| 1 | 可视套管一步法穿刺建立经腹腹腔镜手术观察镜通道的初步经验显示文摘目的 探讨应用可视套管一步法穿刺建立经腹腹腔镜手术观察镜通道的临床价值.方法 选取2015年4-12月120例需行经腹腹腔镜手术患者作为研究对象.所有患者随机分为研究组和对照组,每组60例.研究组男34例,女26例;平均年龄(52.0±11.9)岁;平均体重指数(24.9±2.9) kg/m^2.患者取侧卧位并抬高腰桥,在患侧脐上、腹直肌外缘处预定建立观察镜通道的位置,切开皮肤,采用可视套管在腹腔镜直视下一步法穿刺进入腹腔,置入腹腔镜确认套管进入腹腔后,继续进行后续操作.对照组男36例,女24例;平均年龄(52.9±11.4)岁;平均体重指数(25.2 ±2.4) kg/m^2.患者采用传统逐层切开入腹建立通道的方法.比较两组建立通道的时间、通道漏气率、通道出血率、腹腔镜手术脏器损伤发生率的差异.结果 研究组与对照组相比,通道建立平均时间明显缩短(2.7min vs.15.9 min,P<0.05),通道漏气率明显减少(0 vs.30%,P<0.05).两组均未发生通道出血和腹腔脏器损伤.结论 可视套管一步法穿刺建立经腹腹腔镜手术观察镜通道的方法高效、安全,值得在临床实践中继续探索. | 吴杰英 狄金明 罗云 Rujan Shrestha 方友强 胡成 | 2016 | 中华泌尿外科杂志2016,37,8: | 6 |
| 2 | 回结肠克罗恩病并发肛瘘的CT小肠造影分析显示文摘目的采用CT小肠造影(CTE)探讨克罗恩病回结肠病变与是否伴发肛瘘之间的关系。方法连续收集在本院确诊为克罗恩病回结肠病变活动期且接受CTE及肛管MRI的患者28例,其中肛瘘组16例、无肛瘘组12例,计算患者回盲部及结直肠病变的数目、最大病变的长度及相邻病变间的最小距离,并通过秩和检验分析2组间是否具有差异。结果28例患者中,肛瘘组87.50%(14/16)累及左半结肠或直肠,无肛瘘组50.00%(6/12)累及左半结肠或直肠,差异有统计学意义(Z=-2.135,P<0.05);肛瘘组回结肠克罗恩病的平均病变数目为3.06个,非肛瘘组为2.91个,差异无统计学意义(P>0.05);肛瘘组最大病变长度为(12.79±8.30)cm,无肛瘘组为(7.04±3.09)cm,差异无统计学意义(P>0.05);肛瘘组相邻病变间的最小距离为(5.23±2.98)cm,无肛瘘组为(8.44±2.87)cm,差异有统计学意义(Z=-2.095,P<0.05)。结论克罗恩病是否发生肛瘘与病变位置及相邻病变间的距离有关,而与病变的数目和最大病变长度无关。 | 刘得超 林杨皓 曹务腾 RUJAN Malla 孟晓春 周智洋 | 2017 | 中国医学影像技术2017,33,7: | 6 |
| 3 | 镜下边缘增强缝线桥全层修复肩袖分层撕裂显示文摘[目的]介绍关节镜下边缘增强缝线桥全层修复肩袖分层撕裂的手术技术与初步临床疗效。[方法] 2017年8月~2019年04月对21例肩袖分层撕裂患者进行镜下修复。镜下探明肩袖撕裂的部位,并进行清创,磨钻打磨肱骨大结节,直至磨平、松质骨渗血为止。复位分层肩袖,应用单排固定或内排打结固定加强缝合固定肩袖撕裂两端的止点,全层修复分层撕裂,于足印区缝线桥压紧固定整体肩袖。[结果]所有患者均顺利手术,无感染等并发症。随访时间至少12个月,UCLA评分由术前的(13.86±1.77)分提高到最终随访时的(32.62±1.53)分,Constant-Murley评分由术前(35.05±4.25)分提高到(92.67±3.86)分(P<0.001)。[结论]镜下边缘增强缝线桥全层修复能很好地恢复分层肩袖的整体性和完整性,防治肩袖进一步撕裂,增强肩袖整体的初始强度,改善患者关节功能。 | 张立智 胡孙君 杜守超 Rujan Tuladhar 卫禛 熊文峰 张世民 | 2021 | 中国矫形外科杂志2021,29,10: | 3 |
