1Impact of postoperative omega-3 fatty acid-supplemented parenteral nutrition on clinical outcomes and immunomodulations in colorectal cancer patients显示文摘AIM: To investigate the effect of omega-3 fatty acid parenteral supplementation postoperatively on clinical outcomes and immunomodulation in colorectal cancer patients. METHODS: Forty-two patients undergoing radical colorectal cancer resection with an indication for total parenteral nutrition postoperatively were enrolled in this prospective, double-blind, randomized, controlled study. Patients received total parenteral nutrition supplemented with either soybean oil (LCT; Intralipid, Fresenius-Kabi, SO group, n = 21) or a combination of omega-3 fish oil and soybean oil (LCT:fish oil = 5:1, fish oil; Omegaven, Fresenius-Kabi, FO group, n = 21), up to a total of 1.2 g lipid/kg per day for 7 d postoperatively. A same volume calorie and nitrogen was administrated. Routine blood test, biochemistry, systemic levels of IL-6 and TNF-α, percentage of CD3+, CD4+, and CD8+ lymphocytes were evaluated preoperatively and on postoperative d 1 and 8. Patient outcome was evaluated considering mortality during the hospital stay, length of postoperative hospital stay, and occurrence of infectious complications. RESULTS: Both lipid regimens were well tolerated. No differences between the two groups were noticed in demographics, baseline blood test, biochemistry, serum levels of IL-6 and TNF-α, percentage of CD4+, CD8+ lymphocytes, and ratios of CD4+/CD8+. Compared with those on postoperative d 1, serum IL-6 levels onpostoperative d 8 were significantly depressed in the FO group than in the reference group (-44.43 ± 30.53 vs -8.39 ± 69.08, P = 0.039). Simultaneously, the ratios of CD4+/CD8+ were significantly increased in the FO group (0.92 ± 0.62 vs 0.25 ± 1.22, P = 0.035). In addition, depression of serum TNF-α levels (-0.82 ± 2.71 vs 0.27 ± 1.67, P = 0.125) and elevation of CD3+ and CD4+ lymphocyte percentage (12.85 ± 11.61 vs 3.84 ± 19.62, P = 0.081, 17.80 ± 10.86 vs 9.66 ± 17.55, P = 0.084, respectively) were higher in the FO group than in the reference group. Patients in the FO group trended to need a shorter postoperative hospital stay (17.45 ± 4.80 d vs 19.62 ± 5.59 d, P = 0.19). No statistically significant difference was found when stratified to mortality and occurrence of infectious complications. 2Ultrasonically activated scalpel versus monopolar electrocautery shovel in laparoscopic total mesorectal excision for rectal cancer显示文摘AIM: To investigate the feasibility and safety of monopolar electrocautery shovel (ES) in laparoscopic total mesorectal excision (TME) with anal sphincter preservation for rectal cancer in order to reduce the cost of the laparoscopic operation, and to compare ES with the ultrasonically activated scalpel (US). METHODS: Forty patients with rectal cancer, who underwent laparoscopic TME with anal sphincter preservation from June 2005 to June 2007, were randomly divided into ultrasonic scalpel group and monopolar ES group, prospectively. White blood cells (WBC) were measured before and after operation, operative time, blood loss, pelvic volume of drainage, time of anal exhaust, visual analogue scales (VAS) and surgery-related complications were recorded. RESULTS: All the operations were successful; no one was converted to open procedure. No significant differences were observed in terms of preoperative and postoperative d 1 and d 3 WBC counts (P = 0.493, P = 0.375, P = 0.559), operation time (P = 0.235), blood loss (P = 0.296), anal exhaust time (P = 0.431), pelvicdrainage volume and VAS in postoperative d 1 (P = 0.431, P = 0.426) and d 3 (P = 0.844, P = 0.617) between ES group and US group. The occurrence of surgery-related complications such as anastomotic leakage and wound infection was the same in the two groups. CONCLUSION: ES is a safe and feasible tool as same as US used in laparoscopic TME with anal sphincter preservation for rectal cancer on the basis of the skillful laparoscopic technique and the complete understanding of laparoscopic pelvic anatomy. 3Sorafenib inhibits growth and metastasis of hepatocellular carcinoma by blocking STAT3显示文摘AIM: To investigate the inhibitory role and the underlying mechanisms of sorafenib on signal transducer and activator of transcription 3 (STAT3) activity in hepatocellular carcinoma (HCC).METHODS: Human and rat HCC cell lines were treated with sorafenib. Proliferation and STAT3 dephosphorylation were assessed. Potential molecular mechanisms of STAT3 pathway inhibition by sorafenib were evaluated. In vivo antitumor action and STAT3 inhibition were investigated in an immunocompetent orthotopic rat HCC model.RESULTS: Sorafenib decreased STAT3 phosphorylationat the tyrosine and serine residues (Y705 and S727), but did not affect Janus kinase 2 (JAK2) and phosphatase shatterproof 2 (SHP2), which is associated with growth inhibition in HCC cells. Dephosphorylation of S727 was associated with attenuated extracellular signal-regulated kinase (ERK) phosphorylation, similar to the effects of a mitogen-activated protein kinase (MEK) inhibitor U0126, suggesting that sorafenib induced S727 dephosphorylation by inhibiting MEK/ERK signaling. Meanwhile, sorafenib could also inhibit Akt phosphorylation, and both the phosphatidylinositol-3-kinase (PI3K) inhibitor LY294002 and Akt knockdown resulted in Y705 dephosphorylation, indicating that Y705 dephosphorylation by sorafenib was mediated by inhibiting the PI3K/Akt pathway. 4Quantum secret sharing between multiparty and multiparty with Bell states and Bell measurements显示文摘We present a quantum secret sharing scheme between multiparty (m members in Group 1) and multiparty (n members in Group 2), and analyze its security. This scheme takes EPR pairs in Bell states as quantum resources. In order to obtain the shared key, all members only need to perform Bell measurements, rather than perform any local unitary operation. 