| 1 | Chronic intestinal pseudo-obstruction显示文摘Chronic intestinal pseudo-obstruction (CIPO) is a se- vere digestive syndrome characterized by derangement of gut propulsive motility which resembles mechanical obstruction, in the absence of any obstructive process. Although uncommon in clinical practice, this syndrome represents one of the main causes of intestinal failure and is characterized by high morbidity and mortality. It may be idiopathic or secondary to a variety of diseases. Most cases are sporadic, even though familial forms with either dominant or recessive autosomal inheritance have been described. Based on histological features in- testinal pseudo-obstruction can be classified into three main categories:neuropathies, mesenchymopathies, and myopathies, according on the predominant involvement of enteric neurones, interstitial cells of Cajal or smooth muscle cells, respectively. | 3 | Utility of preoperative systemic inflammatory biomarkers in predicting postoperative complications after pancreaticoduodenectomy: Literature review and single center experience显示文摘BACKGROUND The role of preoperative inflammatory biomarkers(PIBs)in predicting postoperative morbidity has been assessed in colorectal and otorhinolaryngeal surgery.However,data regarding the role that preoperative inflammatory biomarkers have on morbidity after pancreaticoduodenectomiy(PD)are less consistent.AIM To assess the utility of PIBs in predicting postoperative complications after pancreaticoduodenectomy.METHODS A database of 317 consecutive pancreaticoduodenectomies performed from April 2003 to November 2018 has been retrospectively analyzed.Data regarding preoperative neutrophil-to-lymphocyte ratio(NLR),derived NLR and C-reactive protein(CRP),and postoperative complications of 238 cases have been evaluated.Exclusion criteria were:age<18-years-old,previous neoadjuvant treatment,absence of data about PIBs,concomitant hematological disorders,and presence of active infections at the moment of the surgery.PIBs were compared using Mann-Whitney’s test and receiver operating characteristic(ROC)analysis was performed to define the cutoffs.The positive predictive value(PPV)was computed to evaluate the probability to develop complication.P-values<0.05 were considered statistically significant.RESULTS According to the literature findings,only four papers have been published reporting the relation between the inflammatory biomarkers and PD postoperative morbidity.A combination of preoperative and postoperative inflammatory biomarkers in predicting complications after PD and the utility of preoperative NLR in the development of postoperative pancreatic fistula(POPF)have been reported.The combination of PIBs and postoperative day-1 drains amylase has been reported to predict the incidence of POPF.According to our results,CRP values were significantly different between patients who had/did not have postoperative complications and abdominal collections(P<0.05).Notably,patients with preoperative CRP>8.81 mg/dL were at higher risk of both overall complications and abdominal collections(respectively P=0.0037,PPV=0.95,negative predictive value[NPV]=0.27 and P=0.016,PPV=0.59,NPV=0.68).Preoperative derived neutrophil-to-lymphocyte ratio(dNLR)(cut off>1.47)was also a predictor of abdominal collection(P=0.021,PPV=0.48,NPV=0.71).Combining CRP and dNLR,PPV increased to 0.67.NLR(cut off>1.65)was significantly associated with postoperative hemorrhage(P=0.016,PPV=0.17,NPV=0.98).CONCLUSION PIBs may predict complications after PD.During postoperative care,PIB levels could influence decisions regarding the timing of drains removal and the selection of patients who might benefit from second level diagnostic exams. | Alessandro Coppola Vincenzo La Vaccara Lorenza Caggiati Ludovico Carbone Silvia Spoto Massimo Ciccozzi Silvia Angeletti Roberto Coppola Damiano Caputo | 2021 | World Journal of Gastrointestinal Surgery2021,13,10: | 1 |
