1乳腺癌生殖性危险因素与乳腺组织成分的关系显示文摘绝大多数乳腺肿瘤是由乳腺上皮细胞引起的癌变,被认为起源于乳腺间质成分的乳腺肉瘤则较罕见。由钼靶测量的乳腺密度越大,即有越高比例乳腺纤维腺体组织(由上皮和间质组成)的女性,乳腺癌风险越高。与生育相关的生殖因素也被认为是乳腺癌的风险因素,如胎次、初产年龄及母乳喂养等。为研究与生育相关的生殖因素与乳腺腺体组织成分关系,研究人员采用美国护士健康研究队列中经活检证实的良性乳腺疾病(BBD)女性开展研究。获取所有研究对象活检标本,使用深度学习技术在全切片图像上测量每种组织类型(上皮、间质和脂肪)百分比。研究共纳入经活检证实的BBD无癌女性983例,活检年龄范围19~58岁,平均活检年龄42岁。所有样本中上皮组织、间质组织及脂肪组织的比例范围分别为0.5%~52.2%(平均9.1%)、23.6%~99.0%(平均72.4%)及0~71.3%(平均18.5%)。多因素调整的广义线性回归分析结果显示,与已生育女性相比,未生育女性良性乳腺活检标本中上皮组织(β=-0.26,95%CI:-0.41~-0.11)和脂肪组织(β=-0.34,95%CI:-0.54~-0.13)比例较少,而间质组织比例较多(β=0.04,95%CI:0.01~0.08),尤其在绝经前女性中,组织分布比例差异更明显。在已生育女性中,生育胎次与间质组织比例呈负相关(每个孩子β=-0.01,95%CI:-0.02~-0.00);与初产年龄<25岁相比,初产年龄25~29岁女性良性乳腺活检标本中上皮组织比例较高(β=0.12,95%CI:0.03~0.21)。与母乳喂养时间0~1个月相比,母乳喂养持续时间≥24个月与良性乳腺活检标本中降低的脂肪组织比例有关(β=-0.30,95%CI:-0.54~-0.06;P_(趋势性)=0.04)。绝经前女性中,生育胎次多和初产年龄晚与较高比例上皮组织和较低比例间质组织有关。该研究结果也提示了上皮-间质相互作用的重要性。高鹰(编译) Yaghjyan L Austin-Datta RJ Oh H 2022中华健康管理学杂志2022,16,2:3
