| 1 | MicroRNAs:a new ray of hope for diabetes mellitus显示文摘Diabetes mellitus has become one of the most common chronic diseases,thereby posing a major challenge to global health.Characterized by high levels of blood glucose(hyperglycemia),diabetes usually results from a loss of insulin-producing β-cells in the pancreas,leading to a deficiency of insulin(type 1 diabetes),or loss of insulin sensitivity(type 2 diabetes).Both types of diabetes have serious secondary complications,such as microvascular abnormalities,cardiovascular dysfunction,and kidney failure.Various complex factors,such as genetic and environmental factors,are associated with the pathophysiology of diabetes.Over the past two decades,the role of small,single-stranded noncoding microRNAs in various metabolic disorders,especially diabetes mellitus and its complications,has gained widespread attention in the scientific community.Discovered first as an endogenous regulator of development in the nematode Caenorhabditis elegans,these small RNAs post-transcriptionally suppress mRNA target expression.In this review,we discuss the potential roles of different microRNAs in diabetes and diabetes-related complications. | Munish Kumar Sayantan Nath Himanshu K Prasad G D Sharma Yong Li | 2012 | Protein & Cell2012,3,10: | 14 |
| 2 | 起作用的术语和手术后的管理途径适用于肝的外科: 受训练者观点显示文摘 Outcomes in hepatic resectional surgery(HRS) have improved as a result of advances in the understanding of hepatic anatomy,improved surgical techniques, and enhanced peri-operative management.Patients are generally cared for in specialist higher-level ward settings with multidisciplinary input during the initial post-operative period,however,greater acceptance and understanding of HRS has meant that care is transferred,usually after 24-48 h,to a standard ward environment.Surgical trainees will be presented with such patients either electively as part of a hepatobiliary firm or whilst covering the service on-call,and it is therefore important to acknowledge the key points in managing HRS patients.Understanding the applied anatomy of the liver is the key to determining the extent of resection to be undertaken.Increasingly,enhanced patient pathways exist in the post-operative setting requiring focus on the delivery of high quality analgesia,careful fluid balance,nutrition and thromboprophlaxis.Complications can occur including liver,renal and respiratory failure,hemorrhage,and sepsis,all of which require prompt recognition and management.We provide an overview of the relevant terminology applied to hepatic surgery,an approach to the post-operative management,and an aid to developing an awareness of complications so as to facilitate better confidence in this complex subgroup of general surgical patients. | Shahid G Farid K Rajendra Prasad Gareth Morris-Stiff | 2013 | World Journal of Gastrointestinal Surgery2013,5,5: | 2 |
| 3 | Assessment of the diagnostic performance and interobserver variability of endocytoscopy in Barrett's esophagus:A pilot ex-vivo study显示文摘AIM:To investigate a classification of endocytoscopy(ECS)images in Barrett’s esophagus(BE)and evaluate its diagnostic performance and interobserver variability.METHODS:ECS was applied to surveillance endoscopic mucosal resection(EMR)specimens of BE ex-vivo.The mucosal surface of specimen was stained with 1%methylene blue and surveyed with a catheter-type endocytoscope.We selected still images that were most representative of the endoscopically suspect lesion and matched with the final histopathological diagnosis to accomplish accurate correlation.The diagnostic performance and inter-observer variability of the new classification scheme were assessed in a blinded fashion by physicians with expertise in both BE and ECS and inexperienced physicians with no prior exposure to ECS.RESULTS:Three staff physicians and 22 gastroenterology fellows classified eight randomly assigned unknown still ECS pictures(two images per each classification)into one of four histopathologic categories as follows:(1)BEC1-squamous epithelium;(2)BEC2-BE without dysplasia;(3)BEC3-BE with dysplasia;and(4)BEC4-esophageal adenocarcinoma(EAC)in BE.Accuracy of diagnosis in staff physicians and clinical fellows were,respectively,100%and 99.4%for BEC1,95.8%and83.0%for BEC2,91.7%and 83.0%for BEC3,and95.8%and 98.3%for BEC4.Interobserver agreement of the faculty physicians and fellows in classifying each category were 0.932 and 0.897,respectively.CONCLUSION:This is the first study to investigate classification system of ECS in BE.This ex-vivo pilot study demonstrated acceptable diagnostic accuracy and excellent interobserver agreement. | Yutaka Tomizawa Prasad G Iyer Louis M Wongkeesong Navtej S Buttar Lori S Lutzke Tsung-Teh Wu Kenneth K Wang | 2013 | World Journal of Gastroenterology2013,19,46: | 2 |