| 4 | Validations of new cut-offs for surgical drains management and use of computerized tomography scan after pancreatoduodenectomy:The DALCUT trial显示文摘BACKGROUND Postoperative pancreatic fistula(POPF)is the most fearful complication after pancreatic surgery and can lead to severe postoperative complications such as surgical site infections,sepsis and bleeding.A previous study which identified cut-offs of drains amylase levels(DALs)determined on postoperative day(POD)1 and POD3,was able to significantly predict POPF,abdominal collections and biliary fistulas,when related to specific findings detected at the abdominal computerized tomography(CT)scan routinely performed on POD3.AIM To validate the cut-offs of DALs in POD1 and POD3,established during the previous study,to assess the risk of clinically relevant POPF and confirm the usefulness of abdominal CT scan on POD3 in patients at increased risk of abdominal collection.METHODS The DALCUT trial is an interventional prospective study.All patients who will undergo pancreatoduodenectomy(PD)for periampullary neoplasms will be considered eligible.All patients will receive clinical staging and,if eligible for surgery,will undergo routine preoperative evaluation.After the PD,daily DALs will be evaluated from POD1.Drains removal and possible requirement of abdominal CT scans in POD3 will be managed on the basis of the outcome of DALs in the first three postoperative days.RESULTS This prospective study could validate the role of DALs in the management of surgical drains and in assessing the risk or relevant complications after PD.Drains could be removed in POD3 in case of POD1 DALs<666 U/L and POD3 DALs<207 U/L.In case of POD3 DALs≥252,abdominal CT scan will be performed in POD3 to identify abdominal collections≥5 cm.In this latter category of patients,drains could be maintained beyond POD3.CONCLUSION The results of this trial will contribute to a better knowledge of POPF and management of surgical drains. | Damiano Caputo Alessandro Coppola Vincenzo La Vaccara Roberto Passa Ludovico Carbone Massimo Ciccozzi Silvia Angeletti Roberto Coppola | 2022 | World Journal of Clinical Cases2022,10,15: | 0 |

| 6 | Directional radiation enhancement of nanowire quantum dots based on line-array plasmonic antenna coupling显示文摘The integration of a single III-V semiconductor quantum dot with a plasmonic nanoantenna as a means toward efficient single-photon sources(SPEs)is limited due to its weak,wide-angle emission,and low emission rate.These limitations can be overcome by designing a unique linear array of plasmonic antenna structures coupled to nanowire-based quantum dot(NWQD)emitters.A linear array of a coupled device composed of multiple plasmonic antennas at an optimum distance from the quantum dot emitter can be designed to enhance the directionality and the spontaneous emission rate of an integrated single-photon emitter.Finite element modeling has been used to design these compact structures with high quantum efficiencies and directionality of single-photon emission while retaining the advantages of NWQDs.The Purcell enhancement factor of these structures approaches 66.1 and 145.8,respectively.Compared to a single NWQD of the same diameter,the fluorescence was enhanced by 1054 and 2916times.The predicted collection efficiencies approach 85%(numerical aperture,NA=0.5)and 80%(NA=0.5),respectively.Unlike single-photon emitters based on bulky conventional optics,this is a unique nanophotonic single-emission photon source based on a line-array configuration that uses a surface plasmon-enhanced design with minimum dissipation.The designs presented in this work will facilitate the development of SPEs with potential integration with semiconductor optoelectronics. | PEIHANG LI PENG YU JIACHEN SUN ZHIMIN JING JIANG WU LUCAS V.BESTEIRO ROBERTO CAPUTO ARUP NEOGI HONGXING XU ZHIMING WANG | 2022 | Photonics Research2022,10,9: | 0 |
| 7 | Active spatial control of photothermal heating and thermo-actuated convective flow by engineering a plasmonic metasurface with heterodimer lattices显示文摘Thermo-plasmonics, using plasmonic structures as heat sources, has been widely used in biomedical and microfluidic applications. However, a metasurface with single-element unit cells, considered as the sole heat source in a unit cell, functions at a fixed wavelength and has limited control over the thermo-plasmonically induced hydrodynamic effects. Plasmonic metasurfaces with metal disk heterodimer lattices can be viewed to possess two heat sources within a unit cell and are therefore designed to photo-actively control thermal distributions and fluid dynamics at the nanoscale. The locations of heat sources can be switched, and the direction of the convective flow in the central region of the unit cell can be reversed by shifting the wavelength of the excitation source without any change in the excitation direction or physical actuation of the structural elements. The temperature and velocity of a fluid are spatiotemporally controlled by the wavelength selectivity and polarization sensitivity of the plasmonic metasurface. Additionally, we investigate the effects of geometric parameters on the surface lattice resonances and their impact on the temperature and fluid velocity of the optofluidic system.
