero Brokalaki | 2010 | World Journal of Gastroenterology2010,16,46: | 2 |
| 6 | Mechanisms of drug-induced proarrhythmia in clinical practice显示文摘Drug-induced proarrhythmia represents a great challenge for those involved in the development of novel pharmaceuticals and in the regulatory bodies for drug approval as well as for the prescribing clinicians.Our understanding of the mechanisms that underlie druginduced proarrhythmia has grown dramatically over the last two decades.A growing number of cardiac and non-cardiac agents have been shown to alter cardiac repolarization predisposing to fatal cardiac arrhythmias such as ventricular tachycardia or ventricular fibrillation and sudden cardiac death.These agents may induce the phenotype of long QT syndrome and less commonly of short QT syndrome and Brugada syndrome(BS).Although,genetic susceptibility underlie drug-induced proarrhythmia in certain cases,current data are limited regarding this topic.The present review surveys the current published literature on the mechanisms and the offending medical agents that predispose to drug-induced long QT syndrome,short QT syndrome and BS.Drug-induced proarrhythmia should be considered as a predictor of sudden cardiac death and should prompt critical re-evaluation of the risks and benefits of the suspicious medication.Survivors of drug-induced proarrhythmia and family members require careful examination and possibly genetic testing for the presence of a channelopathy.Treating physicians are advised to follow the lists of agents implicated in drug-induced proarrhythmia in order to minimize the risk of arrhythmia and sudden cardiac death. | Arkadia Konstantopoulou Spyros Tsikrikas Dimitrios Asvestas Panagiotis Korantzopoulos Konstantinos P Letsas | 2013 | World Journal of Cardiology2013,5,6: | 2 |

| 9 | α2-Heremans-schmid glycoprotein(fetuin A)downregulation and its utility in inflammatory bowel disease显示文摘AIM To investigate the impact of inflammatory bowel disease (IBD) on α2-Heremans-Schmid Glycoprotein(AHSG/fetuin A) and potential associations with disease and patient characteristics.METHODS AHSG serum levels were determined in treatment-na?ve newly-diagnosed patients, 96 with ulcerative colitis(UC), 84 with Crohn's disease (CD), 62 with diarrheapredominant or mixed irritable bowel syndrome (IBS, D- and M- types) and 180 healthy controls (HC), by an enzyme linked immunosorbent assay (ELISA).All patients were followed for a minimum period of 3 years at the Gastroenterology Department of the University Hospital of Larissa, Greece. C-reactive protein(CRP), anti-glycan antibodies, anti-Saccharomyces cerevisiae mannan antibodies Ig G, anti-mannobioside carbohydrate antibodies Ig G, anti-laminariobioside carbohydrate antibodies Ig G and anti-chitobioside carbohydrate antibodies Ig A were also determined via immunonephelometry and ELISA, respectively.RESULTS The mean ± SE of serum AHSG, following adjustment for confounders, was 0.32 ± 0.02 g/L in IBD, 0.32 ± 0.03 g/L in CD and 0.34 ± 0.03 g/L in UC patients, significantly lower than in IBS patients(0.7 ± 0.018 g/L) and HC (0.71 ± 0.02 g/L) (P < 0.0001, in all cases). AHSG levels were comparable between the CD and UC groups. Based on AHSG levels IBD patients could be distinguished from HC with about 90% sensitivity and specificity. Further adjusted analysis verified the inverse association between AHSG and penetrating, as well as stricturing CD (partial correlation coefficient: -0.45 and -0.33, respectively) (P < 0.05). After adjusting for confounding factors, inverse correlations between AHSG and CRP and the need for anti-TNFα therapy or surgery, were found (partial correlation coefficients:-0.31,-0.33,-0.41, respectively, P < 0.05, in all cases). Finally, IBD individuals who were seropositive, for at least one marker, had AHSG levels falling within the two lower quartiles (OR = 2.86, 95%CI: 1.5-5.44, P < 0.001) while those with at least two serological markers positive exhibited AHSG concentrations within the lowest quartile (OR = 5.03, 95%CI: 2.07-12.21, P < 0.001), after adjusting for age, sex and smoking.CONCLUSION AHSG can be used to distinguish between IBD and IBS patients or HC while at the same time 'predicting' complicated disease behavior, need for therapy escalation and surgery.