5Effect of the cooling rate on plastic deformability of a Zr-based bulk metallic glass显示文摘The present work found the plastic deformability of Zr65Cu17.5Ni10Al7.5 BMG dependent on the cooling rate during the formation from the molten state alloy. The deformation behavior in the compression test of φ 2 mm Zr65Cu17.5Ni10Al7.5 BMGs as-cast or lathed from different sizes as-cast samples was characterized, and they exhibited different plastic strains. The compressive plastic strain increases with the decreasing diameter of the as-cast specimens, i.e. with increasing the cooling rate. 6Efficacies of β-L-D4A against Hepatitis B virus in 2.2.15 cells显示文摘AIM:To investigate the antiviral effect of beta-L-enantiomer of 2',3'-didehydro-2',3'-dideoxyadenosine (β-L-D4A) on 2.2.15 cells transfected with the hepatitis B virus (HBV) genome. METHODS:Lamivudine (3TC) as a positive control. Then, HBV DNA in treated 2.2.15 cells and the Hepatitis B surface antigen (HBsAg) in the culture supernatants were detected to determine the inhibitory effect of β-L-D4A. At the same time, 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) was used to detect the survival ratio of 2.2.15 cells. RESULTS:β-L-D4A has a dose-dependent inhibitory effect on HBV DNA replication;this effect was apparent when the concentration was above 1 mol/L. When β-L-D4A was at the highest concentration, 100 mol/L, the HBsAg inhibition ratio was above 50%. The Therapeutic index (TI) of β-L-D4A was above 2.1. 8Bayesian cognitive trust model based self-clustering algorithm for MANETs显示文摘With the introduction of mobile Ad hoc networks (MANETs), nodes are able to participate in a dynamic network which lacks an underlying infrastructure. Before two nodes agree to interact, they must trust that each will satisfy the security and privacy requirements of the other. In this paper, using the cognition inspired method from the brain informatics (BI), we present a novel approach to improving the search efficiency and scalability of MANETs by clustering nodes based on cognitive trust mechanism. The trust relationship is formed by evaluating the level of trust using Bayesian statistic analysis, and clusters can be formed and maintained autonomously by nodes with only partial knowledge. Simulation experiments show that each node can form and join proper clusters, which improve the interaction performance of the entire network. 9Synthesis of Nanoscale Shell-core Titania Coated Silica Particles in the Presence of Polyether Polyamine and the Phase Transition显示文摘The nanoscale titania coated silica was prepared via a two-step precipitating approach, where the nanoscale silica nuclei were first prepared by passing an aqueous solution of sodium silicate through an ion-exchange resin bed, then coated with the precipitation from hydrolyzed butyl titanate in an ethanol-hexane mixture at a low pH value in the presence of poly(ethylene oxide) polyamine salt(PPA) at a high temperature of 90 ℃. In the second-step precipitating process, the spontaneously precipitated titania shell on the silica nuclei was stabilized in the suspension solution with the help of the adsorption of PPA on the particles. A possible precipitating mechanism was suggested. 11Structural modulation of Cd(Ⅱ) supramolecular frameworks with a versatile 2,4-dipyridyl-type building block and different dicarboxylate ligands显示文摘A series of Cd(Ⅱ) coordination complexes with an elongated 2,4-dipyridyl-type building block trans-1-(2-pyridyl)-2-(4-pyridyl)ethylene (bpe) and different dicarboxylate ligands have been presented. Generally, bpe shows the unidentate coordination mode and serves as the terminal pendant, whereas the dicarboxylate ligands display various binding fashions to interlink the metal centers to form 1-D comb-like chain, ribbon, and fishbone arrays, as well as 2-D 44 and 4.82 layered networks. Notably, the bpe building block is also involved in secondary interactions such as hydrogen bonding and aromatic stacking to construct the resulting supramolecular architectures. 14STRENGTHENING MECHANISMSIN TiAl BASED ALLOYS显示文摘The influence of heat treatment and of thermomechanical processing on the structure and properties of a range of TiAl based alloys has been assessed and in agreement with other reports it has been found that increased refinement of the microstructure leads to improved mechanical strength at room temperature, both for the lamellar and the duplex structures. In the case of alloys cooled rapidly from the alpha phase field the increased refinement in lamellar spacing leads to significant increases in room temperature strength but thermomechanical processing can lead to far greater increases. The origin of this increase in strength in samples with a lamellar structure has been assessed in terms of the ability of dislocations to cross gamma/gamma and gamma/alpha 2 lamellar interfaces. It was concluded that the alpha 2 gamma interfaces and the alpha itself are important factors in strengthening the lamellar alloys. The stability of the various structures developed either by appropriate heat treatments or by thermomechanical processing has been investigated by exposing samples for a range of times at temperatures between 700 and 1 000 ℃. It has been found that the yield strength and the ultimate tensile strength generally decreased by about 20% during high temperature exposure at 700 ℃ for 3 000 h. The detailed behaviour on exposure at 700 ℃ has been found to be a function of alloy composition, with complex precipitates being formed in some alloys, but in all cases the amount of alpha 2 decreased with increased heat treatment time. It has been found that during exposure the alpha 2 lamellae decomposed to gamma phase by a mechanism that can involve the formation of thin gamma lamellae within the original alpha 2 lamellae.M. H.
