1Surgical treatment of synchronous colorectal liver and lung metastases: the usefulness of thoracophrenolaparotomy for single stage resection显示文摘When suitable, surgery still remains the therapeutic option to be preferred for patients carrier of colorectal liver and lung metastases. Since thoracophrenolaparotomy should be helpful during liver resection for some of these patients, simultaneous removal of right lung metastases can be proposed through this approach. Eleven consecutive patients(median age of 53 years) carrier of colorectal liver and lung metastases, underwent single session surgical resection of both liver and right lung lesions by means of J-shaped thoracophrenolaparotomy. The median number of liver metastases removed was 5(range 2-30) and of lung metastases removed was 2(range 1-3). Lung metastases were located in the upper lobe in 1 patient, in the middle lobe in 2, in the lower lobe in 6, and in the upper and lower lobe in 2. Mortality and major morbidity were nil. Two patients had a minor morbidity: one had wound infection and bile leakage treated conservatively and the other had transient fever. Mean overall survival was 24.4 months. 2Safety and feasibility of thullium laser transurethral resection of prostate for the treatment of benign prostatic enlargement in overweight patients显示文摘Objective:We aimed to determine safety and feasibility of thulium laser transurethral vapoenucleation of prostate(ThuVEP)for treatment of obese patients affected by benign prostatic hyperplasia(BPH).Methods:We retrospectively analysed data of 452 patients with BPH who underwent ThuVEP from February 2012 to March 2016 in a single center.Patients were divided into three groups according to body mass index(BMI,kg/m^2):Normal weight(18.5≤BMI<25;Group A),overweight(25≤BMI<30;Group B)and obese(BMI≤30;Group C),for a total of 412 patients evaluable for this study.Preoperative total serum prostate-specific antigen(PSA),digital rectal examination of the prostate,transrectal ultrasound(TRUS),renal ultrasound,urine culture,uroflowmetry,International Prostate Symptoms Score(IPSS),and Quality of Life(QoL)score were analyzed.Post-operative complications,hospital stay and days of catheterization,questionnaires and uroflowmetry at 1 and 3 months after surgery were evaluated.Preoperative data,surgical outcomes,complication rate and clinical outcomes were compared between groups.Results:The median age of patients was 69 years(Interquartile Range[IQR 10]).The preoperative median IPSS among groups was 19(IQR 8.75),20(IQR 10),and 18(IQR 10)respectively.At 1 and 3 months of follow-up,this value was 8(IQR 7),8(IQR 4),7(IQR 5)and 5(IQR 6.25),5(IQR 6),6(IQR 5),respectively(all p between groups>0.05).There was no statistically significant difference among three groups as for hospital stay and days of catheterization(p>0.05).Conclusion:Our results showed that ThuVEP was safe and feasible even in overweight patients with substantially enlarged prostate.Luca Carmignani Maria Chiara Clementi Claudia Signorini Gloria Motta Sebastiano Nazzani Franco Palmisano Elisa De Lorenzis Michele Catellani Alessandro Francesco Mistretta Andrea Conti Valeria Tringali Maria Beatrice Costa Damiano Vizziello 2019Asian Journal of Urology2019,6,3:3

6Comparison of perioperative parameters in one-handed rotational phacoemulsification versus conventional phacoemulsification and femtosecond laser-assisted cataract surgery显示文摘AIM:To compare perioperative parameters of one-handed rotational phacoemulsification technique(one-handed phaco-roll)with each of other two techniques,“Divide et Conquer”and femtosecond laser-assisted cataract surgery(FLACS)METHODS:In this retrospective and comparative cohort study,eyes with uncomplicated cataract(nuclear density grade 2 to 3)treated routinely with one-handed phacoroll(n=23;Group 1)or“Divide et Conquer”(n=23;Group 2)or FLACS(n=23;Group 3)were enrolled.Intraoperative parameters including effective phaco-time(EPt),ultrasound time(USt),aspiration time,surgical time,phacoemulsification(phaco)-power,balanced salt solution(BSS)use,cumulative dissipated energy(CDE)were recorded and compared.Clinical outcomes including best corrected visual acuity(BCVA),corneal endothelial cell density(ECD),endothelial cell loss(ECL),central corneal thickness(CCT)and central macular thickness(CMT),were assessed and compared pre-operatively and at 1mo after surgery.RESULTS:Aspiration and surgical time,and BSS used were lower in Group 1(P<0.01)than other groups.EPt,phaco-power and CDE were lower in Group 1(P<0.05)than Group 2 but not significantly different from Group 3.In Group 1,USt was lower(P<0.05)than Group 2 but higher(P<0.05)than Group 3.BCVA improved in all groups without significant difference between Group 1 and the other ones.No significant differences regarding all post-operative morphologic outcomes(ECD,ECL,CCT,CMT)were reported.No clinical complications occurred.CONCLUSION:One-handed phaco-roll seems to be less time-consuming than“Divide et Conquer”and FLACS and less energy-consuming than“Divide et Conquer”.Furthermore,one-handed phaco-roll seems to have an equal safety profile compared to the other two techniques.Samuele Gigliola Giancarlo Sborgia Alfredo Niro Carmela Palmisano Pasquale Puzo Gianluigi Giuliani Luigi Sborgia Dario Sisto Valentina Pastore Claudio Furino Rossella Donghia Alessandra Sborgia Francesco Boscia Giovanni Alessio 2021International Journal of Ophthalmology(English edition)2021,14,12:2
