| 3 | Computed tomography-based diagnostics might be insufficient in the determination of pancreatic cancer unresectability显示文摘AIM: To inquire into a question of an overestimation of arterial involvement in patients with pancreatic cancer (PC). METHODS: Radiology data were compared with the findings from 51 standard, 58 extended and 17 total pancreaticoduodenectomies; 9 distal resections with celiac artery (CA) excision; and 28 palliations for PC. The survival of 11 patients with controversial computed tomography (CT) and endoscopic ultrasound data with regard to arterial invasion, after R0/R1 procedures (false-positive CT results, Group A), was compared to survival after eight R2 resections (false-negative CT results, Group B) and after 12 bypass procedures for locally advanced cancer (true-positive CT results, Group C).RESULTS: In all of the cases in group A, operative exploration revealed no arterial invasion, which was predicted by CT. The one-year survival in Group A was 88.9%, and the two-year survival was 26.7%, with a median follow-up of 22 mo. One-year survival was not attained in groups B and C, with a significant difference in survival (P a-b = 0.0029, P b-c = 0.003).CONCLUSION: Arterial encasement on CT does not necessarily indicate arterial invasion. Whenever PC is considered unresectable, endoUS should be used. | 4 | 输尿管及膀胱深部浸润型子宫内膜异位症的手术治疗效果显示文摘目的报告泌尿道子宫内膜异位症的手术治疗效果以及探讨保守型手术和根治性手术的选择。方法对5个手术科室参与的输尿管或膀胱深部浸润型子宫内膜异位症外科治疗的CIRENDO前瞻性数据库管理数据进行回顾性研究。对术前、术中和术后数据结果进行分析。结果数据库中的数据显示,30例中有15例为输尿管子宫内膜异位症,14例为膀胱结节病变(bladder nodules),1例同时存在这2种类型的病变。输尿管松解术14例,其中输尿管完全恢复解剖解构10例。4例40岁以上的患者术后出现闭经和可忍受的中度输尿管狭窄,其中3例好转,1例行二次输尿管切除和输尿管膀胱吻合术。4例初次手术行输尿管切除术。5例输尿管标本中发现2例输尿管子宫内膜异位症。4例并发症中有2例与输尿管结节病灶切除有关,2例与膀胱子宫内膜异位病灶切除有关。远期效果较为满意,如疼痛症状显著改善、泌尿系不适症状消失,仅有1例较长时间膀胱神经功能障碍。结论建议保守性手术联合术后假绝经疗法治疗大多数泌尿道子宫内膜异位症。尽管总体结果满意,由于术中可能同时施行其他一些复杂手术如结直肠手术,术后并发症的风险仍不容忽视。 | 陈旭 刘彦 Francisc Rozsnyai Horace Roman Benoit Resch Fabrice Dugardin Juan Berrocal Gérme Descargues Remi Schmied Malik Boukerrou Loc Marpeau CIRENDO Study Group | 2013 | 中国微创外科杂志2013，13，2： | 3 |

| 6 | Prediction of Dynamic and Aerodynamic Characteristics of the Centrifugal Fan with Forward Curved Blades显示文摘The main aim of this paper is determine the centrifugal fan with forward curved blades aerodynamic characteristics based on numerical modeling. Three variants of geometry were investigated. The first, basic 'A' variant contains 12 blades. The geometry of second 'B' variant contains 12 blades and 12 semi-blades with optimal length[1]. The third, control variant 'C' contains 24 blades without semi-blades. Numerical calculations were performed by CFD Ansys. Another aim of this paper is to compare results of the numerical simulation with results of approximate numerical procedure. Applied approximate numerical procedure [2] is designated to determine characteristics of the turbulent flow in the bladed space of a centrifugal-flow fan impeller. This numerical method is an extension of the hydro-dynamical cascade theory for incompressible and inviscid fluid flow. Paper also partially compares results from the numerical simulation and results from the experimental investigation. Acoustic phenomena observed during experiment, during numerical simulation manifested as deterioration of the calculation stability, residuals oscillation and thus also as a flow field oscillation.
