| 1 | Octreotide in Hennekam syndrome-associated intestinal lymphangiectasia显示文摘A number of disorders have been described to cause protein losing enteropathy (PLE) in children. Primary intestinal lymphangiectasia (PIL) is one mechanism leading to PLE. Few syndromes are associated with PIL; Hennekam syndrome (HS) is one of them. The principal treatment for PIL is a high protein, low fat diet with medium chain triglycerides supplementation. Supportive therapy includes albumin infusion. Few publications have supported the use of octreotide to diminish protein loss and minimize hypoalbuminemia seen in PIL. There are no publications on the treatment of PIL with octreotide in patients with HS. We report two children with HS and PLE in which we used octreotide to decrease intestinal protein loss. In one patient, octreotide increased serum albumin to an acceptable level without further need for albumin infusions. The other patient responded more dramatically with near normal serum albumin levels and cessation of albumin infusions. In achieving a good response to octreotide in both patients, we add to the publications supporting the use of octreotide in PIL and suggest that octreotide should be tried in patients with PIL secondary to HS. | 12 | 阿曼Sultanate埃迪卡拉-寒武系Ara群以微生物岩为主的碳酸盐岩储层的沉积相和台地结构显示文摘埃迪卡拉-寒武系Ara群(Ediacaran—CambrianAraGroup)的盐内碳酸盐岩是阿曼盐内“细脉”(stringer)油气区带的一种重要的储层类型,在这个区带中,白云石质碳酸盐岩被包裹在埋深3~7kin的盐体内。这些碳酸盐岩储层受微生物作用的影响显著。虽然Ara群储层的年代大都是前寒武纪末期,但本文所建立的模型可能也适用于在特定环境或者地质历史时期沉积的更年轻的以微生物岩为主的碳酸盐岩储层,例如含盐度比较高、较高等生物不发育的盆地、钙化的无脊椎动物并非碳酸盐重要来源的湖相沉积环境、或者生物大量消亡之后动物群恢复之前的地质时期。众多的碳酸盐岩岩相类型为我们提供了一个很好的背景,来认识在缓坡和镶边陆架环境中发育的以微生物岩为主的碳酸盐岩储层的成因。主要的岩相组合包括碳酸盐岩-蒸发岩过渡带、深水缓坡和陆坡、潮下微生物岩、碎屑结构碳酸盐岩和局限潮缘带碳酸盐岩。,微生物岩被细分为多个岩相,这些岩相对于认识沉积环境的历史和储层物性都具有重要的意义,而且有助于预测有利储集相带的分布。受微生物影响的岩相包括具有大规模凝块结构(clottedtexture)和非常高原始孔隙度(〉50%)的浅水潮下凝块灰岩(thrombolites)、纹层形态表现出厘米尺度变化性的浅水潮下壳瘤纹层岩(pustularlaminites)、纹层形态表现出毫米尺度变化性的较深水潮下波状纹层岩(Crinklylaminites)、和表现出毫米到厘米尺度团状(tufted)结构的潮间簇生纹层岩(tuftedlaminates)。其他储集岩相包括更加常规的颗粒状碳酸盐岩,例如波纹状交错层理颗粒灰岩-泥粒灰岩、丘状交错层理颗粒灰岩-泥粒灰岩、扁平中砾砾岩(flatpebbleconglomerate)、鲕粒和内碎屑颗粒灰岩-泥粒灰岩以及含Cloudina化石的颗粒灰岩-泥粒灰岩。这些岩相毫无例外地都经历了白云石化作用,而且都具有中等到好的储层物性。它们构成了碳酸盐岩台地,台地厚度可达160m,而且在盐类构造运动之前其横向延伸距离可达十几公里到50公里以上,但后来在盐构造运动作用下破碎。、储集岩相的分布与层序地层预测结果一致,在每一个可容纳空间剖面(accommodationprofile)上都可见微生物岩。高位域后期和海进体系域早期有利于发育横向延伸距离更大的凝块灰岩建造,而海进体系域后期到高位体系域早期则有利于发育横向不连续性更强且分隔间化的建造型储集岩相(buildupreservoirfacies)。壳瘤纹层岩与凝块灰岩密切共生,但在海进体系域晚期到高位体系域晚期形成了横向大面积分布的岩席。波状纹层岩形成于海进体系域晚期到高位体系域早期,而且有可能代表着最大海泛期沉积的地层,其间碳酸盐沉积物的输入大幅度减少,从而为来自远洋的有机质的堆积创造了条件。 | John Grotzinger ZuwainaAl—Rawahi | 2014 | 石油地质科技动态2014,0,5: | 0 |
