| 3 | 经食管三维超声心动图引导经皮二尖瓣植入术和主动脉瓣Valve-in-Valve植入术1例显示文摘患者女,80岁,因反复胸闷由外院转入Germany Heart Center Munchen。体格检查:慢性病容,半卧位呼吸,颈静脉轻度怒张,双肺少量湿罗音,心房颤动,主动脉瓣听诊区可闻及3/6级收缩期杂音及轻度舒张期杂音,二尖瓣听诊区可闻及重度舒张期隆隆样杂音。肝肿大,剑突下5 cm,肋下3 cm,双下肢轻度凹陷性水肿。既往史:右侧乳腺癌术后多年,2008年于外院行冠状动脉搭桥术,2013年于外院行经心尖主动脉瓣植入术。心电图提示:慢室率心房颤动;胸部X线检查提示全心扩大,心脏瓣膜钙化严重。门诊以'多瓣膜病变、心功能不全'收入院。入院后实验室检查提示脑利钠肽(BNP)10800 ng/L,肾小球滤过率14 ml/min,促甲状腺素2.8 MU/L,血红蛋白98 g/L。经胸超声心动图提示冠状动脉搭桥术后、主动脉瓣植入术后:主动脉瓣生物瓣损毁,主动脉瓣重度狭窄伴轻度关闭不全,二尖瓣瓣环重度钙化,二尖瓣重度狭窄伴轻度关闭不全,三尖瓣轻度关闭不全,左心室收缩功能正常。多普勒测值提示主动脉瓣有效瓣口面积0.5 cm^2,跨瓣峰值压差56 mmHg(1 mmHg=0.133 kPa),跨瓣平均压差32 mmHg;二尖瓣瓣口面积0.8 cm^2,跨瓣峰值压差28 mmHg;肺动脉压60 mmHg,左室射血分数63%。全动脉CT造影提示全动脉钙化,二尖瓣瓣环钙化,左、右股动脉硬化、迂曲,内径约9 mm。冠状动脉造影提示血管桥通畅。患者在入院第2 d在杂交手术室内行'经皮二尖瓣植入术并Valve-in-valve主动脉瓣植入术'。 | 朱鹏 Magdalena Erlebach Johannes Amadeus Ziegelmueller 胡世军 Rudiger Lange Hendrik Ruge | 2020 | 中华超声影像学杂志2020，29，2： | 4 |
