CD36 expression and lipid metabolism following an oral glucose challenge in South Asians显示文摘AIM:To investigate lipid metabolism and the relationship with monocyte expression of the fatty acid translocase CD36 in South Asians.METHODS:An observational study of South Asians whom as an ethnic group have-a higher risk of developing diabetes. The susceptibility to diabetes is coupled with an earlier and more rapid progression of micro-,and macro-vascular complications.Twentynine healthy South Asian participants [mean age 34.6(8.9) years,76.2% male, mean body-mass index 25.0(5.2) kg/m2] were recruited from an urban residential area of central Birmingham(United Kingdom). The main outcomes measured were post prandial(30 min) and post absorptive(120 min) changes from fasting(0 min) in circulating lipoproteins, lipds and hormones, andmonocyte expression of CD36 post injection of a 75 g oral glucose challenge. The inducements of variations of monocyte CD36 expression were analysed.RESULTS:Our results showed evident changes in monocyte CD36 expression following the glucose challenge(P < 0.001).Non-esterified fatty acids(NEFA) levels decreased progressively during the challenge(P < 0.001),in contrast to increased cholesterol(but not triglyceride) concentrations within very low density lipoprotein(VLDL) and low density lipoprotein subfractions(P < 0.01).Levels of,glucose, serum triglycerides and high density lipoprotein cholesterol remained largely unchanged.Variations of monocyte CD36 were negatively(r =-0.47,P = 0.04) associated to fat from the diet and positively to carbohydrate from the diet(r = 0.65,P < 0.001).CONCLUSION:These data suggest that the initiation of VLDL genesis follows the consumption of glucose within this population, inferring that the sequestration of NEFA from these particles happens due to the increased availability of CD36 receptors. While these are preliminary results,it would appear that lifestyle exposures have a role in moderating the expression of CD36. | Jeetesh V Patel Amitava Banerjee Silvia Montoro-Garcia Eduard Shantsila Mushfique Alam Paul Flinders Kathleen AL Houlton Elizabeth A Hughes Gregory YH Lip Paramjit S Gill | 2015 | World Journal of Diabetes2015,6,7: | 1

Epidemics and primary care in the UK显示文摘IntroduCtIon The emergence of the novel coronavirus disease(COVID-19)in China has resulted in catastrophic events that echo worldwide.Primary care physicians are carrying some of the burden of managing the epidemic alongside secondary care, which is strug-gling to maintain a concerted effort in the face of the mounting pressure of cases.Epidemics affect communities not only biologically but also psychosocially.General practitioners(GPs)in the UK are well posi-tioned to deliver care via a‘whole-person,holistic approach’as they are the first point of contact and provide continuity of care within communities.By reviewing historical epidemics in different countries,we may be able to address the current issues in worldwide by providing pragmatic solutions. In the UK,the successful response to the influenza and severe acute respiratory syndrome(SARS)epidemics was a result of effective collaboration between different organisations. | Mishkat Shehata Shuxia Zhao Paramjit Gill | 2020 | Family Medicine and Community Health2020,8,2: | 0

Multimorbidity and oral health:need for new models of care显示文摘Multimorbidity is defined as patients living with two or more chronic health conditions. The prevalence of multimorbidity is increasing, driven by the ageing population, and represents a major challenge to all healthcare systems because these patients are heavy users of services. The link with oral health is growing although there is need for further robust evidence. There is also need for new models of care to address oral health in patients with multimorbidity. | Balraj Gill Andrew Harris Christopher Tredwin Paramjit Gill | 2020 | Family Medicine and Community Health2020,8,4: | 0
Methodological challenges of cross-language qualitative research with South Asian communities living in the UK显示文摘Objective:We investigate(1)the influence of ethnic, gender, and age concordance with inter-viewers and(2)how expression of qualitative data varies between interviews delivered in English and community languages(Punjabi/Urdu)with monolingual and bilingual participants across three generations of the Indian Sikh and Pakistani Muslim communities living in the UK.Methods:We analyzed and interpreted semi-structured interview transcripts that were designed to collect data about lifestyles, disease management, community practices/beliefs,and social net-works.First,qualitative content analysis was applied to transcripts.Second,a framework was ap-plied as a guide to identify cross-language illustrations where responses varied in length, expression and depth. Results:Participant responses differed by language and topic.First-generation migrants when discussing religion, culture,or family practice were far likelier to use group or community narra-tives and give a longer response, indicating familiarity with or importance of such issues.Ethnic and gender concordance generated greater rapport between researchers and participants centered on community values and practices.Further,open-ended questions that were less direct were better suited for first-generation migrants.Conclusion:Community-based researchers need more time to complete interviews in second languages, need to acknowledge that narratives can be contextualized in both personal and com-munity views, and reframe questions that may lead to greater expression.Furthermore,we detail a number of recommendations with regard to validating the translation of interviews from commu-nity languages to English as well as measures for testing language proficiency. | Manbinder S.Sidhu Farina Kokab Kate Jolly Tom Marshall Nicola K.Gale Paramjit Gill | 2016 | Family Medicine and Community Health2016,4,2: | 0
