5Nationwide trends and predictors of inpatient mortality in 83884 transjugular intrahepatic portosystemic shunt显示文摘AIM: To evaluate and validate the national trends and predictors of in-patient mortality of transjugular intrahepatic portosystemic shunt(TIPS) in 15 years.METHODS: Using the National Inpatient Sample which is a part of Health Cost and Utilization Project, we identified a discharge-weighted national estimate of 83884 TIPS procedures performed in the United States from 1998 to 2012 using international classification of diseases-9 procedural code 39.1. The demographic, hospital and co-morbility data were analyzed using a multivariant analysis. Using multi-nominal logistic regression analysis, we determined predictive factors related to increases in-hospital mortality. Comorbidity measures are in accordance to the Comorbidity Software designed by the Agency for Healthcare Research and Quality.RESULTS: Overall, 12.3% of patients died during hospitalization with downward trend in-hospitalmortality with the mean length of stay of 10.8 ± 13.1 d. Notable, African American patients(OR = 1.809 vs Caucasian patients, P < 0.001), transferred patients(OR = 1.347 vs non-transferred, P < 0.001), emergency admissions(OR = 3.032 vs elective cases, P < 0.001), patients in the Northeast region(OR = 1.449 vs West, P < 0.001) had significantly higher odds of inhospital mortality. Number of diagnoses and number of procedures showed positive correlations with in-hospital death(OR = 1.249 per one increase in number of procedures). Patients diagnosed with acute respiratory failure(OR = 8.246), acute kidney failure(OR = 4.359), hepatic encephalopathy(OR = 2.217) and esophageal variceal bleeding(OR = 2.187) were at considerably higher odds of in-hospital death compared with ascites(OR = 0.136, P < 0.001). Comorbidity measures with the highest odds of in-hospital death were fluid and electrolyte disorders(OR = 2.823), coagulopathy(OR = 2.016), and lymphoma(OR = 1.842).CONCLUSION: The overall mortality of the TIPS procedure is steadily decreasing, though the length of stay has remained relatively constant. 6Utilization of longitudinal ultrasound to quantify joint soft-tissue changes in a mouse model of posttraumatic osteoarthritis显示文摘To assess the utility of longitudinal ultrasound(US) to quantify volumetric changes in joint soft tissues during the progression of posttraumatic osteoarthritis(PTOA) in mice, and validate the US results with histological findings. A longitudinal cohort of 3-month-old wild-type C57 BL/6 male mice received the Hulth-Telhag surgical procedure on right knee to induce PTOA, and sham surgery on their left knee as control. US scans were performed on both knees before, 2, 4, 6, and 8 weeks post-surgery. Joint space volume and Power-Doppler(PD) volume were obtained from US images via Amira software. A parallel cross-sectional cohort of mice was killed at each US time point, and knee joints were subjected to histological analysis to obtain synovial soft-tissue area and OARSI scores. The correlation between US joint space volume and histological synovial soft-tissue area or OARSI score was assessed via linear regression analysis. US images indicated increased joint space volume in PTOA joints over time, which was associated with synovial inflammation and cartilage damage by histology. These changes started from 2 weeks post-surgery and gradually became more severe. No change was detected in sham joints. Increased joint space volume was significantly correlated with increased synovial soft-tissue area and the OARSI score(P o 0.001). PD signal was detected in the joint space of PTOA joints at 6 weeks post-surgery, which was consistent with the location of blood vessels that stained positively for CD31 and alpha-smooth muscle actin in the synovium.This study indicates that US is a cost-effective longitudinal outcome measure of volumetric and vascular changes in joint soft tissues during PTOA progression in mice, which positively correlates with synovial inflammation and cartilage damage.Hao Xu Echoe M Bouta Ronald W Wood Edward M Schwarz Yongjun Wang Lianping Xing 2017Bone Research2017,5,3:7
