| 1 | Co-expression of CDX2 and MUC2 in gastric carcinomas: Correlations with clinico-pathological parameters and prognosis显示文摘AIM: To evaluate the role of CDX2 homeobox protein as a predictor for cancer progression and prognosis as well as its correlation with MUC2 expression. CDX2 represents a transcription factor for various intestinal genes (including MUC2) and thus an important regulator of intestinal differentiation, which could previously be identified in gastric carcinomas and intestinal metaplasia.METHODS: Formalin-fixed and paraffin-embedded tissues from 190 gastric carcinoma patients were stained with monodonal antibodies recognizing CDX2 and MUC2, respectively. Immunoreactivity was evaluated semiquantitatively and statistical analyses including χ2 tests, uni- and multi-variate survival analyses were performed.RESULTS: CDX2 was mostly expressed in a nuclear or supranuclear pattern, whereas MUC2 showed an almost exclusive supranuclear reactivity. Both antigens were present in >80% of areas exhibiting intestinal metaplasia. An immunoreactivity in >5% of the tumor area was observed in 57% (CDX2) or in 21% (MUC2) of the carcinomas. The presence of both molecules did not correlate with WHO,significantly stronger vlUC2 expression in mucinous tumors). CDX2 correlated with a lower pT and pN stage in the subgroups of intestinal and stage I cancers and was associated with MUC2 positivity. A prognostic impact of CDX2 or MUC2 was not observed. CONCLUSION: CDX2 and MUC2 play an important role in the differentiation of normal, inflamed, and neoplastic gastric tissues. | 2 | DEM parameter calibration of cohesive bulk materials using a simple angle of repose test显示文摘The numerical effort of discrete element method (DEM) simulations results in a general idealisation of DEM models that makes the calibration crucial to obtaining realistic simulation results. The angle of repose test has become a standard test for the calibration of DEM parameters of cohesionless bulk materials and is extensively discussed in the literature. One of the most used test methods is the pull-up test of a hollow cylinder filled with bulk material. This paper presents how this basic pull-up test can also be used for the calibration of DEM parameters of cohesive materials by changing the analysis criteria. In contrast to analysing the static angle of repose, the focus lies on the macroscopic flow behaviour during the lifting of the cylinder. Reproducible phases of flow are identified in experiments: the build-up of a stable bulk material column, the convex bending of the column, and the beginning of collapse of the column. Furthermore, the phases are independent of the size and lifting velocity. The convex bending of the bulk-material column combined with the moment of first collapse are introduced as new measurable calibration criteria. These experimental results of wet sand are used for the calibration of related DEM parameters using a simplified JKR cohesion model. | 5 | Therapeutic alternatives for the treatment of type 1 hepatorenal syndrome: A Delphi technique-based consensus显示文摘AIM To propose several alternatives treatment of type 1 hepatorenal syndrome(HRS-1) what is the most severe expression of circulatory dysfunction on patients with portal hypertension.METHODS A group of eleven gastroenterologists and nephrologists performed a structured analysis of available literature.Each expert was designated to review and answer a question.They generated draft statements for evaluation by all the experts.Additional input was obtained from medical community.In order to reach consensus,a modified three-round Delphi technique method was used.According to United States Preventive Services Task Force criteria,the quality of the evidence and level of recommendation supporting each statement was graded.RESULTS Nine questions were formulated.The available evidence was evaluated considering its quality,number of patients included in the studies and the consistency of its results.The generated questions were answered by the expert panel with a high level of agreement.Thus,a therapeutic algorithm was generated.The role of terlipressin and norepinephrine was confirmed as the pharmacologic treatment of choice.On the other hand the use of the combination of octreotide,midodrine and albumin without vasoconstrictors was discouraged.The role of several other options was also evaluated and the available evidence was explored and discussed.Liver transplantation is considered the definitive treatment for HRS-1.The present consensus is an important effort that intends to organize the available strategies based on the available evidence in the literature,the quality of the evidence and the benefits,adverse effects and availability of the therapeutic tools described.CONCLUSION Based on the available evidence the expert panel was able to discriminate the most appropriate therapeutic alternatives for the treatment of HRS-1. | Juan P Arab Juan C Claro Juan P Arancibia Jorge Contreras Fernando Gomez Cristian Munoz Leyla Nazal Eric Roessler Rodrigo Wolff Marco Arrese Carlos Benítez | 2016 | World Journal of Hepatology2016，8，25： | 3 |
