1经皮自体骨髓移植治疗骨不连——骨髓母细胞数量和浓度对治疗的影响显示文摘背景:自髂嵴部位取包含有母细胞成份的骨髓,其可以通过促进骨的愈合来治疗骨不连。然而,有关促进骨修复所需的母细胞的数量、浓度的研究较少。本研究主要探讨母细胞移植治疗骨不连的有效细胞数量和浓度,评估移植后骨痂形成的数量及临床愈合率。 方法:在60例无感染的萎缩性胫骨骨不连患者双侧髂嵴前部吸取骨髓后,经细胞分离器进行浓缩,然后注射入骨折断端。每个骨折部位均注入20cm^3的浓缩骨髓。其母细胞数量通过纤维母细胞集落形成单位数量仪来测定。其形成的矿化骨的数量通过术前CT扫描与注射后4个月复查的CT扫描结果之间的比较来确定。 结果:吸取的骨髓中母细胞计数浓缩前平均为(612±134)个/cm^3(12~1224个/cm^3),浓缩后平均为(2579±1121)个/cm^3(60~6120个/cm^3)。每例骨不连部位注入的纤维母细胞集落形成单位数量平均为51×10^3。60例中有53例骨折愈合,愈合组患者注入的骨髓母细胞数都在1500个/cm^3以上,平均含母细胞数为(54962±17431)个。然而在7例仍然无法愈合的病例中,注入的骨髓母细胞浓度为(634±187)个/cm^3,细胞的总数量为(19324±6843)个,与愈合组相比不愈合组的细胞浓度和数量均较低,两组差异有统计学意义(分别为p=0.001,p〈0.01)。4个月后CT显示矿化的骨痂数量为0.8~5.3cm^3(平均3.1cm^3)。统计学结果显示矿化骨痂的数量和纤维母细胞集落单位的数量及浓度均呈正相关(分别为p=0.04,p=0.01)。而愈合时间和纤维母细胞集落单位的浓度呈负相关(p=0.04)。 结论:经皮自体骨髓移植是治疗胫骨骨干萎缩性骨不连的安全而有效的方法。然而,其有效性与移植骨髓中母细胞的数量有关,自髂嵴获取的骨髓如果不经过浓缩处理,其母细胞数量并不能达到治疗骨不连的理想数目。 可信度:治疗性研究,Ⅲ级,进一步可信度参见作者介绍。PH.
