1Effectiveness of clip-and-snare method using pre-looping technique for gastric endoscopic submucosal dissection显示文摘AIM:To evaluate efficacy and safety of clip-and-snare method using pre-looping technique(CSM-PLT)for gastric endoscopic submucosal dissection(ESD).METHODS:In the CSM-PLT method,a clip attached to the lesion side was strangulated with a snare,followed by application of an appropriate tension to the lesion independent of an endoscope.Twenty consecutive lesions were resected by ESD using CSM-PLT(CSM-PLT group)and compared with a control group,including20 lesions that were resected by conventional ESD.The control group was matched based on the size and location of the lesion,presence of pathologic fibrosis,and experience of endoscopists.Total procedure time of ESD,proportion of en bloc resection,and complications were analyzed.RESULTS:The total procedure time for the CSM-PLT group was significantly shorter than that for the control group(38.5 min vs 59.5 min,P=0.023);all lesions were resected en bloc by ESD.There was no significant difference in complications between the two groups.Moreover,there was no complication in the CSM-PLT group.In one large lesion(size:74 mm)that underwentextensive CSM-PLT during ESD,we used an additional CSM-PLT on another edge of the lesion after achieving submucosal resection to the maximum extent possible during initial CSM-PLT.In two lesions,the snare came off the lesion together with the clip after a sudden pull;nevertheless,ESD was successful in all lesions.CONCLUSION:CSM-PLT was an effective and safe method for gastric ESD.Naohiro Yoshida Hisashi Doyama Ryosuke Ota Yasuhito Takeda Hiroyoshi Nakanishi Kei Tominaga Shigetsugu Tsuji Kenichi Takemura 2016World Journal of Gastrointestinal Endoscopy2016,8,12:3

3Numerical model of local scour considering the unsteady sediment inflow and sediment sorting: Application to scour upstream of slit weir显示文摘The current study proposes a novel framework for the numerical model for estimating the temporal scour considering unsteady sediment inflow and the sediment sorting process. The framework was applied to local scour upstream of a slit weir. The scour model is based on an ordinary nonlinear differential equation derived from sediment continuity and scour rate equations. A one-dimensional(1-D)Boussinesq-type model coupled with nonequilibrium sediment transport was incorporated in the scour model to consider unsteady sediment inflow. In addition, a mixing layer approach was incorporated to determine temporal variations in particle size in the scour hole. The proposed framework was validated using scour experiments conducted under conditions that include non-uniform sediment, live-bed scour,and unsteady flow rate. The simulation results showed satisfactory agreement with experimental results for the scour upstream of a slit weir. The proposed model appropriately explained the hysteresis process for flow unsteadiness, refilling process for live-bed scour, and armoring process for non-uniform sediments. In addition, the model framework proposed in the current study is applicable to local scour around other hydraulic structures(e.g., bridge piers) upon establishing the scour rate equation. 4Experimental investigation on cohesionless sandy bank failure resulting from water level rising显示文摘In the last decade, sediment replenishment forming cohesionless sandy banks below dams has become an increasingly common practice in Japan to compensate for sediment deficits downstream. The erosion process of the placed cohesionless sediment is a combination of lateral toe-erosion and the following mass failure. To explore cohesionless bank failure mechanisms, a series of experiments was done in a soil tank using a compacted sandy soil mass exposed to an increasing water level. Two types of uniform sand(D_(50) = 0.40 mm and 0.17 mm) and two bank heights(50 cm and 25 cm) were used under the condition of a constant bank slope of 75°. The three dimensional(3D) geometry of the bank after failure was measured using a handheld 3D scanner. The motion of bank failure was captured using the particle image velocimetry(PIV) technique, and the matric suction was measured by tensiometers. The compacted sandy soil was eroded by loss of matric suction accompanying the rise in water level which subsequently caused rotational slide and cantilever toppling failure due to destabilization of the bank. The effect of erosion protection resulting from the slumped blocks after these failures is discussed in the light of different failure mechanisms. Tensile strength is analyzed by inverse calculation of cantilever toppling failure events. The tensile strength had non-linear relation with degree of saturation and showed a peak.The findings of the study show that it is important to incorporate the non-linear relation of tensile strength into stability analysis of cantilever toppling failure and prediction of tension crack depth within unsaturated cohesionless banks.Ryosuke Arai Kazuyuki Ota Takahiro Sato Yasushi Toyoda 2018International Journal of Sediment Research2018,33,1:0
5Current state of practice for colonic diverticular bleeding in 37 hospitals in Japan: A multicenter questionnaire study显示文摘AIM To clarify the current state of practice for colonic diverticular bleeding(CDB) in Japan.METHODS We conducted multicenter questionnaire surveys of the practice for CDB including clinical settings(8 questions),diagnoses(8 questions),treatments(7 questions),and outcomes(4 questions) in 37 hospitals across Japan.The answers were compared between hospitals with high and low number of inpatient beds to investigate which factor influenced the answers.RESULTS Endoscopists at all 37 hospitals answered the questions,and the mean number of endoscopists at these hospitals was 12.7.Of all the hospitals,computed tomography was performed before colonoscopy in 67% of the hospitals.The rate of bowel preparation was 46.0%.Early colonoscopy was performed within 24 h in 43.2% of the hospitals.Of the hospitals,83.8% performed clipping as first-line endoscopic therapy.More than half of the hospitals experienced less than 20% rebleeding events after endoscopic hemostasis.No significant difference was observed in the annual number of patients hospitalized for CDB between high-(≥700 beds) and low-volume hospitals.More emergency visits(P=0.012) and endoscopists(P=0.015),and less frequent participation of nursing staff in early colonoscopy(P=0.045) were observed in the high-volume hospitals.CONCLUSION Some practices unique to Japan were found,such as performing computed tomography before colonoscopy,no bowel preparation,and clipping as first-line therapy.Although,the number of staff differed,the practices for CDB were common irrespective of hospital size.Ryota Niikura Naoyoshi Nagata Hisashi Doyama Ryosuke Ota Naoki Ishii Katsuhiro Mabe Tsutomu Nishida Takuto Hikichi Kazuki Sumiyama Jun Nishikawa Toshio Uraoka Shu Kiyotoki Mitsuhiro Fujishiro Kazuhiko Koike 2016World Journal of Gastrointestinal Endoscopy2016,8,20:0
