1Tu-San-Qi(Gynura japonica):the culprit behind pyrrolizidine alkaloid-induced liver injury in China显示文摘Herbs and dietary supplement-induced liver injury(HILI)is the leading cause of drug-induced liver injury in China.Among different hepatotoxic herbs,the pyrrolizidine alkaloid(PA)-producing herb Gynura japonica contributes significantly to HILI by inducing hepatic sinusoidal obstruction syndrome(HSOS),a liver disorder characterized by hepatomegaly,hyperbilirubinemia,and ascites.In China,G.japonica has been used as one of the plant species for Tu-San-Qi and is often misused with non-PA-producing Tu-San-Qi(Sedum aizoon)or even San-Qi(Panax notoginseng)for self-medication.It has been reported that over 50% of HSOS cases are caused by the intake of PA-producing G.japonica.In this review,we provide comprehensive information to distinguish these Tu-San-Qi-related herbal plant species in terms of plant/medicinal part morphologies,medicinal indications,and chemical profiles.Approximately 2156 Tu-San-Qi-associated HSOS cases reported in China from 1980 to 2019 are systematically reviewed in terms of their clinical manifestation,diagnostic workups,therapeutic interventions,and outcomes.In addition,based on the application of our developed mechanism-based biomarker of PA exposure,our clinical findings on the definitive diagnosis of 58 PA-producing Tu-San-Qi-induced HSOS patients are also elaborated.Therefore,this review article provides the first comprehensive report on 2214 PA-producing Tu-San-Qi(G.japonica)-induced HSOS cases in China,and the information presented will improve public awareness of the significant incidence of PA-producing Tu-San-Qi(G.japonica)-induced HSOS and facilitate future prevention and better clinical management of this severe HILI.Lin Zhu Chun-yuan Zhang Dong-ping Li Hu-biao Chen Jiang Ma Hong Gao Yang Ye Ji-yao Wang Peter P.Fu Ge Lin 2021Acta Pharmacologica Sinica2021,42,8:9

3Chemical Diversity Investigation of Hepatotoxic Pyrrolizidine Alkaloids in Qianliguang(Senecio scandens)and Related Species by UHPLC-QTOF-MS显示文摘Objective: Qianliguang(Senecio scandens) is a common Chinese medicinal herb. Qianliguang-containing herbal proprietary products are registered as over-the-counter remedies in China and exported to Western countries. The presence of hepatotoxic pyrrolizidine alkaloids(PAs)has raised concerns about the safety of using Qianliguang and its products. The present study aims at investigation of different types of PAs present in Qianliguang collected from representative locations in China.Methods: In this study, a simple but specific UHPLC-QTOF-MS method for the determination of toxic PAs was developed, based on the characteristic fragment ions specific to different types of PAs. It was successfully applied for the identification and distinguishing of PAs present in Qianliguang and related Senecio species growing in different locations of China.Results: Significant diversity of the PA types and quantities were revealed among the samples tested. The estimated total amounts of toxic PAs in three of the samples exceed the toxic limits of PA intake restricted by WHO, demonstrating the timely and highly demand for regulating both types and quantities of PAs present in Qianliguang.Conclusions: This study provides the methodology for simultaneous identification and quantification of PAs present in herbs without requiring corresponding standards, which could be further used for more systematic investigations of the PA distribution in Qianliguang and other PA-containing herbs.Lin Zhu Na Li Jian-Qing Ruan Peter P.Fu Zhong-Zhen Zhao Ge Lin 2015World Journal of Traditional Chinese Medicine2015,1,2:1