7Sonohysterography: Principles, technique and role in diagnosis of endometrial pathology显示文摘Sonohysterography (SHG), which provides enhanced endometrial visualization during standard transvaginal ultrasonography, is a relatively safe procedure for the evaluation of endometrial pathology. It can be used to evaluate patients with abnormal vaginal bleeding or infertility. This modality offers real time imaging of the endometrium without exposure to ionizing radiation. SHG is typically used in patients for whom standard transvaginal ultrasonography does not show the endometrium well, show a potential abnormality for which further imaging is required, or in patients without endometrial pathology defined on routine transvaginal imaging but in whom there is a strong clinical suspicion of an abnormality. This article will discuss the utility of the sonohysterogram in evaluation of various endometrial pathologies. 8The Changing Cold Regions Network:Observation,diagnosis and prediction of environmental change in the Saskatchewan and Mackenzie River Basins,Canada显示文摘Climate change is causing rapid and severe changes to many Earth systems and processes,with widespread cryospheric,ecological,and hydrological impacts globally,and especially in high northern latitudes.This is of major societal concern and there is an urgent need for improved understanding and predictive tools for environmental management.The Changing Cold Regions Network(CCRN)is a Canadian research consortium with a focus to integrate existing and new experimental data with modelling and remote sensing products to understand,diagnose,and predict changing land,water,and climate,and their interactions and feedbacks over the geographic domain of the Mackenzie and Saskatchewan River Basins in Canada.The network operates a set of 14 unique and focused Water,Ecosystem,Cryosphere and Climate(WECC)observatories within this region,which provide opportunities to observe and understand processes and their interaction,as well as develop and test numerical simulation models,and provide validation data for remote sensing products.This paper describes this network and its observational,experimental,and modelling programme.An overview of many of the recent Earth system changes observed across the study region is provided,and some local insights from WECC observatories that may partly explain regional patterns and trends are described.Several of the model products being developed are discussed,and linkages with the local to international user community are reviewed—In particular,the use of WECC data towards model and remote sensing product calibration and validation is highlighted.Some future activities and prospects for the network are also presented at the end of the paper.Chris M DEBEER Howard S WHEATER William L QUINTON Sean K CAREY Ronald E STEWART Murray D MACKAY Philip MARSH 2015Science China Earth Sciences2015,58,1:5

10Tensile and fracture properties of an Mg-RE-Zn alloy at elevated temperatures显示文摘Magnesium alloy EZ10(Mg-RE-Zn) was deformed in tension at temperatures from 20 up to 520 °C. A rapid decrease of the yield and tensile strength with temperature was observed at temperatures higher than 300 °C. On the other hand, ductility of samples rapidly increased in the same temperature range. Light microscopy and scanning electron microscopy was used to reveal the reason for these behaviours. Intermetallic particles in grain boundaries are responsible for excellent mechanical properties at lower temperatures. 14A novel ZnO/reduced graphene oxide and Prussian blue modified carbon paste electrode for the sensitive determination of Rutin显示文摘A carbon paste modified sensor based on a novel composite of zinc oxide nanoparticles deposited on reduced graphene oxide(ZnO-rGrO) and Prussian blue(PB) was drop-cast(ZnO-rGrO-PB/MCPE) for the sensitive estimation of Rutin(Rtn) at pH 7.0.The high surface area of ZnO-rGrO and electrocatalytic property of PB promotes the oxidation of Rtn. Field emission scanning electron microscope(FE-SEM) and energy-dispersive X-ray spectroscopy(EDX) techniques were employed to confirm the deposition of ZnO-rGrO and PB on carbon paste electrode(CPE). The ability of ZnO-rGrO-PB/MCPE in charge transfer at the interface was investigated using electrochemical impedance spectroscopy(EIS). The heterogeneous rate constant(ks) and the charge transfer coefficient(α) have been calculated as 6.08 s^(-1) and 0.74 respectively. This sensor showed a wide linear response for Rtn from 7.0×10^(-8)to 7.0×10^(-6) M and 7.0×10^(-6) to 1.0×10^(-4) M with a limit of detection(2.05±0.04)×10^(-8) M(S/N=3). The application of ZnO-rGrO-PB/MCPE was found in the analysis of Rtn in fruit juice samples using standard addition method.
